Problématique. Le diabete de type 2 est la maladie metabolique la plus frequente, sa prevalence ne cesse de croitre et parallelement, la population vieillissante est en constante augmentation. Le diagnostic de diabete de type 2 accroit le risque de syndrome geriatrique. Pour prevenir d'eventuelles complications, le traitement du diabete de type 2 est constitue de differentes composantes incluant des automesures de la glycemie capillaire, une prise reguliere du traitement pharmacologique souvent multiples de l'activite physique et enfin le suivi de recommandations pour une alimentation equilibree. Pour la personne agee diabetique de type 2, l'apprentissage de l'application de ces composantes du traitement signifie qu'elle doit proceder a des ajustements dans sa vie quotidienne. Buts. Les buts de cette etude sont : 1) de decrire l'ajustement psychosocial de la personne agee au diabete de type 2, et 2) d'explorer la presence de relations entre differentes variables sociodemographiques et de sante et l'ajustement psychosocial de personnes agees diabetiques de type 2. Méthodologie. Il s'agit d'une etude descriptive correlationnelle. L'echantillon de convenance est compose de 57 participants ages de plus de 65 ans connus pour un diagnostic de diabete de type 2. Les mesures ont ete effectuees par l'utilisation du questionnaire « The Psychological Adjustement to Illness Scale (PAIS-SR) » developpe par Derogatis (1986). Les donnees ont ete recoltees dans un centre ambulatoire universitaire de consultation en diabetologie en Suisse Romande. La théorie de l'adaptation cognitive lors d'événements menaçants (Taylor, 1983) a servi de cadre theorique a l'etude. Résultats. La majorite des participants a l'etude est de sexe masculin et l'age moyen des participants a l'etude est de 71 ans. Les participants ont un ajustement psychosocial a la maladie positif. Le score des signes depressifs est faible, tandis que davantage de difficultes apparaissent dans le domaine des relations sexuelles. Tous les participants ont des comorbidites ou ont developpe des complications. La moyenne de l'hemoglobine glyquee (HbA1c) correspond aux normes preconisees par les experts. Aucune correlation significative n'a pu etre etablie entre le niveau d'ajustement psychosocial, et les variables sociodemographiques et de sante.Conclusion : Les participants a l'etude ont un controle glycemique optimal et presentent un ajustement psychosocial positif. Malgre le fait qu'aucune correlation n'a pu etre determinee entre les differentes variables, la problematique reste importante a etudier. Les complications et les comorbidites associees au diagnostic de diabete de type 2 rendent cette population particulierement vulnerable, et a risque de declin fonctionnel. Peu d'etudes ont permis de determiner les composantes favorisant le processus d'adaptation d'un etat de sante vers un etat de maladie, et ce dans un contexte de vieillissement. La recherche dans ce domaine permettrait de determiner plus precisement des axes de prise en charge et de favoriser la prevention du declin fonctionnel.

The causal mechanism and seasonal evolution of the internal wave field in a deep, warm, monomictic reservoirare examined through the analysis of field observations and numerical techniques. The study period extends fromthe onset of thermal stratification in the spring until midsummer in 2005. During this time, wind forcing wasperiodic, with a period of 24 h (typical of land–sea breezes), and the thermal structure in the lake wascharacterized by the presence of a shallow surface layer overlying a thick metalimnion, typical of small to mediumsized reservoirs with deep outtakes. Basin-scale internal seiches of high vertical mode (ranging from mode V3 toV5) were observed in the metalimnion. The structure of the dominant modes of oscillation changed asstratification evolved on seasonal timescales, but in all cases, their periods were close to that of the local windforcing (i.e., 24 h), suggesting a resonant response. Nonresonant oscillatory modes of type V1 and V2 becamedominant after large frontal events, which disrupted the diurnal periodicity of the wind forcing

Occupational therapy (OT) is a profession concerned with promoting health and well-being through occupation, by enabling handicapped people to participate in the activities of everyday life. OT is part of the clinical rehabilitation of progressive genetic neurodegenerative diseases such as spinocerebellar ataxias; however, its effects have never been determined in these diseases. Our aim was to investigate the effect of OT on both physical disabilities and depressive symptoms of spinocerebellar ataxia type 3 (SCA3) patients. Genomically diagnosed SCA3 patients older than 18 years were invited to participate in the study. Disability, as evaluated by functional independence measurement and Barthel incapacitation score, Hamilton Rating Scale for Depression, and World Health Organization Quality of Life questionnaire (WHOQOL-BREF), was determined at baseline and after 3 and 6 months of treatment. Twenty-six patients agreed to participate in the study. All were treated because OT prevents blinding of a control group. Fifteen sessions of rehabilitative OT were applied over a period of 6 months. Difficult access to food, clothing, personal hygiene, and leisure were some of the main disabilities focused by these patients. After this treatment, disability scores and quality of life were stable, and the Hamilton scores for depression improved. Since no medication was started up to 6 months before or during OT, this improvement was related to our intervention. No association was found between these endpoints and a CAG tract of the MJD1 gene (CAGn), age, age of onset, or neurological scores at baseline (Spearman test). Although the possibly temporary stabilization of the downhill disabilities as an effect of OT remains to be established, its clear effect on depressive symptoms confirms the recommendation of OT to any patient with SCA3 or spinocerebellar ataxia.

Sympathetic ganglion block (SGB) or intravenous regional block (IVRB) has been recommended for pain management in patients with complex regional pain syndrome type I (CRPS-I). Forty-five patients were initially selected but only 43 were accepted for the study. The present study evaluated the efficacy of IVRB produced by combining 70 mg lidocaine with 30 µg clonidine (14 patients, 1 male/13 females, age range: 27-50 years) versus SGB produced by the injection of 70 mg lidocaine alone (14 patients, 1 male/13 females, age range: 27-54 years) or combined with 30 µg clonidine (15 patients, 1 male/14 females, age range: 25-50 years) into the stellate ganglion for pain management in patients with upper extremity CRPS-I. Each procedure was repeated five times at 7-day intervals, and pain intensity and duration were measured using a visual analog scale immediately before each procedure. A progressive and significant reduction in pain scores and a significant increase in the duration of analgesia were observed in all groups following the first three blocks, but no further improvement was obtained following the last two blocks. Drowsiness, the most frequent side effect, and dry mouth occurred only in patients submitted to SGB with lidocaine combined with clonidine. The three methods were similar regarding changes in pain intensity and duration of analgesia. However, IVRB seems to be preferable to SGB due to its easier execution and lower risk of undesirable effects.

Progressive myelopathies can be secondary to inborn errors of metabolism (IEM) such as mucopolysaccharidosis, mucolipidosis, and adrenomyeloneuropathy. The available scale, Japanese Orthopaedic Association (JOA) score, was validated only for degenerative vertebral diseases. Our objective is to propose and validate a new scale addressing progressive myelopathies and to present validating data for JOA in these diseases. A new scale, Severity Score System for Progressive Myelopathy (SSPROM), covering motor disability, sphincter dysfunction, spasticity, and sensory losses. Inter- and intra-rater reliabilities were measured. External validation was tested by applying JOA, the Expanded Disability Status Scale (EDSS), the Barthel index, and the Osame Motor Disability Score. Thirty-eight patients, 17 with adrenomyeloneuropathy, 3 with mucopolysaccharidosis I, 3 with mucopolysaccharidosis IV, 2 with mucopolysaccharidosis VI, 2 with mucolipidosis, and 11 with human T-cell lymphotropic virus type-1 (HTLV-1)-associated myelopathy participated in the study. The mean ± SD SSPROM and JOA scores were 74.6 ± 11.4 and 12.4 ± 2.3, respectively. Construct validity for SSPROM (JOA: r = 0.84, P < 0.0001; EDSS: r = -0.83, P < 0.0001; Barthel: r = 0.56, P < 0.002; Osame: r = -0.94, P < 0.0001) and reliability (intra-rater: r = 0.83, P < 0.0001; inter-rater: r = 0.94, P < 0.0001) were demonstrated. The metric properties of JOA were similar to those found in SSPROM. Several clinimetric requirements were met for both SSPROM and JOA scales. Since SSPROM has a wider range, it should be useful for follow-up studies on IEM myelopathies.

This correlational study was designed to investigate the relationship between self-directed learning and personality type. A sample of 133 graduate and undergraduate education students completed the MBTI and the SDLRS. Two hypotheses were examined: (a) scores on the intuitive scale will account for a significant amount of the variance in the prediction of selfdirected learning readiness and, (b) scores on the introverted scale will account for a significant amount of the variance in self-directed learning readiness. Stepwise multiple regression analyses indicated that psychological type accounts for 28% of the variance in self-directed learning. Support for the first hypothesis was found with 15% of the variance in selfdirected learning accounted for by intuition. The second hypothesis was not supported. Introversion accounted for 13% of the variance but in a negative manner. Results of this study indicate that personality type does influence the ability of the learner to be self-directed in studies. These findings add another dimension for the adult educator to consider when attempting to develop self-directedness in learners.

Le diabète de type 1 (DT1) est une maladie complexe qui requiert une implication importante des patients pour contrôler leur glycémie et ainsi prévenir les complications et comorbidités. L’activité physique (AP) régulière et une attention constante pour les glucides ingérés sont des adjuvants essentiels au traitement insulinique. Nous avons démontré que le questionnaire BAPAD-1, spécifiquement développé pour des adultes atteints de DT1, est un outil valide (validité prédictive, fiabilité interne et reproductibilité) pour définir des barrières associées à l’AP. Bien que le niveau de barrières envers l’AP soit faible, la crainte de l’hypoglycémie est la barrière la plus importante chez cette population. L’adoption d’un mode de vie actif est associée à un profil corporel favorable. Les adultes, avec un DT1 et non diabétique, qui maintiennent un bon niveau d’AP, soit un ratio entre la dépense énergétique totale et celle au repos ≥ 1.7, ont une masse grasse, un indice de masse corporelle et un tour de taille significativement inférieurs à ceux d’adultes moins actifs. Le niveau d’AP peut être estimé au moyen d’un moniteur d’AP comme le SenseWear Armband™. Afin de compléter les études de validation de cet outil, nous avons évalué et démontré la reproductibilité des mesures. Toutefois, la dépense énergétique est sous-estimée durant les 10 premières minutes d’une AP d’intensité modérée sur ergocycle. L’utilisation de cet appareil est donc justifiée pour une évaluation de la dépense énergétique sur de longues périodes. Le calcul des glucides est une méthode largement utilisée pour évaluer la quantité d’insuline à injecter lors des repas. Nous avons évalué dans un contexte de vie courante, sans révision de la technique, la précision des patients pour ce calcul. L’erreur moyenne est de 15,4 ± 7,8 g par repas, soit 20,9 ± 9,7 % du contenu glucidique. L’erreur moyenne est positivement associée à de plus grandes fluctuations glycémiques mesurées via un lecteur de glucose en continu. Une révision régulière du calcul des glucides est probablement nécessaire pour permettre un meilleur contrôle glycémique. Nous avons développé et testé lors d’un essai clinique randomisé contrôlé un programme de promotion de l’AP (PEP-1). Ce programme de 12 semaines inclut une séance hebdomadaire en groupe ayant pour but d’initier l’AP, d’établir des objectifs et d’outiller les adultes atteints de DT1 quant à la gestion de la glycémie à l’AP. Bien que n’ayant pas permis d’augmenter la dépense énergétique, le programme a permis un maintien du niveau d’AP et une amélioration de la condition cardio-respiratoire et de la pression artérielle. À la fin du programme, une plus grande proportion de patients connaissait la pharmacocinétique de l’insuline et une plus grande variété de méthodes pour contrer l’hypoglycémie associée à l’AP était utilisée. En conclusion, le diabète de type 1 engendre des défis quotidiens particuliers. D’une part, le calcul des glucides est une tâche complexe et son imprécision est associée aux fluctuations glycémiques quotidiennes. D’autre part, l’adoption d’un mode de vie actif, qui est associée à un meilleur profil de composition corporelle, est limitée par la crainte des hypoglycémies. Le programme PEP-1 offre un support pour intégrer l’AP dans les habitudes de vie des adultes avec un DT1 et ainsi améliorer certains facteurs de risque cardio-vasculaire.

Nous savons que la grande majorité des étoiles WC9 produit de la poussière à base de carbone. Cette dernière doit se former dans des zones de très haute densité afin de survivre à l’environnement hostile qu’est celui du vent d’une étoile WR. Les étoiles WC appartenant à un système binaire WR + O produisent de la poussière quand les vents des deux étoiles entrent en collision et forment une zone de choc pouvant augmenter la densité du gaz d’un facteur 1000. Par contre, plusieurs étoiles WC9 n’ont, à ce jour, montré aucun signe de la présence d’un compagnon. Le but du projet est de tenter d’identifier un mécanisme alternatif responsable de la formation de poussière dans les étoiles WC9 n’appartenant pas à un système binaire. Nous présentons les résultats d’une campagne d’observation visant à caractériser la variabilité spectroscopique d’un échantillon de huit étoiles WC9 et une étoile WC8d. Nos résultats indiquent que la majorité des étoiles montrent des variations à grande échelle dans la raie d’émission C III 5696, soit à un niveau d’au moins 5% du flux de la raie et que les structures dans le vent ont une dispersion de vitesses de l’ordre de 150-300 km/s. De manière générale, les variations de vitesse radiales sont anti-corrélées avec le coefficient d’asymétrie de la raie, ce qui semble infirmer la présence d’un compagnon. Des observations en photométrie de l’étoile WR103 montrent une période de 9.1 ± 0.6 jours qui s’accorde avec les variations spectroscopiques et qui ne semble pas, de manière évidente, d’origine binaire.

Cette thèse est divisée en trois chapitres. Le premier explique comment utiliser la méthode «level-set» de manière rigoureuse pour faire la simulation de feux de forêt en utilisant comme modèle physique pour la propagation le modèle de l'ellipse de Richards. Le second présente un nouveau schéma semi-implicite avec une preuve de convergence pour la solution d'une équation de type Hamilton-Jacobi anisotrope. L'avantage principal de cette méthode est qu'elle permet de réutiliser des solutions à des problèmes «proches» pour accélérer le calcul. Une autre application de ce schéma est l'homogénéisation. Le troisième chapitre montre comment utiliser les méthodes numériques des deux premiers chapitres pour étudier l'influence de variations à petites échelles dans la vitesse du vent sur la propagation d'un feu de forêt à l'aide de la théorie de l'homogénéisation.

Objective: to assess, in a randomized sample of measurements made with the NBAS scale, in preterm and / or low birth weight babies, some psychometric characteristics of the same as her construct validity and internal consistency. Materials and methods: we designed a study of assessment of Psychometric properties. From a study of cross-sectional measurements, there were made assessment of the statistic behavior of the items, individually and in their internal consistency, then, there were carried out, type exploratory and confirmatory factor analysis. Results: we find that, in the analysis of preterm or low birth weight babies, the scale remains largely, the overall structure of factors proposed since 1982, in relation to the dimensions of habituation, social interactive and complementary. The items that make up the dimensions of SNA and motor systems are integrated. Also, we found Cronbach alpha values for internal consistency, which reflects higher to medium correlation levels in the behavioral and reflex items. Conclusion: the NBAS scale reflects characteristics of construct validity and internal consistency, which credits it as a useful tool in clinical assessment of neurodevelopmental effects of preterm or low birth weight babies. It is necessary to analyze the structure of the scale when used for purposes of scientific research.

A preoccupation with competition often dominates the study of governance. A focus on competition often unnecessarily precludes the possibility that regional institutions can suspend competition in certain areas and facilitate cooperation among potential rivals, thereby potentially contributing to their mutual success. In many ways companies cooperating through these types of networks have a greater degree of flexibility than firms which are forced to rely solely on hierarchies or markets for solutions to their problems. In order to fully understand how such networks work, this article first parses out differences in definitions of networks in order to understand how the type of network mentioned above actually differs from other uses of this term. Then it develops a theory of governance that goes beyond hierarchies and markets by demonstrating how this type of network can lead to reductions in transaction costs. This claim is illustrated on hand from examples of alternative forms of organization in Germany and Italy.

The Met Office Unified Model is run for a case observed during Intensive Observation Period 18 (IOP18) of the Convective Storms Initiation Project (CSIP). The aims are to identify the physical processes that lead to perturbation growth at the convective scale in response to model-state perturbations and to determine their sensitivity to the character of the perturbations. The case is strongly upper-level forced but with detailed mesoscale/convective-scale evolution that is dependent on smaller-scale processes. Potential temperature is perturbed within the boundary layer. The effects on perturbation growth of both the amplitude and typical scalelength of the perturbations are investigated and perturbations are applied either sequentially (every 30 min throughout the simulation) or at specific times. The direct effects (within one timestep) of the perturbations are to generate propagating Lamb and acoustic waves and produce generally small changes in cloud parameters and convective instability. In exceptional cases a perturbation at a specific gridpoint leads to switching of the diagnosed boundary-layer type or discontinuous changes in convective instability, through the generation or removal of a lid. The indirect effects (during the entire simulation) are changes in the intensity and location of precipitation and in the cloud size distribution. Qualitatively different behaviour is found for strong (1K amplitude) and weak (0.01K amplitude) perturbations, with faster growth after sunrise found only for the weaker perturbations. However, the overall perturbation growth (as measured by the root-mean-square error of accumulated precipitation) reaches similar values at saturation, regardless of the perturbation characterisation.

ATSR-2 active fire data from 1996 to 2000, TRMM VIRS fire counts from 1998 to 2000 and burn scars derived from SPOT VEGETATION ( the Global Burnt Area 2000 product) were mapped for Peru and Bolivia to analyse the spatial distribution of burning and its intra- and inter-annual variability. The fire season in the region mainly occurs between May and October; though some variation was found between the six broad habitat types analysed: desert, grassland, savanna, dry forest, moist forest and yungas (the forested valleys on the eastern slope of the Andes). Increased levels of burning were generally recorded in ATSR-2 and TRMM VIRS fire data in response to the 1997/1998 El Nino, but in some areas the El Nino effect was masked by the more marked influences of socio-economic change on land use and land cover. There were differences between the three global datasets: ATSR-2 under-recorded fires in ecosystems with low net primary productivities. This was because fires are set during the day in this region and, when fuel loads are low, burn out before the ATSR-2 overpass in the region which is between 02.45 h and 03.30 h. TRMM VIRS was able to detect these fires because its overpasses cover the entire diurnal range on a monthly basis. The GBA2000 product has significant errors of commission (particularly areas of shadow in the well-dissected eastern Andes) and omission (in the agricultural zone around Santa Cruz, Bolivia and in north-west Peru). Particular attention was paid to biomass burning in high-altitude grasslands, where fire is an important pastoral management technique. Fires and burn scars from Landsat Thematic Mapper (TM) and Enhanced Thematic Mapper (ETM) data for a range of years between 1987 and 2000 were mapped for areas around Parque Nacional Rio Abiseo (Peru) and Parque Nacional Carrasco (Bolivia). Burn scars mapped in the grasslands of these two areas indicate far more burning had taken place than either the fires or the burn scars derived from global datasets. Mean scar sizes are smaller and have a smaller range in size between years the in the study area in Peru (6.6-7.1 ha) than Bolivia (16.9-162.5 ha). Trends in biomass burning in the two highland areas can be explained in terms of the changing socio-economic environments and impacts of conservation. The mismatch between the spatial scale of biomass burning in the high-altitude grasslands and the sensors used to derive global fire products means that an entire component of the fire regime in the region studied is omitted, despite its importance in the farming systems on the Andes.

Twenty-five small soil-filled perspex boxes arranged in a square, with dwarf sunflowers growing in them, were used to study micro-scale advection. Hydrological heterogeneity was introduced by applying two different amounts of irrigation water (low-irrigation, L, versus high-irrigation, H). The nine central boxes (4 H, 4 L and I bare box) were precision weighing lysimeters, yielding diurnal measurements of evaporation. After the onset of soil water stress, a large difference in latent heat flux (up to 4-fold) was observed between the lysimeters of the H and L treatments, mainly caused by large differences between H and L stomatal conductance values. This resulted in micro-advection, causing H soil-sunflower systems to evaporate well above equilibrium latent heat flux. The occurrence of micro-advective enhancement was reflected in large values of the Priestley-Taylor constant (often larger than 2.0) and generally negative values of sensible heat flux for the H treatment. There are now considerable expectations that semi-distributed models are useful tools for supporting catchment water quality management. However, insufficient attention has been given to evaluating the uncertainties inherent to this type of model, especially those associated with the spatial disaggregation of the catchment. The Integrated Nitrogen in Catchments model (INCA) is subjected to an extensive regionalised sensitivity analysis in application to the River Kennet, part of the groundwater-dominated upper Thames catchment, UK The main results are: (1) model output was generally insensitive to land-phase parameters, very sensitive to groundwater parameters, including initial conditions, and significantly sensitive to in-river parameters; (2) INCA was able to produce good fits simultaneously to the available flow, nitrate and ammonium in-river data sets; (3) representing parameters as heterogeneous over the catchment (206 calibrated parameters) rather than homogeneous (24 calibrated parameters) produced a significant improvement in fit to nitrate but no significant improvement to flow and caused a deterioration in ammonium performance; (4) the analysis indicated that calibrating the flow-related parameters first, then calibrating the remaining parameters (as opposed to calibrating all parameters together) was not a sensible strategy in this case; (5) even the parameters to which the model output was most sensitive suffered from high uncertainty due to spatial inconsistencies in the estimated optimum values, parameter equifinality and the sampling error associated with the calibration method; (6) soil and groundwater nutrient and flow data are needed to reduce. uncertainty in initial conditions, residence times and nitrogen transformation parameters, and long-term historic data are needed so that key responses to changes in land-use management can be assimilated. The results indicate the general, difficulty of reconciling the questions which catchment nutrient models are expected to answer with typically limited data sets and limited knowledge about suitable model structures. The results demonstrate the importance of analysing semi-distributed model uncertainties prior to model application, and illustrate the value and limitations of using Monte Carlo-based methods for doing so.
