Summary Purpose: Status epilepticus (SE) that is resistant to two antiepileptic compounds is defined as refractory status epilepticus (RSE). In the few available retrospective studies, estimated RSE frequency is between 31% and 43% of patients presenting an SE episode; almost all seem to require a coma induction for treatment. We prospectively assessed RSE frequency, clinical predictors, and outcome in a tertiary clinical setting. Methods: Over 2 years we collected 128 consecutives SE episodes (118 patients) in adults. Clinical data and their relationship to outcome (mortality and return to baseline clinical conditions) were analyzed. Results: Twenty-nine of 128 SE episodes (22.6%) were refractory to first- and second-line antiepileptic treatments. Severity of consciousness impairment and de novo episodes were independent predictors of RSE. RSE showed a worse outcome than non-RSE (39% vs. 11% for mortality; 21% vs. 63% for return to baseline clinical conditions). Only 12 patients with RSE (41%) required coma induction for treatment. Discussion: This prospective study identifies clinical factors predicting the onset of SE refractoriness. RSE appears to be less frequent than previously reported in retrospective studies; furthermore, most RSE episodes were treated outside the intensive care unit (ICU). Nonetheless, we confirm that RSE is characterized by high mortality and morbidity.

O objetivo deste trabalho foi avaliar o efeito da suplementação com sais proteinados sobre o desempenho de novilhos em pastagem nativa diferida, no Rio Grande do Sul, Brasil. Os suplementos de sal proteinado avaliados foram: com uréia, com amiréia, com amiréia mais levedura e sal mineral. O experimento teve duração de 118 dias e foram utilizadas 8 parcelas com área de 7,5 ha, cada uma com 8 novilhos, de peso médio 264 kg, com idade de 18 meses, em delineamento completamente casualizado. A pastagem apresentou valores médios de 6,8% de proteína bruta, 73,3% de fibra em detergente neutro, e 42,5% de digestibilidade in vitro da matéria orgânica, sem que fossem detectadas diferenças significativas entre tratamentos. O ganho médio diário (0,287 kg) dos animais suplementados com o sal proteinado com amiréia e levedura, foi superior ao apresentado pelos animais que consumiram sal mineralizado (0,019 kg) mas não houve diferenças entre uréia (0,159 kg) e amiréia (0,124 kg). O consumo diário dos suplementos proteinados (0,400 kg) foi superior ao consumo do suplemento mineral (0,038 kg). A adição de levedura ativa, ao sal proteinado formulado com amiréia, melhora o desempenho de novilhos em pastagem nativa diferida.

Abstract Background: Effective promotion of exercise could result in substantial savings in healthcare cost expenses in terms of direct medical costs, such as the number of medical appointments. However, this is hampered by our limited knowledge of how to achieve sustained increases in physical activity. Objectives: To assess the effectiveness of a Primary Health Care (PHC) based physical activity program in reducing the total number of visits to the healthcare center among inactive patients, over a 15-month period. Research Design: Randomized controlled trial. Subjects: Three hundred and sixty-two (n = 362) inactive patients suffering from at least one chronic condition were included. One hundred and eighty-three patients (n = 183; mean (SD); 68.3 (8.8) years; 118 women) were randomly allocated to the physical activity program (IG). One hundred and seventy-nine patients (n = 179; 67.2 (9.1) years; 106 women) were allocated to the control group (CG). The IG went through a three-month standardized physical activity program led by physical activity specialists and linked to community resources. Measures: The total number of medical appointments to the PHC, during twelve months before and after the program, was registered. Self-reported health status (SF-12 version 2) was assessed at baseline (month 0), at the end of the intervention (month 3), and at 12 months follow-up after the end of the intervention (month 15). Results: The IG had a significantly reduced number of visits during the 12 months after the intervention: 14.8 (8.5). The CG remained about the same: 18.2 (11.1) (P = .002). Conclusions: Our findings indicate that a 3-month physical activity program linked to community resources is a shortduration, effective and sustainable intervention in inactive patients to decrease rates of PHC visits. BACKGROUND:: Voltage-gated sodium channels dysregulation is important for hyperexcitability leading to pain persistence. Sodium channel blockers currently used to treat neuropathic pain are poorly tolerated. Getting new molecules to clinical use is laborious. We here propose a drug already marketed as anticonvulsant, rufinamide. METHODS:: We compared the behavioral effect of rufinamide to amitriptyline using the Spared Nerve Injury neuropathic pain model in mice. We compared the effect of rufinamide on sodium currents using in vitro patch clamp in cells expressing the voltage-gated sodium channel Nav1.7 isoform and on dissociated dorsal root ganglion neurons to amitriptyline and mexiletine. RESULTS:: In naive mice, amitriptyline (20 mg/kg) increased withdrawal threshold to mechanical stimulation from 1.3 (0.6-1.9) (median [95% CI]) to 2.3 g (2.2-2.5) and latency of withdrawal to heat stimulation from 13.1 (10.4-15.5) to 30.0 s (21.8-31.9), whereas rufinamide had no effect. Rufinamide and amitriptyline alleviated injury-induced mechanical allodynia for 4 h (maximal effect: 0.10 ± 0.03 g (mean ± SD) to 1.99 ± 0.26 g for rufinamide and 0.25 ± 0.22 g to 1.92 ± 0.85 g for amitriptyline). All drugs reduced peak current and stabilized the inactivated state of voltage-gated sodium channel Nav1.7, with similar effects in dorsal root ganglion neurons. CONCLUSIONS:: At doses alleviating neuropathic pain, amitriptyline showed alteration of behavioral response possibly related to either alteration of basal pain sensitivity or sedative effect or both. Side-effects and drug tolerance/compliance are major problems with drugs such as amitriptyline. Rufinamide seems to have a better tolerability profile and could be a new alternative to explore for the treatment of neuropathic pain.

BACKGROUND: Findings from randomised trials have shown a higher early risk of stroke after carotid artery stenting than after carotid endarterectomy. We assessed whether white-matter lesions affect the perioperative risk of stroke in patients treated with carotid artery stenting versus carotid endarterectomy. METHODS: Patients with symptomatic carotid artery stenosis included in the International Carotid Stenting Study (ICSS) were randomly allocated to receive carotid artery stenting or carotid endarterectomy. Copies of baseline brain imaging were analysed by two investigators, who were masked to treatment, for the severity of white-matter lesions using the age-related white-matter changes (ARWMC) score. Randomisation was done with a computer-generated sequence (1:1). Patients were divided into two groups using the median ARWMC. We analysed the risk of stroke within 30 days of revascularisation using a per-protocol analysis. ICSS is registered with controlled-trials.com, number ISRCTN 25337470. FINDINGS: 1036 patients (536 randomly allocated to carotid artery stenting, 500 to carotid endarterectomy) had baseline imaging available. Median ARWMC score was 7, and patients were dichotomised into those with a score of 7 or more and those with a score of less than 7. In patients treated with carotid artery stenting, those with an ARWMC score of 7 or more had an increased risk of stroke compared with those with a score of less than 7 (HR for any stroke 2·76, 95% CI 1·17-6·51; p=0·021; HR for non-disabling stroke 3·00, 1·10-8·36; p=0·031), but we did not see a similar association in patients treated with carotid endarterectomy (HR for any stroke 1·18, 0·40-3·55; p=0·76; HR for disabling or fatal stroke 1·41, 0·38-5·26; p=0·607). Carotid artery stenting was associated with a higher risk of stroke compared with carotid endarterectomy in patients with an ARWMC score of 7 or more (HR for any stroke 2·98, 1·29-6·93; p=0·011; HR for non-disabling stroke 6·34, 1·45-27·71; p=0·014), but there was no risk difference in patients with an ARWMC score of less than 7. INTERPRETATION: The presence of white-matter lesions on brain imaging should be taken into account when selecting patients for carotid revascularisation. Carotid artery stenting should be avoided in patients with more extensive white-matter lesions, but might be an acceptable alternative to carotid endarterectomy in patients with less extensive lesions. BACKGROUND & AIMS: Recent studies have described a major impact of genetic variations near the IL28B gene on the natural course and outcome of antiviral therapy in chronic hepatitis C. We therefore, aimed to explore the impact of donor and recipient genotypes of these polymorphisms on hepatitis C virus (HCV) liver graft reinfection. METHODS: Donor and recipient genotypes of IL28B rs12979860C>T single nucleotide polymorphism were determined in 91 patients with HCV liver graft reinfection, 47 of whom were treated with pegylated interferon-α (PEG-IFN-α) and ribavirin. IL28B genetic polymorphisms were correlated with the natural course and treatment outcome of recurrent hepatitis C. RESULTS: Patients requiring liver transplantation due to end-stage chronic hepatitis C appeared to be selected toward the adverse genotypes rs12979860 CT/TT compared to non-transplanted HCV-infected patients (p=0.046). Patients with the donor genotype rs12979860 CC had higher peak ALT and HCV RNA serum concentrations than those with CT/TT (p=0.04 and 0.06, respectively). No association was observed between ALT/HCV RNA serum concentrations and recipient genotypes (p>0.3). More important, donor IL28B rs12979860 CC vs. CT/TT genotypes were associated with rapid, complete early, and sustained virologic response (RVR, cEVR, SVR) to treatment with PEG-IFN-α and ribavirin (p=0.003, 0.0012, 0.008, respectively), but weaker associations of recipient genotypes with RVR, cEVR, and SVR were observed as well (p=0.0046, 0.115, 0.118, respectively). CONCLUSIONS: We provide evidence for a dominant, but not exclusive impact of the donor rather than the recipient IL28B genetic background on the natural course and treatment outcome of HCV liver graft reinfection.

O objetivo deste trabalho foi comparar o crescimento e a habilidade competitiva de dois biótipos de capim-colchão (Digitaria ciliaris), um resistente (R) e outro suscetível (S) aos herbicidas inibidores da acetil coenzima A carboxilase. O crescimento dos biótipos foi determinado pela coleta da matéria seca das plantas, aos 14, 21, 25, 28, 34, 42, 49, 57, 65, 72, 78, 86, 101, 111 e 118 dias após emergência (DAE). Os dados de massa de matéria seca foram ajustados ao modelo logístico e, também, utilizados para a obtenção da taxa de crescimento absoluto. Para avaliar a habilidade competitiva intra-específica e interespecífica, foram instalados cinco experimentos com o uso do método substitutivo. Compararam-se os biótipos R e S entre si e cada um destes com a cultura da soja, quando semeada no mesmo dia ou sete dias após a semeadura das plantas daninhas. As proporções de plantas entre as espécies ou biótipos utilizados foram: 5:0; 4:1; 3:2; 2:3; 1:4 e 0:5. Os biótipos de capim-colchão apresentaram acúmulo de matéria seca, crescimento absoluto e competição interespecífica semelhantes, e a redução da matéria seca da soja foi similar na presença dos biótipos R e S, o que sugere que ambos os biótipos de capim-colchão possuem a mesma adaptabilidade ecológica.

Introduction: La troponine est un marqueur biologique bien reconnupour sa cardiospécificité et la détection précoce de la nécrosemyocardique. Le dosage de la troponine fait partie de la stratificationdu syndrome coronarien aigu (SCA) et est fréquemment utilisé dans lebilan initial lors de douleurs thoraciques. Cependant, d'autrespathologies sont également associées à une élévation de la troponine.Le but de ce travail est de décrire les diagnostics retenus chez despatients admis aux urgences avec une douleur thoracique et uneélévation de la troponine.Méthode: Durant une période de 18 mois (de novembre 2008 à mai2010), 1242 patients admis aux urgences avec une douleur thoraciqueont eu un dosage de la troponine positive (>= 0,1 microg/l). Undiagnostic de SCA a été posé chez 709 patients (57%). Chez les533 autres patients (43%) un autre diagnostic a été retenu.Résultats: Les 533 patients (295 hommes, âge moyen 71 ± 19 anset 238 femmes, 77 ±16 ans) présentaient une valeur de troponinemoyenne (TM) à 1,12 ± 4 microg/l. Parmi ce collectif, 192 patients(36%) avaient un diagnostic d'insuffisance cardiaque (TM 1,18microg/l), 118 patients (22%) un diagnostic autre (anémie, AVC, chutesans traumatisme) (TM 1,65 microg/l), 46 patients (8%) présentaientune pneumonie (TM 0,95 microg /l), 45 patients (8%) une arythmiecardiaque sans signes de décompensation cardiaque (TM 0,42 microg/l), 35 patients (6,5%) un traumatisme (TM 0,52 microg /l), 25 patients(4,7%) une insuffisance rénale sévère (TM 0,74 microg /l), 23 patients(4%) une insuffisance respiratoire (BPCO, fibrose pulmonaire, autrespneumopathies) (TM 1,63 microg /l), 23 patients (4,3%) un contexteinfectieux (choc septique, bactériémie) (TM 0,96 microg /l), 18 patients(3%) une embolie pulmonaire (TM 0,30 microg /l) et 8 patients (1,5%)un Takotsubo ou angor de prinzmétal (TM 1,73 microg /l).A signaler que la TM pour les 709 patients présentant un diagnostic deSCA était de 3,61 microg /l.Conclusion: Ce travail montre que près de 50% des patients arrivantaux urgences avec une douleur thoracique et une troponine positive ontun diagnostic autre qu'un SCA.
