Puede parecer una obviedad pero, aún así, resultaría pertinente comenzar nuestra comunicación expresando que religiones a nuestro alcance hay más de una. De entrada, nos damos cuenta de que el hecho de hablar de «religión» para referirnos al catolicismo, el budismo o el Islam representa una conquista cultural porque implica englobarlas a todas dentro de una misma categoría: Aquello que da sentido. Y hoy, más que en otras épocas, tenemos que hablar de religiones porque la realidad contemporánea lo exige al son de un pluralismo religioso que muestra cambios en el papel y resalta la importancia de las distintas formas de religiosidad. Simultáneamente a este hecho, parecería que se da una progresiva secularización de la sociedad, que paradójicamente cabe combinar con un incremento del número de creencias y una mayor difusión de éstas. Estos dos elementos, pluralismo y secularización, son el germen de un proyecto de investigación cuyos resultados pretendemos mostrar en esta ponencia. Dicho estudio se ha llevado a cabo por el grupo de investigación ISOR, Investigaciones en Sociología de la Religión de la Universidad Autónoma de Barcelona. El estudio perseguía justamente facilitar estrategias, instrumentos e información al personal de los centros educativos, para un reconocimiento mutuo entre los actores en situaciones de diversidad religiosa. El objetivo se enmarcaba en un interés más general: Demostrar hasta qué punto la pluralidad religiosa convierte las situaciones cotidianas y evidentes en situaciones “cuestionadas” que exigen adaptación. Este trabajo de investigación partió de un análisis exhaustivo de la diversidad religiosa en diferentes ámbitos educativos enfatizando tanto aquello que es evidente como lo que pasa desapercibido. La información utilizada procede de un análisis detallado de 26 entrevistas en profundidad realizadas a miembros de distintos centros educativos catalanes, seleccionados según los siguientes criterios: el tipo de centro (infantil, primaria, secundaria), la titularidad del centro (público, privado, concertado), la diversidad territorial, y la presencia proporcionalmente significativa de alumnado de tradiciones religiosas minoritarias. El resultado de nuestra investigación nos permite afirmar que, teniendo en cuenta sólo los elementos religiosos, podemos constatar que en la inmensa mayoría de las ocasiones la diversidad religiosa no plantea problemas graves en los centros educativos. Sin embargo, al tiempo que hacemos esta afirmación, no podemos obviar la generalización de un conjunto de situaciones potencialmente conflictivas por la diversidad religiosa y que, pese a todo, son invisibilizadas, entre otras razones, por el capital cultural de las familias, el dominio que éstos tienen del idioma, o la incomprensión de los códigos de relación.

Uno de los debates más candentes de la situación socio-política europea es el de la inmigración y las políticas de identidad. A causa de acontecimientos recientes, como la “Ley del Velo” o los disturbios en Francia en el 2005, el discurso que domina discusiones políticas y educativas sobre la inmigración, es el de una crisis de “los modelos de integración”, particularmente en lo que concierne a las segundas generaciones de inmigrantes. La Oficina Central de Estadística en Holanda, por ejemplo, ha publicado datos que indican que los jóvenes de segunda generación turcos y marroquíes se sienten menos “en casa” en el país, que sus propios padres (2006). Una pregunta importante es: ¿Cómo se llega a ese sentimiento de alienación? Aunque esta ponencia no puede abarcar esta pregunta en toda su complejidad, lo que sí ofrece es la rara oportunidad de examinar las interacciones de la vida cotidiana de un grupo de niños/as marroquíes con sus compañeros/as españoles durante la jornada escolar. Investigar las relaciones que los niños/as inmigrantes tienen con otros niños/as es crucial porque las identidades que desarrollan estos niños/as y su propio sentido de quiénes son, dependen, no sólo de las interacciones que tienen en su vida privada, sino también de las reacciones de otros compañeros/as y de sus interacciones en la escuela y otras instituciones (Appiah, 2006; Taylor, 1994). Es importante entender este fenómeno porque estos/as niños/as van a crecer o bien sintiendose miembros de pleno derecho de la sociedad española, o bien con un acusado sentido de alienación y no-pertenencia. Por lo tanto, el análisis de estas interacciones es una ventana que puede ofrecernos un resquicio para dilucidar como estos procesos de exclusión y alienación podrían empezar en la infancia y la pre-adolescencia. Este análisis es parte de un estudio etnográfico de socialización lingüística desarrollado en 12 meses, durante los cuales investigué la vida cotidiana de los niños/as de seis familias marroquíes en un pueblo del suroeste español. Este estudio consistía en grabar las rutinas de interacción en las que los niños/as participaban. Para este trabajo, he analizado 17 horas de video recogidos en la escuela primaria del pueblo. El análisis se centra en las prácticas lingüísticas y no-verbales que organizan formas de exclusión social de niños/as inmigrantes marroquíes por parte de sus compañeros/as españoles, a pesar del discurso público de inclusion, tolerancia y respeto que caracterizan los planes curriculares oficiales del centro escolar. A través de estas prácticas, se mostrará como a los niños/as inmigrantes se les adscriben características negativas y se les asignan posiciones para un trato diferencial.

Aquesta recerca analitza els conflictes i les articulacions entre el dret local i el dret estatal que tenen lloc a les comunitats rurals de Madagascar a l’hora de resoldre els conflictes a l’àmbit local. L’anàlisi es porta a terme a partir d’un cas etnogràfic concret que permet descriure aquesta pluralitat jurídica. A la present memòria es descriu el treball realitzat en el recorregut de la investigació i dóna compte de la gestació del projecte fins a l’actualitat, moment en el qual la recerca es troba en el seu darrer punt de redacció de la tesi.

El objetivo del presente trabajo es realizar un análisis regional y sectorial del Balance Neto de agua virtual asociada al comercio entre la región de Andalucía y el resto de España para el año 2005. Para ello, desarrollamos una metodología que nos permite efectuar un análisis comparativo del uso del agua en ambas regiones, en cuanto al impacto de su producción sectorial y de su demanda final. Incluimos en el marco metodológico el nuevo concepto de Agua Virtual, ya que está tomando gran relevancia en la arena de la Gestión Hídrica. La base del marco metodológico es el análisis Input-Output. En particular, construimos un modelo Input-Output Multi-Regional (MRIO) para las regiones consideradas. Esta herramienta es el método más utilizado para los estudios en los que se asigna la responsabilidad del uso de recursos de acuerdo a la demanda final, localizada territorialmente, de cada rama productiva. Entre otras ventajas, nos permite analizar los vínculos interregionales e intersectoriales de las regiones consideradas. La incorporación de la técnica de la integración vertical o subsistemas a nivel interregional nos permite un enfoque alternativo para el Balance Neto resultante, en el que puede examinarse la importancia de una determinada rama productiva de acuerdo a su influencia en el resto de sectores de las diversas regiones. No tenemos constancia de que este enfoque haya sido utilizado con anterioridad en los análisis MRIO aplicados al estudio de los impactos medioambientales incorporados en el comercio. Tampoco la tenemos sobre la aplicación de los MRIO de forma general para estos impactos a nivel interregional de la economía española. Los estudios previos sobre el uso del agua de la región andaluza se han centrado en el comercio internacional ante la dificultad de hacer un análisis exhaustivo a nivel sectorial utilizando las estadísticas de transporte por carretera. Sin embargo, en términos monetarios, el comercio interior de la región con el resto de España supone un 52% de las “exportaciones” y un 62% de las “importaciones”. Por lo tanto, a nivel práctico, este trabajo aporta a los estudios desarrollados con anterioridad la esencial perspectiva del comercio interior. Los resultados esperados contribuyen a mejorar una información de extrema relevancia para una posible reforma estructural de la economía y el comercio interregional andaluz acorde a un mejor uso del recurso agua.

Vestidors nus presenta una descripció etnogràfica centrada en el vestidor femení d'una piscina pública. Les línies teòriques d'estudi parteixen de l'espai físic i la seva relació amb el comportament humà, tenint en compte les qualitats biològiques, culturals, socials i de gènere.

Els membres de l'ètnia bara de Madagascar, eminentment ramadera, viuen la seva cultura entorn d'un animal, el zebú, que no només és moneda de canvi i força per al transport, sinó tot un símbol per a aquest poble, i entorn del qual se segueixen tot un seguit de rituals. L'estudi es centra en el robatori de zebús, com a ritual de pas de l'edat jove a l'adulta. Tanmateix, el poble bara és en l'actualitat víctima d'un prejudici lamentable, ja que s'associa amb els lladres de bestiar en el context del problema social i econòmic actual de bandits armats i el robatori de l'espècie bovina a gran escala.Passant d'allò general a allò concret, l'estudi s'estructura de la següent manera: en primer lloc, es fa una aproximació inicial a l'ètnia bara i a la seva cultura lligada al seu ramat, fent esment al fenomen de la desforestació que devasta el país i que guarda certa relació amb l'objecte de recerca; posteriorment, es passa a explicar en què consisteix el robatori de zebús, les diverses motivacions que el provoquen i la seva evolució fins a l'actualitat.

In April 2009, in response to the WHO's alert due to the existence of human infection cases with a new AH1N1 influenza virus, known as swine flu, Andalusian Health Authorities trigger an specific action plan. The surveillance actions developped provided us with appropriate clinical, epidemiological and virological characteristics of the disease. During the first few days, contingency plans were set up based on epidemiological surveillance and outbreak control measures were adopted through early alert and rapid response systems. After phase 6 was declared, influenza sentinel and severe cases surveillance were used in order to plan healthcare services, to reduce transmission and to identify and protect the most vulnerable population groups. Behaviour of pandemic influenza in Andalusia was similar to that observed in the rest of the world. Atack rate was similar to a seasonal flu and the peak was reached at the 46th/2009 week. Most of them were mild cases and affected particularly to young people. The average age of hospitalised patients was 32. Prior pulmonary disease, smoking and morbid obesity (BMI>40) were the most common pathologies and risk factors in severe cases. An impact scenario of pandemic wave in Andalusia, with an expected attack rate from 2 to 5%, was prepared considering watt observed in the southern hemisphere. Characteristics of the epidemic concerning its extent, severity and mortality rate were adjusted to this scenario.

The Department of Health of the Government of Andalusia provides professionals of the Andalusian Public Health Care System a collaborative working environment (Entorno colaborativo de trabajo [ECT]) based on the principles of web 2.0. The ECT is organized into communities, understood as sets of people with a common interest who share a space with its own information and collaboration tools. This space is managed and powered autonomously by the communities themselves. This paper analyzes the use and degree of implementation of the ECT, considering the user communities and activity statistics in 2009 and 2010. From the data obtained we deduce that instrumental services have easier acceptance than collaboration and knowledge management services; content generation is focused on a small number of users; and communities associated with organizational units have less development than those associated with work areas or projects.

Background: Most mortality atlases show static maps from count data aggregated over time. This procedure has several methodological problems and serious limitations for decision making in Public Health. The evaluation of health outcomes, including mortality, should be approached from a dynamic time perspective that is specific for each gender and age group. At the moment, researches in Spain do not provide a dynamic image of the population’s mortality status from a spatio-temporal point of view. The aim of this paper is to describe the spatial distribution of mortality from all causes in small areas of Andalusia (Southern Spain) and evolution over time from 1981 to 2006. Methods: A small-area ecological study was devised using the municipality as the unit for analysis. Two spatiotemporal hierarchical Bayesian models were estimated for each age group and gender. One of these was used to estimate the specific mortality rate, together with its time trends, and the other to estimate the specific rate ratio for each municipality compared with Spain as a whole. Results: More than 97% of the municipalities showed a diminishing or flat mortality trend in all gender and age groups. In 2006, over 95% of municipalities showed male and female mortality specific rates similar or significantly lower than Spanish rates for all age groups below 65. Systematically, municipalities in Western Andalusia showed significant male and female mortality excess from 1981 to 2006 only in age groups over 65. Conclusions: The study shows a dynamic geographical distribution of mortality, with a different pattern for each year, gender and age group. This information will contribute towards a reflection on the past, present and future of mortality in Andalusia.

Background: Cardiovascular diseases are ranked among the leading causes of death in the industrialized countries. This study is aimed at ascertaining the mortality trends by ischemic heart disease (IHD) and cerebrovascular diseases (CVD) in Andalusia within the 1975-2004 period. Method: Based on the official IHD and CVD death statistics and the related populations, the gross rates (GR) and age-adjusted rates (TS) and the Potential Years of Life Lost (PYLL) were calculated. To quantify the trends and their change points, a joinpoint regression analysis was made. Results: The number of IHD deaths for females rose from 2,086 deaths in 1975 to 3,336 in 2004, the TS having dropped from 74.29 to 50.94 deaths/100,000 females, the PYLL having dropped from 173.65 years to 90.56 years/100,000 females. The number of deaths for males rose from 2,854 deaths in 1975 to 4,085 in 2004, the TS having dropped from 147, 67 to 104.96 deaths /100,000 males. The PYLL showed a like behaviour from the first to the last year of the series, showing values of 716.46 and 460.04 years / 100,000 males. For the IHD in females, the number of deaths in absolute numbers dropped from 4,712 to 4,221, the TS having dropped from 166.00 to 62.08 deaths in females, and the PYLL from 338.08 to 87.63 years / 100,000 females. For males, the number of deaths dropped from 3,714 to 2,951, the TS from 206.88 deaths /100,000 males in 1975 to 76.12 /100,000 males in 2004, and the PYLL dropping from 533.12 to 182.38 years / 100,000 males. Conclusions: The trend in mortality due to IHD was not constant either among females or males, although it has always been a downward trend, the drop being statistically significant. The drop in the CVD has been such a major one that both the absolute numbers and the gross rates are lower for the most recent years that the first years in the series studied despite the aging of Andalusia’s population.

Currently equality plans are an effective tool to incorporate equal opportunities in human resource management of both companies and public administrations. They are also the appropriate mechanism to detect possible disparities and gender gaps in employment. Configured as an ordered set of actions and measures taken after a diagnosis of the situation and help organizations to promote equal treatment and opportunities between women and men in the workplace, and help eliminate discrimination based on sex. In this regard, the Directorate General of Health Planning and Innovation of the Ministry of Health, intends to promote and facilitate the development and implementation of equality plans in institutions and centers of the Andalusian Public Health System. This has raised the production and dissemination of a methodological guide that supports the design of equality plans in unison to allow both the completion of the monitoring process as the development and practical implementation of those plans.

Background. The study of the severity of occupational injuries is very important for the establishment of prevention plans. The aim of this paper is to analyze the distribution of occupational injuries by a) individual factors b) work place characteristics and c) working conditions and to analyze the severity of occupational injuries by this characteristics in men and women in Andalusia. Methods. Injury data came from the accident registry of the Ministry of Labor and Social Issues in 2003. Dependent variable: the severity of the injury: slight, serious, very serious and fatal; the independent variables: the characteristics of the worker, company data, and the accident itself. Bivariate and multivariate analysis were done to estimate the probability of serious, very serious and fatal injury, related to other variables, through odds ratio (OR), and using a 95% confidence interval (CI 95%). Results. The 82,4% of the records were men and 17,6% were women, of whom the 78,1% are unskilled manual workers, compared to 44,9% of men. The men belonging to class I have a higher probability of more severe lesions (OR = 1,67, 95% CI = 1,17 – 2,38). Conclusions. The severity of the injury is associated with sex, age and type of injury. In men it is also related with the professional situation, the place where the accident happened, an unusual job, the size and the characteristics of the company and the social class, and in women with the sector

Background The epidemic of HIV/AIDS and treatments that have emerged to alleviate, have brought about a shift in the burden of disease from death to quality of life/disability. The aim was to determine which factors are associated with improvements in the level of health of male and female patients with HIV/AIDS in Andalusia, in terms of disability-adjusted life years. Methods Descriptive study based on a sample group of 8800 people on the Andalusian AIDS register between 1983 and 2004. Dependent variables: Life lost due to premature mortality (YLL), years lost due to disability (YLD) and disability-adjusted life years (DALY). Independent variables: vital state, sex, age at the time of diagnosis, age at the time of death, transmission category, province of residence, AIDS-indicator disease and the period of diagnosis. A bivariate analysis was carried out to find out if the health level variables changed in accordance with the independent variables. Using the independent variables which had a statistically significant link with the level of health variables, a multivariate linear regression model, disaggregated by gender, was constructed. Results Amongst the women, we found a model which explained the level of health of 64.9%: a link was found between a higher level of health (lower DALYs) and not intravenous drug use, the province of residence, being diagnosed during the HAART era and older age at the time of diagnosis. Amongst the men, we found a model which explained the level of health of 64.4%: a link was found between a higher level of health (lower DALYs) and intravenous drug use, the province of residence, being diagnosed during the HAART era and older age at the time of diagnosis. Conclusion A higher level of health (lower DALY) amongst both men and women was found to be linked to not be intravenous drug user, the province of residence, being diagnosed during the HAART era and older age at the time of diagnosis.

Background Demand for home care services has increased considerably, along with the growing complexity of cases and variability among resources and providers. Designing services that guarantee co-ordination and integration for providers and levels of care is of paramount importance. The aim of this study is to determine the effectiveness of a new case-management based, home care delivery model which has been implemented in Andalusia (Spain). Methods Quasi-experimental, controlled, non-randomised, multi-centre study on the population receiving home care services comparing the outcomes of the new model, which included nurse-led case management, versus the conventional one. Primary endpoints: functional status, satisfaction and use of healthcare resources. Secondary endpoints: recruitment and caregiver burden, mortality, institutionalisation, quality of life and family function. Analyses were performed at base-line, and at two, six and twelve months. A bivariate analysis was conducted with the Student's t-test, Mann-Whitney's U, and the chi squared test. Kaplan-Meier and log-rank tests were performed to compare survival and institutionalisation. A multivariate analysis was performed to pinpoint factors that impact on improvement of functional ability. Results Base-line differences in functional capacity – significantly lower in the intervention group (RR: 1.52 95%CI: 1.05–2.21; p = 0.0016) – disappeared at six months (RR: 1.31 95%CI: 0.87–1.98; p = 0.178). At six months, caregiver burden showed a slight reduction in the intervention group, whereas it increased notably in the control group (base-line Zarit Test: 57.06 95%CI: 54.77–59.34 vs. 60.50 95%CI: 53.63–67.37; p = 0.264), (Zarit Test at six months: 53.79 95%CI: 49.67–57.92 vs. 66.26 95%CI: 60.66–71.86 p = 0.002). Patients in the intervention group received more physiotherapy (7.92 CI95%: 5.22–10.62 vs. 3.24 95%CI: 1.37–5.310; p = 0.0001) and, on average, required fewer home care visits (9.40 95%CI: 7.89–10.92 vs.11.30 95%CI: 9.10–14.54). No differences were found in terms of frequency of visits to A&E or hospital re-admissions. Furthermore, patients in the control group perceived higher levels of satisfaction (16.88; 95%CI: 16.32–17.43; range: 0–21, vs. 14.65 95%CI: 13.61–15.68; p = 0,001). Conclusion A home care service model that includes nurse-led case management streamlines access to healthcare services and resources, while impacting positively on patients' functional ability and caregiver burden, with increased levels of satisfaction.

Despite medical advances, mortality in infective endocarditis (IE) is still very high. Previous studies on prognosis in IE have observed conflicting results. The aim of this study was to identify predictors of in-hospital mortality in a large multicenter cohort of left-sided IE.Methods An observational multicenter study was conducted from January 1984 to December 2006 in seven hospitals in Andalusia, Spain. Seven hundred and five left-side IE patients were included. The main outcome measure was in-hospital mortality. Several prognostic factors were analysed by univariate tests and then by multilogistic regression model. Results.The overall mortality was 29.5% (25.5% from 1984 to 1995 and 31.9% from 1996 to 2006; Odds Ratio 1.25; 95% Confidence Interval: 0.97-1.60; p = 0.07). In univariate analysis, age, comorbidity, especially chronic liver disease, prosthetic valve, virulent microorganism such as Staphylococcus aureus, Streptococcus agalactiae and fungi, and complications (septic shock, severe heart failure, renal insufficiency, neurologic manifestations and perivalvular extension) were related with higher mortality. Independent factors for mortality in multivariate analysis were: Charlson comorbidity score (OR: 1.2; 95% CI: 1.1-1.3), prosthetic endocarditis (OR: 1.9; CI: 1.2-3.1), Staphylococcus aureus aetiology (OR: 2.1; CI: 1.3-3.5), severe heart failure (OR: 5.4; CI: 3.3-8.8), neurologic manifestations (OR: 1.9; CI: 1.2-2.9), septic shock (OR: 4.2; CI: 2.3-7.7), perivalvular extension (OR: 2.4; CI: 1.3-4.5) and acute renal failure (OR: 1.69; CI: 1.0-2.6). Conversely, Streptococcus viridans group etiology (OR: 0.4; CI: 0.2-0.7) and surgical treatment (OR: 0.5; CI: 0.3-0.8) were protective factors.Conclusions Several characteristics of left-sided endocarditis enable selection of a patient group at higher risk of mortality. This group may benefit from more specialised attention in referral centers and should help to identify those patients who might benefit from more aggressive diagnostic and/or therapeutic procedures.
