Aims  To undertake further psychometric validation of the W-BQ28 to determine its suitability for use in adults with Type 2 diabetes in the UK using data from the AT.LANTUS follow-on study.

Methods  A total of 353 people with Type 2 diabetes participated in the AT.LANTUS Follow-on study, completing measures of well-being (W-BQ28), treatment satisfaction (DTSQ) and self-care (SCI-R). Confirmatory factor analyses was used to confirm the W-BQ28 structure and internal consistency reliability was assessed. Additional statistical tests were conducted to explore convergent, divergent and known-groups validity. Minimal important differences were calculated using distribution and anchor-based techniques.

Results  Structure of the W-BQ28 (seven four-item subscales plus 16-item generic and 12-item diabetes-specific scales) was confirmed (comparative fit index = 0.917, root mean square error of approximation (RMSEA) = 0.057). Internal consistency reliability was satisfactory (four-item subscales: alpha = 0.73–0.90; 12/16-item scales: α = 0.84–0.90). Convergent validity was supported by expected moderate to high correlations (rs = 0.35–0.67) between all W-BQ28 subscales (except Energy); divergent validity was supported by expected low to moderate correlations with treatment satisfaction (rs = −0.03–0.52) and self-care (rs = 0.02–0.22). Known-groups validity was supported with statistically significant differences by sex, age and HbA1c for expected subscales. Minimal important differences were established (range 0.14–2.90).

Conclusions  The W-BQ28 is a valid and reliable measure of generic and diabetes-specific well-being in Type 2 diabetes in the UK. Confirmation of the utility of W-BQ28 (including establishment of minimal important differences) means that its use is indicated in research and clinical practice.

This chapter begins by defining the constructs of happiness and subjective well-being in terms of taxonomy and conceptual frameworks. Issues of measurement are addressed separately for qualitative and quantitative methodology. Two special areas of measurement difficulty are then discussed: the validity of data derived from low-functioning populations and cross-cultural measurement equivalence. Finally specific scales are considered in terms of their consistency with the foregoing analysis.

Objective

To examine the cost-effectiveness of Be Active Eat Well (BAEW), a large, multifaceted, community-based capacity-building demonstration program that promoted healthy eating and physical activity for Australian children aged 4-12 years between 2003 and 2006.

Conclusion
BAEW was affordable and cost-effective, and generated substantial spin-offs in terms of activity beyond funding levels. Elements fundamental to its success and any potential cost efficiencies associated with scaling-up now require identification.

Aim
To examine the emotional well-being of severely obese Australians with type 2 diabetes, along with markers of social and economic disadvantage, using the Diabetes MILES – Australia dataset.
Methods
Diabetes MILES – Australia was a national survey of 3338 adults with diabetes that focused on psychosocial issues; 1795 had type 2 diabetes and reported BMI. We extracted data regarding depression (PHQ-9), anxiety (GAD-7), obesity- and diabetes- related comorbidities, and demographics. The severely obese group (SOG) (BMI ≥ 35; median BMI = 41.6) constituted 530 (30%) of the type 2 diabetes respondents and was matched with 530 controls (CG) (BMI < 35; median BMI = 28.2). Within- and between- group trends were examined.
Results
The SOG had higher depression scores (median (IQR) 6.0 (3–12)) than CG (5.0 (2–10)); p < 0.001, and were more likely to report moderate-severe depressive symptoms (37% versus 27%; p < 0.001). The groups did not differ on anxiety. The SOG, compared with the CG, were more likely to live alone (21% versus 17%), receive a disability pension (21% versus 15%), earn ≤$40.000/year (51% versus 41%; all p < 0.05), and were less likely to be employed (46% versus 53%), university or higher educated (17% versus 26%), or have health insurance (50% versus 60%; all p ≤ 0.01). Moderate-severe depression was positively associated with cumulative stressors of severe obesity, socioeconomic disadvantage, and obesity- and diabetes- related comorbidity.
Conclusions
Severely obese people living with type 2 diabetes have cumulative stressors related to health, disability, demographic and socioeconomic factors, and impaired emotional well-being.

This chapter concerns the idea that Subjective Wellbeing (SWB) is managed by a system of psychological devices which have evolved for this purpose. It is proposed that this management is actually directed at the protection of Homeostatically Protected Mood, as the major component of SWB. We normally experience HPMood as a combination of contentment, happiness and arousal. A theoretical description of this construct is offered that can account for many of the commonly observed empirical characteristics of SWB data. It is further proposed that when homeostasis fails, due to the overwhelming nature of a negative challenge, people lose contact with HPMood and experience the domination of negative rather than positive affect. If this condition is chronic, people experience the clinical condition we call depression.

Australian and Victorian Government policies encourage settlement in regional areas for international migrants, refugees and internal migrants. Migrants to regional areas are diverse in terms of their area or country of origin, skills and occupation, family status and other demographic characteristics. The regional cities to which they migrate are also varied in terms of their community resources, social and cultural capital. The objective shared by all of these cities is for migrants to engage successfully with their new communities. Just how this occurs is the subject of debate and a lack of clarity. This therefore calls for a sound, theoretically informed understanding of how employers and community groups (formal and informal) can effectively assist migrants to make social connections in regional cities, and practical strategies which respond to these insights. The well-established social determinants of health tell us that the more socially included, connected and stable workforce and their families are, the better will be their physical and mental health and wellbeing.


People in Australia generally move to live near family and friends; for better access to work or work opportunities; or to live in an attractive neighbourhood. Policies and programs intended to assist with settlement tend to be short term and project based. Good practice in assisting migrants make social connections however is long term and embedded into the community. Workplaces and community groups that are already established, and groups that migrants or others tend to form naturally, are good examples of such best practice. Workplaces, local government, institutions such as schools, community spaces and other organisations can also assist in the settling in process and can complement formal and informal community groups, once a sound evidence base is established.

This is the second paper to emerge from a research project running over 2011-2012 at the Alfred Deakin Research Institute (ADRI), Deakin University in Geelong. The first Working Paper (No. 32) (Jackson et al., 2012) located the research theoretically. This second Working Paper will report on the research itself, its methods and outcomes as well as policy implications. The first section of this paper will briefly outline the project before considering those who have migrated to Geelong in the past two to five years: to investigate why they moved to Geelong; how they made connections and with whom; and, what was the value of those connections (Section 2). The third section of the paper examines how employers, non-government organisations (NGOs) and other facilitators effectively assist migrants to make social connections. The fourth and fifth sections look at the barriers to making connections but also those things – organisation and policies - that facilitated settling in. Section six summarises the findings and makes a series of policy recommendations for individuals, organisations and government on how to better the prospects for migrant in regional centres.
