La malnutrició està present en el 30% dels pacients hospitalitzats. Aquest estat patològic causa una major aparició de complicacions intrahospitalàries, prolongació de l'estada hospitalària, i augment de la mortalitat. El nostre objectiu va ser avaluar el maneig de l'estat nutricional en el nostre servei (Medicina digestiva, de l'hospital La Fe). Per aconseguir-ho, es va realitzar un estudi observacional d'una cohort de pacients ingressats en el nostre servei, amb posterior anàlisi de les dades necessàries per a avaluar l'estat nutricional a nivell hospitalari (valors antropomètrics, valors analítics, proves de cribatge de risc malnutricional). S'han trobat dades que indiquen falta de conscienciació sobre la malnutrició per part del personal sanitari, i la influència negativa de la malnutrició sobre el pacient ingressat, encara que aquesta relació no aconsegueix nivells de significació estadística, sent necessari un augment de la grandària de la mostra per a obtindre dades significatives.

Donada la manca de consens davant el tractament de la uveítis crònica anterior (UCA) associada a la artritis idiopàtica juvenil (AIJ), aquest treball té com a objectiu valorar la possible eficàcia de la leflunomida (LFN) en aquesta malaltia. Per a això es va valorar la resposta al tractament de forma retrospectiva de 13 pacients amb UCA associada a AIJ de la cohort de 417 pacients de l'Hospital Universitari Vall d´Hebron. Els resultats obtinguts suggereixen que la LFN pot ser una alternativa vàlida en pacients amb AIJ no només com a tractament de l'afectació articular sinó també de l'afectació ocular.

L’objectiu del present estudi va ser estudiar a la població afecta de GMSI en l’àrea de salut 6 de València, així com la incidència de la mateixa. Es tracta d’un estudi descriptiu transversal retrospectiu observacional dels pacients amb aquesta patologia que van acudir al servei d’Hematologia de l’hospital de referència. Quant als resultats, encara que les dades analítiques i evolutives van ser similars a sèries anteriors (subtipus predominant IgG, 7.5% de progressió), el nombre de pacients i la incidència va ser menor de l'esperada, la qual cosa va restar pes estadístic a l'extrapolació dels resultats.

From January 1995 to August 1997 we evaluated prospectively the clinical presentation, laboratory findings and short-term survival of smear-positive pulmonary tuberculosis (TB) patients who sought care at our hospital. After providing informed, written consent, the patients were interviewed and laboratory tests were performed. Information about survivorship and death was collected through September 1998. Eighty-six smear-positive pulmonary TB patients were enrolled; 26.7% were HIV-seropositive. Seventeen HIV-seronegative pulmonary TB patients (19.8%) presented chronic diseases in addition to TB. In the multiple logistic regression analysis a CD4+ cell count <= 200 cell/mm³ was independently associated with HIV seropositivity. In the Cox regression model, fitted to all patients, HIV seropositivity and age > or = 50 years were independently associated with decreased survival. Among HIV-seronegative persons, the presence of an additional disease increased the risk of death of almost six-fold. Use of antiretroviral drugs was associated with a lower risk of death among HIV-seropositive smear-positive pulmonary TB patients (RH = 0.32, 95% CI 0.10-0.92). In our study smear-positive pulmonary TB patients had a low short-term survival rate that was strongly associated with HIV infection, age and co-morbidities. Therapy with antiretroviral drugs reduced the short-term risk of death among HIV-seropositive patients after TB diagnosis.

Aim. The study aimed at describing the evolution over a 6-year period of patients leaving the emergency department (ED) before being seen ("left without being seen" or LWBS) or against medical advice ("left against medical advice" or LAMA) and at describing their characteristics. Methods. A retrospective database analysis of all adult patients who are admitted to the ED, between 2005 and 2010, and who left before being evaluated or against medical advice, in a tertiary university hospital. Results. During the study period, among the 307,716 patients who were registered in the ED, 1,157 LWBS (0.4%) and 1,853 LAMA (0.9%) patients were identified. These proportions remained stable over the period. The patients had an average age of 38.5 ± 15.9 years for LWBS and 41.9 ± 17.4 years for LAMA. The median time spent in the ED before leaving was 102.4 minutes for the LWBS patients and 226 minutes for LAMA patients. The most frequent reason for LAMA was related to the excessive length of stay. Conclusion. The rates of LWBS and LAMA patients were low and remained stable. The patients shared similar characteristics and reasons for leaving were largely related to the length of stay or waiting time.

The main goal of psychosocial rehabilitation is to compensate the vulnerability underlying psychiatric disorders through intermediate institutions when the persistence and recurrence of these disorders have led to social and professional exclusion. Intermediate institutions refer to services which allow transition between the state of dependence on the hospital to the state of relative autonomy in social community. Psychosocial rehabilitation is a comprehensive approach which link the type of interventions: treatment, rehabilitation and support integrated in multimodal and individualized programs. A study of the out-patients followed by the rehabilitation unit of the psychiatric department in Lausanne has shown that provision of services is divided into 60% for rehabilitation, 20% for treatment and 20% for support independently of the psychiatric disorders. The implementation of these programs necessitates institutional support from psychiatric hospital to outpatient clinics through different types of facilities in order to offer a medical and psychosocial device of rehabilitation into the community

The Minister for Health requested the Chief Medical Officer to prepare a Report for him on issues that arose following a Primetime Investigates programme relating to Portlaoise Hospital Maternity Services (PHMS) on 30th January 2014. This Report provides a preliminary assessment of PHMS focusing on perinatal deaths (2006-date) and related matters. Through a series of recommendations it sets out the need for further examination or actions where the findings of this preliminary assessment suggest such a need.
