Objective - To compare hemodynamic, clinicopathologic, and gastrointestinal motility effects and recovery characteristics of halothane and isoflurane in horses undergoing arthroscopic surgery. Animals - 8 healthy adult horses. Procedure - Anesthesia was maintained with isoflurane or halothane (crossover study). At 6 intervals during anesthesia and surgery, cardiopulmonary variables and related derived values were recorded. Recovery from anesthesia was assessed; gastrointestinal tract motility was subjectively monitored for 72 hours after anesthesia. Horses were administered chromium, and fecal chromium concentration was used to assess intestinal transit time. Venous blood samples were collected for clinicopathologic analyses before and 2, 24, and 48 hours after anesthesia. Results - Compared with halothane-anesthetized horses, cardiac index, oxygen delivery, and heart rate were higher and systemic vascular resistance was lower in isoflurane-anesthetized horses. Mean arterial blood pressure and the dobutamine dose required to maintain blood pressure were similar for both treatments. Duration and quality of recovery from anesthesia did not differ between treatments, although the recovery periods were somewhat shorter with isoflurane. After isoflurane anesthesia, gastrointestinal motility normalized earlier and intestinal transit time of chromium was shorter than that detected after halothane anesthesia. Compared with isoflurane, halothane was associated with increases in serum aspartate transaminase and glutamate dehydrogenase activities, but there were no other important differences in clinicopathologic variables between treatments. Conclusions and clinical relevance - Compared with halothane, isoflurane appears to be associated with better hemodynamic stability during anesthesia, less hepatic and muscle damage, and more rapid return of normal intestinal motility after anesthesia in horses undergoing arthroscopic procedures.

The purpose of this article was to report a series of 23 renal transplant recipients with histologically proven and immunohistochemically confirmed cytomegalovirus (CMV) lesions in the gastrointestinal tract (GIT) and to assess the risk factors associated with severe disease/mortality. CMV patients (n=23) were allocated into two groups: those who died (n=6) and those considered cured (n=17). Overall mortality rate was 26% (6/23). Initial symptoms suggestive of lower GIT involvement were observed in all death cases and in 35.3% of those cured (p=0.01). Enterorrhagia was seen in 83.3% of the patients who died. Death risk increased twofold (RR 2 [1.13-3.52], p=0.01) when symptoms of lower GIT involvement were initially observed and sixfold when enterrohagia was present (RR 6 [1.1-35.9], p=0.001). Among death cases, mean time at diagnosis was significantly more distant (2002±2.9×2008±1.6, p=0.04). The difference in mortality rates seen as service practices changed along the years demonstrates the importance of early diagnosis. © 2011 John Wiley & Sons A/S.

Over the last decade, several studies were conducted on the gastrointestinal changes associated to chronic heart failure. This article presents a literature review on the physiopathology and clinical consequences of pathological digestive changes of heart failure patients. Structural and functional abnormalities of the gastrointestinal tract, such as edema of absorptive mucosa and intestinal bacterial overgrowth, have been leading to serious clinical consequences. Some of these consequences are cardiac cachexia, systemic inflammatory activation and anemia. These conditions, alone or in combination, may lead to worsening of the pre-existing ventricular dysfunction. Although currently there is no therapy specifically earmarked for gastrointestinal changes associated to heart failure, the understanding of digestive abnormalities is germane for the prevention and management of systemic consequences.

O câncer do trato gastrointestinal tem sua importância no perfil de mortalidade do Brasil, estando entre os dez mais incidentes do país. A detecção precoce garante uma melhor qualidade de vida para os doentes oncológicos, porém frequentemente estes chegam aos centros de tratamento em fase avançada da doença. O estudo objetiva investigar as dificuldades de acesso ao diagnóstico e tratamento para os pacientes com câncer gastrointestinal atendidos pelo Sistema Único de Saúde. Com este intuito, realizou-se uma pesquisa observacional descritiva e sob a forma de um questionário foram coletados dados de pacientes em tratamento em dois hospitais públicos de Belém, no período de março a junho de 2013. Preencheram os critérios de inclusão 122 pacientes que foram agrupados em diferentes trajetórias de atendimento. Além disso, foram também obtidas informações registradas nos prontuários desses pacientes. A análise dos dados demonstrou que o diagnóstico da doença em 68,1% foi realizado pelo médico generalista; a maior dificuldade, nessa fase, foi o acesso ao diagnóstico gerando gastos com exames, pois a maioria dos pacientes (68,9%) não realizou exames especializados através do Sistema Único de Saúde, mas com recursos próprios. Nos centros/ unidades de referência em oncologia, as dificuldades relatadas por 56 pacientes começam com a marcação da consulta médica, ocorrendo demora do agendamento pela instituição para 94,6% desses doentes. A falta de leito para internação foi apontada como o maior entrave (54.4%) para iniciar a terapêutica cirúrgica, particularmente para o câncer gástrico e de cólon e reto. A análise das trajetórias percorridas pelos doentes, desde o inicio dos sintomas até o atendimento na unidade de referência, revela que o diagnóstico da doença em 50% dos pacientes ocorreu somente após 10 meses do inicio dos sintomas, e o tratamento iniciou só depois de 90 dias do diagnóstico. O tempo que os pacientes permanecem sintomáticos sem um diagnóstico impacta negativamente no prognóstico. Nesta pesquisa, os casos de câncer gástrico e de cólon e reto foram diagnosticados tardiamente (estádio IV e IIIB) e, por conseguinte o tratamento não ocorreu no prazo desejável.

Introdução: O câncer consiste em um problema de saúde publica mundial, com estimativa de 27 milhões de casos novos e 17 milhões de mortes por câncer no ano de 2030. No Brasil, as estimativas para o câncer no ano de 2014, apontam a ocorrência de aproximadamente 580 mil casos novos. As fluoropirimidinas são usadas nos principais esquemas quimioterápicos direcionados a tumores do trato gastrointestinal. Nos últimos anos muito se tem investigado sobre causas de respostas individuais diferenciadas em relação ao tratamento quimioterápico; dessa forma têm-se buscado uma terapia individualizada que possa maximizar a eficácia dos medicamentos e minimizar os efeitos adversos associados aos fármacos. Objetivamos buscar a associação do INDEL (rs16430) do gene TYMS com o padrão de resposta ao tratamento oncológico de fármacos com base em fluoropirimidinas, de maneira a contribuir para o desenvolvimento da medicina personalizada. Material: Estudadas 151 amostras de sangue periférico de pacientes oncológicos tratados com fluoropirimidinas, da população da região amazônica brasileira com elevado grau de mistura interétinica. Foi genotipado um polimorfismo INDEL (rs16430) no gene TYMS envolvido na resposta ao tratamento com uso de fluoropirimidinas. Resultados: A investigação relatou que a maioria dos pacientes tinha doença avançada no momento do diagnóstico; 32,7% receberam tratamento com intenção paliativa; e 22,8% tratamento neoadjuvante. Nossos resultados evidenciam que o polimorfismo INDEL no gene TYMS demonstrou ter um efeito de proteção à progressão tumoral (p=0,033). Pacientes tratados com fluoropirimidinas que eram homozigotos selvagens (INS/INS) apresentaram uma proteção à progressão tumoral de 24% comparado com outros genótipos desse polimorfismo. As estimativas de ancestralidade genômica global da amostra investigada foram: 62,4% europeia; 25,2% nativo americana e 12,4% africana. Foi possível estabelecer uma correlação inversa entre o aumento da ancestralidade ameríndia e a presença de metástase (p=0,024). Conclusão: Estudos farmacogenéticos podem proporcionar uma terapia personalizada reduzindo mortalidade por toxicidades e aumentando a eficácia terapêutica, desta forma proporcionando um tratamento oncológico com melhores resultados clínicos.

Endoscopy of the gastrointestinal tract is a specialized minimally invasive diagnostic method and with both diagnostic and therapeutic possibilities. It is divided into two major groups, the rigid and flexible endoscopy. The first most widely used for visualization of body cavities that do not have a light, like the chest, for example, and the second for those with light as the gastrointestinal tract. In Brazil there are few professionals, veterinary hospitals or clinics that offer this type of service. Thus, endoscopy is an area of expertise of the veterinarian who is in huge expansion and has shown great applicability with excellent results in both clinical medicine and surgical small animals

Seed dispersal is a key process in the life cycle of plant species and may determine the dynamics of their populations. The passage of the fruit from the gastrointestinal tract brings effects on energy and nutritional rewards for the bird. The retention time of the seeds is an essential factor for the dispersion, affecting the dispersal distance. Some factors determine it, as the size of the bird, degree of frugivory, chemical composition of the fruit, the number and size of seeds. The study sought to characterize the Morus nigra seed retention time in three species of thrushes and compare them with other species of birds and plants. The blackberry, exotic species with socio-economic importance in Brazil, produces aggregate fruit having up to 60 seeds. Have been kept in captivity individuals leucomelas Turdus rufiventris thrush, thrush and amaurochalinus, native and omnivorous species, important dispersers in degraded areas, inhabiting the urban and the natural environment. In 274 samples of feces was recorded about 500 mulberry seeds. The first record of seeds was in 15 minutes (n = 2) and the last to 115 minutes (n = 4) after eating the fruit. The average time of gastrointestinal transit of seed for the three species was 52 minutes and 80% were defecated to 65 minutes post ingestion. Taking into consideration the type of fruit and the species of bird, there is great variation in the retention time when compared to other species of birds, being in general slower. That way, can be increase the degree of dispersion of the seed to more distant areas of the mother plant, increasing the chances of survival of seedlings, according to the hypothesis Janzen and Connell. The Turdus It showed efficient dispersers Morus nigra seeds, and inhabits natural and urbanized environment, the possibility of exchange of these seeds between environments by the action of thrushes

The effect of inulin and/or okara flour on Lactobacillus acidophilus La-5 and Bifidobacterium animalis Bb-12 viability in a fermented soy product (FSP) and on probiotic survival under in vitro simulated gastrointestinal conditions were investigated throughout 28 days of storage at 4 °C. Employing a 22 design, four FSP trials were produced from soymilk fermented with ABT-4 culture (La-5, Bb-12, and Streptococcus thermophilus): FSP (control); FSP-I (with inulin, 3 g/100 mL of soymilk); FSP-O (with okara, 5 g/100 mL); FSP-IO (with inulin + okara, ratio 3:5 g/100 mL). Probiotic viabilities ranged from 8 to 9 log cfu/g during the 28 days of storage, and inulin and/or okara flour did not affect the viability of La-5 and Bb-12. Bb-12 resistance to the artificial gastrointestinal juices was higher than for La-5, since the Bb-12 and La-5 populations decreased approximately 0.6 log cfu/g and 3.8 log cfu/g, respectively, throughout storage period. Even though the protective effect of inulin and/or okara flour on probiotic microorganisms was not significant, when compared to a fresh culture, the FSP matrix improved Bb-12 survival on day 1 of storage and may be considered a good vehicle for Bb-12 and could play an important role in probiotic protection against gastrointestinal juices. © 2013 Elsevier Ltd.

Schwannoma is a tumor derived from Schwann cells which usually arises in the upper extremities, trunk, head and neck, retroperitoneum, mediastinum, pelvis, and peritoneum. However, it can arise in the gastrointestinal tract, including biliary tract. We present a 24-year-old male patient with obstructive jaundice, whose investigation with computed tomography abdomen showed focal wall thickening in the common hepatic duct, difficult to differentiate with hilar adenocarcinoma. He was diagnosed intraoperatively schwannoma of common bile duct and treated with local resection. The patient recovered well without signs of recurrence of the lesion after 12 mo. We also reviewed the common bile duct schwannoma related in the literature and evaluated the difficulty in pre and intraoperative differential diagnosis with adenocarcinoma hilar. Resection is the treatment of choice for such cases and the tumor did not recur in any of the resected cases. (C) 2012 Baishideng. All rights reserved.

The association between anisotropic magnetoresistive (AMR) sensor and AC biosusceptometry (ACB) to evaluate gastrointestinal motility is presented. The AMR-ACB system was successfully characterized in a bench-top study, and in vivo results were compared with those obtained by means of simultaneous manometry. Both AMR-ACB and manometry techniques presented high temporal cross correlation between the two periodicals signals (R = 0.9 +/- 0.1; P < 0.05). The contraction frequencies using AMR-ACB were 73.9 +/- 7.6 mHz and using manometry were 73.8 +/- 7.9 mHz during the baseline (r = 98, p < 0.05). The amplitude of contraction using AMR-ACB was 396 +/- 108 mu T.s and using manometry were 540 +/- 198 mmHg.s during the baseline. The amplitudes of signals for AMR-ACB and manometric recordings were similarly increased to 86.4% and 89.3% by neostigmine, and also decreased to 27.2% and 21.4% by hyoscine butylbromide in all animals, respectively. The AMR-ACB array is nonexpensive, portable, and has high-spatiotemporal resolution to provide helpful information about gastrointestinal tract.

Mitogen-activated protein kinase (MAPK) pathways are activated by several stimuli and transduce the signal inside cells, generating diverse responses including cell proliferation, differentiation, migration and apoptosis. Each MAPK cascade comprises a series of molecules, and regulation takes place at different levels. They communicate with each other and with additional pathways, creating a signaling network that is important for cell fate determination. In this review, we focus on ERK, JNK, p38 and ERK5, the major MAPKs, and their interactions with PI3K-Akt, TGFβ/Smad and Wnt/β-catenin pathways. More importantly, we describe how MAPKs regulate cell proliferation and differentiation in the rapidly renewing epithelia that lines the gastrointestinal tract and, finally, we highlight the recent findings on nutritional aspects that affect MAPK transduction cascades.

Serotonin (5-hydroxytryptamine, 5-HT) is involved in gastrointestinal tract (GIT) motor functions through binding to specific receptors located in the GIT walls. The objectives of the current study were to compare mRNA levels and binding sites of 5-HT(4) receptors (5-HTR(4)) in smooth muscle layers from the fundus abomasi, pylorus, ileum, cecum, proximal loop of the ascending colon (PLAC), and external loop of the spiral colon (ELSC) of healthy dairy cows, and to verify whether mRNA and protein expression were correlated. Smooth muscle samples were prepared by scraping the mucosa and submucosa from full-thickness intestinal wall samples. The mRNA levels of 5-HTR(4) were measured by real-time PCR and expressed relative to those of the housekeeping gene glyceraldehyde phosphate dehydrogenase. Binding studies were performed using the 5-HTR(4) antagonist [(3)H]GR113808. The mRNA levels of 5-HTR(4) were affected (P < 0.05) by location along the GIT. The mRNA levels of 5-HTR(4) in the ELSC and the ileum were greater than in the PLAC (P = 0.05 and P = 0.07, respectively) but similar to those of all other locations. The competitive binding of [(3)H]GR113808 to suspended membranes from the fundus abomasi, pylorus, cecum, and ELSC was best fit by a 2-site receptor model, whereas it was best fit by a 1-site receptor model in the ileum and PLAC. The mRNA levels and numbers of 5-HTR(4) were not correlated (r = 0.14; P = 0.71). In conclusion, mRNA and binding sites for 5-HTR(4) are present in the smooth muscle layer of the entire GIT of dairy cows and may play a role with respect to motility. The effects of activation of this receptor subtype may be different among GIT locations due to differences in the amount of high- relative to low-affinity binding sites.

Colonisation of the gastrointestinal tract by anaerobic bacteria, protozoa, trematodes, cestodes and/or nematodes and other infectious pathogens, including viruses, represents a major cause of morbidity and mortality in Africa, South America and southeast Asia, as well as other parts of the world. Nitazoxanide is a member of the thiazolide class of drugs with a documented broad spectrum of activity against parasites and anaerobic bacteria. Moreover, the drug has recently been reported to have a profound activity against hepatitis C virus infection. In addition, nitazoxanide exhibits anti-inflammatory properties, which have prompted clinical investigations for its use in Crohn's disease. Studies with nitazoxanide derivatives have determined that there must be significantly different mechanisms of action acting on intracellular versus extracellular pathogens. An impressive number of clinical studies have shown that the drug has an excellent bioavailability in the gastrointestinal tract, is fast acting and highly effective against gastrointestinal bacteria, protozoa and helminthes. A recent Phase II study has demonstrated viral response (hepatitis C) to monotherapy, with a low toxicity and an excellent safety profile over 24 weeks of treatment. Pre-clinical studies have indicated that there is a potential for application of this drug against other diseases, not primarily affecting the liver or the gastrointestinal tract.
