BACKGROUND Cancer survivors are advised to follow lifestyle recommendations on diet, physical activity, and body fatness proposed by the World Cancer Research Fund/American Institute of Cancer Research (WCRF/AICR) for cancer prevention. Previous studies have demonstrated that higher concordance with these recommendations measured using an index score (the WCRF/AICR score) was associated with lower cancer incidence and mortality. The aim of this study was to evaluate the association between pre-diagnostic concordance with WCRF/AICR recommendations and mortality in colorectal cancer (CRC) patients. METHODS The association between the WCRF/AICR score (score range 0-6 in men and 0-7 in women; higher scores indicate greater concordance) assessed on average 6.4 years before diagnosis and CRC-specific (n = 872) and overall mortality (n = 1,113) was prospectively examined among 3,292 participants diagnosed with CRC in the European Prospective Investigation into Cancer and Nutrition (EPIC) cohort (mean follow-up time after diagnosis 4.2 years). Multivariable Cox proportional hazard models were used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) for mortality. RESULTS The HRs (95% CIs) for CRC-specific mortality among participants in the second (score range in men/women: 2.25-2.75/3.25-3.75), third (3-3.75/4-4.75), and fourth (4-6/5-7) categories of the score were 0.87 (0.72-1.06), 0.74 (0.61-0.90), and 0.70 (0.56-0.89), respectively (P for trend <0.0001), compared to participants with the lowest concordance with the recommendations (category 1 of the score: 0-2/0-3). Similar HRs for overall mortality were observed (P for trend 0.004). Meeting the recommendations on body fatness and plant food consumption were associated with improved survival among CRC cases in mutually adjusted models. CONCLUSIONS Greater concordance with the WCRF/AICR recommendations on diet, physical activity, and body fatness prior to CRC diagnosis is associated with improved survival among CRC patients.

Oleoylethanolamide (OEA) is an agonist of the peroxisome proliferator-activated receptor α (PPARα) and has been described to exhibit neuroprotective properties when administered locally in animal models of several neurological disorder models, including stroke and Parkinson's disease. However, there is little information regarding the effectiveness of systemic administration of OEA on Parkinson's disease. In the present study, OEA-mediated neuroprotection has been tested on in vivo and in vitro models of 6-hydroxydopamine (6-OH-DA)-induced degeneration. The in vivo model was based on the intrastriatal infusion of the neurotoxin 6-OH-DA, which generates Parkinsonian symptoms. Rats were treated 2 h before and after the 6-OH-DA treatment with systemic OEA (0.5, 1, and 5 mg/kg). The Parkinsonian symptoms were evaluated at 1 and 4 wk after the development of lesions. The functional status of the nigrostriatal system was studied through tyrosine-hydroxylase (TH) and hemeoxygenase-1 (HO-1, oxidation marker) immunostaining as well as by monitoring the synaptophysin content. In vitro cell cultures were also treated with OEA and 6-OH-DA. As expected, our results revealed 6-OH-DA induced neurotoxicity and behavioural deficits; however, these alterations were less severe in the animals treated with the highest dose of OEA (5 mg/kg). 6-OH-DA administration significantly reduced the striatal TH-immunoreactivity (ir) density, synaptophysin expression, and the number of nigral TH-ir neurons. Moreover, 6-OH-DA enhanced striatal HO-1 content, which was blocked by OEA (5 mg/kg). In vitro, 0.5 and 1 μM of OEA exerted significant neuroprotection on cultured nigral neurons. These effects were abolished after blocking PPARα with the selective antagonist GW6471. In conclusion, systemic OEA protects the nigrostriatal circuit from 6-OH-DA-induced neurotoxicity through a PPARα-dependent mechanism.

BACKGROUND The high prevalence of women that do not reach the recommended level of physical activity is worrisome. A sedentary lifestyle has negative consequences on health status and increases health care costs. The main objective of this project is to assess the cost-effectiveness of a primary care-based exercise intervention in perimenopausal women. METHODS/DESIGN The present study is a Randomized Controlled Trial. A total of 150 eligible women will be recruited and randomly assigned to either a 16-week exercise intervention (3 sessions/week), or to usual care (control) group. The primary outcome measure is the incremental cost-effectiveness ratio. The secondary outcome measures are: i) socio-demographic and clinical information; ii) body composition; iii) dietary patterns; iv) glycaemic and lipid profile; v) physical fitness; vi) physical activity and sedentary behaviour; vii) sleep quality; viii) quality of life, mental health and positive health; ix) menopause symptoms. All outcomes will be assessed at baseline and post intervention. The data will be analysed on an intention-to-treat basis and per protocol. In addition, we will conduct a cost effectiveness analysis from a health system perspective. DISCUSSION The intervention designed is feasible and if it proves to be clinically and cost effective, it can be easily transferred to other similar contexts. Consequently, the findings of this project might help the Health Systems to identify strategies for primary prevention and health promotion as well as to reduce health care requirements and costs. A person's physical and social environment is considered as an influencing factor in terms of rates of engagement in physical activity. This study analyses the influence of socio-demographic, physical and social environmental factors on physical activity reported in the adult population in Andalusia. This is a cross-sectional study using data collected in the Andalusia Health Survey in 1999 and 2003. In addition to the influence of the individual's characteristics, if there are no green spaces in the neighbourhood it is less likely that men and women will take exercise (OR = 1.26; 95% CI = 1.13, 1.41). Likewise, a higher local illiteracy rate also has a negative influence on exercise habits in men (OR = 1.39; 95% CI = 1.21, 1.59) and in women (OR = 1.22; 95% CI = 1.07, 1.40). Physical activity is influenced by individuals' characteristics as well as by their social and physical environment, the most disadvantaged groups are less likely to engage in physical activity.

O presente estudo relacionado ao processo ensino-aprendizagem da técnica de palpação e mensuração uterina, teve como objetivos, analisar a influência da prática em relação à habilidade e analisar o conhecimento adquirido da técnica dos estudantes de Graduação em Enfermagem. Aplicando o "modelo padrão" elaborado para este estudo, os estudantes foram avaliados em 2 etapas; os resultados obtidos permitiram concluir pela importância da prática no processo ensino-aprendizagem de uma habilidade perceptivo-motora, verificada através da melhora do desempenho dos estudantes em relação à habilidade e ao conhecimento adquirido.

Diante da vivência em tratar de indivíduos que apresentam lesão medular, nos deparamos com a problemática envolvida no cotidiano dessas pessoas após uma situação de trauma súbito e inesperado. Com a intenção de prestar cuidados efetivos a estas pessoas nos preocupamos, neste estudo, em identificar a situação pela qual elas estão passando, se houve mudanças e quais foram elas, tanto em nível psíquico como físico e social. Para tanto, optou-se pela realização de urna entrevista individual semi-estruturada, utilizando-se o recurso da gravação, o instrumento de coleta de dados consta de dados de identificação e dados a respeito da vida do entrevistado. O discurso livre com gravação e posterior transcrição caracteriza um estudo de caso de natureza, sobretudo, exploratória. Os dados obtidos receberam uma análise temática utilizando concepções sobre qualidade de vida segundo FORATTINI (1991). Neste estudo foram identificadas uma deficiência física e uma série de incapacidades. A deambulação é a primeira perda notada pelo paciente após a lesão e a sua primeira questão ao profissional mais próximo dele, é que se estabeleça um canal de comunicação que permita esta expressão. Mas devemos lembrar do processo possível de reabilitação que não aborda somente a busca pela locomoção, permeando o reaprendizado nesta nova situação de vida. Constatamos modificações em todas as esferas (física, social, psicológica), sendo que as mais afetadas, segundo os dados colhidos, foram a psicológica e a social devido às alterações físicas. A principal sensação de perda refere-se ao papel social, sendo que esta é caracterizada, principalmente, pela impossibilidade de retornar ao trabalho. As mudanças nas concepções de auto-imagem e auto-estima também foram identificadas, em menor grau, pela sensação de incapacidade. A perda física mais importante e compreensivelmente mais relatada refere-se a perda de sensibilidade e capacidade motora dos membros.

O traumatismo raquimedular é uma agressão à medula espinhal que pode ocasionar danos neurológicos, tais como alteração das funções motora, sensitiva e autônoma. Este artigo tem como propósito relatar detalhadamente as principais complicações clínicas resultantes desse tipo de lesão, e apresentar as intervenções assistenciais de enfermagem que possam auxiliar na promoção do bem estar e na melhoria da qualidade de vida dos pacientes, seja em caráter de acometimento já instalado ou profilático.
