BackgroundIn Switzerland assisted suicide is legal if no self-interest is involved.AimsTo compare the strength and direction of associations with sociodemographic factors between assisted and unassisted suicides.MethodWe calculated rates and used Cox and logistic regression models in a longitudinal study of the Swiss population.ResultsAnalyses were based on 5 004 403 people, 1301 assisted and 5708 unassisted suicides from 2003 to 2008. The rate of unassisted suicides was higher in men than in women, rates of assisted suicides were similar in men and women. Higher education was positively associated with assisted suicide, but negatively with unassisted. Living alone, having no children and no religious affiliation were associated with higher rates of both.ConclusionsSome situations that indicate greater vulnerability such as living alone were associated with both assisted and unassisted suicide. Among the terminally ill, women were more likely to choose assisted suicide, whereas men died more often by unassisted suicide.

El resultado de la evolución de los años noventa muestra a una China con un alto grado de apertura económica, asociaciones estratégicas y crecimiento como potencia emergente en el Sistema Internacional. Para el gobierno de Ju Hintao (2003) la Política Exterior China se integraba a la economía global y sus instituciones dejando de lado el bajo perfil en el Sistema Internacional. La inserción de Beijing en la economía mundial implicó la búsqueda de nuevos socios comerciales que suplieran el suministro de materias primas para el gigante asiático. Frente la apertura, la economía China necesitaba fortalecer su mercado para competir en la estructura económica del Sistema Internacional. Para dicho objetivo, el gobierno de Beijing proyectó una campaña para fomentar programas para el desarrollo, el beneficio mutuo y la práctica entre iguales; marco que se dio principalmente en África rico en materias primas. A partir de lo anterior, el realismo defensivo como sustento teórico de la monografía traza enuncia un nuevo patrón en las relaciones de seguridad regional e internacional con base en la igualdad y el respeto entre las distintas soberanías nacionales.

El objetivo fue determinar la variación batimétrica y temporal de la composición funcional por gremios tróficos del grupo Poliquetos. Se colectaron muestras de macrobentos e información oceanográfica en tres estaciones ubicadas en la bahía (BH, 35 m), plataforma interna (PI, 65 m) y plataforma externa (PE, 117 m), de febrero 2003 a octubre 2008. Los resultados indicaron que la variabilidad estacional está sujeta a cambios de mayor escala, como el incremento de la producción primaria en primavera y verano, posibilitando un aumento de los depositívoros superficiales (SDF) en la BH durante primavera, por efecto de la acumulación de materia orgánica fitoplanctónica que llega al fondo. Asimismo, la disminución de los depositívoros superficiales (SDF) y el aumento de los suspensívoros (SF) en verano, en la BH y PI, se pueden explicar debido al incremento de la descarga de partículas terrígenas de origen vegetal, que afectan físicamente al sustrato. En tanto, los consumidores de interfaz (IF) que dominaron en verano/otoño en la PE, donde la influencia continental es menor, pueden haber sido favorecidos por la mayor calidad de flujo acumulado de fitodetritus en los sedimentos y a un menor contenido de OD, lo cual disminuyó quizás la presión por predación y competencia.

Les recherches en relations industrielles portent rarement sur les politiques publiques du marché du travail. Lorsqu’elles existent, ces recherches s’intéressent généralement à l’efficacité de ces politiques. Or, cette thèse illustre l’importance de comprendre comment se forment les lois et les règlements. Afin de mieux comprendre le processus d’élaboration des politiques publiques, une étude de cas a été réalisée dans le domaine des retraites au Québec et au Canada entre 2003 et 2008. Les retraites ont été ciblées en raison des bouleversements socio-économiques que connaissait le Canada à l’époque, notamment le vieillissement de la population et les fluctuations des rendements des fonds de retraite. Ce sujet a également été retenu car il s’agit d’une compétence partagée entre les paliers gouvernementaux fédéral et provinciaux ainsi que partagée entre le secteur public et privé.Afin d’étudier la question, des théories en relations industrielles et en sciences économiques ont été considérées. C’est toutefois la théorie des courants multiples du politologue américain John Kingdon qui a été retenu comme fondement théorique de cette thèse. Cette théorie, qui postulait que les acteurs voulant influencer les politiques publiques devaient attirer l’attention sur des problèmes, contrôler le débat sur les politiques considérées et se faire des appuis, a été confirmée. La contribution majeure de cette thèse a été d’identifier les différents types d’acteurs impliqués dans le processus d’élaboration de politiques publiques de retraite, les stratégies qu’ils ont employées pour y parvenir et les résultats de leurs efforts.

Introducción: el riesgo de desarrollar cáncer de seno durante la vida es del 13,4% (1 de cada 7 mujeres) y la posibilidad de morir por la enfermedad después del diagnostico es cercana al 30%. Pacientes y Métodos: es un estudio de cohorte abierta retrospectiva en el que se analizó la sobrevida según los factores pronósticos de las pacientes con cáncer de seno del hospital militar central en el periodo de enero de 2003 a diciembre de 2008. Los factores pronósticos son: Edad, estadío del tumor al momento del diagnóstico, Grado de diferenciación del tumor, presencia de metástasis al momento del diagnóstico, presencia de metástasis, número de sitios de metástasis, erb2, presencia de ganglios afectados, número de ganglios positivos, receptores estrogénicos, receptores de progestágeno, tratamiento con trastuzumab, tratamiento con hormonoterapia; el análisis estadístico se realizó a partir de la herramienta de recolección de datos, esta base de datos fue trasladada al programa SPSS. Resultados: participaron 171 mujeres. La presencia de receptores para estrógenos positivos se correlaciona con una mayor sobrevida con una diferencia estadísticamente significativa (p=0.015). Durante el periodo de tiempo del estudio fallecieron 23 pacientes (13.4%), de las cuales 20 (86%) presentaban Carcinoma Canalicular Infiltrante y 21 (91%) presentaban estadios avanzados del carcinoma. Conclusiones: las características demográficas de nuestra población son similares a lo publicado en la literatura, sin variantes estadísticamente significativas frente a los hallazgos internacionales. En nuestro análisis hubo una fuerte correlación de la presencia de estrógenos positivos en relación al tiempo de sobrevida.

Background: The state of the HIV epidemic in the Philippines has been described as "low and slow", which is in stark contrast to many other countries in the region. A review of the conditions for HIV spread in the Philippines is necessary. Methods: We evaluated the current epidemiology, trends in behaviour and public health response in the Philippines to identify factors that could account for the current HIV epidemic, as well as to review conditions that may be of concern for facilitating an emerging epidemic. Results: The past control of HIV in the Philippines cannot be attributed to any single factor, nor is it necessarily a result of the actions of the Filipino government or other stakeholders. Likely reasons for the epidemic's slow development include: the country's geography is complicated; injecting drug use is relatively uncommon; a culture of sexual conservatism exists; sex workers tend to have few clients; anal sex is relatively uncommon; and circumcision rates are relatively high. In contrast, there are numerous factors suggesting that HIV is increasing and ready to emerge at high rates, including: the lowest documented rates of condom use in Asia; increasing casual sexual activity; returning overseas Filipino workers from high-prevalence settings; widespread misconceptions about HIV/AIDS; and high needle-sharing rates among injecting drug users. There was a three-fold increase in the rate of HIV diagnoses in the Philippines between 2003 and 2008, and this has continued over the past year. HIV diagnoses rates have noticeably increased among men, particularly among bisexual and homosexual men (114% and 214% respective increases over 2003-2008). The average age of diagnosis has also significantly decreased, from approximately 36 to 29 years. Conclusions: Young adults, men who have sex with men, commercial sex workers, injecting drug users, overseas Filipino workers, and the sexual partners of people in these groups are particularly vulnerable to HIV infection. There is no guarantee that a large HIV epidemic will be avoided in the near future. Indeed, an expanding HIV epidemic is likely to be only a matter of time as the components for such an epidemic are already present in the Philippines.

Objective To determine if a clinic-based behavioral intervention program for low-income mid-life women that emphasizes use of community resources will increase moderate intensity physical activity (PA) and improve dietary intake. Methods Randomized trial conducted from May 2003 to December 2004 at one community health center in Wilmington, NC. A total of 236 women, ages 40–64, were randomized to receive an Enhanced Intervention (EI) or Minimal Intervention (MI). The EI consisted of an intensive phase (6 months) including 2 individual counseling sessions, 3 group sessions, and 3 phone calls from a peer counselor followed by a maintenance phase (6 months) including 1 individual counseling session and 7 monthly peer counselor calls. Both phases included efforts to increase participants' use of community resources that promote positive lifestyle change. The MI consisted of a one-time mailing of pamphlets on diet and PA. Outcomes, measured at 6 and 12 months, included the comparison of moderate intensity PA between study groups as assessed by accelerometer (primary outcome) and questionnaire, and dietary intake assessed by questionnaire and serum carotenoids (6 months only). Results For accelerometer outcomes, follow-up was 75% at 6 months and 73% at 12 months. Though moderate intensity PA increased in the EI and decreased in the MI, the difference between groups was not statistically significant (p = 0.45; multivariate model, p = 0.08); however, moderate intensity PA assessed by questionnaire (92% follow-up at 6 months and 75% at 12 months) was greater in the EI (p = 0.01; multivariate model, p = 0.001). For dietary outcomes, follow-up was 90% for questionnaire and 92% for serum carotenoids at 6 months and 74% for questionnaire at 12 months. Dietary intake improved more in the EI compared to the MI (questionnaire at 6 and 12 months, p < 0.001; serum carotenoid index, p = 0.05; multivariate model, p = 0.03). Conclusion The EI did not improve objectively measured PA, but was associated with improved self-reported and objective measures of dietary intake.

It is now widely acknowledged that student mental well-being is a critical factor in the tertiary student learning experience and is important to student learning success. The issue of student mental well-being also has implications for effective student transition out of university and into the world of work. It is therefore vital that intentional strategies are adopted by universities both within the formal curriculum, and outside it, to promote student well-being and to work proactively and preventatively to avoid a decline in student psychological well-being. This paper describes how the Queensland University of Technology Law School is using animation to teach students about the importance for their learning success of the protection of their mental well-being. Mayer and Moreno (2002) define an animation as an external representation with three main characteristics: (1) it is a pictorial representation, (2) it depicts apparent movement, and (3) it consists of objects that are artificially created through drawing or some other modelling technique. Research into the effectiveness of animation as a tool for tertiary student learning engagement is relatively new and growing field of enquiry. Nash argues, for example, that animations provide a “rich, immersive environment [that] encourages action and interactivity, which overcome an often dehumanizing learning management system approach” (Nash, 2009, 25). Nicholas states that contemporary millennial students in universities today, have been immersed in animated multimedia since their birth and in fact need multimedia to learn and communicate effectively (2008). However, it has also been established, for example through the work of Lowe (2003, 2004, 2008) that animations can place additional perceptual, attentional, and cognitive demands on students that they are not always equipped to cope with. There are many different genres of animation. The dominant style of animation used in the university learning environment is expository animation. This approach is a useful tool for visualising dynamic processes and is used to support student understanding of subjects and themes that might otherwise be perceived as theoretically difficult and disengaging. It is also a form of animation that can be constructed to avoid any potential negative impact on cognitive load that the animated genre might have. However, the nature of expository animation has limitations for engaging students, and can present as clinical and static. For this reason, the project applied Kombartzky, Ploetzner, Schlag, and Metz’s (2010) cognitive strategy for effective student learning from expository animation, and developed a hybrid form of animation that takes advantage of the best elements of expository animation techniques along with more engaging short narrative techniques. First, the paper examines the existing literature on the use of animation in tertiary educational contexts. Second, the paper describes how animation was used at QUT Law School to teach students about the issue of mental well-being and its importance to their learning success. Finally, the paper analyses the potential of the use of animation, and of the cognitive strategy and animation approach trialled in the project, as a teaching tool for the promotion of student learning about the importance of mental well-being.

The number of Finnish pupils attending special education has increased for more than a decade (Tilastokeskus 1999, 2000, 2001, 2003, 2004, 2005a, 2006b, 2007b, 2008b, 2008e, 2009b; Virtanen ja Ratilainen 1996). In the year 2007 nearly third of Finnish comprehensive school pupils took part in special needs education. According to the latest statistics, in the autumn of 2008 approximately 47 000 pupils have been admitted or transferred to special education and approximately 126 000 pupils received part-time special education during the 2007 - 2008 academic year. (Tilastokeskus 2008b, 2009b.) The Finnish special education system is currently under review. The Reform, both in legislation and in practice, began nationwide in the year 2008 (e.g. Special education strategy document, November 2007 and the development project Kelpo). The aim of the study was the statistical description of the Finnish special education system and on the other hand to gain a deeper understanding about the Finnish special education system and its quantitative increase, by analysis based on the nationwide statistical information. Earlier studies have shown that the growth in special education is affected by multiple independent variables and cannot be solely explained by the pupil characteristics. The statistical overview and analysis have been carried out in two parts. In the first part, the description and analysis were based on statistical time series from the academic year 1979 -1980 until 2008. While, in the second, more detailed description and analysis, based on comparable time series from 1995 to 2008 and from 2001-2002 to 2007-2008, is presented. Historical perspective was one part of this study. There was an attempt to find reasons explaining the observed growth in the special needs education from late 1960s to 2008. The majority of the research was based on the nationwide statistics information. In addition to this, materials including educational legislation literature, different kind of records of special education and preceding studies were also used to support the research. The main results of the study, are two statistical descriptions and time series analysis of the quantitative increase of the special needs education. Further, a summary of the plausible factors behind the special education system change and its quantitative increase, is presented. The conclusions coming from the study can be summarised as follows: the comparable statistical time series analysis suggests that the growth in special education after the year 1999 could be a consequence of the changes in the structure of special education and that new group of pupils have been directed to special needs education. A atividade de pesquisa em saúde é uma área de interesse e importância para o desenvolvimento de uma sociedade. Ela pode contribuir no processo de redução das desigualdades na saúde. A constituição de agendas de prioridades em pesquisa, enquanto um instrumento técnico-político indutor de financiamentos de pesquisas alinhadas às necessidade sociais e sanitárias, seria uma das estratégias a serem utilizadas na busca dessa redução. O objetivo deste trabalho foi mapear a aplicação dos recursos financeiros aplicados em pesquisa e desenvolvimento em saúde pelo Ministério da Saúde (MS) durante o período 2003-2005, conforme Agenda Nacional de Prioridades de pesquisa em Saúde (ANPPS), estabelecida em 2004. Utilizaram-se os dados procedentes de pesquisa realizada com a finalidade primária de mensurar os fluxos de recursos investidos em P&D/S no país no período, sendo considerados apenas os investimentos oriundos do próprio MS. Foi computado apenas o financiamento direto em pesquisa e efetivamente pago, excluindo-se dispêndios com salários. As pesquisas forma categorizadas segundo as 24 Subagendas da ANPPS por dois pesquisadores, independentemente, com as discordâncias sendo resolvidas por consenso. O volume de recursos aplicados pelo MS no período foi cerca de R$ 409,7 milhões. Em termos da distribuição dos recursos, segundo as Subagendas componentes da ANPPS, os dado apontaram para uma concentração de fomento em cinco Subagendas principais: Doenças Transmissíveis, Complexo produtivo da Saúde, Pesquisa clínica, Assistência farmacêutica e Doenças não transmissíveis. Somadas, elas representaram investimentos da ordem de R$ 324 milhões para o período 2003-2005. Todas as 24 Subagendas receberam algum tipo de financiamento no período, embora alguns tenham sido de pequena monta. No período sob análise, ocorreram os passos iniciais para a construção e institucionalização da Agenda. Dessa maneira, o estudo realizado pode servir para estabelecer um ponto inicial (como um marco zero) para posteriores estudos sobre o potencial indutor deste instrumento, contribuindo para avaliações acerca da aproximação entre os investimentos em P&D/S no país e as necessidades sanitárias da população e, para o desenvolvimento do SUS.

La multiplication des formations professionnelles universitaires (FPU) a poussé plusieurs chercheurs à s’intéresser aux caractéristiques de ces formations, leur perméabilité à une multitude de pressions étant l’une des plus fréquemment relevées. Ainsi, les unités responsables de FPU sont confrontées à des pressions diverses et souvent contradictoires. Si les écrits scientifiques sur les FPU témoignent bien de cette réalité, ceux-ci nous informent moins bien de la manière dont les unités responsables de ce type de formation répondent à ces pressions. Cette thèse a donc fait appel à plusieurs concepts de l’approche institutionnelle issue de la sociologie des organisations pour analyser l’évolution récente de la FPU destinée aux directions d’établissement scolaire (DES) du Québec, un champ qui a connu d’importantes transformations au cours des vingt-cinq dernières années. Construite sur une étude de cas interprétative dite à « unités enchâssées » (Yin, 2003), cette thèse s’est intéressée à l’évolution de cette formation dans deux unités universitaires francophones : le Département d’administration et fondements de l’éducation de l’Université de Montréal et le Département de gestion de l’éducation et de la formation de l’Université de Sherbrooke. Couvrant la période allant des années universitaires 1988-1989 à 2008-2009, elle repose sur une analyse du discours produit par les deux unités sélectionnées, et, dans une moindre mesure, par les organisations qui composent le champ organisationnel de la formation des DES au Québec. Pour ce faire, trois corpus documentaires distincts ont été assemblés et une série d’entrevues (dix par unités) ont été réalisées auprès d’informateurs-clés (doyens, directeurs de département/section, responsables de formation). Les résultats montrent comment ces unités tendent à se rendre isomorphes à leur environnement, et comment cela se fait en réponse à des pressions institutionnelles et de compétition diverses émanant d’un champ organisationnel en pleine transformation. En fait, poussée par des changements plus profonds touchant l’administration scolaire, cette transformation amène un champ organisationnel plus structuré, où les attentes concernant la FPU destinée aux DES sont plus explicites. Cela n’est pas sans conséquence sur l’évolution de la formation dans les deux unités. En effet, celle-ci connaît des changements importants, dont plusieurs convergent autour d’une logique de professionnalisation, d’un archétype spécifique de formation (un continuum de formation de 2e cycle, au cœur duquel se trouve un diplôme de deuxième cycle) et d’outils conséquents (conditions d’admission et populations étudiantes élargies; flexibilité dans la structure du programme et professionnalisation des activités; équipes enseignantes plus diversifiées). Les deux unités n’apparaissent cependant pas impuissantes devant ces pressions. Les résultats témoignent d’un certain niveau d’agence des deux unités, qui déploient un éventail de stratégies en réaction à ces pressions. Ces stratégies évoluent au cours de la période observée et visent surtout à gérer la situation de « pluralisme institutionnel » à laquelle elles sont confrontées, notamment entre les pressions externes de nature plus professionnalisantes, et les pressions intraorganisationnelles de nature plus académisantes. Ainsi, plusieurs des stratégies et tactiques composant la typologie d’Oliver (1991) ont été observées, les stratégies de compromis et de manipulation occupant, dans les deux unités, une place de plus en plus importante au gré de l’évolution du champ. La mise en œuvre de ces stratégies vise surtout à maintenir la légitimité de leur offre de formation devant des pressions plurielles et parfois contradictoires. Les résultats montrent aussi que la nature de l’entrepreneuriat institutionnel en place détermine en grande partie les stratégies qu’elles déploient. Cet « entrepreneuriat » est au cœur de l’évolution de la formation. Cependant, les résultats montrent aussi comment celui-ci est en partie contraint ou, a contrario, stimulé par les choix historiques qui ont été faits par les unités et leur université, et par l’empreinte et les dépendances de sentier qui en découlent. Ces résultats apportent un éclairage « institutionnaliste » sur la manière dont deux unités universitaires ont réagi, à une période donnée, aux pressions diverses provenant de leur environnement. Ils brossent un portrait complexe et nuancé qui vient à la fois (1) approfondir notre compréhension de cette spécificité des FPU, (2) approfondir notre compréhension de l’évolution récente de la FPU destinée aux DES québécoises, et (3) confirmer la puissance d’analyse de plusieurs concepts tirés de l’approche institutionnelle.

El objetivo general de esta investigación es analizar el proceso de incorporación del gobierno electrónico en Colombia, así como evaluar el impacto de las TIC en asuntos del gobierno y su relación con los ciudadanos, puesto que en la práctica se desea incrementar la participación en asuntos públicos y una inclusión ciudadana a través de las TIC. El éxito del Gobierno en Línea dependerá de la transparencia y confianza creciente entre el gobierno y los ciudadanos y toma como referencia la estrategia de Gobierno en Línea 2008-2010. Será usada, una metodología exploratoria y descriptiva sobre la base de documentos bibliográficos y publicaciones periódicas y académicas.

El interés principal de este estudio de caso es analizar y caracterizar el fenómeno del Crimen Organizado (CO) en México y Colombia, y su manifestación a través de estructuras criminales como carteles de droga (en el caso mexicano), y BACRIM, FARC y grupos insurgentes (en el caso colombiano), para luego analizar y evaluar cuáles han sido las tendencias de convergencia y divergencia en el marco de cooperación de estos países en torno al tráfico de droga en un periodo comprendido entre el 2003 al 2010.
