Escherichia coli variants in periprosthetic joint infection: diagnostic challenges with sessile bacteria and sonication.

The diagnostic yield of prosthetic joint-associated infection is hampered by the phenotypic change of bacteria into a sessile and resistant form, also called biofilm. With sonication, adherent bacteria can be dislodged from the prosthesis. Species identification may be difficult because of their variations in phenotypic appearance and biochemical reaction. We have studied the phenotypic, genotypic, and biochemical properties of Escherichia coli variants isolated from a periprosthetic joint infection. The strains were collected from synovial fluid, periprosthetic tissue, and fluid from the explanted and sonicated prosthesis. Isolates from synovial fluid revealed a normal phenotype, whereas a few variants from periprosthetic tissue and all isolates from sonication fluid showed different morphological features (including small-colony variants). All isolates from sonication fluid were beta-galactosidase negative and nonmotile; most were indole negative. Because of further variations in biochemical properties, species identification was false or not possible in 50% of the isolates included in this study. In contrast to normal phenotypes, variants were resistant to aminoglycosides. Typing of the isolates using pulsed-field gel electrophoresis yielded nonidentical banding patterns, but all strains were assigned to the same clonal origin when compared with 207 unrelated E. coli isolates. The bacteria were repeatedly passaged on culture media and reanalyzed. Thereafter, most variants reverted to normal phenotype and regained their motility and certain biochemical properties. In addition, some variants displayed aminoglycoside susceptibility after reversion. Sonication of an explanted prosthesis allows insight into the lifestyle of bacteria in biofilms. Since sonication fluid also reveals dislodged sessile forms, species identification of such variants may be misleading.

Cet ouvrage offre un regard inédit en se fondant sur des sources jusqu'alors inexplorées provenant de la justice militaire. Le Code pénal militaire punit en effet les relations sexuelles entre hommes depuis 1928, et la mobilisation générale amène à un paroxysme permettant de dévoiler un ordinaire habituellement masqué. Quelque 120 affaires sont instruites par la justice militaire avec le concours des polices militaires et civiles, des autorités communales et, parfois, du Service sanitaire de l'armée ou de médecins civils. Ces affaires permettent de mettre en évidence, au-delà de l'interdiction de l'homosexualité au sein de la troupe, une marge de liberté dans les grandes villes, mais aussi la surveillance policière et un opprobre social tenace.¦La relative tolérance envers l'homosexualité s'inscrit comme un compromis entre un progressisme et un conservatisme politique, juridique, voire médical, qui diffère selon les régions du pays ou l'origine sociale des prévenus.

Yingti/ruanti (hardware/software). La création d'un hall culturel hakka à Taiwan

Dans le cadre de la politique multiculturelle de protection des minorités menée par le gouvernement taïwanais, le Comité ministériel aux affaires hakka (keweihui) a planifié dix-huit « halls culturels hakkas » dans chaque comté et municipalité. L'article retrace le processus de création d'un de ces halls, dans le nord de l'île de Taiwan, au cours de la décennie 2000. L'aménagement du hall procède à l'incorporation d'artistes locaux dans une tradition nationale hakka réinventée. Il est montré comment sa genèse s'est produite à l'intersection d'une politique impulsée par « le haut » et d'un projet émanant d'acteurs locaux. L'article met en évidence comment les catégories yingti (hardware) et ruanti (software) président à sa fabrication et impliquent une opposition et une complémentarité entre « contenant » et « contenu », entre « État » et « société », qui agissent à toutes les étapes de sa mise en oeuvre.

Luc, pionnier de l'historiographie chrétienne

Qui a écrit la première histoire du christianisme ? La recherche historienne moderne a rarement hésité à pointer le doigt vers Eusèbe de Césarée, ainsi que le rappellent dans ce dossier les contributions de M. Fédou et de M. Heinzelmann. Une telle reconnaissance devait amener une dégradation de la qualité historienne de l'oeuvre lucanienne, évangile et Actes des Apôtres, reconnue jusqu'au XVIIIe siècle. Ainsi, jusque dans les années 60 du XX' siècle, la recherche sur cette oeuvre devait être dominée par cette tonalité lourde. Une révision de ces attitudes, dont certaines se sont manifestées dès le début de ce même siècle, s'est depuis engagée, grâce notamment à une meilleure connaissance et acceptation des caractéristiques de l'historiographie ancienne. C'est donc un véritable historien qui se révèle désormais dans l'oeuvre lucanienne, légitimant du même coup le concept d'histoire théologique, et rendant à Luc sa place, la première, bien avant Eusèbe de Césarée.

Need of reduced and harmonized bureaucracy in multi-centre clinical research - a case-report from a Swiss trial

Introduction We launched an investigator-initiated study(ISRCTN31181395) to evaluate the potential benefit of pharmacokinetic-guided dosage individualization of imatinib for leukaemia patients followed in public and private sectors. Following approval by the research ethics committee (REC) of the coordinating centre, recruitment throughout Switzerland necessitated to submit the protocol to 11 cantonal RECs.Materials and Methods We analysed requirements and evaluation procedures of the 12 RECs with associated costs.Results 1-18 copies of the dossier, in total 4300 printed pages, were required (printing/posting costs: ~300 CHF) to meet initial requirements. Meeting frequencies of RECs ranged between 2 weeks and 2 months, time from submission to first feedback took 2-75 days. Study approval was obtained from a chairman, a subor the full committee, the evaluation work being invoiced by 0-1000 CHF (median: 750 CHF, total: 9200 CHF). While 5 RECs gave immediate approval, the other 6 rose in total 38 queries before study release, mainly related to wording in the patient information, leading to 7 different final versions approved. Submission tasks employed an investigator half-time over about 6 months.Conclusion While the necessity of clinical research evaluation by independent RECs is undisputed, there is a need of further harmonization and cooperation in evaluation procedures. Current administrative burden is indeed complex, time-consuming and costly. A harmonized electronic application form, preferably compatible with other regulatory bodies and European countries, could increase transparency, improve communication, and encourage academic multi-centre clinical research in Switzerland.

N-Terminal Modifications Improve the Receptor Affinity and Pharmacokinetics of Radiolabeled Peptidic Gastrin-Releasing Peptide Receptor Antagonists: Examples of 68Ga- and 64Cu-Labeled Peptides for PET Imaging.

Gastrin-releasing peptide receptors (GRPrs) are overexpressed on a variety of human cancers, providing the opportunity for peptide receptor targeting via radiolabeled bombesin-based peptides. As part of our ongoing investigations into the development of improved GRPr antagonists, this study aimed at verifying whether and how N-terminal modulations improve the affinity and pharmacokinetics of radiolabeled GRPr antagonists. METHODS: The potent GRPr antagonist MJ9, Pip-d-Phe-Gln-Trp-Ala-Val-Gly-His-Sta-Leu-NH2 (Pip, 4-amino-1-carboxymethyl-piperidine), was conjugated to 1,4,7-triazacyclononane, 1-glutaric acid-4,7 acetic acid (NODAGA), and 1,4,7-triazacyclononane-1,4,7-triacetic acid (NOTA) and radiolabeled with (68)Ga and (64)Cu. The GRPr affinity of the corresponding metalloconjugates was determined using (125)I-Tyr(4)-BN as a radioligand. The labeling efficiency of (68)Ga(3+) was compared between NODAGA-MJ9 and NOTA-MJ9 in acetate buffer, at room temperature and at 95°C. The (68)Ga and (64)Cu conjugates were further evaluated in vivo in PC3 tumor xenografts by biodistribution and PET imaging studies. RESULTS: The half maximum inhibitory concentrations of all the metalloconjugates are in the high picomolar-low nanomolar range, and these are the most affine-radiolabeled GRPr antagonists we have studied so far in our laboratory. NODAGA-MJ9 incorporates (68)Ga(3+) nearly quantitatively (>98%) at room temperature within 10 min and at much lower peptide concentrations (1.4 × 10(-6) M) than NOTA-MJ9, for which the labeling yield was approximately 45% under the same conditions and increased to 75% at 95°C for 5 min. Biodistribution studies showed high and specific tumor uptake, with a maximum of 23.3 ± 2.0 percentage injected activity per gram of tissue (%IA/g) for (68)Ga-NOTA-MJ9 and 16.7 ± 2.0 %IA/g for (68)Ga-NODAGA-MJ9 at 1 h after injection. The acquisition of PET images with the (64)Cu-MJ9 conjugates at later time points clearly showed the efficient clearance of the accumulated activity from the background already at 4 h after injection, whereas tumor uptake still remained high. The high pancreas uptake for all radiotracers at 1 h after injection was rapidly washed out, resulting in an increased tumor-to-pancreas ratio at later time points. CONCLUSION: We have developed 2 GRPr antagonistic radioligands, which are improved in terms of binding affinity and overall biodistribution profile. Their promising in vivo pharmacokinetic performance may contribute to the improvement of the diagnostic imaging of tumors overexpressing GRPr.
