OBJECTIVE To assess the 5-year survival of metal-ceramic and all-ceramic tooth-supported fixed dental prostheses (FDPs) and to describe the incidence of biological, technical and esthetic complications. METHODS Medline (PubMed), Embase and Cochrane Central Register of Controlled Trials (CENTRAL) searches (2006-2013) were performed for clinical studies focusing on tooth-supported FDPs with a mean follow-up of at least 3 years. This was complemented by an additional hand search and the inclusion of 10 studies from a previous systematic review [1]. Survival and complication rates were analyzed using robust Poisson's regression models to obtain summary estimates of 5-year proportions. RESULTS Forty studies reporting on 1796 metal-ceramic and 1110 all-ceramic FDPs fulfilled the inclusion criteria. Meta-analysis of the included studies indicated an estimated 5-year survival rate of metal-ceramic FDPs of 94.4% (95% CI: 91.2-96.5%). The estimated survival rate of reinforced glass ceramic FDPs was 89.1% (95% CI: 80.4-94.0%), the survival rate of glass-infiltrated alumina FDPs was 86.2% (95% CI: 69.3-94.2%) and the survival rate of densely sintered zirconia FDPs was 90.4% (95% CI: 84.8-94.0%) in 5 years of function. Even though the survival rate of all-ceramic FDPs was lower than for metal-ceramic FDPs, the differences did not reach statistical significance except for the glass-infiltrated alumina FDPs (p=0.05). A significantly higher incidence of caries in abutment teeth was observed for densely sintered zirconia FDPs compared to metal-ceramic FDPs. Significantly more framework fractures were reported for reinforced glass ceramic FDPs (8.0%) and glass-infiltrated alumina FDPs (12.9%) compared to metal-ceramic FDPs (0.6%) and densely sintered zirconia FDPs (1.9%) in 5 years in function. However, the incidence of ceramic fractures and loss of retention was significantly (p=0.018 and 0.028 respectively) higher for densely sintered zirconia FDPs compared to all other types of FDPs. CONCLUSIONS Survival rates of all types of all-ceramic FDPs were lower than those reported for metal-ceramic FDPs. The incidence of framework fractures was significantly higher for reinforced glass ceramic FDPs and infiltrated glass ceramic FDPs, and the incidence for ceramic fractures and loss of retention was significantly higher for densely sintered zirconia FDPs compared to metal-ceramic FDPs.

The objective of the present review was to summarize the evidence available on the temporal sequence of hard and soft tissue healing around titanium dental implants in animal models and in humans. A search was undertaken to find animal and human studies reporting on the temporal dynamics of hard and soft tissue integration of titanium dental implants. Moreover, the influence of implant surface roughness and chemistry on the molecular mechanisms associated with osseointegration was also investigated. The findings indicated that the integration of titanium dental implants into hard and soft tissue represents the result of a complex cascade of biological events initiated by the surgical intervention. Implant placement into alveolar bone induces a cascade of healing events starting with clot formation and continuing with the maturation of bone in contact with the implant surface. From a genetic point of view, osseointegration is associated with a decrease in inflammation and an increase in osteogenesis-, angiogenesis- and neurogenesis-associated gene expression during the early stages of wound healing. The attachment and maturation of the soft tissue complex (i.e. epithelium and connective tissue) to implants becomes established 6-8 weeks following surgery. Based on the findings of the present review it can be concluded that improved understanding of the mechanisms associated with osseointegration will provide leads and targets for strategies aimed at enhancing the clinical performance of titanium dental implants.

When considering data from many trials, it is likely that some of them present a markedly different intervention effect or exert an undue influence on the summary results. We develop a forward search algorithm for identifying outlying and influential studies in meta-analysis models. The forward search algorithm starts by fitting the hypothesized model to a small subset of likely outlier-free studies and proceeds by adding studies into the set one-by-one that are determined to be closest to the fitted model of the existing set. As each study is added to the set, plots of estimated parameters and measures of fit are monitored to identify outliers by sharp changes in the forward plots. We apply the proposed outlier detection method to two real data sets; a meta-analysis of 26 studies that examines the effect of writing-to-learn interventions on academic achievement adjusting for three possible effect modifiers, and a meta-analysis of 70 studies that compares a fluoride toothpaste treatment to placebo for preventing dental caries in children. A simple simulated example is used to illustrate the steps of the proposed methodology, and a small-scale simulation study is conducted to evaluate the performance of the proposed method. PURPOSE Digital developments have led to the opportunity to compose simulated patient models based on three-dimensional (3D) skeletal, facial, and dental imaging. The aim of this systematic review is to provide an update on the current knowledge, to report on the technical progress in the field of 3D virtual patient science, and to identify further research needs to accomplish clinical translation. MATERIALS AND METHODS Searches were performed electronically (MEDLINE and OVID) and manually up to March 2014 for studies of 3D fusion imaging to create a virtual dental patient. Inclusion criteria were limited to human studies reporting on the technical protocol for superimposition of at least two different 3D data sets and medical field of interest. RESULTS Of the 403 titles originally retrieved, 51 abstracts and, subsequently, 21 full texts were selected for review. Of the 21 full texts, 18 studies were included in the systematic review. Most of the investigations were designed as feasibility studies. Three different types of 3D data were identified for simulation: facial skeleton, extraoral soft tissue, and dentition. A total of 112 patients were investigated in the development of 3D virtual models. CONCLUSION Superimposition of data on the facial skeleton, soft tissue, and/or dentition is a feasible technique to create a virtual patient under static conditions. Three-dimensional image fusion is of interest and importance in all fields of dental medicine. Future research should focus on the real-time replication of a human head, including dynamic movements, capturing data in a single step.

Objectives. The purpose of this thesis is to understand the underlying socioeconomic characteristics affecting dental insurance coverage, yearly dental visits, and factors related to visiting a dentist in Mexico among border region residents. Methods. Using data from the Border Epidemiological Study of Aging, dental utilization in the previous 12 months, dental visits to Mexico, and dental insurance (proxy) were calculated utilizing logistic regression. Three different models were utilized for the dependent variables adjusting for diverse socioeconomic characteristics such as gender, age, marital status, income, education, years of residence in the United States (for immigrants), English proficiency, general health status, employment and dental insurance. Results. After adjustment, diverse variables were significant for the three different models calculated. Conclusion. Although the Mexican health market constitutes a viable option for dental services for border residents, dental insurance and dental yearly visits were lower in this region when compared to national averages. ^

Objectives. The purpose of this study was to elucidate behavioral determinants (prevailing attitudes and beliefs) of hand hygiene practices among undergraduate dental students in a dental school. ^ Methods. Statistical modeling using the Integrative Behavioral Model (IBM) prediction was utilized to develop a questionnaire for evaluating behavioral perceptions of hand hygiene practices by dental school students. Self-report questionnaires were given to second, third and fourth year undergraduate dental students. Models representing two distinct hand hygiene practices, termed "elective in-dental school hand hygiene practice" and "inherent in-dental school hand hygiene practice" were tested using linear regression analysis. ^ Results. 58 responses were received (24.5%); the sample mean age was 26.6 years old and females comprised 51%. In our models, elective in-dental school hand hygiene practice and inherent in-dental school hand hygiene practice, explained 40% and 28%, respectively, of the variance in behavioral intention. Translation of community hand hygiene practice to the dental school setting is the predominant driver of elective hand hygiene practice. Intended elective in-school hand hygiene practice is further significantly predicted by students' self-efficacy. Students' attitudes, peer pressure of other dental students and clinic administrators, and role modeling had minimal effects. Inherent hand hygiene intent was strongly predicted by students' beliefs in the benefits of the activity and, to a lesser extent, role modeling. Inherent and elective community behaviors were insignificant. ^ Conclusions. This study provided significant insights into dental student's hand hygiene behavior and can form the basis for an effective behavioral intervention program designed to improve hand hygiene compliance.^

Dental caries is the most common chronic disease worldwide. It is characterized by the demineralization of tooth enamel caused by acid produced by cariogenic dental bacteria growing on tooth surfaces, termed bacterial biofilms. Cariogenesis is a complex biological process that is influence by multiple factors and is not attributed to a sole causative agent. Instead, caries is associated with multispecies microbial biofilm communities composed of some bacterial species that directly influence the development of a caries lesion and other species that are seemingly benign but must contribute to the community in an uncharacterized way. Clinical analysis of dental caries and its microbial populations is challenging due to many factors including low sensitivity of clinical measurement tools, variability in saliva chemistry, and variation in the microbiota. Our laboratory has developed an in vitro anaerobic biofilm model for dental carries to facilitate both clinical and basic research-based analyses of the multispecies dynamics and individual factors that contribute to cariogenicity. The rational for development of this system was to improve upon the current models that lack key elements. This model places an emphasis on physiological relevance and ease of maintenance and reproducibility. The uniqueness of the model is based on integrating four critical elements: 1) a biofilm community composed of four distinct and representative species typically associated with dental caries, 2) a semi-defined synthetic growth medium designed to mimic saliva, 3) physiologically relevant biofilm growth substrates, and 4) a novel biofilm reactor device designed to facilitate the maintenance and analysis. Specifically, human tooth sections or hydroxyapatite discs embedded into poly(methyl methacrylate) (PMMA) discs are incubated for an initial 24 hr in a static inverted removable substrate (SIRS) biofilm reactor at 37°C under anaerobic conditions in artificial saliva (CAMM) without sucrose in the presence of 1 X 106 cells/ml of each Actinomyces odontolyticus, Fusobacterium nucleatum, Streptococcus mutans, and Veillonella dispar. During days 2 and 3 the samples are maintained continually in CAMM with various exposures to 0.2% sucrose; all of the discs are transferred into fresh medium every 24 hr. To validate that this model is an appropriate in vitro representation of a caries-associated multispecies biofilm, research aims were designed to test the following overarching hypothesis: an in vitro anaerobic biofilm composed of four species (S. mutans, V. dispar, A. odontolyticus, and F. nucleatum) will form a stable biofilm with a community profile that changes in response to environmental conditions and exhibits a cariogenic potential. For these experiments the biofilms as described above were exposed on days 2 and 3 to either CAMM lacking sucrose (no sucrose), CAMM with 0.2% sucrose (constant sucrose), or were transferred twice a day for 1 hr each time into 0.2% sucrose (intermittent sucrose). Four types of analysis were performed: 1) fluorescence microscopy of biofilms stained with Syto 9 and hexidium idodine to determine the biofilm architecture, 2) quantitative PCR (qPCR) to determine the cell number of each species per cm2, 3) vertical scanning interferometry (VSI) to determine the cariogenic potential of the biofilms, and 4) tomographic pH imaging using radiometric fluorescence microscopy after exposure to pH sensitive nanoparticles to measure the micro-environmental pH. The qualitative and quantitative results reveal the expected dynamics of the community profile when exposed to different sucrose conditions and the cariogenic potential of this in vitro four-species anaerobic biofilm model, thus confirming its usefulness for future analysis of primary and secondary dental caries.

The demand of new services, the emergence of new business models, insufficient innovation, underestimation of customer loyalty and reluctance to adopt new management are evidence of the deficiencies and the lack of research about the relations between patients and dental clinics. In this article we propose the structure of a model of Relationship Marketing (RM) in the dental clinic that integrates information from SERVQUAL, Customer Loyalty (CL) and activities of RM and combines the vision of dentist and patient. The first pilot study on dentists showed that: they recognize the value of maintaining better patients however they don't perform RM actions to retain them. They have databases of patients but not sophisticated enough as compared to RM tools. They perceive that the patients value "Assurance" and "Empathy" (two dimensions of service quality). Finally, they indicate that a loyal patient not necessarily pays more by the service. The proposed model will be validated using Fuzzy Logic simulation and the ultimate goal of this research line is contributing a new definition of CL.

Esta tesis doctoral propone un modelo de comportamiento del paciente de la clínica dental, basado en la percepción de la calidad del servicio (SERVQUAL), la fidelización del paciente, acciones de Marketing Relacional y aspectos socioeconómicos relevantes, de los pacientes de clínicas dentales. En particular, el estudio de campo se lleva a cabo en el ámbito geográfico de la Comunidad de Madrid, España, durante los años 2012 y 2013. La primera parte del proceso de elaboración del modelo está basada en la recolección de datos. Para ello, se realizaron cinco entrevistas a expertos dentistas y se aplicaron dos tipos encuestas diferentes: una para el universo formado por el conjunto de los pacientes de las clínicas dentales y la otra para el universo formado el conjunto de los dentistas de las clínicas dentales de la Comunidad de Madrid. Se obtuvo muestras de: 200 encuestas de pacientes y 220 encuestas de dentistas activos colegiados en el Ilustre Colegio Oficial de Odontólogos y Estomatólogos de la I Región Madrid. En la segunda parte de la elaboración del modelo, se realizó el análisis de los datos, la inducción y síntesis del modelo propuesto. Se utilizó la metodología de modelos gráficos probabilísticos, específicamente, una Red Bayesiana, donde se integraron variables (nodos) y sus dependencias estadísticas causales (arcos dirigidos), que representan el conocimiento obtenido de los datos recopilados en las encuestas y el conocimiento derivado de investigaciones precedentes en el área. Se obtuvo una Red Bayesiana compuesta por 6 nodos principales, de los cuales dos de ellos son nodos de observación directa: “Revisit Intention” y “SERVQUAL”, y los otros cuatro nodos restantes son submodelos (agrupaciones de variables), estos son respectivamente: “Attitudinal”, “Disease Information”, “Socioeconomical” y “Services”. Entre las conclusiones principales derivadas del uso del modelo, como herramientas de inferencia y los análisis de las entrevistas realizadas se obtiene que: (i) las variables del nodo “Attitudinal” (submodelo), son las más sensibles y significativas. Al realizarse imputaciones particulares en las variables que conforman el nodo “Attitudinal” (“RelationalMk”, “Satisfaction”, “Recommendation” y “Friendship”) se obtienen altas probabilidades a posteriori en la fidelidad del paciente de la clínica dental, medida por su intención de revisita. (ii) En el nodo “Disease Information” (submodelo) se destaca la relación de dependencia causal cuando se imputa la variable “Perception of disease” en “SERVQUAL”, demostrando que la percepción de la gravedad del paciente condiciona significativamente la percepción de la calidad del servicio del paciente. Como ejemplo destacado, si se realiza una imputación en la variable “Clinic_Type” se obtienen altas probabilidades a posteriori de las variables “SERVQUAL” y “Revisit Intention”, lo que evidencia, que el tipo de clínica dental influye significativamente en la percepción de la calidad del servicio y en la fidelidad del paciente (intención de revisita). (iii) En el nodo “Socioeconomical” (submodelo) la variable “Sex” resultó no ser significativa cuando se le imputaban diferentes valores, por el contrario, la variable “Age” e “Income” mostraban altas variabilidades en las probabilidades a posteriori cuando se imputaba alguna variable del submodelo “Services”, lo que evidencia, que estas variables condicionan la intención de contratar servicios (“Services”), sobretodo en las franjas de edad de 30 a 51 años en pacientes con ingresos entre 3000€ y 4000€. (iv) En el nodo “Services” (submodelo) los pacientes de las clínicas dentales mostraron altas probabilidades a priori para contratar servicios de fisiotrapia oral y gingival: “Dental Health Education” y “Parking”. (v) Las variables de fidelidad del paciente medidas desde su perspectiva comportamental que fueron utilizadas en el modelo: “Visit/year” “Time_clinic”, no aportaron información significativa. Tampoco, la variable de fidelidad del cliente (actitudinal): “Churn Efford”. (vi) De las entrevistas realizadas a expertos dentistas se obtiene que, los propietarios de la clínica tradicional tienen poca disposición a implementar nuevas estrategias comerciales, debido a la falta de formación en la gestión comercial y por falta de recursos y herramientas. Existe un rechazo generalizado hacia los nuevos modelos de negocios de clínicas dentales, especialmente en las franquicias y en lo que a políticas comerciales se refiere. Esto evidencia una carencia de gerencia empresarial en el sector. Como líneas futuras de investigación, se propone profundizar en algunas relaciones de dependencia (causales) como SERVQUALServices; SatisfactionServices; RelationalMKServices, Perception of diseaseSatisfaction, entre otras. Así como, otras variables de medición de la fidelidad comportamental que contribuyan a la mejora del modelo, como por ej. Gasto del paciente y rentabilidad de la visita. ABSTRACT This doctoral dissertation proposes a model of the behavior of the dental-clinic customer, based on the service-quality perception (SERVQUAL), loyalty, Relational Marketing and some relevant socio-economical characteristics, of the dental-clinic customers. In particular, the field study has been developed in the geographical region of Madrid, Spain during the years 2012 and 2013. The first stage of the preparation of the model consist in the data gathering process. For this purpose, five interviews where realized to expert dentists and also two different types of surveys: one for the universe defined by the set of dental-clinic patients and the second for the universe defined by the set of the dentists of the dental clinics of the Madrid Community. A sample of 200 surveys where collected for patients and a sample of 220 surveys where collected from active dentists belonging to the Ilustre Colegio Oficial de Odontólogos y Estomatólogos de la I Región Madrid. In the second stage of the model preparation, the processes of data-analysis, induction and synthesis of the final model where performed. The Graphic Probabilistic Models methodology was used to elaborate the final model, specifically, a Bayesian Network, where the variables (nodes) and their statistical and causal dependencies where integrated and modeled, representing thus, the obtained knowledge from the data obtained by the surveys and the scientific knowledge derived from previous research in the field. A Bayesian Net consisting on six principal nodes was obtained, of which two of them are directly observable: “Revisit Intention” y “SERVQUAL”, and the remaining four are submodels (a grouping of variables). These are: “Attitudinal”, “Disease Information”, “Socioeconomical” and “Services”. The main conclusions derived from the model, as an inference tool, and the analysis of the interviews are: (i) the variables inside the “Attitudinal” node are the most sensitive and significant. By making some particular imputations on the variables that conform the “Attitudinal” node (“RelationalMk”, “Satisfaction”, “Recommendation” y “Friendship”), high posterior probabilities (measured in revisit intention) are obtained for the loyalty of the dental-clinic patient. (ii) In the “Disease Information” node, the causal relation between the “Perception of disease” and “SERVQUAL” when “Perception of disease” is imputed is highlighted, showing that the perception of the severity of the patient’s disease conditions significantly the perception of service quality. As an example, by imputing some particular values to the “Clinic_Type” node high posterior probabilities are obtained for the “SERVQUAL” variables and for “Revisit Intention” showing that the clinic type influences significantly in the service quality perception and loyalty (revisit intention). (iii) In the “Socioeconomical” variable, the variable “Sex” showed to be non-significant, however, the “Age” variable and “Income” show high variability in its posterior probabilities when some variable from the “Services” node where imputed, showing thus, that these variables condition the intention to buy new services (“Services”), especially in the age range from 30 to 50 years in patients with incomes between 3000€ and 4000€. (iv) In the “Services” submodel the dental-clinic patients show high priors to buy services such as oral and gingival therapy, Dental Health Education and “Parking” service. (v) The obtained loyalty measures, from the behavioral perspective, “Visit/year” and “Time_clinic”, do not add significant information to the model. Neither the attitudinal loyalty component “Churn Efford”. (vi) From the interviews realized to the expert dentists it is observed that the owners of the traditional clinics have a low propensity to apply new commercial strategies due to a lack of resources and tools. In general, there exists an opposition to new business models in the sector, especially to the franchise dental model. All of this evidences a lack in business management in the sector. As future lines of research, a deep look into some statistical and causal relations is proposed, such as: SERVQUALServices; SatisfactionServices; RelationalMKServices, Perception of diseaseSatisfaction, as well as new measurement variables related to attitudinal loyalty that contribute to improve the model, for example, profit per patient and per visit.

One to two percent of all children are born with a developmental disorder requiring pediatric hospital admissions. For many such syndromes, the molecular pathogenesis remains poorly characterized. Parallel developmental disorders in other species could provide complementary models for human rare diseases by uncovering new candidate genes, improving the understanding of the molecular mechanisms and opening possibilities for therapeutic trials. We performed various experiments, e.g. combined genome-wide association and next generation sequencing, to investigate the clinico-pathological features and genetic causes of three developmental syndromes in dogs, including craniomandibular osteopathy (CMO), a previously undescribed skeletal syndrome, and dental hypomineralization, for which we identified pathogenic variants in the canine SLC37A2 (truncating splicing enhancer variant), SCARF2 (truncating 2-bp deletion) and FAM20C (missense variant) genes, respectively. CMO is a clinical equivalent to an infantile cortical hyperostosis (Caffey disease), for which SLC37A2 is a new candidate gene. SLC37A2 is a poorly characterized member of a glucose-phosphate transporter family without previous disease associations. It is expressed in many tissues, including cells of the macrophage lineage, e.g. osteoclasts, and suggests a disease mechanism, in which an impaired glucose homeostasis in osteoclasts compromises their function in the developing bone, leading to hyperostosis. Mutations in SCARF2 and FAM20C have been associated with the human van den Ende-Gupta and Raine syndromes that include numerous features similar to the affected dogs. Given the growing interest in the molecular characterization and treatment of human rare diseases, our study presents three novel physiologically relevant models for further research and therapy approaches, while providing the molecular identity for the canine conditions.

Introduction: Computer-Aided-Design (CAD) and Computer-Aided-Manufacture (CAM) has been developed to fabricate fixed dental restorations accurately, faster and improve cost effectiveness of manufacture when compared to the conventional method. Two main methods exist in dental CAD/CAM technology: the subtractive and additive methods. While fitting accuracy of both methods has been explored, no study yet has compared the fabricated restoration (CAM output) to its CAD in terms of accuracy. The aim of this present study was to compare the output of various dental CAM routes to a sole initial CAD and establish the accuracy of fabrication. The internal fit of the various CAM routes were also investigated. The null hypotheses tested were: 1) no significant differences observed between the CAM output to the CAD and 2) no significant differences observed between the various CAM routes. Methods: An aluminium master model of a standard premolar preparation was scanned with a contact dental scanner (Incise, Renishaw, UK). A single CAD was created on the scanned master model (InciseCAD software, V2.5.0.140, UK). Twenty copings were then fabricated by sending the single CAD to a multitude of CAM routes. The copings were grouped (n=5) as: Laser sintered CoCrMo (LS), 5-axis milled CoCrMo (MCoCrMo), 3-axis milled zirconia (ZAx3) and 4-axis milled zirconia (ZAx4). All copings were micro-CT scanned (Phoenix X-Ray, Nanotom-S, Germany, power: 155kV, current: 60µA, 3600 projections) to produce 3-Dimensional (3D) models. A novel methodology was created to superimpose the micro-CT scans with the CAD (GOM Inspect software, V7.5SR2, Germany) to indicate inaccuracies in manufacturing. The accuracy in terms of coping volume was explored. The distances from the surfaces of the micro-CT 3D models to the surfaces of the CAD model (CAD Deviation) were investigated after creating surface colour deviation maps. Localised digital sections of the deviations (Occlusal, Axial and Cervical) and selected focussed areas were then quantitatively measured using software (GOM Inspect software, Germany). A novel methodology was also explored to digitally align (Rhino software, V5, USA) the micro-CT scans with the master model to investigate internal fit. Fifty digital cross sections of the aligned scans were created. Point-to-point distances were measured at 5 levels at each cross section. The five levels were: Vertical Marginal Fit (VF), Absolute Marginal Fit (AM), Axio-margin Fit (AMF), Axial Fit (AF) and Occlusal Fit (OF). Results: The results of the volume measurement were summarised as: VM-CoCrMo (62.8mm3 ) > VZax3 (59.4mm3 ) > VCAD (57mm3 ) > VZax4 (56.1mm3 ) > VLS (52.5mm3 ) and were all significantly different (p presented as areas with different colour. No significant differences were observed at the internal aspect of the cervical aspect between all groups of copings. Significant differences (p< M-CoCrMo Internal Occlusal, Internal Axial and External Axial 2 ZAx3 > ZAx4 External Occlusal, External Cervical 3 ZAx3 < ZAx4 Internal Occlusal 4 M-CoCrMo > ZAx4 Internal Occlusal and Internal Axial The mean values of AMF and AF were significantly (p M-CoCrMo and CAD > ZAx4. Only VF of M-CoCrMo was comparable with the CAD Internal Fit. All VF and AM values were within the clinically acceptable fit (120µm). Conclusion: The investigated CAM methods reproduced the CAD accurately at the internal cervical aspect of the copings. However, localised deviations at axial and occlusal aspects of the copings may suggest the need for modifications in these areas prior to fitting and veneering with porcelain. The CAM groups evaluated also showed different levels of Internal Fit thus rejecting the null hypotheses. The novel non-destructive methodologies for CAD/CAM accuracy and internal fit testing presented in this thesis may be a useful evaluation tool for similar applications.

Objective: To examine sociodemographic and dental factors for associations with dental sealant placement in children and adolescents aged 6-18 years old. Methods: Secondary data analysis of 2011-2012 NHANES data was conducted. Multiple logistic regression models were used to assess relationships between predictor variables and sealant presence. Results: More than a third (37.1%) of children and adolescents have at least one sealant present; 67.9% of children compared with 40.4% of adolescents. Racial/ethnic differences exist, with Non-Hispanic black youth having the lowest odds of having sealants. Sealant placement odds vary by presence of dental home; the magnitude of the odds varies by age group. Those with untreated decay have lower odds of having sealants than those who do not have untreated decay (child OR: 2.6, 95% CI: 1.83-3.72; adolescent OR: 3.9, 95% CI: 2.59-6.07). Conclusion: Disparities exist in odds of sealant prevalence across racial/ethnic groups, income levels, and dental disease and visit characteristics. Further research is necessary to understand the reasons for these differences and to inform future interventions.

Purpose:
To identify economic evaluation models and parameters that could be replicated or adapted to construct a generic model to assess cost-effectiveness of and prioritise a wide range of community-based oral disease prevention programmes in an Australian context.
Methods:
The literature search was conducted using MEDLINE, ERIC, PsycINFO, CINHAL (EBSCOhost), EMBASE (Ovid), CRD, DARE, NHSEED, HTA, all databases in the Cochrane library, Scopus and ScienceDirect databases from their inception to November 2012.
Results:
Thirty-three articles met the criteria for inclusion in this review (7 were Australian studies, 26 articles were international). Existing models focused primarily on dental caries. Periodontal disease, another common oral health problem, was lacking. Among caries prevention studies, there was an absence of clear evidence showing continuous benefits from primary through to permanent dentition and the long-term effects of oral health promotion.
Conclusion:
No generic model was identified from previous studies that could be immediately adopted or adapted for our purposes of simulating and prioritising a diverse range of oral health interventions for Australian children and adolescents. Nevertheless, data sources specified in the existing Australian-based models will be useful for developing a generic model for such purposes.
