Des parents semblent ignorer la limite entre ce que l'on est en droit de montrer et ce que l'on doit traiter avec pudeur. Une sorte d'ambiguïté les amène à être négligents et par là à tolérer des comportements ou des propos grivois de leurs enfants ou à en rire. Les thérapeutes sont invités à entreprendre un travail pédagogique de guidance parentale quand les frontières de l'intime sont floues et quand la confusion touche les jeunes devenant vulnérables et symptomatiques. Les auteurs exposent un cas d'anorexie chez une adolescente, dont la souffrance se trouve masquée par une série de comportements désinhibés. La surexcitation qui en découle peut apparaître comme une solution défensive pour lutter contre la dépression liée à l'expérience de séparation et de deuil. Pour favoriser la construction de représentations des territoires de l'intime plus respectueuses de la vulnérabilité des adolescents, les auteurs ont aussi proposé la sculpture familiale.Parental guidance and respect of intimacy. The therapists are invited to undertake an educational work of parental guidance when the boderlines of the intimate are hazy and when the confusion is affecting young people who become vulnerable and symptomatic. The authors present an anorexia case of a teenager whose distress is concealed by a series of uninhibited behaviours. The ensuing overexcitement could seem to be a defensive solution to fight against the depression related to parting and mourning experience. To encourage the building of representations of the territories of the intimate which would be more respectful of the teenagers vulnerability, the authors also suggest the technique of family sculpture.

This preliminary study aims to analyze the therapeutic alliance in a crosscultural triadic setting, where there is a therapist and a client who speak a different language but are able to interact thanks to an interpreter/cultural mediator. The participants' (therapists, clients, interpreters) representations associated with the notion of therapeutic alliance, and the level of alliance between each group was obtained and compared. Clients (N = 9) were all from Albanese origin. The results show that the three groups of participants give particular meanings to the alliance and tend to evaluate their alliance level differently. The interpreter's mediating role in the construction of the therapeutic alliance is discussed.

Cet article présente une recherche menée dans le cadre d'une consultation ambulatoire pour couples et familles. Les multiples niveaux impliqués dans une thérapie de couple ou de famille constituent une difficulté pour définir les variables pertinentes permettant d'évaluer l'efficacité d'une intervention thérapeutique systémique. Le but de notre recherche est d'évaluer l'efficacité à court et moyen terme d'une intervention systémique brève (ISB), qui consiste en un suivi thérapeutique de six séances maximum, en évaluant son impact au niveau de: (1) la symptomatologie individuelle; (2) la satisfaction conjugale; (3) la qualité des relations parentale et co-parentale et (4) le fonctionnement familial global. Les différents niveaux sont évalués par des auto-questionnaires avant et après l'ISB ainsi qu'après trois mois de catamnèse. L'alliance thérapeutique est également mesurée par auto-questionnaire (WAI) après chaque séance. Les premiers résultats sur un échantillon pilote de N = 10 couples/familles montrent, d'une part, une efficacité globale de l'ISB pour la plupart des variables mesurées, et, d'autre part, une efficacité plus grande pour les femmes que pour les hommes. Aucun effet thérapeutique n'est observé pour la symptomatologie individuelle des hommes et pour le fonctionnement familial global, suggérant que l'impact de l'ISB diffère selon les niveaux mesurés. L'importance de l'alliance thérapeutique pour la réussite thérapeutique est également confirmée.

We investigated the moderating effect of family relationships on the links between maternal postpartum depression and child symptoms in a low-risk community sample of families with 3-month-old infants (n = 57). The level of maternal depression was assessed by the Montgomery-Asberg Depression Rating Scale from a clinical interview, child symptoms by the Symptom Check List completed by both parents, and family relationships by direct observation of father-mother-baby interactions (Lausanne Trilogue Play). Families were categorized as high coordination or low coordination from their overall coordination level throughout the play. Results showed no significant links between maternal depression level and child symptoms reported by both parents. Mothers with a high depressive level in high coordination families tended to report more symptoms in their child than did mothers with lower depressive scores, whereas this link was not found in low coordination families. Prevention perspectives and clinical implications of these results are discussed.

Defense mechanisms as a central notion of psychoanalysis have inspired various levels of interest in research in psychotherapy and psychopathology. Defense specificities have only recently been investigated systematically with regard to several clinical diagnoses, such as affective and personality disorders. For the present study, 30 inpatients diagnosed with Bipolar Affective Disorder I (BD) were interviewed. An observer-rater method, the Defense Mechanisms Rating Scales (DMRS), applied to session-transcripts, of assessment of defenses was used. A matched, nonclinical control group was introduced. Defense specificities in BD encompass a set of 5 immature defenses, of which omnipotence is linked with symptom level. The level of the therapeutic alliance is predicted by mature defenses. These results are discussed with regard to the psychological vulnerability of BD, and treatment implications for psychodynamic psychotherapy with such challenging patients are evoked.

OBJECTIVE: The aim of this study was to evaluate the impact of communication skills training (CST) on working alliance and to identify specific communicational elements related to working alliance. METHODS: Pre- and post-training simulated patient interviews (6-month interval) of oncology physicians and nurses (N=56) who benefited from CST were compared to two simulated patient interviews with a 6-month interval of oncology physicians and nurses (N=57) who did not benefit from CST. The patient-clinician interaction was analyzed by means of the Roter Interaction Analysis System (RIAS). Alliance was measured by the Working Alliance Inventory - Short Revised Form. RESULTS: While working alliance did not improve with CST, generalized linear mixed effect models demonstrated that the quality of verbal communication was related to alliance. Positive talk and psychosocial counseling fostered alliance whereas negative talk, biomedical information and patient's questions diminished alliance. CONCLUSION: Patient-clinician alliance is related to specific verbal communication behaviors. PRACTICE IMPLICATIONS: Working alliance is a key element of patient-physician communication which deserves further investigation as a new marker and efficacy criterion of CST outcome.
