Estudo exploratório, descritivo e quantitativo que busca identificar e descrever as atividades do familiar que cuida de pessoas incapacitadas por AVC no domicílio. A amostra constituiu-se de 35 cuidadores familiares, predominantemente mulheres, esposas ou filhas. Os cuidados realizados relacionam-se ao grau de incapacidade do familiar. As orientações recebidas denotam falta de compromisso dos profissionais com a continuidade do cuidado. O cuidador aprende a cuidar no cotidiano, com base na observação e auxílio à enfermagem na internação. As dificuldades pautam-se no esforço físico, na desinformação e no medo, no constrangimento e na vergonha em lidar com o corpo do outro. O estudo enfoca a necessidade do preparo para a alta hospitalar, salienta a família como espaço concreto para o cuidado; o aumento da expectativa de vida é uma constatação.

BACKGROUND: Control of hemorrhage in patients with active bleeding from rupture of the aortic arch is difficult, because of the location of the bleeding and the impossibility of cross-clamping the aorta without interfering with cerebral perfusion. A precise and swift plan of management helped us salvage some patients and prompted us to review our experience. METHODS: Six patients with active bleeding of the aortic arch in the mediastinum and pericardial cavity (5 patients) or left pleural cavity (1 patient), treated between 1992 and 1996, were reviewed. Bleeding was reduced by keeping the mediastinum under local tension (3 patients) or by applying compression on the bleeding site (2 patients), or both (1 patient) while circulatory support, retransfusion of aspirated blood, and hypothermia were established. The diseased aortic arch was replaced during deep hypothermic circulatory arrest, which ranged from 25 to 40 minutes. In 3 patients, the brain was further protected by retrograde (2 patients) or antegrade (1 patient) cerebral perfusion. RESULTS: Hemorrhage from the aortic arch was controlled in all patients. Two patients died postoperatively, one of respiratory failure and the other of abdominal sepsis. Recovery of neurologic function was assessed and complete in all patients. The 4 survivors are well 8 to 49 months after operation. CONCLUSIONS: An approach relying on local tamponade to reduce bleeding, rapid establishment of circulatory support and hypothermia, retransfusion of aspirated blood, and swift repair of the aortic arch under circulatory arrest allows salvage of patients with active bleeding from an aortic arch rupture.

BACKGROUND: Developmental venous anomaly (DVA) is considered a benign and asymptomatic cerebrovascular malformation, and only isolated cases of symptomatic DVAs are described in the literature. Even more rarely will these symptoms come from an intraparenchymal hemorrhage caused by the DVA. METHODS AND RESULTS: We present two symptomatic DVAs by intracerebellar hemorrhage. CONCLUSION: The diagnostic steps regarding the pathophysiology of DVA hemorrhage and the treatment options are discussed.

In spite of the fact that the Ars predicandi populo is attributed to Eiximenis in only one of the three extant manuscripts, there is no doubt that it is one of his works.First of all, the internal references to this treatise in the pages of Terç and Dotzè del Crestià are an irrefutable testimony and, secondly, the presence of an autographvolume of sermons which begins with the Ars in the inventory of the Eiximenis library made shortly after his death. The writing of the Ars and the collection of sermons which introduces it must be dated prior to the beginning of the Crestià encyclopedia (1379) because of the complexity that entailed the writing of a collection of three perhaps four volumes of sermons. Due to a series of formal and codicological reasons it must be concluded that the chapter entitled De consiliis circa predicacionem cannot be attributed to Eiximenis and thus be excluded from the Ars

A study of the spermiogenesis and spermatozoa of Helicolenus dactylopterus was conducted. Females of this species have the capacity to store sperm within their ovaries, and male gametes have a considerable cytoplasmic mass surrounding their heads to survive the long period of intraovarian sperm storage. Our observations show that early spermatids are round-shaped cells and have a spherical nucleus with diffuse chromatin. The nuclear volume decreases as a result of progressive chromatin condensation during spermiogenesis, causing the nucleus to take on a U-shape. Flagellar insertion is not central to the nucleus but consistently occurs at an oblique angle towards one side of it. The flagellum is inserted into the nuclear fossa, without subsequent nuclear rotation. In mature spermatozoa, the flagellum is adjacent to the nucleus. A comparison of the spermatozoa in the testicular lobules and those in the intraovarian storage structures suggests that the increase in volume of the cytoplasmic mass may occur in the posterior region of the testis, in the testicular duct. Spermatozoa enter the ovary in groups that reach the ovarian lumen and are surrounded by the ovarian epithelium for storage in sperm storage crypts

In 1345, the unique worship of Saint Charlemagne was institutionalized in the cathedral of Gerona. The roots (and for many, the reasons) of this celebration are found in the existence of an old local legend, the well-known testimonies to which go back to the eleventh century. Nevertheless, a lengthy analysis of the facts that marked the life of the cathedral during the long permanence Amau de Montrodon had there—first in quality of canon (1297-1335) and later as bishop (1333-1348)- make it possible for us to verify that the promotion of the cult to San Charlemagne falls within a complex program of ideological and symbolic exaltation of the See of Girona that was designed by this notable and restlessly active ecclesiastic

Las características sensoriales, y las propiedades nutricionales del aceite de oliva virgen han incrementado la demanda de aceite de oliva a nivel mundial. Recientemente, las investigaciones sobre la biodiversidad oleícola y la composición química del aceite de oliva se han multiplicado, concediendo un gran interés a la calidad de su perfil organoléptico. En el presente estudio se ha evaluado el perfil de compuestos volátiles y en su caso de compuestos fenólicos y pigmentos mediante técnicas convencionales de GC-MS y LC-MS, en relación con el perfil sensorial y en función de distintos factores tecnológicos y ambientales, tales como las condiciones de extracción, el perfil microbiológico de la materia prima, operaciones pre-proceso y la zona de implantación. Se ha estudiado el comportamiento de los numerosos analitos con impacto sensorial, en función de estos factores, permitiendo identificar posibles marcadores químicos de aceites con distintas características. Además, tal y como ocurre con otros alimentos, el comercio del aceite de oliva se ha orientado a favor de las producciones que se caracterizan por sus orígenes y sus cualidades. La espectrometría de masas de alta resolución se ha aplicado para la determinación directa del fingerprint glicerolipídico de los aceites de oliva, llevando a cabo un análisis a nivel molecular sin necesidad de fraccionamiento previo y lográndose elevadas sensibilidad, precisión, tiempos cortos de análisis y mínima manipulación de la muestra. Los resultados obtenidos demuestran las potencialidades de la espectrometría de masas de alta resolución para la caracterización y autentificación del aceite de oliva virgen.

La creciente adaptación de los medios de comunicación al entorno digital hace conveniente la utilización de metodologías de análisis que permitan determinar la calidad de sus sitios web. En este artículo, a partir del estudio de las características de las webs de algunos de los medios de comunicación más importantes a nivel internacional, se propone una metodología para la evaluación específica de este tipo de sitios web. El modelo propuesto se compone de treinta y seis indicadores, organizados en torno a ocho parámetros que permiten valorar, especialmente, la adaptación del medio de comunicación a la interacción de la Web 2.0. Otros aspectos a los que también se presta atención, pero en menor medida, son la arquitectura de la información, la usabilidad, la accesibilidad o las herramientas de interacción y comunicación que éste pone a disposición de sus usuarios.

RÉSUMÉ Introduction. Les hémopathies représentent une cause rare d'accident vasculaire cérébral (AVC), faisant l'objet de peu de publications, mais sont très fréquemment recherchées après un AVC par de coûteux bilans dont la rentabilité reste à définir. Matériel et Méthodes. Dans le registre lausannois des AVC, nous nous sommes intéressés de façon rétrospective aux dossiers des patients hospitalisés entre 1979 et 2001 pour un premier AVC ischémique artériel, dont la cause a été attribuée à une pathologie hématologique. Sur 4 697 patients, 22 (0,47 p. 100) ont vu leur AVC imputé à l'une des causes hématologiques suivantes : maladie de Vaquez , polyglobulie secondaire , thrombocytémie essentielle , thrombocytose secondaire , myélome multiple , MD , déficit en protéine S , syndrome anticorps antiphospholipides , hyperhomocystéinémie . Chaque hémopathie retrouvée a donné lieu à une revue de la littérature. Conclusion. À la lumière de ces données, nous concluons qu'une formule sanguine représente un bilan hématologique de dépistage suffisant pour l'immense majorité des patients hospitalisés pour un premier AVC artériel ischémique. Les recherches d'anticorps antiphospholipides, de thrombophilies héréditaires et d'hyperhomocystéinémie peuvent être limitées à des cas sélectionnés. SUMMARY Cerebral infarction of arterial origin and haematological causation: the Lausanne experience and a review of the literature. Introduction. Hematological diseases are seldom found as the etiology of ischemic strokes, but are frequently investigated by expensive laboratory tests after a first cerebral vascular event. Methods. In the Lausanne Stroke Registry, we retrospectively reviewed the cases of patients hospitalized between 1979 and 2001 for a first ischemic arterial stroke which was attributed to a hematological etiology. Of 4697 patients, 22 (0.47 per cent) had a stroke due to one of the following hematological pathology: polycythemia vera, secondary polycythemia, essential thrombocytemia, secondary thrombocytosis, multiple myeloma, CIVD, protein S deficiency, antiphospholipid antibody syndrome, moderate homocysteinemia. A literature review was undertaken for each hemopathy. Conclusion. In light of the results of these data, we concluded that a complete blood count provides sufficient hematological screening for the majority of patients hospitalized for an arterial stroke. The antiphospholipid antibody syndrome is a rare cause of cerebral infarction, which needs to be investigated in young patients, in cases of multiple or recurring stroke or in the presence of a typical history. Inherited thrombophilias are not a significant risk factor for arterial cerebral infarction and their investigation is only warranted for a sub-group of young patients with a cryptogenic stroke, in which group the prevalence is slightly increased. Moderate homocysteinemia must be considered as a cerebrovascular risk factor of minor importance, but potentially treatable by a substitution of vitamin B12, B 6 and folates. The efficacy of this substitution in the prevention of cardiovascular events needs yet to be demonstrated.
