BACKGROUND: Aromatase inhibitors provide superior disease control when compared with tamoxifen as adjuvant therapy for postmenopausal women with endocrine-responsive early breast cancer. PURPOSE: To present the design, history, and analytic challenges of the Breast International Group (BIG) 1-98 trial: an international, multicenter, randomized, double-blind, phase-III study comparing the aromatase inhibitor letrozole with tamoxifen in this clinical setting. METHODS: From 1998-2003, BIG 1-98 enrolled 8028 women to receive monotherapy with either tamoxifen or letrozole for 5 years, or sequential therapy of 2 years of one agent followed by 3 years of the other. Randomization to one of four treatment groups permitted two complementary analyses to be conducted several years apart. The first, reported in 2005, provided a head-to-head comparison of letrozole versus tamoxifen. Statistical power was increased by an enriched design, which included patients who were assigned sequential treatments until the time of the treatment switch. The second, reported in late 2008, used a conditional landmark approach to test the hypothesis that switching endocrine agents at approximately 2 years from randomization for patients who are disease-free is superior to continuing with the original agent. RESULTS: The 2005 analysis showed the superiority of letrozole compared with tamoxifen. The patients who were assigned tamoxifen alone were unblinded and offered the opportunity to switch to letrozole. Results from other trials increased the clinical relevance about whether or not to start treatment with letrozole or tamoxifen, and analysis plans were expanded to evaluate sequential versus single-agent strategies from randomization. LIMITATIONS: Due to the unblinding of patients assigned tamoxifen alone, analysis of updated data will require ascertainment of the influence of selective crossover from tamoxifen to letrozole. CONCLUSIONS: BIG 1-98 is an example of an enriched design, involving complementary analyses addressing different questions several years apart, and subject to evolving analytic plans influenced by new data that emerge over time.

Most research on sexual orientation and alcohol use in the United States has found higher rates of alcohol use and abuse among gay men and lesbians. Studies from other countries have found smaller or no differences between sexual minority and heterosexual women and men. The present study used general population survey data from 14 countries to examine high-volume and risky single-occasion drinking by sexual orientation. Data from 248 gay men and lesbians and 3720 heterosexuals were analyzed in a case-control design. In several countries partnered or recently partnered gay men and lesbians had no greater risk of heavy drinking or engaging in heavy drinking than heterosexual controls. Only lesbians in North America showed higher risk for both indicators. Future general population health research should include larger samples of gays and lesbians and use more comprehensive measures of sexual orientation for investigating the prevalence of health risk factors.

OBJECTIVE: The aim of this work was to assess loss to follow-up (LTFU) in EuroSIDA, an international multicentre observational cohort study. METHODS: LTFU was defined as no follow-up visit, CD4 cell count measurement or viral load measurement after 1 January 2006. Poisson regression was used to describe factors related to LTFU. RESULTS: The incidence of LTFU in 12 304 patients was 3.72 per 100 person-years of follow-up [95% confidence interval (CI) 3.58-3.86; 2712 LTFU] and varied among countries from 0.67 to 13.35. After adjustment, older patients, those with higher CD4 cell counts, and those who had started combination antiretroviral therapy all had lower incidences of LTFU, while injecting drug users had a higher incidence of LTFU. Compared with patients from Southern Europe and Argentina, patients from Eastern Europe had over a twofold increased incidence of LTFU after adjustment (incidence rate ratio 2.16; 95% CI 1.84-2.53; P<0.0001). A total of 2743 patients had a period of >1 year with no CD4 cell count or viral load measured during the year; 743 (27.1%) subsequently returned to follow-up. CONCLUSIONS: Some patients thought to be LTFU may have died, and efforts should be made to ascertain vital status wherever possible. A significant proportion of patients who have a year with no follow-up visit, CD4 cell count measurement or viral load measurement subsequently return to follow-up.

SECTION III PLAN Cette étude sera divisée en six chapitres. Le premier retracera l'évolution historique des enquêtes parlementaires, aux niveaux fédéral et cantonal. Ce survol servira de cadre empirique pour les discussions plus spécifiques qui le suivront. A cette occasion, une attention particulière sera accordée au principe de séparation des pouvoirs et aux fonctions que cette institution remplit au sein de notre système démocratique. L'objet de l'enquête sera défini de manière plus détaillée dans un chapitre second. Il s'agira d'étudier l'étendue du champ d'investigation ouvert aux députés, afin de déterminer si certaines thématiques sont, dès l'abord, soustraites à leur compétence. La troisième partie de ce travail portera sur l'établissement des faits. Les diverses possibilités s'offrant aux parlementaires pour élucider les événements litigieux seront répertoriées puis analysées, afin d'évaluer leur importance réelle dans la pratique des commissions. A ces moyens d'investigation seront opposées les garanties de procédure reconnues au représentant du Gouvernement et aux personnes concernées par l'enquête. Le quatrième chapitre s'intéressera ainsi au droit d'être entendu et aux règles assurant l'équité de la procédure. Le cinquième chapitre sera le complément procédural des deux précédents. Il s'intéressera aux diverses voies de droit, judiciaires ou non, permettant d'obtenir un contrôle sur les travaux de la commission. Il sera conçu comme un catalogue des moyens de redressement qu'un particulier ou une autorité peuvent utiliser, lorsqu'une contestation les oppose aux parlementaires enquêteurs. Ce chapitre donnera aussi l'occasion d'étudier certains mécanismes introduits par la récente révision de l'organisation judiciaire fédérale. Enfin, le sixième et dernier chapitre traitera des problèmes de coordination posés par la poursuite, parallèlement aux recherches parlementaires, de procédures concurrentes portant sur le même objet. Un concours sera tour à tour envisagé avec une instruction pénale puis avec des procédures internes à l'administration, telles qu'une procédure disciplinaire ou une enquête administrative.

General internal medicine (GIM) has flourished in the United States (U.S.). Unlike other subspecialties of internal medicine, however, GIM's evolution has not been global in scope, but rather appears to have occurred in isolation within countries. Here, we describe international models of GIM from Canada, Switzerland, Australia/New Zealand, Argentina, and Japan, and compare these with the U.S. model. There are notable differences in the typical clinical roles assumed by General Internists across these 7 countries, but also important overlap in clinical and academic domains. Despite this overlap, there has been a relative lack of contact among General Internists from these and other countries at a truly international GIM meeting; the time is now for increased international exchange and the "globalization" of GIM.

This study tested for the measurement equivalence of a four-factor measure of career indecision (Career Indecision Profile-65 [CIP-65]) between a U.S. sample and two international samples; one composed of French-speaking young adults from France and Switzerland and the other of Italian ado- lescents. Previous research had supported the four-factor structure of the CIP-65 in both the United States and Iceland but also showed that items on two of the four scales may be interpreted differently by young adults growing up in these two countries. This study extends previous research by testing whether the four CIP-65 factors are measured equivalently in two additional international samples. Results largely supported the configural and metric invariance of the CIP-65 in the United States and international samples, but several scales showed a lack of scalar invariance. Some explanations are offered for these findings along with suggestions for future research and implications for practice.

"Metric Training For The Highway Industry", HR-376 was designed to produce training materials for the various divisions of the Iowa DOT, local government and the highway construction industry. The project materials were to be used to introduce the highway industry in Iowa to metric measurements in their daily activities. Five modules were developed and used in training over 1,000 DOT, county, city, consultant and contractor staff in the use of metric measurements. The training modules developed deal with the planning through operation areas of highway transportation. The materials and selection of modules were developed with the aid of an advisory personnel from the highway industry. Each module is design as a four hour block of instruction and a stand along module for specific types of personnel. Each module is subdivided into four chapters with chapter one and four covering general topics common to all subjects. Chapters two and three are aimed at hands on experience for a specific group and subject. This module includes: Module 1 - Basic Introduction to the Use of International Units of Measurement. This module is designed for use by all levels of personnel, primarily office staff, and provides a basic background in the use of metric measurements in both written and oral communications.

Cette étude se penche sur le parcours de 24jeunes filles présentant des troubles alimentairesatypiques, suivies dans le cadre d'ungroupe thérapeutique.Les troubles des conduites alimentairesatypiques (TCAA) représentent une catégoriediagnostique émergente, relativement peudécrite. Ils regroupent des tableauxcliniques ne satisfaisant pas entièrementaux critères des troubles alimentairestypiques anorexie et boulimie. Les TCAAreprésentent la majorité des demandes deconsultations pour troubles alimentaires,concernent une grande proportion de lapopulation non-consultante et constituentun enjeu de santé publique prioritaire.Les résultats cliniques de cette étude décriventdes troubles importants, induisant une souffranceintense, et non des troubles résiduels.Le passage d'une catégorie de trouble à uneautre se voit confirmé, ainsi que l'existencede formes stables, les troubles atypiquespouvant représenter des moments dans leparcours de la maladie tout comme destroubles durables.En terme de classification, il est proposé deparler d'anorexie atypique, de type restrictifou avec crises, de boulimie atypique, purgativeou non, et d'hyperphagie boulimique.L'utilité du groupe thérapeutique ressortclairement.Cette prise en charge thérapeutique convientdavantage à des patientes présentant untrouble alimentaire sur le versant boulimiqueque sur le versant restrictif sans pertede contrôle.Repérer et prendre en charge ces troublesalimentaires atypiques qui demeurentsouvent banalisés s'avère une priorité.Les aspects ayant trait à l'addiction serontplus particulièrement abordés dans le cadrede cette communication
