INTRODUCTION: Even before the 2009 pandemics, influenza in healthcare workers (HCW) was a known threat to patient safety, while Influenza vaccine coverage in the same group was generally low. Identification of predictors for HCW adherence to Influenza vaccination has challenged infection control committees. METHODS: Our group conducted a cross-sectional survey in December 2007, interviewing 125 HCWs from a teaching hospital to identify adherence predictors for Influenza vaccination. The outcomes of interest were: A - adherence to the 2007 vaccination campaign; B - adherence to at least three yearly campaigns in the past five years. Demographic and professional data were assessed through univariate and multivariate analysis. RESULTS: Of the HCWs interviewed, 43.2% were vaccinated against Influenza in 2007. However, only 34.3% of HCWs working in healthcare for more than five years had adhered to at least three of the last five vaccination campaigns. Multivariate analysis showed that working in a pediatric unit (OR = 7.35, 95%CI = 1.90-28.44, p = 0.004) and number of years in the job (OR = 1.32, 95%CI = 1.00-1.74, p = 0.049) were significant predictors of adherence to the 2007 campaign. Physicians returned the worst outcome performances in A (OR = 0.40, 95%CI = 0.16-0.97, p = 0.04) and B (OR = 0.17, 95%CI = 0.05-0.60, p = 0.006). CONCLUSIONS: Strategies to improve adherence to Influenza vaccination should focus on physicians and newly-recruited HCWs. New studies are required to assess the impact of the recent Influenza A pandemics on HCW-directed immunization policies.

INTRODUCTION: Multidrug-resistant Pseudomonas aeruginosa is a major threat in healthcare settings. The use of antimicrobials can influence the incidence of resistant strains by direct and indirect mechanisms. The latter can be addressed by ecological studies. METHODS: Our group attempted to analyze the relation between the use of antipseudomonal drugs and the incidence of MDR-PA among 18 units from a 400-bed teaching hospital. The study had a retrospective, ecological design, comprising data from 2004 and 2005. Data on the use of four antimicrobials (amikacin, ciprofloxacin, ceftazidime and imipenem) were tested for correlation with the incidence of MDR-PA (defined as isolates resistant to the four antimicrobials of interest) in clinical cultures. Univariate and multivariate linear regression analyses were performed. RESULTS: Significant correlations were determined between use and resistance for all antimicrobials in the univariate analysis: amikacin (standardized correlation coefficient = 0.73, p = 0.001); ciprofloxacin (0.71, p = 0.001); ceftazidime (0.61, p = 0.007) and imipenem (0.87, p < 0.001). In multivariate analysis, only imipenem (0.67, p = 0.01) was independently related to the incidence of multidrug-resistant strains. CONCLUSIONS: These findings share similarities with those reported in individual-based observational studies, with possible implications for infection control.

Domestication of plants and plant breeding have dramatically eroded the allelic variations of crop species which led to an increasing susceptibility of crop plants to environmental stresses, diseases and pests. Drought is a major environmental stress factor that affects the growth and development of plants so the selection of tolerant genotypes becomes increasingly important with respect to the predicted effects of global warming. In this study, several genotypes of Spelt (Triticum aestivum var. spelta) were tested under low water supply in soil with the aim of to find Spelt genotypes more resistant than wheat to these conditions, and select them so that in future may be used to improve wheat crops. Morphological analyses were performed and mineral and enzymatic analyses and also dry matter production were calculated. Our results suggests that the genotypes Sp53, Sp96, Sp912, Sp757 and Sp804 are a potential ones to use in breeding programs to improve wheat production. Under drought, these genotypes had growth efficiency of 38%, 45%, 64%, 37%, and 31% respectively and also showed higher biomass than modern wheat and were also mineralogical richer. The genotypes Sp96 and Sp912 showed highest activity of all antioxidants enzymes tested. This work proves that Spelt is a good wheat to continue to study in order to improve wheat crops in dry areas and consequently increase the quality of life and health of the populations living in those areas.

INTRODUCTION: Epidemiological studies concerning HCV genotypic distribution in the Brazilian Amazon are scarce. Thus, this study determined the patterns of distribution of HCV genotypes among different exposure categories in the State of Pará, Brazilian Amazon. METHODS: A cross-sectional study was conducted on 312 HCV-infected individuals belonging to different categories of exposure, who were attended at the HEMOPA, CENPREN and a private hemodialysis clinic in Belém. They were tested for HCV antibodies using an immunoenzymatic test, RNA-HCV, using real-time PCR and HCV genotyping through phylogenetic analysis of the 5' UTR. The population groups were epidemiologically characterized according to data collected in a brief interview or medical consultation. RESULTS: Genotype 1 predominated in all the different categories of HCV exposure. HCV genotypic distribution among blood donors comprised genotypes 1 (94%) and 3 (6%). All patients with chronic hematologic diseases had HCV genotype 1. The genotypic distribution in illicit-drug users comprised genotypes 1 (59.6%) and 3 (40.4%). In patients under hemodialysis, genotypes 1 (90.1%), 2 (3.3%), and 3 (6.6%) were detected. Finally, the frequency of genotypes 1 and 3 was significantly different between the groups: BD and DU, PUH and DU, PUH and PCHD and PCHD and DU. CONCLUSIONS: The genotypic frequency and distribution of HCV in different categories of exposure in the State of Pará showed a predominance of genotype 1, regardless of the possible risk of infection.

Nowadays, the consumption of goods and services on the Internet are increasing in a constant motion. Small and Medium Enterprises (SMEs) mostly from the traditional industry sectors are usually make business in weak and fragile market sectors, where customized products and services prevail. To survive and compete in the actual markets they have to readjust their business strategies by creating new manufacturing processes and establishing new business networks through new technological approaches. In order to compete with big enterprises, these partnerships aim the sharing of resources, knowledge and strategies to boost the sector’s business consolidation through the creation of dynamic manufacturing networks. To facilitate such demand, it is proposed the development of a centralized information system, which allows enterprises to select and create dynamic manufacturing networks that would have the capability to monitor all the manufacturing process, including the assembly, packaging and distribution phases. Even the networking partners that come from the same area have multi and heterogeneous representations of the same knowledge, denoting their own view of the domain. Thus, different conceptual, semantic, and consequently, diverse lexically knowledge representations may occur in the network, causing non-transparent sharing of information and interoperability inconsistencies. The creation of a framework supported by a tool that in a flexible way would enable the identification, classification and resolution of such semantic heterogeneities is required. This tool will support the network in the semantic mapping establishments, to facilitate the various enterprises information systems integration.

INTRODUÇÃO: O aumento da prevalência de isolados de enterococos em hospitais, particularmente Enterococcus resistente à vancomicina (VRE), é importante por causa da limitada terapia antimicrobiana efetiva para o tratamento de infecções enterocócicas. MÉTODOS: O presente trabalho apresentou uma investigação retrospectiva de dados de suscetibilidade in vitro quantitativa para uma variedade de antimicrobianos frente aos isolados de Enterococcus spp. e avaliação da associação de resistência entre os agentes antimicrobianos apontados como escolha para o tratamento de infecções causadas por VRE, através do cálculo do risco relativo. RESULTADOS: Dos 156 isolados de enterococos, 40 (25,6%) foram resistentes a três ou mais antimicrobianos, incluindo 7,7% (n = 12/156) resistentes à vancomicina. A associação de resistência elevada foi mais pronunciada entre os isolados de VREs com antimicrobianos alternativos e primários para o tratamento de infecções causadas por estes patógenos, incluindo ampicilina (100%, RR = 7,2), estreptomicina (90,9%, RR = 4,9), rifampicina (91,7%, RR = 3,1) e linezolida (50%, RR = 11,5), apesar da alta taxa de suscetibilidade a esta droga (94,9%). CONCLUSÕES: A resistência associada significativa aos antimicrobianos de primeira escolha e alternativos, usados no tratamento de infecções graves por cepas com o fenótipo VRE e que requerem um regime terapêutico combinado, evidencia alternativas terapêuticas ainda mais limitadas na instituição analisada.

Este estudo debruça-‐se sobre a presença das Organizações Não-‐ Governamentais nos media, partindo de uma análise a quatro meios de comunicação social nacionais. As ONG são das entidades em que os cidadãos mais confiam, como aliás indicam alguns barómetros de confiança (Edelman, 2012). Apesar disto as ONG raramente são capa de jornal, destaque de telejornal ou protagonistas de reportagens, e os seus profissionais, na maior parte das vezes, não são identificados como especialistas. O contacto com estas entidades define-‐ se por alguma falta de frequência, e pouca presença nos media. Os jornalistas trabalham num contexto cada vez mais limitador, menos criativo. E as ONG não estão a conseguir aproveitar este espaço que muitos jornalistas deixam em branco, de investigação, de exploração, e mesmo de novidade. Este estudo pretende apurar a relação entre os media e as fontes de informação não-‐governamentais, e como estão representadas nos media. Já que o papel de fonte de informação preferencial fica regularmente guardado para outras instituições e entidades. Para conseguir responder a algumas questões que nos pareciam essenciais, no sentido de tentar contribuir para um espaço público e participativo mais plural, procurámos esclarecer as dinâmicas atuais das ONG enquanto fontes de informação. Considerámos, por isso, determinante descrever o processo de produção das notícias, e o modo como este constrói a agenda e reflete a realidade. Desenvolvemos o conceito histórico e social do conceito de Organização Não-‐ Governamental Desenvolvemos uma análise minuciosa e detalhada das notícias publicadas durante três anos, entre 2009 e 2011, no jornal “Público”, agência Lusa, RTP e TSF, para identificar as características mais relevantes das notícias cuja fonte de informação é uma ONG. Paralelamente à análise dos meios de comunicação, apurámos ainda a pesquisa através da realização de entrevistas com profissionais da área não-‐governamental e jornalistas que nos apoiaram na construção de um trabalho empírico mais conclusivo e completo. Concluímos que mediante ONG mais ágeis e fortes, do ponto de vista da comunicação, parece haver uma resposta por parte dos media. Assim cremos que as ONG são reflectidas pelos media como instituições credíveis, e é esta mesma credibilidade que facilita o seu acesso aos media. Mas, os jornalistas, por seu lado, demonstram estar muito mais atentos ao trabalho das ONG se estas, além de credíveis, forem ágeis nas respostas correspondendo assim às necessidades impostas pela atualidade informativa, produzirem conteúdos com relevância jornalística, e dotados de valores-‐notícia.

A Parte I teve como objectivo a pesquisa de novos constituintes bioactivos da planta Solanum cernuum Vell. Identificaram-se, pela primeira vez no género Solanum, três triterpenos com um esqueleto de 31-norcicloartanona((+)-cicloeucalenona, (+)-24-oxo-31 norcicloartanona e (+)-24(1,3-dioxetano)-31-norcicloartanona) e quatro alcalóides com uma unidade (2-aminopirrolidin-1-il)carboxamidina acilada com o ácido isoferúlico assumindo configurações E e Z (cernumidina,isocernumidina, cernumidina B e isocernumidina B). A (+)-24-oxo-31-norcicloartanona apresentou actividade anti-neoplásica selectiva para as células de pulmão NCIH460, actividade anti-inflamatória relevante e uma inibição acentuada da expressão da COX-2. A (+)-24(1,3-dioxetano)-31-norcicloartanona apresentou também actividade para as células de leucemia K562. A cernumidina provocou inibição da interleuquina-8 (IL-8) em células do carcinoma do cólon HT-29 superior a 50%. Foram também identificados quercitrina, afzelina, hiperina, ácido ferúlico e Nacetildopamina. Na Parte II desenvolveram-se estudos sobre metodologias de síntese de novos cromóforos derivados da 7-amino-4-metilcumarina. Foi realizado um estudo da reacção de Buchwald-Hartwig para a introdução de grupo(s) electrodoador(es) na posição 7 e de um grupo electroatractor na posição 3 com rendimentos razoáveis. Foram obtidos os compostos 7-(4-metoxifenilamino)-4-metil-2H-cromen-2-ona, 4-metil-7-(fenilamino)- 2H-cromen-2-ona, 3-bromo-7-(4-metoxi-fenilamino)-4-metil-2H-cromen-2-ona e 3- bromo-4-metil-7-(fenilamino)- 2H-cromen-2-ona. Os espectros de absorção destes quatro cromóforos apresentaram c.d.o. máximos desviados para a zona do vermelho. Para a expansão do sistema π pela introdução de um grupo etinileno como ponte conjugada na posição 3 foi necessário a activação desta posição. Para tal, procedeu-se à bromação da N-(4-metil-2-oxo-2H-cromen-7-il)octanamida seguida da reacção de Sonogashira-Hagihara com o álcool propagílico. A oxidação posterior ao aldeído formou a N-(4-metil-2-oxo-3-(3-oxoprop-1- inil)2H-cromen-7-il)octanamida. Numa aproximação convergente foi testada a síntese do ácido 2-cianopent-2-en-4-óico (sem sucesso) para posterior reacção de Sonogashira-Hagihara com 3-bromo-N-(4-metil-2-oxo-2Hcromen-7-il)octanamida.

Portugal, having responsibilities at European level, needs to ensure compliance with European standards, particularly with regard to the European Security Plan for Critical Infrastructures. National critical infrastructures should be a focus of attention with regard to the management of public risks, since these represent "a set of services that are essential to the functioning of the country and the functioning of the forces that ensure national defense." (Soares, 2008) This contribution on national critical infrastructures (CI) has the essential objective of clarifying the development of the strategy adopted by Portugal in pursuit of the security of these fundamental infrastructures. The goal lies not only through producing a descriptive document, but also carry a brief confrontation between the legal framework related to these subjects and the reality in which the Critical Infrastructure Operators and the National Civil Protection Authority (ANPC) operate. It is intended, in this sense, to understand the development of the project for the national security program of critical infrastructures and what effects of its measures on operators. As for the methodology, we followed a methodological strategy, where we combine the literature with data obtained through semi-structured interviews. Portugal, being a geographically peripheral country and having no record of incidents capable of causing major contingencies in key services for the normal development of society, does not have a structured and regulator plan that substantiates the need for operators responsible for CI to invest in security. This same approach is expected at the State level, believing that even though this theme has be widely explored by international institutions, Portugal has not yet tried to give the attention it deserves. Without the existence of an institution and a regulatory system, CI operators can become less available to comply with the legal framework.

This study uses a VAR methodology to evaluate the impact of the macroeconomic conditions and money supply in the fluctuation of nonperforming loans for the Portuguese economy. Additionally, the feedback effect of nonperforming loans growth to the economy and specially to the credit supply is analised. The study is motived by the hypothesis that loan quality is procyclical and that the fast growth of credit supply has a positive relation with the growth of nonperforming loans. The hypothesis that nonperforming loans reinforce economic fragilities and credit market frictions is also tested. Empirical results corroborate both hypothesis presented. Hence, it was possible to establish that the macroeconomic conditions measured by GDP and unemployment and the fast growth of credit supply contribute to the development of nonperforming loans. Furthermore, the growth of nonperforming loans reinforces the economic cycle, as it contributes to the deterioration of macroeconomic conditions and creates frictions in the credit market that may results in a credit crunch.

RESUMO: Enthesitis is the hallmark of spondyloarthritis (SpA), and is observed in all subtypes. Wide information on SpA abnormalities, including synovitis, tendinitis and enthesitis, can be efficiently perceived by Doppler ultrasound. Furthermore, several studies on imaging of enthesis showed that imaging techniques are better than clinical examination to detect enthesis alterations; and vascularized enthesitis detected by Doppler ultrasound appears to be a valuable diagnostic tool to confirm SpA diagnosis. However, data published until now concerning entheseal elementary alterations that characterize SpA enthesitis (enthesis inflammatory activity) or enthesopathy (permanent structural changes) reflect rather the authors’ empiric opinion than a methodological validation process. In this sense it seems crucial to identify elementary entheseal lesions associated with activity or damage, in order to improve monitoring and treatment response in SpA patients. The development of better assessment tools is today a challenge and a need in SpA. The first study of this thesis focused on the analysis of the reliability of inter-lector and inter-ultrasonography equipment of Madrid sonography enthesitis index (MASEI). Fundamental data for the remaining unrolling project validity. In the second and third studies we concerned about two entheseal elemental lesions: erosions and bursa. In literature erosions represent a permanent structural damage, being useful for monitoring joint injury, disease activity and therapeutic response in many rheumatic diseases; and to date, this concept has been mostly applied in rheumatoid arthritis (RA). Unquestionably, erosion is a tissue-related damage and a structural change. However, the hypothesis that we decided to test was if erosions represent a permanent structural change that can only grow and worsen over time, as occurs in RA, or a transitory alteration. A longitudinal study of early SpA patients was undertaken, and the Achilles enthesis was used as a model. Our results strongly suggested that previously detected erosions could disappear during the course of the disease, being consistent with the dynamic behavior of erosion over time. Based on these striking results it seems reasonable to suggest that the new-bone formation process in SpA could be associated with the resolution of cortical entheseal erosion over time. These results could also be in agreement with the apparent failure of anti-tumor necrosis factor (TNF) therapies to control bone proliferation in SpA; and with the relation of TNF-α, Dickkopf-related protein 1 (Dkk-1) and the regulatory molecule of the Wnt signaling pathway in the bone proliferation in SpA. In the same model, we then proceeded to study the enthesis bursa. Interestingly, the Outcome Measures in Rheumatology Clinical Trials (OMERACT) enthesopathy definition does not include bursa as an elementary entheseal lesion. Nonetheless, bursa was included in 46% of the enthesis studies in a recently systematic literature review, being in agreement with the concept of “synovio-entheseal complex” that includes the link between enthesitis and osteitis in SpA. It has been clarified in recent data that there is not only a close functional integration of the enthesis with the neighboring bone, but also a connection between enthesitis and synovitis. Therefore, we tried to assess the prevalence and relevance of the bursa-synovial lesion in SpA. Our findings showed a significant increase of Achilles bursa presence and thickness in SpA patients compared to controls (healthy/mechanical controls and RA controls). These results raise awareness to the need to improve the enthesopathy ultrasonographic definition. In the final work of this thesis, we have explored new perspectives, not previously reported, about construct validity of enthesis ultrasound as a possible activity outcome in SpA. We performed a longitudinal Achilles enthesis ultrasound study in patients with early SpA. Achilles ultrasound examinations were performed at baseline, six- and twelve-month time periods and compared with clinical outcome measures collected at basal visit. Our results showed that basal erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are higher in patients with Doppler signal in enthesis, and even that higher basal ESR, CRP and Ankylosing Spondylitis Disease Activity Score (ASDAS) predicted a higher Doppler signal (an ultrasound alteration accepted as representative of inflammation) six months later. Patients with very high disease activity assessed by ASDAS (>3.5) at baseline had significantly higher Achilles total ultrasound score verified at the same time; and ASDAS <1.3 predicted no Doppler signal at six and twelve months. This seems to represent a connection between classical biomarkers and clinical outcomes associated with SpA activity and Doppler signal, not only at the same time, but also for the following months. Remarkably, patients with inactive disease (ASDAS < 1.3) at baseline had no Doppler signal at six and twelve months. These findings reinforce the potential use of ultrasound related techniques for disease progression assessment and prognosis purposes. Intriguingly, Ankylosing Spondylitis Disease Activity Index (BASDAI) didn’t show significant differences between different cut-offs concerning ultrasound lesions or Doppler signal, while verified with ASDAS. These results seem to indicate that ASDAS reflects better than BASDAI what happens in the enthesis. The work herein discussed clearly shows the potential utility of ultrasound in enthesis assessment in SpA patients, and can be important for the development of ultrasound activity and structural damage scores for diagnosis and monitoring purposes. Therefore, local promotion of this technique constitutes a medical intervention that is worth being tested in SpA patients for diagnosis, monitoring and prognosis purposes.

Este trabalho aborda a conceito de desordem psíquica na obra de Galeno (129-216). A primeira parte enquadra o pensamento de Galeno na visão da Filosofia e Medicina em vigor no início do séc. II d.C. De seguida descrevo a concepção Hipocrática de epilepsia, e a abordagem que Platão desenvolve no Timeu e Fedro acerca da loucura, para de seguida abordar a concepção de doença em Galeno, onde são analisados os conceitos de nosêma, diathesis, pathos e energeia. Após este excurso descrevo o ponto de vista de Galeno acerca da controvérsia sobre a localização da parte dirigente da alma (hêgemonikon) que se dividia entre defensores do encefalocentrismo (Platão, Hipócrates e Herófilo) e do cardiocentrismo (Aristóteles e os Estóicos). De modo a aprofundar a compreensão de Galeno acerca deste tema descrevo o trabalho anatómico-fisiológico desenvolvido pelos médicos Alexandrinos Erasistrato e Herófilo, activos no sec. III a. C., que descobriram, através de dissecação de animais e muito provavelmente de humanos, o papel dos nervos e tendões nas atividades cognitivas e sensório-motoras. Esta foi uma descoberta central para a argumentação de Galeno acerca da interacção mente-corpo assim como para a descrição das desordens psíquicas. Posteriormente apresento a metodologia de Galeno no que concerne ao processo de diagnóstico e etiologia, essencial para se compreender como é possível aceder a ‘entidades’ não visíveis, como o hêgemonikon e as suas diferentes faculdades: ‘imaginação’, memória e raciocínio. Por fim, analiso alguns casos clínicos de pacientes afectados por desordens do hêgemonikon, a saber: phrenitis, mania e melancolia. Os principais textos objecto de análise são: Acerca dos Lugares Afectados, Acerca das Teses de Hipócrates e Platão e Que as Faculdades da Alma Seguem as Disposições do Corpo. Todavia, outros textos de Galeno serão convocados consoante a necessidade de analisar os conceitos que me proponho compreender, entre eles Acerca do Método Terapêutico e Acerca da Utilidade das Partes.
