BACKGROUND: Health professionals need to be integrated more effectively in clinical research to ensure that research addresses clinical needs and provides practical solutions at the coal face of care. In light of limited evidence on how best to achieve this, evaluation of strategies to introduce, adapt and sustain evidence-based practices across different populations and settings is required. This project aims to address this gap through the co-design, development, implementation, evaluation, refinement and ultimately scale-up of a clinical research engagement and leadership capacity building program in a clinical setting with little to no co-ordinated approach to clinical research engagement and education.

METHODS/DESIGN: The protocol is based on principles of research capacity building and on a six-step framework, which have previously led to successful implementation and long-term sustainability. A mixed methods study design will be used. Methods will include: (1) a review of the literature about strategies that engage health professionals in research through capacity building and/or education in research methods; (2) a review of existing local research education and support elements; (3) a needs assessment in the local clinical setting, including an online cross-sectional survey and semi-structured interviews; (4) co-design and development of an educational and support program; (5) implementation of the program in the clinical environment; and (6) pre- and post-implementation evaluation and ultimately program scale-up. The evaluation focuses on research activity and knowledge, attitudes and preferences about clinical research, evidence-based practice and leadership and post implementation, about their satisfaction with the program. The investigators will evaluate the feasibility and effect of the program according to capacity building measures and will revise where appropriate prior to scale-up.

DISCUSSION: It is anticipated that this clinical research engagement and leadership capacity building program will enable and enhance clinically relevant research to be led and conducted by health professionals in the health setting. This approach will also encourage identification of areas of clinical uncertainty and need that can be addressed through clinical research within the health setting.

AIM: Existing practice strategies for actively involving patients in care during hospitalisation are poorly understood. The aim of this study was to explore how healthcare professionals engaged patients in communication associated with care transitions.

METHOD: An instrumental, collective case study approach was used to generate empirical data about patient transitions in care. A purposive sample of key stakeholders representing (i) patients and their families; (ii) hospital discharge planning team members; and (iii) healthcare professionals was recruited in five Australian health services. Individual and group semi-structured interviews were conducted to elicit detailed explanations of patient engagement in transition planning. Interviews lasted between 30 and 60 minutes and were digitally recorded and transcribed verbatim. Data collection and analysis were conducted simultaneously and continued until saturation was achieved. Thematic analysis was undertaken.

RESULTS: Five themes emerged as follows: (i) organisational commitment to patient engagement; (ii) the influence of hierarchical culture and professional norms on patient engagement; (iii) condoning individual healthcare professionals' orientations and actions; (iv) understanding and negotiating patient preferences; and (v) enacting information sharing and communication strategies. Most themes illustrated how patient engagement was enabled; however, barriers also existed.

CONCLUSION: Our findings show that strong organisational and professional commitment to patient-centred care throughout the organisation was a consistent feature of health services that actively engaged patients in clinical communication. Understanding patients' needs and preferences and having both formal and informal strategies to engage patients in clinical communication were important in how this involvement occurred.

Industry expects a creative and innovative academic practice that provides students with valuable practical knowledge focused on graduate ready skills for future careers. The learning environment in engineering is inadequate for students to become a skillful graduate. The practical role of engineering is gained through working on real world problems in an industry collaborative environment through projects. Industry academia collaboration seems to be actively increasing in the development of engineering education in various parts of the globe. The close relationship between industry and academia is a vital component of the engineering pedagogy to improve student engagement in industry through projects. By engaging students with industry, students will acquire global perspective about the core attributes expected in future engineering jobs. In today’s large-scale industrial market, companies tend to prefer graduates with design skills attained through the project approach. Thus, universities should open their doors and accept the challenges of interacting with students with industrial experiences and expectations. This paper is focused on improving student industry engagement through project/design oriented curriculum. Through quantitative and qualitative research, the paper shows the industry perspectives and students views on university and industry collaboration. The research results show that students and industry can possibly maintain their engagement by providing regular feedback, reviewing goals and objectives, improving communication, keeping focused, and sharing a similar vision.

This work demonstrates a model-driven approach to the development of care plan systems, amenable to: (a) a flexible and extensible definition of care plan scope; and (b) deployment of care plan viewing and tracking functionality to a wide range of physical computing devices. The approach utilises a care plan domain model from which guideline implementers formulate care plan templates aligning to specific clinical guidelines. A clinical end user would subsequently constrain that template (e.g., selecting a subset of available activities and specific targets) to create a care plan instance for an individual patient. An XML care plan visualisation definition created using the Marama tool is transformed to OpenLaszlo script from which Shockwave Flash objects can be compiled, creating Flash applications that run on a variety of hardware for both clinical and patient users. The approach is illustrated with respect to an overweight and obesity guideline.

BACKGROUND: Despite increased use of dietary pattern methods in nutritional epidemiology, there have been few direct comparisons of methods. Older adults are a particularly understudied population in the dietary pattern literature. This study aimed to compare dietary patterns derived by principal component analysis (PCA) and cluster analysis (CA) in older adults and to examine their associations with socio-demographic and health behaviours. METHODS: Men (n = 1888) and women (n = 2071) aged 55-65 years completed a 111-item food frequency questionnaire in 2010. Food items were collapsed into 52 food groups and dietary patterns were determined by PCA and CA. Associations between dietary patterns and participant characteristics were examined using Chi-square analysis. The standardised PCA-derived dietary patterns were compared across the clusters using one-way ANOVA. RESULTS: PCA identified four dietary patterns in men and two dietary patterns in women. CA identified three dietary patterns in both men and women. Men in cluster 1 (fruit, vegetables, wholegrains, fish and poultry) scored higher on PCA factor 1 (vegetable dishes, fruit, fish and poultry) and factor 4 (vegetables) compared to factor 2 (spreads, biscuits, cakes and confectionery) and factor 3 (red meat, processed meat, white-bread and hot chips) (mean, 95 % CI; 0.92, 0.82-1.02 vs. 0.74, 0.63-0.84 vs. -0.43, -0.50- -0.35 vs. 0.60 0.46-0.74, respectively). Women in cluster 1 (fruit, vegetables and fish) scored highest on PCA factor 1 (fruit, vegetables and fish) compared to factor 2 (processed meat, hot chips cakes and confectionery) (1.05, 0.97-1.14 vs. -0.14, -0.21- -0.07, respectively). Cluster 3 (small eaters) in both men and women had negative factor scores for all the identified PCA dietary patterns. Those with dietary patterns characterised by higher consumption of red and processed meat and refined grains were more likely to be Australian-born, have a lower level of education, a higher BMI, smoke and did not meet physical activity recommendations (all P < 0.05). CONCLUSIONS: PCA and CA identified comparable dietary patterns within older Australians. However, PCA may provide some advantages compared to CA with respect to interpretability of the resulting dietary patterns. Older adults with poor dietary patterns also displayed other negative lifestyle behaviours. Food-based dietary pattern methods may inform dietary advice that is understood by the community.

 This research constructs an approach to engagement with Bama, Indigenous people from the Upper Mitchell River Watershed to develop a model of increased engagement and improved care of natural resource environments. It demonstrates that using Indigenous knowledge and experience can lead to better outcomes for both the land and its peoples.

Classification of electrocardiogram (ECG) data stream is essential to diagnosis of critical heart conditions. It is vital to accurately detect abnormality in the ECG in order to prevent possible beginning of life-threatening cardiac symptoms. In this paper, we focus on identifying premature ventricular contraction (PVC) which is one of the most common heart rhythm abnormalities. We use "Replacing" strategy to check the effects of each individual heartbeat on the variation of principal directions. Based on this idea, an online PVC detection method is proposed to classify the new arriving PVC beats in the real-time and online manner. The proposed approach is tested on the MIT-BIH arrhythmia database (MIT-BIH-AR). The PVC detection accuracy was 98.77%, with the sensitivity and positive predictivity of 96.12% and 86.48%, respectively. These results are an improvement on previous reported results for PVC detection. In addition, our proposed method is effective in terms of computation time. The average execution time of our proposed method was 3.83 s for a 30 min ECG recording. It shows the capability of the classifier to detect abnormal PVCs in online manner.

O MHC (Major Histocompatibility Complex) é um sistema genético importante para a manutenção de espécies ameaçadas, uma vez que baixa variabilidade para locos MHC tem sido associada a uma menor capacidade de resposta a doenças e diminuição do sucesso reprodutivo. Deste modo, pesquisas sobre a variabilidade genética do MHC têm demonstrado ser bastante informativas em estudos populacionais voltados para aspectos referentes à conservação. No presente trabalho foi investigada a variabilidade genética do MHC para três espécies de mamíferos marinhos (toninha, baleia franca austral e lobo marinho sul-americano) do sul do Brasil, com intensa mortalidade provocada por atividades humanas atuais ou passadas. As amostras foram coletadas de animais mortos encalhados na costa, de animais capturados acidentalmente por barcos pesqueiros, e também através de um sistema de biópsia. A região variável do exon 2 do gene DQB do MHC foi amplificada por PCR (Polymerase Chain Reaction) em 109 amostras de toninhas (Rio de Janeiro n=32, Rio Grande do Sul n=52, Argentina n=25), 35 amostras de lobo marinho sul-americano e 30 amostras de baleia franca austral, utilizando-se um par de primers heterólogos. O fragmento resultante de 172 pares de bases foi analisado quanto ao polimorfismo de seqüência através da técnica de SSCP (Polimorfismo de Conformação de Fita Simples) em todas as amostras de toninha e de lobo marinho sul-americano e 14 amostras de baleia franca austral. Dificuldades associadas à amplificação resultaram em padrões de SSCP pouco informativos para as amostras de lobo marinho sul-americano e baleia franca austral Todas as amostras de toninha apresentaram um padrão de pelo menos 4 bandas por indivíduo. As 4 bandas de um único indivíduo do Rio Grande do Sul foram seqüenciadas, tendo sido possível verificar que 2 seqüências relacionadas ao genes DQB estão sendo amplificadas com estes primers. Pelas análises de SSCP foi possível detectar ausência de variabilidade para as amostras de toninha provenientes do Rio de Janeiro e diferenciá-las da população da Argentina, que é polimórfica. A população do Rio Grande do Sul parece apresentar níveis intermediários de variação em relação aos extremos da distribuição da espécie. Analisando as três populações amostradas, conclui-se que a espécie apresenta baixos níveis de variabilidade para o loco DQB, a exemplo do que é reportado para os genes de MHC de outros mamíferos marinhos.

The government has two objectives in this economy: make the states invest in thepriority sector and equalize wealth among states. Applying the model of the Principal-Agent Problem, we obtain that the federal system may not increase society 's wellfare when the states not necessarily invest in its respective thepriority sector. We also obtain that it is possible to implement an optimal mechanism where government equalize wealth among states without cost and can make states invest in thepriority sector.

A separação do controle da gestão leva a um conflito de objetivos entre acionistas e gestores, conflito este já identificado pela teoria da agência. Assim como ocorre em empresas privadas, as empresas públicas também apresentam este clássico problema. Enquanto os controladores últimos das empresas públicas (a sociedade) gostariam que estas maximizassem o bem estar social, seus gestores têm como objetivo maximizar suas próprias funções utilidade. O alinhamento de objetivos através de contratos de incentivos é uma das possíveis soluções para se resolver este problema de agência. As empresas públicas possuem nas promoções e cargos comissionados (modelo de Prendergast) uma possível solução para este dilema. Ocorre que nas empresas públicas o contrato de promoções é distorcido pela influência política, fato que diminui o poder de alinhamento de interesses que haveria em outras situações. Esta dissertação tem, portanto, como objetivo analisar, através de um modelo principal-agente, o atual modelo de promoções ofertado pelo governo (principal). Na primeira parte é apresentado um breve arcabouço teórico e, na segunda, analisamos os principais contratos de incentivo existentes. No terceiro capítulo, através de um modelo principal-agente, mostramos como a influência política distorce os incentivos que haveria neste tipo de contratos em outras situações. Na parte final deste trabalho indicamos possíveis caminhos para reduzir o problema de agência existente.
