Duchenne muscular dystrophy (DMD) is a severe, progressive disease first described by Meryon in 1852 and later by Guillaume Duchene. It is the most common and severe form of childhood muscular dystrophy, affecting 1 in 3500 live male births. Is caused by an X—linked recessive genetic disorder resulting in a deficiency of the dystrophin protein, responsible for linking contractile proteins to the sarcolemma. Diagnosis is not always easy and the first symptoms are often related to weakness and difficulty or delay in acquiring the ability to perform simple activities. Progressive weakness leads to the use of compensatory strategies in order to maintain the ability to walk and perform other activities. Respiratory muscles are also affected and the complications resulting from its impairments are frequently the cause of early death of these patients. The advances in DMD management has increased life expectancy of these children with the need for adequate care in adulthood. DMD manifestations include muscle weakness, contractures, respiratory and cardiac complications. Some authors also refer that one-third of patients have difficulties with learning and delayed global development because the gene that encodes dystrophyn expresses various dystrophin isoforms that are found in Schwann and Purkinje celis in the brain. Body functions and structure impairments like muscle weakness, contractures and reduced range of motion lead to limitations in activities, i.e., impairments affect the performance of tasks by the individual. In a physiotherapist’s point of view analysing these limitations is mandatory because physiotherapy’s final purpose is to restore or preserve the ability to perform ADL and to improve quality of life.

The conjugate margins system of the Gulf of Lion and West Sardinia (GLWS) represents a unique natural laboratory for addressing fundamental questions about rifting due to its landlocked situation, its youth, its thick sedimentary layers, including prominent palaeo-marker such as the MSC event, and the amount of available data and multidisciplinary studies. The main goals of the SARDINIA experiment, were to (i) investigate the deep structure of the entire system within the two conjugate margins: the Gulf of Lion and West Sardinia, (ii) characterize the nature of the crust, and (iii) define the geometry of the basin and provide important constrains on its genesis. This paper presents the results of P-wave velocity modelling on three coincident near-vertical reflection multi-channel seismic (MCS) and wide-angle seismic profiles acquired in the Gulf of Lion, to a depth of 35 km. A companion paper [part II Afilhado et al., 2015] addresses the results of two other SARDINIA profiles located on the oriental conjugate West Sardinian margin. Forward wide-angle modelling of both data sets confirms that the margin is characterised by three distinct domains following the onshore unthinned, 33 km-thick continental crust domain: Domain I is bounded by two necking zones, where the crust thins respectively from 30 to 20 and from 20 to 7 km over a width of about 170 km; the outermost necking is imprinted by the well-known T-reflector at its crustal base; Domain II is characterised by a 7 km-thick crust with anomalous velocities ranging from 6 to 7.5 km/s; it represents the transition between the thinned continental crust (Domain I) and a very thin (only 4-5 km) "atypical" oceanic crust (Domain III). In Domain II, the hypothesis of the presence of exhumed mantle is falsified by our results: this domain may likely consist of a thin exhumed lower continental crust overlying a heterogeneous, intruded lower layer. Moreover, despite the difference in their magnetic signatures, Domains II and III present the very similar seismic velocities profiles, and we discuss the possibility of a connection between these two different domains.

Geophysical data acquired on the conjugate margins system of the Gulf of Lion and West Sardinia (GLWS) is unique in its ability to address fundamental questions about rifting (i.e. crustal thinning, the nature of the continent-ocean transition zone, the style of rifting and subsequent evolution, and the connection between deep and surface processes). While the Gulf of Lion (GoL) was the site of several deep seismic experiments, which occurred before the SARDINIA Experiment (ESP and ECORS Experiments in 1981 and 1988 respectively), the crustal structure of the West Sardinia margin remains unknown. This paper describes the first modeling of wide-angle and near-vertical reflection multi-channel seismic (MCS) profiles crossing the West Sardinia margin, in the Mediterranean Sea. The profiles were acquired, together with the exact conjugate of the profiles crossing the GoL, during the SARDINIA experiment in December 2006 with the French R/V L'Atalante. Forward wide-angle modeling of both data sets (wide-angle and multi-channel seismic) confirms that the margin is characterized by three distinct domains following the onshore unthinned, 26 km-thick continental crust : Domain V, where the crust thins from 26 to 6 km in a width of about 75 km; Domain IV where the basement is characterized by high velocity gradients and lower crustal seismic velocities from 6.8 to 7.25 km/s, which are atypical for either crustal or upper mantle material, and Domain III composed of "atypical" oceanic crust.The structure observed on the West Sardinian margin presents a distribution of seismic velocities that is symmetrical with those observed on the Gulf of Lion's side, except for the dimension of each domain and with respect to the initiation of seafloor spreading. This result does not support the hypothesis of simple shear mechanism operating along a lithospheric detachment during the formation of the Liguro-Provencal basin.

As freguesias de Lisboa sofreram profundas alterações ao longo dos séculos XIX e XX. Não só foram alteradas em número, mas também na sua dimensão geográfica. A última grande reforma, datada de 1959, modificou de tal forma os contornos das freguesias lisboetas que inviabiliza a utilização dos contornos actuais para a realização de estudos no passado. Procurando dar resposta a esta problemática e com base em cartografia histórica, foram desenhadas as freguesias em três períodos (1826, 1852 e 1909) sendo incluídas num sistema de informação geográfica, o que possibilita não só a análise da evolução administrativa da capital, mas também o estudo de muitas variáveis históricas, de base paroquial, para todo o século XIX e início do século XX.

AIMS: To investigate the long-term effects of efavirenz on cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein (LDL-C) and triglycerides (TG). METHODS: Thirty-four HIV-infected patients who commenced efavirenz therapy were monitored for 36 months. RESULTS: In patients with baseline HDL-C<40 mg.dL-1 an increase in HDL-C from 31+/-1 mg.dL-1 to 44+/-2 mg.dL-1 (95% confidence interval 5.9, 21.9, P<0.01) was observed and remained throughout the follow-up period. Median efavirenz plasma concentration was 1.98 mg.L-1 and a direct correlation between percentage of HDL-C variation or TC/HDL-C ratio and efavirenz plasma concentrations was found. CONCLUSIONS: There is evidence of a long-term and concentration-dependent beneficial effect of efavirenz on HDL-C in HIV-infected patients.

Actualmente, o desafio da reimplantação do membro superior pós-amputação tornou-se uma realidade alcançável e minuciosamente aperfeiçoada nas últimas décadas, e em permanente evolução. A opção cirúrgica de reimplantação deve ter em conta não apenas a análise exclusiva da viabilidade do reimplante, mas, fundamentalmente o seu potencial de recuperação funcional a longo prazo. Apresenta-se o caso clínico de um jovem de 18 anos, fumador, transferido do Hospital do Barreiro, vítima de acidente de trabalho, com traumatismo por corte, do qual resultou amputação distal do antebraço direito. O tempo de isquemia quente foi de 4horas, tendo sido submetido a cirurgia de reimplantação conjunta por Ortopedia e Cirurgia Plástica e Reconstrutiva (CPR) para reimplantação. Na sequência da cirurgia, foi precocemente referenciado a Medicina Física e de Reabilitação (MFR), realizando um programa de reabilitação funcional sequencial. Este trabalho visa enfatizar a importância do papel da MFR num precoce, criterioso e extenso programa de reabilitação, factor fundamental na recuperação funcional e prognóstico a longo prazo destas lesões e prevenção de complicações.

In his Answer to the Question: What is Enlightenment (1784), Kant puts forward his belief that the vocation to think freely, which humankind is endowed with, is bound to make sure that “the public use of reason” will at last act “even on the fundamental principles of government and the state [will] find it agreeable to treat man – who is now more than a machine – in accord with his dignity”. The critical reference to La Mettrie (1747), by opposing the machine to human dignity, will echo, in the dawn of the 20th century, in Bergson’s attempt to explain humor. Besides being exclusive to humans, humor is also a social phenomenon. Freud (1905) assures that pleasure originated by humor is collective, it results from a “social process”: jokes need an audience, a “third party”, in order to work and have fun. Assuming humor as a social and cultural phenomenon, this paper intends to sustain that it played a role in the framing of the public sphere and of public opinion in Portugal during the transition from Absolute Monarchy to Liberalism. The search for the conditions which made possible the critical exercise of sociability is at the root of the creation of the public sphere in the sense developed by Habermas (1962), whose perspective, however, has been questioned by those who point 2 out the alleged idealism of the concept – as opposed, for example, to Bakhtin (1970), whose work stresses diversity and pluralism. This notwithstanding, the concept of public sphere is crucial to the building of public opinion, which is, in turn, indissoluble from the principle of publicity, as demonstrated by Bobbio (1985). This paper discusses the historical evolution of the concept of public opinion from Ancient Greece doxa, through Machiavelli’s “humors” (1532), the origin of the expression in Montaigne (1580) and the contributions of Hobbes (1651), Locke (1690), Swift (1729), Rousseau (1762) or Hume (1777), up to the reflection of Lippman (1922) and Bourdieu’s critique (1984). It maintains that humor, as it appears in Portuguese printed periodicals from 1797 (when Almocreve de Petas was published for the first time) to the end of the civil war (1834) – especially in those edited by José Daniel Rodrigues da Costa but also in O Piolho Viajante, by António Manuel Policarpo da Silva, or in the ones written by José Agostinho de Macedo, as well as in a political “elite minded” periodical such as Correio Braziliense –, contributed to the framing of the public sphere and of public opinion in Portugal.

O casal de suíços, Heinrich (1795–1866) e Cécile Däniker-Haller (1816–1887), com seus filhos, viveu no Rio de Janeiro entre 1828 e 1852. Comerciantes prósperos, cosmopolitas e melômanos, deixaram grande quantidade de informações sobre suas atividades de música em ambiente doméstico daquele período, mas também depois de terem voltado a se estabelecer na Suíça. O Genealogisches Archiv der Helene und Cécile Rübel Familienstiftung (FA HCR), na cidade de Zurique, documenta a história desta família. As informações registradas pelo casal estudado, principalmente em forma de cartas e diários, permitem-nos conhecer e analisar a prática musical em ambiente doméstico das famílias estrangeiras no Rio de Janeiro, e demonstrar quais eram os repertórios por eles utilizados. Entre as conclusões tiradas, inclui-se o fato de que o romantismo alemão teria feito sua entrada no ambiente musical brasileiro através dos salões musicais das famílias estrangeiras, exatamente como foi o caso em outras cidades do mundo não germânico. Nos documentos deixados pela família Däniker-Haller encontram-se os mais antigos testemunhos até hoje encontrados da execução de Lieder de Schubert em ambiente doméstico no Rio de Janeiro. Por outro lado, a prática da Hausmusik na cidade de Zurique vem descrita e comparada àquela carioca. A análise destes relatos também nos permite descrever a diferença que havia entre os eventos musicais em ambiente doméstico íntimo e informal, e os "salões musicais", onde diletantes se produziam em esfera semi-pública previamente preparados e ensaiados, e a marcante diferença no repertório destes dois tipos de eventos sociais.

Muitas das espécies de insetos associadas ao plantios de eucalipto são consideradas pragas, enquanto outras ainda não apresentam uma relação definida com estas florestas. Amostragens quinzenais da entomofauna, realizadas com armadilhas luminosas, no período de agosto/90 a julho/91, revelaram a existência de 263 espécies de Lepidoptera associadas aos plantios de eucalipto da região de Açailândia (Maranhão). Destas, sete são consideradas pragas primarias: Eupseudosoma aberransSchaus, 1905 e Eupseudosoma involutaSepp, 1852 (Arctiidae), "Thyirrteina leucoceraeaRindge, 1961, Glenaspp., e Oxydia veuliaCramer, 1779 (Geometridae); Nystalea nyseusCramer, 1775 (Notodontidae); Sarsina violascensHerrich-Schaeffer, 1856 (Lymantriidae) e seis pragas sencundárias: Automeris illustrisWalker, 1855 Automenis sp., Hyperchiria incisaWalker, 1855 e Hyfesiasp. (Satumiidae), Cosmosoma augeLinné, 1767 (Amatidae) e idalus affinis(Arctiidae).

Este estudo pioneiro pretende associar a qualidade da aliança terapêutica estabelecida entre os pais e o terapeuta com a qualidade da relação terapêutica entre a criança e o terapeuta. O objectivo foi perceber como estas duas variáveis se podem influenciar entre si. A amostra era constituída por 21 díades pai/mãe filho que teriam de individualmente avaliar a sua relação terapêutica estabelecida com o terapeuta. As dimensões avaliadas foram a qualidade da relação, os objectivos e as tarefas do processo terapêutico. Os resultados obtidos não encontraram uma associação entre as relações terapêuticas globais dos pais e dos filhos com o terapeuta. Todavia, algumas correlações secundárias foram encontradas. Orientações e recomendações para futuras replicações do presente estudo são discutidas.
