Abstract: Since ancient times, people have attributed a variety of health benefits to moderate consumption of fermented beverages such as wine and beer, often without any scientific basis. There is evidence that excessive or binge alcohol consumption is associated with increased morbidity and mortality, as well as with work related and traffic accidents. On the contrary, at the moment, several epidemiological studies have suggested that moderate consumption of alcohol reduces overall mortality, mainly from coronary diseases. However, there are discrepancies regarding the specific effects of different types of beverages (wine, beer and spirits) on the cardiovascular system and cancer, and also whether the possible protective effects of alcoholic beverages are due to their alcoholic content (ethanol) or to their non-alcoholic components (mainly polyphenols). Epidemiological and clinical studies have pointed out that regular and moderate wine consumption (one to two glasses a day) is associated with decreased incidence of cardiovascular disease (CVD), hypertension, diabetes, and certain types of cancer, including colon, basal cell, ovarian, and prostate carcinoma. Moderate beer consumption has also been associated with these effects, but to a lesser degree, probably because of beer"s lower phenolic content. These health benefits have mainly been attributed to an increase in antioxidant capacity, changes in lipid profiles, and the anti-inflammatory effects produced by these alcoholic beverages. This review summarizes the main protective effects on the cardiovascular system and cancer resulting from moderate wine and beer intake due mainly to their common components, alcohol and polyphenols.

Nicotine (NIC), the main psychostimulant compound of smoked tobacco, exerts its effects through activation of central nicotinic acetylcholine receptors (nAChR), which become up-regulated after chronic administration. Recent work has demonstrated that the recreational drug 3,4-methylenedioxymethamphetamine (MDMA) has affinity for nAChR and also induces up-regulation of nAChR in PC 12 cells. Tobacco and MDMA are often consumed together. In the present work we studied the in vivo effect of a classic chronic dosing schedule of MDMA in rats, alone or combined with a chronic schedule of NIC, on the density of nAChR and on serotonin reuptake transporters. MDMA induced significant decreases in [3H]paroxetine binding in the cortex and hippocampus measured 24 h after the last dose and these decreases were not modified by the association with NIC. In the prefrontal cortex, NIC and MDMA each induced significant increases in [3H]epibatidine binding (29.5 and 34.6%, respectively) with respect to saline-treated rats, and these increases were significantly potentiated (up to 72.1%) when the two drugs were associated. Also in this area, [3H]methyllycaconitine binding was increased a 42.1% with NIC + MDMA but not when they were given alone. In the hippocampus, MDMA potentiated the a7 regulatory effects of NIC (raising a 25.5% increase to 52.5%) but alone was devoid of effect. MDMA had no effect on heteromeric nAChR in striatum and a coronal section of the midbrain containing superior colliculi, geniculate nuclei, substantia nigra and ventral tegmental area. Specific immunoprecipitation of solubilised receptors suggests that the up-regulated heteromeric nAChRs contain a4 and b2 subunits. Western blots with specific a4 and a7 antibodies showed no significant differences between the groups, indicating that, as reported for nicotine, up-regulation caused by MDMA is due to post-translational events rather than increased receptor synthesis.

The analysis of the activity of neuronal cultures is considered to be a good proxy of the functional connectivity of in vivo neuronal tissues. Thus, the functional complex network inferred from activity patterns is a promising way to unravel the interplay between structure and functionality of neuronal systems. Here, we monitor the spontaneous self-sustained dynamics in neuronal cultures formed by interconnected aggregates of neurons (clusters). Dynamics is characterized by the fast activation of groups of clusters in sequences termed bursts. The analysis of the time delays between clusters' activations within the bursts allows the reconstruction of the directed functional connectivity of the network. We propose a method to statistically infer this connectivity and analyze the resulting properties of the associated complex networks. Surprisingly enough, in contrast to what has been reported for many biological networks, the clustered neuronal cultures present assortative mixing connectivity values, meaning that there is a preference for clusters to link to other clusters that share similar functional connectivity, as well as a rich-club core, which shapes a"connectivity backbone" in the network. These results point out that the grouping of neurons and the assortative connectivity between clusters are intrinsic survival mechanisms of the culture.

Hypertension is a major public health problem and a leading cause of death and disability in both developed and developing countries, affecting onequarter of the world"s adult population. Our aim was to evaluate whether the consumption of gazpacho, a Mediterranean vegetable-based cold soup rich in phytochemicals, is associated with lower blood pressure (BP) and/or reduced prevalence of hypertension in individuals at high cardiovascular risk. Methods and results: We selected 3995 individuals (58% women, mean age 67 y) at high cardiovascular risk (81% hypertensive) recruited into the PREDIMED study. BP, weight, and dietary and physical activity data were collected. In multivariate linear regression analyses, after adjustment, moderate and high gazpacho consumption categories were associated with reduced mean systolic BP of 1.9 mm Hg [95% confidence interval (CI): 3.4; 0.6] and 2.6 mm Hg (CI: 4.2; 1.0), respectively, and reduced diastolic BP of 1.5 mm Hg (CI: 2.3; 0.6) and 1.9 mm Hg (CI: 2.8; 1.1). By multiple-adjusted logistic regression analysis, gazpacho consumption was associated with a lower prevalence of hypertension, with OR Z 0.85 (CI: 0.73; 0.99) for each 250 g/week increase and OR Z 0.73 (CI: 0.55; 0.98) for high gazpacho consumption groups compared to the no-consumption group. Conclusions: Gazpacho consumption was inversely associated with systolic and diastolic BP and prevalence of hypertension in a cross-sectional Mediterranean population at high cardiovascular risk. The association between gazpacho intake and reduction of BP is probably due to synergy among several bioactive compounds present in the vegetable ingredients used to make the recipe.

Background: Few clinical studies have focused on the alcoholindependent cardiovascular effects of the phenolic compounds of red wine (RW). Objective: We aimed to evaluate the effects of ethanol and phenolic compounds of RW on the expression of inflammatory biomarkers related to atherosclerosis in subjects at high risk of cardiovascular disease. Design: Sixty-seven high-risk, male volunteers were included in a randomized, crossover consumption trial. After a washout period, all subjects received RW (30 g alcohol/d), the equivalent amount of dealcoholized red wine (DRW), or gin (30 g alcohol/d) for 4 wk. Before and after each intervention period, 7 cellular and 18 serum inflammatory biomarkers were evaluated. Results: Alcohol increased IL-10 and decreased macrophage-derived chemokine concentrations, whereas the phenolic compounds of RW decreased serum concentrations of intercellular adhesion molecule- 1, E-selectin, and IL-6 and inhibited the expression of lymphocyte function-associated antigen 1 in T lymphocytes and macrophage-1 receptor, Sialil-Lewis X, and C-C chemokine receptor type 2 expression in monocytes. Both ethanol and phenolic compounds of RW downregulated serum concentrations of CD40 antigen, CD40 ligand, IL-16, monocyte chemotactic protein-1, and vascular cell adhesion molecule-1. Conclusion: The results suggest that the phenolic content of RW may modulate leukocyte adhesion molecules, whereas both ethanol and polyphenols of RW may modulate soluble inflammatory mediators in high-risk patients. The present work describes the development of a fast and robust analytical method for the determination of 53 antibiotic residues, covering various chemical groups and some of their metabolites, in environmental matrices that are considered important sources of antibiotic pollution, namely hospital and urban wastewaters, as well as in river waters. The method is based on automated off-line solid phase extraction (SPE) followed by ultra-high-performance liquid chromatography coupled to quadrupole linear ion trap tandem mass spectrometry (UHPLC–QqLIT). For unequivocal identification and confirmation, and in order to fulfill EU guidelines, two selected reaction monitoring (SRM) transitions per compound are monitored (the most intense one is used for quantification and the second one for confirmation). Quantification of target antibiotics is performed by the internal standard approach, using one isotopically labeled compound for each chemical group, in order to correct matrix effects. The main advantages of the method are automation and speed-up of sample preparation, by the reduction of extraction volumes for all matrices, the fast separation of a wide spectrum of antibiotics by using ultra-high-performance liquid chromatography, its sensitivity (limits of detection in the low ng/L range) and selectivity (due to the use of tandem mass spectrometry) The inclusion of β-lactam antibiotics (penicillins and cephalosporins), which are compounds difficult to analyze in multi-residue methods due to their instability in water matrices, and some antibiotics metabolites are other important benefits of the method developed. As part of the validation procedure, the method developed was applied to the analysis of antibiotics residues in hospital, urban influent and effluent wastewaters as well as in river water samples

Newly generated olfactory receptor axons grow from the peripheral to the central nervous system aided by olfactory ensheathing cells (OECs). Thus, OEC transplantation has emerged as a promising therapy for spinal cord injuries and for other neural diseases. However, these cells do not present a uniform population, but, instead, a functionally heterogeneous population that exhibits a variety of responses including adhesion, repulsion and crossover during cell-cell and cell-matrix interactions. Some studies report that the migratory properties of OECs are compromised by inhibitory molecules and potentiated by chemical gradients. Here, we demonstrated that rodent OECs express all the components of the Nogo Receptor complex and that their migration is blocked by Myelin. Next, we used cell tracking and traction force microscopy to analyze OEC migration and its mechanical properties over Myelin. Our data relate the absence of traction force of OEC with lower migratory capacity, which correlates with changes in the F-Actin cytoskeleton and focal adhesion distribution. Lastly, OEC traction force and migratory capacity is enhanced after cell incubation with the Nogo Receptor inhibitor NEP1-40.

Objetivo: Este artículo presenta los resultados del análisis del peso que cada dominio competencial del instrumento COM-VA© tiene en función del área asistencial hospitalaria en la que el profesional de enfermería presta sus cuidados. Método: El formulario de recogida de datos para la variable"peso" contenía una matriz con los dominios competenciales del instrumento COM-VA© y las diferentes áreas de cuidados. Los participantes debían asignar un peso a cada dominio, repartiendo el porcentaje total en las diferentes competencias. El análisis de los datos incluyó la descriptiva de las variables demográficas. Las medidas de tendencia central fueron empleadas para el análisis de los pesos competenciales. Las diferencias se consideraron significativas si p < 0,05. Resultados: Se obtuvieron un total del 78,7% de respuestas correctamente cumplimentadas. Los resultados de la ponderación media de cada una de las competencias varían ostensiblemente en función del área de cuidados. Las diferencias de peso competencial por área de cuidados son estadísticamente significativas, excepto para la competencia 1:"Cuidar" y 6:"Trabajar en equipo y adaptarse a un entorno cambiante". Conclusiones: Es recomendable incluir en los sistemas de evaluación de competencias la variable ponderación por área, para garantizar una evaluación de la ejecución competencial más ajustada y equitativa.

El contexto organizativo para la prestación de cuidados incluye los factores organizativos y estructurales que facilitan la práctica profesional enfermera y tienen un impacto significativo en los resultados de los pacientes y de los centros. Objetivo: analizar el contexto organizativo para la prestación de cuidados en los hospitales del sistema público de salud. Método: los datos se recogieron en los talleres organizados por la Coordinación de Enfermería del Institut Català de la Salut, empleando un cuestionario que contenía los elementos del Nursing Work Index-Revised. La estrategia de análisis es eminentemente descriptiva, incluyendo también la exploración de la correlación entre las subescalas del instrumento. Resultados: se analizaron 405 cuestionarios. El resultado principal del estudio indica un bajo grado de desarrollo del contexto organizativo en los hospitales estudiados. Los factores organizativos más desarrollados son la"Autonomía" y el"Control sobre la práctica enfermera". Los factores organizativos menos desarrollados incluyen el"Reconocimiento profesional" y la"Formación". Ninguno de los factores evaluados obtuvo una puntuación sobresaliente. Conclusiones: el Nursing Work Index-Revised es una buena herramienta para efectuar una estimación aproximada del contexto organizativo para la prestación de cuidados. Los gestores deberían hacer una profunda reflexión sobre el coste de no considerar los aspectos que facilitan la práctica enfermera en los hospitales.

Objetivo: analizar la relación entre los elementos del instrumento COM_VA©(competencia y valoración) de evaluación de competencias asistenciales con los niveles de pericia, de aprendiz a experto, mediante el establecimiento del umbral de competencia. Método: estudio descriptivo transversal con una muestra de más de 400 enfermeras que respondieron a un cuestionario estructurado en el que debían asociar a cada elemento competencial un nivel de pericia, siendo 1 aprendiz y 5 expertos. El análisis estadístico se realizó con la determinación de medidas de tendencia central (porcentajes, media, varianza, moda y desviación típica). Resultados: los elementos competenciales que requieren un mayor grado de pericia se incluyen en las competencias: (4) contribuir a garantizar la seguridad y el proceso asistencial, (2) valorar, diagnosticar y abordar situaciones clínicas cambiantes, (3) ayudar al paciente a cumplir el tratamiento y (5) facilitar los procesos de adaptación y afrontamiento. El umbral mínimo identificado es de 2.3 (principiante) y el máximo de 3,9 (proeficiente). Ninguno de los elementos competenciales se asocia a un umbral extremo (aprendiz/ experto). Conclusiones: el umbral de pericia identifica nivel mínimo necesario de interacción de conocimientos, habilidades y actitudes para la toma de decisiones en la práctica asistencial. Los elementos identificados con un mayor umbral de pericia se corresponden con contenidos competenciales que precisan de una desarrollada habilidad de juicio clínico, una marcada capacidad analítica, un amplio conocimiento disciplinar y una importante capacidad de juicio intuitivo.

Objetivos: Este artículo tiene un doble objetivo: presentar la segunda parte del marco conceptual empleado en el desarrollo del proyecto COM VA© y los resultados del proceso de definición de un instrumento de competencias asistenciales del profesional de enfermería en el ámbito hospitalario, que enfatiza la participación de los profesionales, la reflexión crítica individual y grupal, y el compromiso profesional. El marco conceptual explora los significados de los términos toma de decisiones y ámbito de responsabilidad, competencias, estándares y normas de la práctica, dominio competencial, elemento competencial y matriz de competencias. Material y método: Para el proceso de definición del marco competencial se empleó el modelo de investigación-acción participativa. La muestra incluyó a 487 profesionales de enfermería hospitalarios. Los resultados del análisis definen el ámbito de competencias asistenciales en 6 dominios: 1) cuidar al enfermo; 2) valorar, diagnosticar y abordar situaciones clínicas cambiantes; 3) ayudar al enfermo a cumplir el tratamiento; 4) contribuir a garantizar la seguridad y el proceso asistencial; 5) facilitar el proceso de adaptación y afrontamiento, y 6) trabajar en equipo y adaptarse a un entorno cambiante. Cada dominio queda finalmente definido por 5 elementos competenciales que explican su significado. Cada elemento competencial contiene una definición que incluye las principales intervenciones y actuaciones enfermeras que lo configuran y las conductas que conforman los criterios de valoración de la ejecución competencial.

Objetivo: explorar la identificación de los factores individuales de complejidad de cuidados en enfermos hospitalizados. Método: se empleó un diseño cualitativo de investigación-acción participativa con el método de análisis directo de contenido de los breves relatos de los participantes, enfermeras y enfermeros de los hospitales del lnstituto Catalán de la Salud, sobre casos en los que hubieran experimentado situaciones de complejidad. La suficiencia muestral se estableció a partir del criterio de saturación de la información. Los participantes fueron invitados a formar parte de una ronda de talleres/sesiones de discusión que se hicieron durante 18 meses. Uno de los investigadores recogía por escrito las opiniones de los participantes, mientras el otro moderaba el debate y realizaba preguntas reflexivas sobre el contenido de la propuesta. Posteriormente se invitaba a los participantes a organizarse en pequeños grupos para discutir y registrar en un fomulario individual, breves narrativas sobre casos en los que hubieran experimentado situaciones de complejidad. Resultados: el número de relatos breves incluidos en el análisis final fue de 287. Los factores individuales de complejidad de cuidados incluye cinco dominios: (1) evolutivo, (2) mental y cognitivo, (3) psicoemocional, (4) sociocultural y (5) comorbilidad y complicaciones. La complejidad individual de cuidados se estructura fuentes, factores y especificaciones. Conclusiones: de los cinco ejes de complejidad identificados en el Modelo Vectorial de Complejidad de Safford, cuatro coinciden parcialmente con el análisis presentado. La arquitectura de la complejidad de cuidados debería incluir una consideración multiperspectiva, incluyendo los ejes de complejidad individual, terapéutica-procedimental y organizativa.
