OBJETIVO: estudar os efeitos de técnicas fisioterápicas sobre os desconfortos músculo-esqueléticos na gestação. MÉTODOS: estudo coorte prospectivo, com 71 gestantes nulíparas, de baixo risco, distribuídas conforme a participação (estudo; n=38) ou não (controle; n=33). O Programa Multidisciplinar de Preparo para o Parto e Maternidade constou de 10 encontros (da 18ª à 38ª semana), com atividades educativas, fisioterápicas e de interação. Compararam-se, por questionário específico, no início e final do programa, a ocorrência (presença ou ausência), as características (local, tipo, intensidade, freqüência e duração) e a evolução dos desconfortos músculo-esqueléticos. As médias aferidas na avaliação inicial foram comparadas pela análise de variância (ANOVA) seguida pelo teste do F. Para o estudo entre as proporções, verificadas no início e no final do programa, utilizou-se o teste do c². A significância estatística foi definida pelo limite de 5% (p<0,05). RESULTADOS: no início do programa, 63,6% das gestantes do grupo controle e 84,2% do Estudo relataram sintomas músculo-esqueléticos (p=0,05), caracterizados por dor na região lombossacra. No grupo controle predominaram a intensidade leve (18,2%) e grave (18,4%), e no grupo estudo, a grave (36,8%) e isolada ou associada (31,6%). No final, o grupo controle tinha sintomas de intensidade grave (60,6%), com freqüência diária (42,4%) e duração maior que três horas (69,7%) (p<0,05). O grupo estudo referia intensidade leve (57,9%) e freqüência quinzenal (50,0%) com duração máxima de uma hora (55,3%) (p<0,05). A evolução dos sintomas foi diferenciada, confirmando-se piora em 63,6% das gestantes do Controle e melhora em 65,8% das participantes do Programa (p<0,05). CONCLUSÕES: as técnicas fisioterápicas do Programa Multidisciplinar de Preparo para o Parto e Maternidade se relacionaram à diminuição da intensidade, freqüência e duração e à melhor evolução dos desconfortos músculo-esqueléticos na gestação.

Edema de Reinke é doença crônica da laringe na qual a camada superficial da lâmina própria é expandida por muco espesso conferindo-lhe aspecto gelatinoso. Relaciona-se ao tabagismo e acomete, preferencialmente mulheres, as quais apresentam a voz mais grave. Suas características histológicas nem sempre conseguem diferenciá-lo das demais lesões benignas da laringe, havendo necessidade de técnicas histológicas adicionais. OBJETIVOS: Estudar a imunoexpressão da fibronectina, do colágeno IV e da laminina no edema de Reinke por meio de técnicas imunoistoquímicas. Estudo prospectivo. MATERIAL E MÉTODOS: Blocos histológicos de 60 casos cirúrgicos de edema de Reinke foram resgatados, submetidos a novos cortes e às reações imunoistoquímicas para fibronectina, laminina e colágeno IV pelo método da Avidina Biotina Peroxidase. Todos os pacientes eram fumantes e adultos, sendo 50 mulheres e 10 homens. RESULTADOS: As análises da imunoexpressão da fibronectina, do colágeno IV e da laminina foram mais expressivas no endotélio dos vasos (68,33%, 76,66%, 73,33%, respectivamente), e menos relevantes na membrana basal (25,0%, 5,0% e 3,3%, respectivamente). CONCLUSÕES: No edema de Reinke, a imunoexpressão da fibronectina, da laminina e do colágeno IV na membrana basal não apresentam relevância, havendo predomínio desses anticorpos no endotélio do vasos.

Foram avaliadas as técnicas radiográficas dentárias intra (TIB) e extrabucal (TEB) em 50 cães com doença periodontal, no intuito de padronizar os procedimentos de diagnóstico dessa síndrome. A TIB revelou que 16 animais não apresentaram lesões ósseas visíveis, enquanto a TEB apontou que 39 pacientes foram negativos para as mesmas lesões. em resumo, a TIB foi mais eficaz na detecção de sinais radiográficos, especialmente as chamadas lesões finas, que a TEB, sendo a técnica de escolha na síndrome periodontal.

Foram realizados quatro estudos de simulação para verificar a distribuição de inversas de variáveis com distribuição normal, em função de diferentes variâncias, médias, pontos de truncamentos e tamanhos amostrais. As variáveis simuladas foram GMD, com distribuição normal, representando o ganho médio diário e DIAS, obtido a partir da inversa de GMD, representando dias para se obter determinado peso. em todos os estudos, foi utilizado o sistema SAS® (1990) para simulação dos dados e para posterior análise dos resultados. As médias amostrais de DIAS foram dependentes dos desvios-padrão utilizados na simulação. As análises de regressão mostraram redução da média e do desvio-padrão de DIAS em função do aumento na média de GMD. A inclusão de um ponto de truncamento entre 10 e 25% do valor da média de GMD reduziu a média de GMD e aumentou a de DIAS, quando o coeficiente de variação de GMD foi superior a 25%. O efeito do tamanho dos grupos nas médias de GMD e DIAS não foi significativo, mas o desvio-padrão e CV amostrais médios de GMD aumentaram com o tamanho do grupo. em virtude da dependência entre a média e o desvio-padrão e da variação observada nos desvios-padrão de DIAS em função do tamanho do grupo, a utilização de DIAS como critério de seleção pode diminuir a acurácia da variação. Portanto, para a substituição de GMD por DIAS, é necessária a utilização de um método de análise robusto o suficiente para a eliminação da heterogeneidade de variância.

Background: The purpose of this experimental study was to evaluate the collagen fiber distribution histologically after phenytoin, cyclosporin, or nifedipine therapy and to correlate it with collagen I and matrix metalloproteinase (MMP)-1 and -2 gene expression levels.Methods: Gingival samples from the canine area were obtained from 12 male monkeys (Cebus apella). The mesial part of each sample was assessed by reverse transcription-polymerase chain reaction, whereas the distal part was processed histologically for picrosirius red and hematoxylin and eosin stainings, as well as for collagen IV immunostaining. One week after the first biopsy, the animals were assigned to three groups that received daily oral dosages of cyclosporin, phenytoin, or nifedipine for 120 days. Additional gingival samples were obtained on days 52 and 120 of treatment from two animals from each group on the opposite sides from the first biopsies.Results: Picrosirius red staining showed a predominance of mature collagen fibers in the control group. Conversely, there was an enlargement of areas occupied by immature collagen fibers in all groups at days 52 and 120, which was not uniform over each section. There was a general trend to lower levels of MMP-1 gene expression on day 52 and increased levels on day 120. Phenytoin led to increased levels of MMP-2 and collagen I gene expression on day 120, whereas the opposite was observed in the nifedipine group.Conclusion: Cyclosporin, phenytoin, and nifedipine led to phased and drug-related gene expression patterns, resulting in impaired collagen metabolism, despite the lack of prominent clinical signs.

Background: Very limited information is available from in vivo studies about whether smoking and/or nicotine affect gingival tissues in the absence of plaque. The purpose of this study is to evaluate the effect of the systemic administration of nicotine in the proliferation and counting of fibroblast-like cells in the gingival tissue of rats.Methods: Thirty adult male Wistar rats were randomly assigned into two groups to receive subcutaneous injections of a saline solution (control group = group C) or nicotine solution (group N; 3 mg/kg) twice a day. The animals were euthanized 37, 44, or 51 days after the first subcutaneous injection. Specimens were routinely processed for serial histologic sections. Five fields of view in the connective tissue adjacent to the gingival epithelium and above the alveolar bone crest of the maxillary first molar were selected for the counting of fibroblast-like cells. Data were statistically analyzed (P<0.05).Results: The intergroup analysis detected a lower number of fibroblast-like cells in group N compared to group C on days 37 (2.65 +/- 1.41 and 6.67 +/- 3.25, respectively), 44 (2.70 +/- 1.84 and 8.57 +/- 2.37, respectively), and 51(2.09 +/- 1.41 and 7.49 +/- 2.60, respectively) (P<0.05). The quantification of fibroblast-like cells showed no significant difference (P >0.05) in the intragroup analysis of control and nicotine throughout experimental periods. In the intergroup analysis, group N had reduced proliferating cell nuclear antigen positive fibroblasts compared to group C in all periods (P<0.05).Conclusion: The daily systemic administration of nicotine negatively affected, in vivo, the number and proliferation of fibroblast-like cells in the gingival tissue of rats. Gingival overgrowth (GO) may be related to the frequent use of certain medications, such as cyclosporin, phenytoin (PHT), and nifedipine, and is therefore denominated drug-induced GO. This article reports a case of a patient who with chronic periodontitis made use of PHT and presented generalized GO. A 30-year-old man with GO was referred to the clinic of the Universidade Estadual Paulista, Brazil. The complaint was poor aesthetics because of the GO. The patient had a medical history of a controlled epileptic state, and PHT was administered as an anticonvulsant medication. The clinical examination showed generalized edematous gingival tissues and presence of bacterial plaque and calculus on the surfaces of the teeth. The diagnosis was GO associated with PHT because no other risk factors were identified. Treatment consisted of meticulous oral hygiene instruction, scaling, root surface instrumentation, prophylaxis, and daily chlorhexidine mouth rinses. After this stage, periodontal surgery was performed, and histopathologic evaluation was made. The patient has been under control for 3 years after the periodontal surgery, and up to the present time, there has been no recurrence. It can be concluded that PHT associated with the presence of irritants favored gingival growth and that the association of nonsurgical and surgical periodontal therapies was effective in the treatment of GO. Besides, motivating the patient to maintain oral hygiene is a prerequisite for the maintenance of periodontal health.

Background: the incidence of gingival overgrowth (GO) associated with the use of cyclosporin A (CsA) is controversial. In the present study, we determined the incidence of GO in Brazilian renal transplant patients treated with CsA and the possible associations between periodontal and pharmacological variables.Methods: the test group consisted of 20 renal transplant patients, and the control group included 20 non-transplant patients. Periodontal conditions were evaluated based on the plaque index (PI), gingival index (GI), probing depth (PD), and the rate of gingival overgrowth, together with pharmacological variables (daily CsA dose and duration of treatment).Results: A significant difference in PI (P<0.0001) and PD (P<0.0001) was observed between groups, while GI (P=0.15) did not differ significantly. Using the Pearson correlation coefficient, a significant correlation was observed not only between GI (P<0.001; r=0.8141) and GO, but also for PD (P<0.001; r=0.866) and GO. The other correlations were not statistically significant.Conclusions: We conclude that GO induced by CsA may vary according to the individual sensitivity of each patient and may or may not be correlated with other local factors (periodontal variables).

Desquamative gingivitis (DG) is a fairly common disorder in which the gingivae show chronic desquamation. Originally considered to be related to hormonal changes at menopause, since many of the patients are middle-aged women, DG is now recognized to be mainly a manifestation of a number of disorders ranging from vesiculobullous diseases to adverse reactions to a variety of chemicals or allergens. Desquamative gingivitis can be an important early clinical manifestation of serious systemic diseases such as pemphigus vulgaris. The authors present a case that illustrates the importance of a specific diagnosis in patients with desquamative gingival lesions previously treated for 6 months as classical gingivitis. Gingival biopsy showed histologic patterns typical of pemphigus vulgaris. The patient was treated with systemic and topical corticosteroids in association with miconazole the patient is now under control with low-close systemic corticosteroids. Proper recognition of lesions in the oral mucosa leads, in several situations, to an early diagnosis of a systemic disease.

Gingival recession was created in six mongrel dogs. The dogs were divided into two groups based on treatment: group 1-AlloDerm only, group 2-AlloDerm + Emdogain. The histologic results were compared. At the end of the study, the mean values were, for groups I and 2, respectively: 0.06 and 0.32 mm for cementum regeneration; -0.75 and -0.86 mm for bone regeneration; -2.15 and -3.11 mm for attachment level; and 4.90 and 5.51 mm for defect extent. The epithelial formation parameter was 2.88 mm in group 1 and 2.15 mm in group 2, which was a statistically significant difference. It could be concluded that Emdogain did not result in beneficial effects when associated with AlloDerm.

Background: Gingival fibromatosis is a rare condition characterized by a generalized enlargement of the buccal and lingual aspects of the attached and marginal gingiva.Methods: This case report describes the periodontal management of a 13-year-old female patient with gingival fibromatosis associated with Zimmermann-Laband syndrome. The patient presented with gingival enlargement involving the maxillary and the mandibular arches, anterior open bite, and non-erupted teeth. Periodontal treatment included gingivectomy in all four quadrants.Results: Histopathologic evaluation of the excised tissue supported the diagnosis of gingival flbromatosis. A significant improvement in esthetic appearance and eruption of the non-erupted teeth were obtained. The patient was referred for appropriate orthodontic treatment and has been closely followed for the earliest signs of recurrence of gingival enlargement.Conclusions: the successful therapy for gingival fibromatosis depends on correctly identifying the etiological factors and improving the impaired function and esthetic appearance through surgical intervention and adjunctive orthodontics. Maintaining treatment results depends on preservation of periodontal health.
