Extraction and reconstruction of rectal wall structures from an ultrasound image is helpful for surgeons in rectal clinical diagnosis and 3-D reconstruction of rectal structures from ultrasound images. The primary task is to extract the boundary of the muscular layers on the rectal wall. However, due to the low SNR from ultrasound imaging and the thin muscular layer structure of the rectum, this boundary detection task remains a challenge. An active contour model is an effective high-level model, which has been used successfully to aid the tasks of object representation and recognition in many image-processing applications. We present a novel multigradient field active contour algorithm with an extended ability for multiple-object detection, which overcomes some limitations of ordinary active contour models—"snakes." The core part in the algorithm is the proposal of multigradient vector fields, which are used to replace image forces in kinetic function for alternative constraints on the deformation of active contour, thereby partially solving the initialization limitation of active contour for rectal wall boundary detection. An adaptive expanding force is also added to the model to help the active contour go through the homogenous region in the image. The efficacy of the model is explained and tested on the boundary detection of a ring-shaped image, a synthetic image, and an ultrasound image. The experimental results show that the proposed multigradient field-active contour is feasible for multilayer boundary detection of rectal wall

Regular monitoring of wastewater characteristics is undertaken on most wastewater treatment plants. The data acquired during this process are usually filed and forgotten. However, systematic analysis of these data can provide useful insights into plant behaviour. Conventional graphical techniques are inadequate to give a good overall picture of how wastewater characteristics vary, with time and along the lagoon system. An approach based on the use of contour plots was devised that largely overcomes this problem. Superimposition of contour plots for different parameters can be used to gain a qualitative understanding of the nature and strength of relationships between the parameters. This is illustrated in an analysis of monitoring data for lagoon 115 East at the Western Treatment Plant, near Melbourne, Australia. In this illustrative analysis, relationships between ammonia removal rates and parameters such as chlorophyll a level and temperature are explored using a contour plot superimposition approach. It is concluded that this approach can help improve our understanding, not only of lagoon systems, but of other wastewater treatment systems as well.

PURPOSE. It is well documented that myopia is associated with an increase in axial length or, more specifically, in vitreous chamber depth. Whether the transverse dimensions of the eye also increase in myopia is relevant to further understanding of its development. METHODS. The posterior retinal surface was localized in two-dimensional space in both eyes of young adult white and Taiwanese-Chinese iso- and anisomyopes (N = 56), from measured keratometry, A-scan ultrasonography, and central and peripheral refraction (±35°) data, with the aid of a computer modeling program designed for this purpose. Anisomyopes had 2 D or more interocular difference in their refractive errors, with mean values in their more myopic eyes of -5.57 D and in their less myopic eyes of -3.25 D, similar to the means of the two isomyopic groups. The derived retinal contours for the more and less myopic eyes were compared by way of investigating ocular shape changes that accompany myopia, in the posterior region of the vitreous chamber. The presence and size of optic disc crescents were also investigated as an index of retinal stretching in myopia. RESULTS. Relative to the less myopic eyes of anisometropic subjects, the more myopic eyes were more elongated and also distorted into a more prolate shape in both the white and Chinese groups. However, the Chinese eyes showed a greater and more uniform relative expansion of the posterior retinal surface in their more myopic eyes, and this was associated with larger optic disc crescents. The changes in the eyes of whites displayed a nasal-temporal axial asymmetry, reflecting greater enlargement of the nasal retinal sector. CONCLUSIONS. Myopia is associated with increased axial length and a prolate shape. This prolate shape is consistent with the proposed idea that axial and transverse dimensions of the eye are regulated differently. The observations that ocular shape changes are larger but more symmetrical in Chinese eyes than in eyes of whites warrant further investigation.

This paper presents a novel intonation modelling approach and demonstrates its applicability using the Standard Yorùbá language. Our approach is motivated by the theory that abstract and realised forms of intonation and other dimensions of prosody should be modelled within a modular and unified framework. In our model, this framework is implemented using the Relational Tree (R-Tree) technique. The R-Tree is a sophisticated data structure for representing a multi-dimensional waveform in the form of a tree. Our R-Tree for an utterance is generated in two steps. First, the abstract structure of the waveform, called the Skeletal Tree (S-Tree), is generated using tone phonological rules for the target language. Second, the numerical values of the perceptually significant peaks and valleys on the S-Tree are computed using a fuzzy logic based model. The resulting points are then joined by applying interpolation techniques. The actual intonation contour is synthesised by Pitch Synchronous Overlap Technique (PSOLA) using the Praat software. We performed both quantitative and qualitative evaluations of our model. The preliminary results suggest that, although the model does not predict the numerical speech data as accurately as contemporary data-driven approaches, it produces synthetic speech with comparable intelligibility and naturalness. Furthermore, our model is easy to implement, interpret and adapt to other tone languages.

This study explored the role of formant transitions and F0-contour continuity in binding together speech sounds into a coherent stream. Listening to a repeating recorded word produces verbal transformations to different forms; stream segregation contributes to this effect and so it can be used to measure changes in perceptual coherence. In experiment 1, monosyllables with strong formant transitions between the initial consonant and following vowel were monotonized; each monosyllable was paired with a weak-transitions counterpart. Further stimuli were derived by replacing the consonant-vowel transitions with samples from adjacent steady portions. Each stimulus was concatenated into a 3-min-long sequence. Listeners only reported more forms in the transitions-removed condition for strong-transitions words, for which formant-frequency discontinuities were substantial. In experiment 2, the F0 contour of all-voiced monosyllables was shaped to follow a rising or falling pattern, spanning one octave. Consecutive tokens either had the same contour, giving an abrupt F0 change between each token, or alternated, giving a continuous contour. Discontinuous sequences caused more transformations and forms, and shorter times to the first transformation. Overall, these findings support the notion that continuity cues provided by formant transitions and the F0 contour play an important role in maintaining the perceptual coherence of speech.

Taken as a policy framework, active aging ranks high on most supranational bodies’ agenda. The new political economy of aging portrays “active” citizenship amongst seniors as a key challenge for the years to come. Our research focuses on, first, elderly women’s everyday ‘active’ practices, their meaning and purpose, in the context of Quebec’s active aging policy framework; and second, their day-to-day practical citizenship experiences. Informed by discourse analysis and a narrative approach, the life stories of women 60 to 70 years of age allowed for the identification of a plethora of distinctive old age activity figures. More specifically, four activity figures were identified by which respondents materialize their routine active practices, namely: (1) paid work; (2) voluntary and civic engagement; (3) physical activity; and (4) caregiving. Set against Quebec’s active aging policy framework, these patterns and set of practices that underpin them are clearly in tune with government’s dominant perspectives. Respondents’ narratives also show that active aging connotes a range of ‘ordinary’ activities of daily living, accomplished within people’s private worlds and places of proximity. Despite nuances, tensions and opposition found in dominant public discourse, as well as in active aging practices, a form of counter-discourse does not emerge from respondents’ narratives. To be active is normally the antithesis of immobility and dependence. Thus, to see oneself as active in old age draws on normative, positive assumptions about old age quite difficult to refute; nevertheless, discourses also raise identity and relational issues. In this respect, social inclusion issues cut across all active aging practices described by respondents. Moreover, a range of individual aims and quests underpin activity pattern. Such quests express respondents’ subjective interactions with their social environment; including their actions’ meaning and sense of social inclusiveness in old age. A first quest relates to personal identity and social integration to the world; a second one concerns giving; a third centers on the search for authenticity; whereas the fourth one is connected to a desire for freedom. It is through the objectivising of active practices and related existential pursuits that elderly woman recognize themselves as active citizens, rooted in the community, and variously contributing to society. Accordingly, ‘active’ citizenship experiences are articulated in a dialogic manner between the dimensions of ‘doing’, ‘active’ social practices, and ‘being’ in relation to others, within a context of interdependence. A proposed typology allows for the modeling of four ‘active’ citizenship figures. Overall, despite the role played by power relations and social inequality in structuring aging experiences, in everyday life ‘old age citizenship’ appears as a relational process, embedded in a set of social relations and practices involving individuals, families and communities, whereby elderly women are able to express a sense of agency within their social world.

L’insuffisance mitrale (IM) fonctionnelle est une complication fréquente des cardiopathies, causée par la dilatation du ventricule gauche (VG) qui empêche la valve de se fermer. L’insuffisance aortique (IA) est une condition associée à des valves mitrales (VM) inhabituellement grandes, et relativement peu d’IM malgré des VG très dilatés. Cet élargissement de la VM a le potentiel de prévenir l’IM dans les VG dilatés. Les mécanismes sont cependant peu compris : il n’est pas clair s’il s’agit d’une croissance active ou d’un étirement passif des feuillets. Également, le timing de l’adaptation valvulaire n’est pas connu. Notre hypothèse est que l’agrandissement de la valve mitrale en IA est un phénomène actif avec réactivation des mécanismes de croissance embryonnaire. Cent-onze rats ont été divisés en deux groupes : IA (perforation aortique) et contrôle. Les animaux ont été sacrifiés à 48 h, 1 semaine et 3 mois après la création du modèle. Des échocardiographies ont évalué la sévérité de l’IA, la présence d’IM et les dimensions du VG. Les valves ont été prélevées pour analyses microscopiques et moléculaires. La création de l’insuffisance aortique a entrainé une dilatation et une hypertrophie du VG. Malgré cette dilatation rapide du VG, aucun animal n’a développé de l’IM fonctionnelle. À l’échographie, le feuillet antérieur mitral était significativement plus long dans les groupes IA. Par microscopie, les feuillets étaient plus épais dés la première semaine. L’IA était associée à une surexpression de collagène α-SMA (un marqueur de myofibroblastes), TGF-β1 et MMP-2 dans le tissu valvulaire dès la première semaine. Les valves exposées à l’IA étaient également positives pour ces différents facteurs dés les premiers jours. L’agrandissement de la VM est un phénomène actif qui survient rapidement après la création de l’IA, en parallèle de la dilatation du VG. La stimulation de cette croissance dans d’autres pathologies pourrait contribuer à prévenir l’IM fonctionnelle.

La fin du traitement actif et le début de la phase de survie correspondent à une période de transition qui présente de nombreux défis pour la personne survivante au cancer, les soignants et le système de santé. La phase de survie au cancer est une phase distincte mais négligée du continuum de soins. Pour faciliter la transition de la fin du traitement actif vers la survie et optimiser la coordination des soins de suivi, la mise en place d’un plan de soins de suivi (PSS) est proposée. Le but de cette recherche est de développer, de mettre en place et d’évaluer si un plan de soins de suivi (PSS) permet de répondre aux besoins globaux, de diminuer la détresse émotionnelle et de favoriser les comportements d’autogestion de santé de femmes atteintes du cancer de l’endomètre (FACE) lors de la transition de la fin du traitement actif vers la survie au cancer. Elle comprend deux phases distinctes. La première phase visait d’abord le développement d’un plan de soins de suivi (PSS) pour des femmes atteintes du cancer de l’endomètre avec traitements adjuvants. La sélection du contenu du PSS a été faite à partir de la recension des écrits et des données recueillies lors d’entrevues individuelles avec 19 FACE, 24 professionnels de la santé travaillant avec cette clientèle et quatre gestionnaires de proximité en oncologie. Cette première phase avait également pour but la validation du contenu du PSS auprès de dix professionnels de la santé impliqués dans l’étude. La seconde phase consistait à évaluer la faisabilité, l’acceptabilité du PSS et à en pré-tester l’utilité à répondre aux besoins globaux, à diminuer la détresse émotionnelle (peur de la récidive) et à favoriser l’autogestion de santé auprès d’un groupe de 18 femmes atteintes du cancer de l’endomètre avec traitements adjuvants à la fin du traitement actif vers la survie. Sur le plan de la faisabilité, les résultats suggèrent que la mise en place du PSS comporte des défis en termes de temps, de ressources et de coordination pour l’infirmière pivot en oncologie (IPO). Concernant l’acceptabilité du PSS, les FACE le perçoivent comme un outil d’information utile qui favorise la communication avec le médecin de famille ou d’autres professionnels de la santé. Les IPO soutiennent sa valeur ajoutée à la fin du traitement et soulignent que la discussion du contenu du PSS fait ressortir des éléments de surveillance et de suivi essentiels à prendre en compte pour la phase de survie et permet de mettre l’emphase sur l’autogestion de sa santé. Pour les médecins de famille, le PSS est un outil d’information pour les survivantes qui favorise la réassurance, la communication et la continuité des soins entre professionnels de la santé. Enfin, pour ce qui est de l’utilité du PSS à répondre aux besoins globaux, les résultats suggèrent que l’ensemble des besoins sont plus satisfaits trois mois après la fin des traitements pour le groupe ayant reçu un PSS. Bien que la peur de récidive du cancer (PRC) s’améliore au suivi de trois mois pour le groupe exposé au PSS, 55% des FACE conservent un score cliniquement significatif de 13 à la sous-échelle de sévérité de peur de récidive à la fin du traitement et 42% au suivi de trois mois. Les comportements d’autogestion de santé s’améliorent entre la fin du traitement et le suivi de trois mois pour le groupe exposé un PSS. Considérant ces résultats, la démarche soutient la pertinence de mettre en place un PSS à la fin du traitement actif pour les FACE pour répondre à des besoins d’information, favoriser la communication et la continuité des soins avec les professionnels de la santé et les comportements d’autogestion de santé dans la phase de survie. Cependant, des contraintes de temps, de ressources et de coordination doivent être prises en compte pour sa mise en place dans le milieu clinique.
