Background Expenditure on dental and oral health services in Australia is $3.4 billion AUD annually. This is the sixth highest health cost and accounts for 7 % of total national health expenditure. Approximately 49 % of Australian children aged 6 years have caries experience in their deciduous teeth and this is rising. The aetiology of dental caries involves a complex interplay of individual, behavioural, social, economic, political and environmental conditions, and there is increasing interest in genetic predisposition and epigenetic modification. Methods The Oral Health Sub-study; a cross sectional study of a birth cohort began in November 2012 by examining mothers and their children who were six years old by the time of initiation of the study, which is ongoing. Data from detailed questionnaires of families from birth onwards and data on mothers’ knowledge, attitudes and practices towards oral health collected at the time of clinical examination are used. Subjects’ height, weight and mid-waist circumference are taken and Body Mass Index (BMI) computed, using an electronic Bio-Impedance balance. Dental caries experience is scored using the International Caries Detection and Assessment System (ICDAS). Saliva is collected for physiological measures. Salivary Deoxyribose Nucleic Acid (DNA) is extracted for genetic studies including epigenetics using the SeqCap Epi Enrichment Kit. Targets of interest are being confirmed by pyrosequencing to identify potential epigenetic markers of caries risk. Discussion This study will examine a wide range of potential determinants for childhood dental caries and evaluate inter-relationships amongst them. The findings will provide an evidence base to plan and implement improved preventive strategies.

Diagnostic radiology represents the largest man-made contribution to population radiation doses in Europe. To be able to keep the diagnostic benefit versus radiation risk ratio as high as possible, it is important to understand the quantitative relationship between the patient radiation dose and the various factors which affect the dose, such as the scan parameters, scan mode, and patient size. Paediatric patients have a higher probability for late radiation effects, since longer life expectancy is combined with the higher radiation sensitivity of the developing organs. The experience with particular paediatric examinations may be very limited and paediatric acquisition protocols may not be optimised. The purpose of this thesis was to enhance and compare different dosimetric protocols, to promote the establishment of the paediatric diagnostic reference levels (DRLs), and to provide new data on patient doses for optimisation purposes in computed tomography (with new applications for dental imaging) and in paediatric radiography. Large variations in radiation exposure in paediatric skull, sinus, chest, pelvic and abdominal radiography examinations were discovered in patient dose surveys. There were variations between different hospitals and examination rooms, between different sized patients, and between imaging techniques; emphasising the need for harmonisation of the examination protocols. For computed tomography, a correction coefficient, which takes individual patient size into account in patient dosimetry, was created. The presented patient size correction method can be used for both adult and paediatric purposes. Dental cone beam CT scanners provided adequate image quality for dentomaxillofacial examinations while delivering considerably smaller effective doses to patient compared to the multi slice CT. However, large dose differences between cone beam CT scanners were not explained by differences in image quality, which indicated the lack of optimisation. For paediatric radiography, a graphical method was created for setting the diagnostic reference levels in chest examinations, and the DRLs were given as a function of patient projection thickness. Paediatric DRLs were also given for sinus radiography. The detailed information about the patient data, exposure parameters and procedures provided tools for reducing the patient doses in paediatric radiography. The mean tissue doses presented for paediatric radiography enabled future risk assessments to be done. The calculated effective doses can be used for comparing different diagnostic procedures, as well as for comparing the use of similar technologies and procedures in different hospitals and countries.

This thesis comprises four intercomplementary parts that introduce new approaches to brittle reaction layers and mechanical compatibility of metalloceramic joints created when fusing dental ceramics to titanium. Several different methods including atomic layer deposition (ALD), sessile drop contact angle measurements, scanning acoustic microscopy (SAM), three-point bending (TPB, DIN 13 927 / ISO 9693), cross-section microscopy, scanning electron microscopy (SEM), and energy dispersive X-ray spectroscopy (EDS) were employed. The first part investigates the effects of TiO2 layer structure and thickness on the joint strength of the titanium-metalloceramic system. Samples with all tested TiO2 thicknesses displayed good ceramics adhesion to Ti, and uniform TPB results. The fracture mode was independent of oxide layer thickness and structure. Cracking occurred deeper inside titanium, in the oxygen-rich Ti[O]x solid solution surface layer. During dental ceramics firing TiO2 layers dissociate and joints become brittle with increased dissolution of oxygen into metallic Ti and consequent reduction in the metal plasticity. To accomplish an ideal metalloceramic joint this needs to be resolved. The second part introduces photoinduced superhydrophilicity of TiO2. Test samples with ALD deposited anatase TiO2 films were produced. Samples were irradiated with UV light to induce superhydrophilicity of the surfaces through a cascade leading to increased amount of surface hydroxyl groups. Superhydrophilicity (contact angle ~0˚) was achieved within 2 minutes of UV radiation. Partial recovery of the contact angle was observed during the first 10 minutes after UV exposure. Total recovery was not observed within 24h storage. Photoinduced ultrahydrophilicity can be used to enhance wettability of titanium surfaces, an important factor in dental ceramics veneering processes. The third part addresses interlayers designed to restrain oxygen dissolution into Ti during dental ceramics fusing. The main requirements for an ideal interlayer material are proposed. Based on these criteria and systematic exclusion of possible interlayer materials silver (Ag) interlayers were chosen. TPB results were significantly better in when 5 μm Ag interlayers were used compared to only Al2O3-blasted samples. In samples with these Ag interlayers multiple cracks occurred inside dental ceramics, none inside Ti structure. Ag interlayers of 5 μm on Al2O3-blasted samples can be efficiently used to retard formation of the brittle oxygen-rich Ti[O]x layer, thus enhancing metalloceramic joint integrity. The most brittle component in metalloceramic joints with 5 μm Ag interlayers was bulk dental ceramics instead of Ti[O]x. The fourth part investigates the importance of mechanical interlocking. According to the results, the significance of mechanical interlocking achieved by conventional surface treatments can be questioned as long as the formation of the brittle layers (mainly oxygen-rich Ti[O]x) cannot be sufficiently controlled. In summary in contrast to former impressions of thick titanium oxide layers this thesis clearly demonstrates diffusion of oxygen from sintering atmosphere and SiO2 to Ti structures during dental ceramics firing and the following formation of brittle Ti[O]x solid solution as the most important factors predisposing joints between Ti and SiO2-based dental ceramics to low strength. This among other predisposing factors such as residual stresses created by the coefficient of thermal expansion mismatch between dental ceramics and Ti frameworks can be avoided with Ag interlayers.

The aim of the present study was to assess oral health and treatment needs among Iranian adults with diabetes according to socio-demographic status, oral hygiene, diabetes related factors, and to investigate the relation between these determinants and oral health. Moreover, the effect of an educational oral health promotion intervention on their oral health and periodontal treatment needs was studied. The target population comprised adults with diabetes in Tehran, Iran. 299 dentate patients with diabetes, who were regular attendants to a diabetic clinic, were selected as the study subjects. Data collection was performed through a clinical dental examination and self-administered structured questionnaire. The questionnaire covered information of the subject s social background, medical history, oral health behaviour and smoking. The clinical dental examinations covered the registration of caries experience (DMFT), community periodontal index (CPI) and plaque index (PI). The intervention provided the adults with diabetes dental health education through a booklet. Reduction in periodontal treatment needs one year after the baseline examination was used as the main outcome. A high prevalence of periodontal pockets among the study population was found; 52% of the participants had periodontal pockets with a pocket depth of 4 to 5 mm and 35% had periodontal pockets with pocket depth of 6 mm or more. The mean of the DMFT index was 12.9 (SD=6.1), being dominated by filled teeth (mean 6.5) and missing teeth (mean 5.0). Oral self-care among adults with diabetes was inadequate and poor oral hygiene was observed in more than 80% of the subjects. The educational oral health promotion decreased periodontal treatment needs more in the study groups than in the control group. The poor periodontal health, poor oral hygiene and insufficient oral self-care observed in this study call for oral health promotion among adult with diabetes. An educational intervention showed that it is possible to promote oral health behaviour and to reduce periodontal treatment needs among adults with diabetes. The simplicity of the model used in this study allows it to be integrated to diabetes programmes in particular in countries with a developing health care system.

Tsunami waves of the Sumatra-Andaman earthquake on 26 December 2004 claimed approximately 230 000 lives and started the biggest identification operation in Interpol's history. The aim of this study was to resolve methods of the identification and results received. The viewpoint is mainly that of forensic odontology, but also includes other means of identification and results of the medico-legal examination performed in Finland. Of the 5395 victims in Thailand, approximately 2 400 were foreigners from 36 nations including 177 Finnish nationals. Additionally, a Finnish woman perished in Sri Lanka and a severely injured man after the evacuation in a hospital. The final numbers of missing persons and dead bodies registered in the Information Management Centre in Phuket,Thailand, were 3 574 ante-mortem (AM) and 3 681 post-mortem (PM) files. The number of identifications by December 2006 was 3 271 or 89% of the victims registered. Of Finnish victims, 172 have been identified in Thailand and 163 repatriated to Finland. One adult and four children are still missing. For AM data, a list of Finnish missing persons including 178 names was published on 30 December 2004. By February 2005 all useful dental AM data were available. Five persons on the list living in Finland lacked records. Based on the AM database, for the children under age 18 years (n=60) dental identification could be established for 12 (20%). The estimated number for adults (n=112) was 96 (86%). The final identification rate, based on PM examinations in Finland, was 14 (25%) for children (n= 56) and 98 (90%) for adults (n= 109). The number of Finnish victims identified by dental methods, 112 (68%), was high compared to all examined in Thailand (43%). DNA was applied for 26 Finnish children and for 6 adults, fingerprints for 24 and 7, respectively. In 12 cases two methods were applied. Every victim (n=165) underwent in Finland a medico-legal investigation including an autopsy with sampling specimens for DNA, the toxicological and histological investigation. Digital radiographs and computed tomography were taken of the whole body to verify autopsy findings and bring out changes caused by trauma, autolysis, and sampling for DNA in Thailand. Data for identification purposes were also noted. Submersion was the cause of death for 101 of 109 adults (92.7%), and trauma for 8 (7.3%). Injuries were 33 times contributing factors for submersion and 3 times for trauma-based death. Submersion was the cause of death for 51 (92.7%) children and trauma for 4 (7.3%). Injuries were in 3 cases contributing factors in submersion and once in trauma-based death. The success of the dental identification of Finnish victims is mainly based on careful registration of dental records, and on an education program from 1999 in forensic odontology.

A raspagem subgengival e o alisamento radicular constituem o "padrão ouro" e o tratamento de eleição para a periodontite; porém, é um procedimento difícil de ser executado, que requer um intenso treinamento e que pode expor a dentina, causando hipersensibilidade dentinária pela remoção excessiva de cemento, ou produzir defeitos, como sulcos e ranhuras, além de deixar cálculo residual e não conseguir atingir toda as superfície radicular. Recentemente, um gel a base de papaína e cloramina foi introduzido no mercado (Papacárie), utilizado no tratamento da remoção de dentina cariada. Este gel poderia auxiliar na remoção do cálculo subgengival com menor desgaste do cemento. O objetivo deste trabalho foi comparar a eficácia e analisar a superfície radicular na utilização de um gel à base de papaína e cloramina, associado ao alisamento radicular, na região subgengival. Após receberem instruções de higiene oral, raspagem supragengival e polimento coronário, 18 pacientes com periodontite crônica, 6 mulheres e 12 homens, com idade média de 51 anos (8) foram tratados num modelo de boca dividida. O tratamento-teste foi constituído pela aplicação do gel na área subgengival por 1 min., seguida pelo alisamento radicular; o tratamento-controle foi constituído pela raspagem subgengival e alisamento radiculares. A terapia foi executada por 3 operadoras e os exames inicial, de 28 dias e 3 meses, foram realizados por um único examinador. Quatro dentes nunca tratados de dois outros pacientes (2 incisivos centrais inferiores e 2 premolares), com indicação para extração, foram submetidos ao tratamento teste e controle e, após a exodontia, analisados em microscopia eletrônica de varredura (MEV). Ao longo dos 3 meses, os resultados demonstraram significativa melhora nos parâmetros clínicos: sangramento à sondagem, profundidade de bolsa e ganho de inserção, tanto no lado-teste, como no lado-controle, principalmente aos 28 dias; mas não foi observada significância estatística quando ambas as formas de terapia foram comparadas. O índice de placa médio permaneceu alto ao longo do estudo. A análise do MEV demonstrou que o tratamento-teste deixou uma maior quantidade de cálculo residual sobre a superfície radicular; porém, áreas livres de cálculo também foram observadas. No tratamento-controle, verificaram-se regiões mais profundas não atingidas pelas curetas, áreas livres de cálculo e um sulco produzido pela cureta. Concluiu-se que tanto o tratamento-teste, como o controle, foram eficazes no tratamento da periodontite crônica nos 3 meses observados.

A odontologia moderna utiliza métodos e técnicas ultraconservadores no intuito de corrigir os diversos tipos de alterações cromáticas observadas clinicamente. Os meios empregados baseiam-se na utilização de substâncias químicas à base de peróxidos presentes em diversas concentrações. O presente estudo objetivou avaliar a microestrutura de três resinas compostas fotossensíveis submetidas à aplicação de um agente clareador a base de peróxido de hidrogênio a 35% (Whiteness HP Maxx - fabricante: FGM), ativado por uma fonte híbrida de energia luminosa (Aparelho de Laser-Led Whitening Lase, fabricante: DMC). Para isso, foram confeccionados 30 corpos de prova (CDP) 10 para cada grupo, no formato de discos, com 13 mm de diâmetro e 2,0 mm de espessura em uma matriz de teflon e aço inox, fotoativados por um aparelho de luz halógena convencional (Optilux 401 - Demetron/UR) por 40 segundos com densidade de potência média igual a 450 mW/cm2. Os grupos foram dispostos da seguinte forma: Grupo 1 - resina microparticulada (Durafill VS - fabricante: Heraeus Kulzer); Grupo 2 - resina micro-híbrida (Esthet-X - fabricante: Dentsply); e Grupo 3 resina nanoparticulada (Filtek Supreme XT fabricante: 3M ESPE). Todos os materiais restauradores utilizados eram da cor A2. Após serem submetidos à sequência de acabamento e polimento os CDP foram armazenados por sete dias em saliva artificial, limpos em ultra-som, envelhecidos artificialmente de acordo com a norma ASTM G 154. Os CDP dos três grupos foram aleatoriamente divididos em 2 subgrupos (ST sem tratamento e CT com tratamento) e finalmente submetidos aos experimentos. Os CDP dos subgrupos 1-ST, 2- ST e 3-ST foram triturados (SPEX SamplePrep 8000-series, marca: Mixer/Mills) seguido pela verificação dos materiais por meio de um espectrômetro (marca/modelo: Shimadzu EDX 720) para certificação da ausência de elementos pertencentes ao meio de moagem e por fim foram levados a um difrator de raios-X (marca / modelo: Philips -PW 3040 -X'Celerator- 40kV; 30mA; (λ): CuKα; 0,6; 0,2mm; 0,05 (2θ); 2s; 10-90 (2θ). Em seguida os CDP dos subgrupos 1-CT, 2- CT e 3-CT foram tratados com o peróxido de hidrogênio de acordo com o protocolo do fabricante para a fonte híbrida luminosa de energia selecionada, totalizando 9 aplicações de 10 minutos, onde eram respeitados os tempos de 3 minutos de ativação por 20 segundos de descanso, finalizando 10 minutos em cada aplicação. Mediante a este tratamento, os CDP dos subgrupos CT eram verificados e avaliados pelo mesmo método descrito anteriormente. Após interpretação gráfica, análise comparativa por meio do processamento digital das imagens no programa KS400 3.0 (Carl Zeiss Vision) e análise de concordância por cinco avaliadores calibrados utilizando um escore, pôde-se concluir que houve degradação estrutural e que as estruturas cristalinas das resinas estudadas foram afetadas de forma distinta quando tratadas pelo peróxido de hidrogênio; onde observou-se que: Grupo 1 > Grupo 3 > Grupo 2. Foi sugerido a realização de novos estudos, relacionados à interação do peróxido de hidrogênio às resinas compostas.

O objetivo deste estudo foi comparar in vitro o desgaste dental de pré-molares (PM) e molares (M) e sua relação com o valor de dureza Vickers dos materiais utilizados como antagonistas em uma máquina de abrasão simulada para provocar o desgaste nos dentes testados. Os materiais antagonistas utilizados foram VeraBond II (liga de Ni-Cr), Solidex (resina composta) e IPS Empress 2 (cerâmica). Para cada ensaio de dureza, foram preparados seis corpos-de-prova de cada material, os quais foram polidos sob refrigeração, com ciclo de 20 min para cada granulação. Num microdurômetro (HMV-2), foram realizadas três mossas por quadrante, cada uma sob carga de 19,614 N por 30 s, totalizando 12 mossas de base quadrada com ângulo de 136 entre os planos. O teste de abrasão foi realizado numa máquina simuladora de abrasão, freqüência de 265 ciclos/min e 4,4 Hz, com um percurso do antagonista de 10 mm à velocidade de 88 mm/s. Cada dente foi testado em oposição a um antagonista (foram 6 pares dente/material para cada grupo), em água deionizada, sob carga de 5 N, por 150 min, num total de 39.750 ciclos. Foram utilizados dezenove dentes 1 pré-molares, dezenove 3 molares e confeccionados doze antagonistas em cada material em forma de pastilha. Cada grupo de seis dentes foi testado em oposição a seis antagonistas do mesmo material. Ademais, um dente 1 pré-molar (PM) e um dente 3 molar (M) foram testados em oposição ao Plexiglass. Com relação ao desgaste do esmalte dentário (PM+M) segundo o material antagonista, o teste de Kruskal-Wallis evidenciou diferença significativa com p-valor < 0,001 e o teste de Mann-Whitney evidenciou diferença significativa nas comparações PM+M/resina X PM+M/metal (p-valor < 0,001), PM+M/resina X PM+M/cerâmica (p-valor < 0,001) e PM+M/metal X PM+M/cerâmica (p-valor = 0,002). A análise isolada, considerando pré-molares e molares separadamente, encontrou diferença significativa em relação ao desgaste do esmalte dentário no teste de Kruskall-Wallis, porém não detectou diferença significativa no teste de comparação múltipla Mann-Whitney quando comparou o desgaste sofrido pelo PM/metal em relação ao PM/cerâmica. Em relação à dureza Vickers detectou-se diferença significativa da dureza dos materiais no teste de Kruskall-Wallis (p-valor < 0,001) e também no teste de Mann-Whitney nas comparações múltiplas com p-valor = 0,002. Comparando-se a dureza com a perfilometria, observou-se uma correlação estatisticamente significativa (p ≤ 0,05) na correlação negativa (ρ= -0,829) entre a dureza do metal como material antagonista e o desgaste do esmalte dentário do dente molar. Os resultados sugeriram que todo material restaurador indireto estudado causou desgaste ao esmalte dentário quando submetido a forças de simulação de abrasão com carga. Embora tenha sido observada correlação entre dureza e resistência à abrasão, essa correlação foi pouco significativa.

Based on the Coulomb friction model, the frictional motion model of workpiece relating to the polishing pad was presented in annular polishing. By the dynamic analysis software, the model was simulated and analysed. The conclusions from the results were that the workpiece did not rotate steadily. When the angular velocity of ring and the direction were the same as that of the polishing pad, the angular velocity of workpiece hoicked at the beginning and at the later stage were the same as that of the polishing pad before contacting with the ring. The angular velocity of workpiece vibrated at the moment of contacting with the ring. After that the angular velocity of workpiece increased gradually and fluctuated at a given value, while the angular velocity of ring decreased gradually and also fluctuated at a given value. Since the contact between the workpiece and the ring was linear, their linear velocities and directions should be the same. But the angular velocity of workpiece was larger than that of the polishing pad on the condition that the radius of the workpiece was less than that of the ring. This did not agree with the pure translation principle and the workpiece surface could not be flat, either. Consequently, it needed to be controlled with the angular velocity of ring and the radii of the ring and the workpiece, besides friction to make the angular velocity of workpiece equal to that of the polishing pad for obtaining fine surface flatness of the workpiece.
