II. Es van analitzar mostres de microdiàlisi cerebral in vivo de pacients d’ictus isquèmic pel descobriment de biomarcadors associats a la malaltia cerebrovascular. Es van obtenir microdialitzats (MDs) de la zona infartada, de la zona del periinfart i de la zona contralateral no afectada. Els MDs es van comparar, qualitativament i quantitativament, mitjançant tècniques de proteómica basades en marcatge isobàric i espectrometria de masses. Complementàriament, alguns candidats es van estudiar mitjançant immunotransferència en mostres de teixit de necròpsies cerebrals de pacients morts a causa d’un ictus isquèmic. Finalment, hem començat a realitzar ELISAs per la determinació d’aquests candidats en mostres de sang.

S'ha pretés en tot moment fer una lectura de la novel·la en clau femenina per tal de demostrar que la figura central de l'heroi actua i evoluciona generalment gràcies a o a causa de la voluntat, les decisions o les accions de les dones que troba en la seua aventura. No hi pot haver cavalleria sense amor; tot cavaller ha de lluitar per la seua dama tant al camp de batalla com a la cort.

En los transplantes de progenitores hematopoyéticos, la sangre de cordón umbilical es una fuente establecida de células madre hematopoyéticas que presenta como mayor ventaja una menor incidencia de enfermedades de injerto contra el huésped. Sin embargo, el bajo número de células madre obtenidas de una sola unidad limita su utilización a un número reducido de pacientes. Las células madre hematopoyéticas se definen por su capacidad de automantenimiento y reconstitución de todo el sistema hematopoyético de un huésped trasplantado. En ratón, la combinación de los marcadores de superficie Lin- LSK junto con los marcadores de la familia SLAM, ha permitido establecer una jerarquía en las poblaciones de células madre y progenitores hematopoyéticos. Sin embargo, la población de células madre hematopoyéticas humanas CD34+CD38- es heterogénea y las subpoblaciones de progenitores y células madre no están bien establecidas. Uno de los objetivos de este trabajo es determinar si los marcadores de la familia SLAM podrían redefinir la población de células madre hematopoyéticas humanas CD34+CD38- de forma similar a lo sucedido en ratón. En este trabajo se describe una nueva población de progenitores hematopoyéticos en sangre de cordón umbilical caracterizada por el fenotipo CD34+CD38-CD150+CD135-. Lon ensayos realizados tanto in vitro como in vivo han demostrado que esta población esta formada por células con capacidad de autorrenovación, de diferenciación a todos los linajes hematopoyéticos, y de reconstitución a corto y largo plazo de un modelo murino inmunodeficiente irradiado. Por otro lado, con la finalidad de obtener un número suficiente de progenitores hematopoyéticos para ser trasplantados, se han estudiado diferentes sistemas de expansión in vitro. Se ha observado que el ácido valproico (un inhibidor de las histona deacetilasas) y la activación de la vía de Notch, promueven el mantenimiento y expansión de los progenitores hematopoyéticos reduciendo los procesos de diferenciación.

In order to explain the speed of Vesicular Stomatitis Virus VSV infections, we develop a simple model that improves previous approaches to the propagation of virus infections. For VSV infections, we find that the delay time elapsed between the adsorption of a viral particle into a cell and the release of its progeny has a veryimportant effect. Moreover, this delay time makes the adsorption rate essentially irrelevant in order to predict VSV infection speeds. Numerical simulations are in agreement with the analytical results. Our model satisfactorily explains the experimentally measured speeds of VSV infections

The management of patients scheduled for surgery with a coronary stent, and receiving 1 or more antiplatelet drugs, has many controversies. The premature discontinuation of antiplatelet drugs substantially increases the risk of stent thrombosis (ST), myocardial infarction, and cardiac death, and surgery under an altered platelet function could also lead to an increased risk of bleeding in the perioperative period. Because of the conflict in the recommendations, this article reviews the current antiplatelet protocols after positioning a coronary stent, the evidence of increased risk of ST associated with the withdrawal of antiplatelet drugs and increased bleeding risk associated with its maintenance, the different perioperative antiplatelet protocols when patients are scheduled for surgery or need an urgent operation, and the therapeutic options if excessive bleeding occurs.

Studies show that feeding patterns, environmental conditions, and pet management mainly affect animal health, behavior and welfare. However, little epidemiologic data exist about pet dog feeding habits and management and canine behavior, especially aggression. Moreover, it is well known that serotonin is involved in aggression: a diet low in tryptophan (serotonin precursor) with high levels of neutral amino-acids could decrease the central levels of serotonin and increase the expression of aggression and impulsiveness in several domestic animals. oreover, brain levels of serotonin in humans and other species can be increased by physical activity. In this study we alysed the effects of diet management, supplementation and regular physical activity on oncentration of serotonin in a population of non aggressive dogs. The general aim of the study is to determine the effect of environmental factors on the dogs’ feeding habits as well as the relationship between the type of food provided and management and behavioral problems in dogs. The specific objectives were: characterization and comparison of different physiological and blood parameters between dogs with and without behavior pathologies. Observation of the impact of the diet and managment factors (exercise, education) on those parameters and on dog’s behavior, with particular attention to aggression. This study is divided into three phases, according with the objectives. Firstly, characterization of the dogs’ characteristics and serics profiles on the basis of the blood tests. Comparison between the values noticed on animals without any behavior pathology and animals with behavior disorder to point out the difference between the biochemical parameters of the two groups. Secondly, the purpose of the second phase is to verify whether a type of food is able to affect the biochemical paramenters of a population of non-aggressive dogs. Thirdly, the goal is to evaluate the effects of different diet factors, management and physical exercise on the canine aggression.

Este trabajo recoge el estudio, diseño, fabricación y análisis de dos dipolos de media onda para aplicaciones Wi-fi, con objeto de caracterizar sus respectivas ganancias mediante el uso de dos sistemas de medida distintos, el primero basado en el análisis del balance de enlace entre antenas mediante un analizador de redes y el segundo mediante una celda TEM. Para obtener un correcto funcionamiento de los dipolos, resulta de vital importancia conseguir un buen ajuste de las dimensiones de los mismos durante el desarrollo práctico del proyecto, consiguiendo una máxima transferencia de potencia y un ancho de banda suficientemente amplio para asegurar que las antenas presenten una buena adaptación en la banda de los 2.4 GHz – 2.5 GHz.

Los sofisticados procesos de transfusión de sangre que conocemos hoy son el resultado de un largo y complicado recorrido de muchos siglos en los que diferentes científicos buscaron superar sus problemas inherentes de compatibilidad sanguínea, coagulación, conservación y otros más. El grupo empresarial Grifols se comprometió desde su nacimiento -a inicios del siglo XX- con el desarrollo científico y tecnológico de los hemoderivados, atravesando procesos históricos llenos de dificultades que incluyen todo el período del franquismo, la Segunda Guerra Mundial y las posteriores crisis económicas hasta establecerse actualmente en una de las principales empresas internacionales de esta industria. El presente trabajo constituye un análisis sistemático de los factores que han contribuido al éxito competitivo de este holding internacional y que ahora busca ampliar sus fronteras con nuevas alternativas en el área de la salud.

BACKGROUND Functional brain images such as Single-Photon Emission Computed Tomography (SPECT) and Positron Emission Tomography (PET) have been widely used to guide the clinicians in the Alzheimer's Disease (AD) diagnosis. However, the subjectivity involved in their evaluation has favoured the development of Computer Aided Diagnosis (CAD) Systems. METHODS It is proposed a novel combination of feature extraction techniques to improve the diagnosis of AD. Firstly, Regions of Interest (ROIs) are selected by means of a t-test carried out on 3D Normalised Mean Square Error (NMSE) features restricted to be located within a predefined brain activation mask. In order to address the small sample-size problem, the dimension of the feature space was further reduced by: Large Margin Nearest Neighbours using a rectangular matrix (LMNN-RECT), Principal Component Analysis (PCA) or Partial Least Squares (PLS) (the two latter also analysed with a LMNN transformation). Regarding the classifiers, kernel Support Vector Machines (SVMs) and LMNN using Euclidean, Mahalanobis and Energy-based metrics were compared. RESULTS Several experiments were conducted in order to evaluate the proposed LMNN-based feature extraction algorithms and its benefits as: i) linear transformation of the PLS or PCA reduced data, ii) feature reduction technique, and iii) classifier (with Euclidean, Mahalanobis or Energy-based methodology). The system was evaluated by means of k-fold cross-validation yielding accuracy, sensitivity and specificity values of 92.78%, 91.07% and 95.12% (for SPECT) and 90.67%, 88% and 93.33% (for PET), respectively, when a NMSE-PLS-LMNN feature extraction method was used in combination with a SVM classifier, thus outperforming recently reported baseline methods. CONCLUSIONS All the proposed methods turned out to be a valid solution for the presented problem. One of the advances is the robustness of the LMNN algorithm that not only provides higher separation rate between the classes but it also makes (in combination with NMSE and PLS) this rate variation more stable. In addition, their generalization ability is another advance since several experiments were performed on two image modalities (SPECT and PET).

La infección por el Virus de Inmunodeficiencia Humano (VIH) y el Síndrome de Inmunodeficiencia adquirida (SIDA) afecta a millones de personas en todo el mundo, y constituye una amenaza a la salud y la vida de muchas otras más, sobre todo en países en vías de desarrollo. Existe un gran interés en el desarrollo de nuevas metodologías analíticas para el diagnóstico de dicha enfermedad de forma rápida, económica y fuera del ámbito del laboratorio por personal no especializado. Los biosensores son dispositivos ideales para cubrir esta demanda analítica facilitando la toma de decisiones y permitiendo un uso racional de técnicas analíticas confirmatorias más costosas. Se plantea el diseño de una estrategia magneto-ELISA con detección óptica así como un dispositivo magneto biosensor electroquímico para el diagnóstico de SIDA a través del recuento de células marcadoras de la enfermedad presentes en la sangre. Ambas estrategias se basan en la captura inmunomagnética de linfocitos T CD4+ con partículas magnéticas modificadas con anticuerpos monoclonales específicos (anti-CD3). La detección de las células capturadas se realiza con un anticuerpo primario anti-CD4 marcado con biotina (antiCD4-biotina) y con un conjugado de estreptavidina y de la enzima HRP (peroxidasa de rábano picante). La unión de esta enzima al anticuerpo primario se realiza a través del complejo biotina/estreptavidina. Se proponen dos tipos de sistemas de detección: óptico y electroquímico. Esto se logra mediante la elección adecuada del sustrato para cada sistema planteado. El dispositivo biosensor basados en un transductor electroquímico renovable y magnético acoplado a partículas magnéticas específicas para las células marcadoras de la enfermedad, consigue la simplificación metodológica y facilita la transferencia de la tecnología hacia la fabricación de un biokit diagnóstico en el ámbito clínico. La potencial aplicación de los dispositivos analíticos propuestos en este trabajo tienen un interés social elevado por su idoneidad para realizar análisis, rápidos, económicos y en el ámbito de la propia consulta médica.

Purpose: Iron overload (IO) has been associated with increased cardiovascular risk (CVR) and metabolic syndrome (MS) in the general population; both elevated CVR and MS are frequent in HIV- patients. Our aim was to analyze the prevalence of IO in a cohort of asymptomatic patients with HIV infection, and related factors. Methods: Cross-sectional study of a cohort of HIV outpatients in regular follow-up. Demographic, epidemiological, clinical, analytical and therapeutic data were collected. Patients completed a questionnaire about CVR factors and 10-year CV disease risk estimation (Framingham score), underwent a physical exam, and a fasting blood analysis. IO was defined as a plasma ferritin level higher than 200 m/L in women and 300 m/L in men. Results: 571 patients (446 men, 125 women), with a mean age of 43.2 years, sexual transmission of HIV in 68.5%, median CD4 count 474 cell/μL (IQR: 308-666), and 36.3% Aids cases 86.2% were on antiretroviral therapy (ART), and 74.8% of them had undetectable HIV viral load 14.6% met MS criteria, and mean CVR at 10 years was 6.67%. IO was detected in 11% of cases. Patients with IO were more immunosuppressed (CD4 count 369 vs 483/μL, p<0.0001), presented a higher prevalence of detectable HIV viral load (17.6% vs 8.9%; p<0.005), and of Aids cases (14.9% vs 8.7%; p<0.023), and lower plasma levels of cholesterol, HDLc and LDLc (154 vs 183, 34 vs 43, 93 vs 110 mg/dL, respectively; p<0.0001. In the multivariate analysis, the only related factor was CD4 count <350 cell/μL (OR 2.86, 95% CI 1.6-4.9; p<0.0001). IO was not associated with CVR nor with MS. Conclusions: IO is not uncommon in HIV patients, and it is only related with immunosuppression defined as CD4 count <350 cell/ mL, and in contrast to general population, it is not related with increased CVR nor with MS.

Como parte del proyecto A.P.A.T. (Asistencia y seguimiento al Paciente Anciano Traumatizado), desarrollamos un protocolo para optimizar la hemoglobina preoperatoria mediante ferroterapia endovenosa, en pacientes con fractura de cadera susceptible de intervención quirúrgica. Los objetivos del estudio fueron describir el perfil anémico de estos pacientes y determinar la influencia de la administración de hierro endovenoso sobre la transfusión de sangre alogénica (TSA), el número total de concentrados de hematíes administrados y la estancia media hospitalaria.

Treball de recerca on analitzo els elements socials i culturals que influeixen en la vivència de la maternitat, en el cas de tres dones. Em proposo intentar copsar de quina forma tres dones que han estat mares al segle XXI experimenten la seva pròpia maternitat i així intentar dilucidar fins a quin punt aquestes tres dones viuen aquesta situació personal present, en clau de passat. Quan escric passat, em refereixo al segle XIX, quan es va institucionalitzar el concepte de l'ideal maternal.

There are very few disease-specific studies focusing on outcomes of umbilical cord blood transplantation for Philadelphia chromosome-positive acute lymphoblastic leukemia. We report the outcome of 45 patients with Philadelphia chromosome-positive acute lymphoblastic leukemia who underwent myeloablative single unit cord blood transplantation from unrelated donors within the GETH/GITMO cooperative group. Conditioning regimens were based on combinations of thiotepa, busulfan, cyclophospamide or fludarabine, and antithymocyte globulin. At the time of transplantation, 35 patients (78%) were in first complete remission, four (8%) in second complete remission and six (14%) in third or subsequent response. The cumulative incidence of myeloid engraftment was 96% at a median time of 20 days and significantly better for patients receiving higher doses of CD34(+) cells. The incidence of acute grade II-IV graft-versus-host disease was 31%, while that of overall chronic graft-versus-host disease was 53%. Treatment-related mortality was 17% at day +100 and 31% at 5 years. The 5-year relapse, event-free survival and overall survival rates were 31%, 36% and 44%, respectively. Although the event-free and overall survival rates in patients without BCR/ABL transcripts detectable at time of transplant were better than those in whom BCR/ABL transcripts were detected (46% versus 24% and 60% versus 30%, respectively) these differences were not statistically significant in the univariate analysis (P=0.07). These results demonstrate that umbilical cord blood transplantation from unrelated donors can be a curative treatment for a substantial number of patients with Philadelphia chromosome-positive acute lymphoblastic leukemia.

BACKGROUND Measurement of HbA1c is the most important parameter to assess glycemic control in diabetic patients. Different point-of-care devices for HbA1c are available. The aim of this study was to evaluate two point-of-care testing (POCT) analyzers (DCA Vantage from Siemens and Afinion from Axis-Shield). We studied the bias and precision as well as interference from carbamylated hemoglobin. METHODS Bias of the POCT analyzers was obtained by measuring 53 blood samples from diabetic patients with a wide range of HbA1c, 4%-14% (20-130 mmol/mol), and comparing the results with those obtained by the laboratory method: HPLC HA 8160 Menarini. Precision was performed by 20 successive determinations of two samples with low 4.2% (22 mmol/mol) and high 9.5% (80 mmol/mol) HbA1c values. The possible interference from carbamylated hemoglobin was studied using 25 samples from patients with chronic renal failure. RESULTS The means of the differences between measurements performed by each POCT analyzer and the laboratory method (95% confidence interval) were: 0.28% (p<0.005) (0.10-0.44) for DCA and 0.27% (p<0.001) (0.19-0.35) for Afinion. Correlation coefficients were: r=0.973 for DCA, and r=0.991 for Afinion. The mean bias observed by using samples from chronic renal failure patients were 0.2 (range -0.4, 0.4) for DCA and 0.2 (-0.2, 0.5) for Afinion. Imprecision results were: CV=3.1% (high HbA1c) and 2.97% (low HbA1c) for DCA, CV=1.95% (high HbA1c) and 2.66% (low HbA1c) for Afinion. CONCLUSIONS Both POCT analyzers for HbA1c show good correlation with the laboratory method and acceptable precision.
