A anestesia regional em ortopedia apresenta vantagens claras na estabilidade hemodinâmica, perfusão do território cirúrgico e analgesia de qualidade superior. o objectivo deste estudo foi avaliar o tipo e frequência de técnicas realizadas no ano de 2012 em anestesia para cirurgia ortopédica pediátrica. Um total de 662 crianças agendadas para cirurgia electiva foram retrospectivamente estudadas no que diz respeito às técnicas regionais utilizadas. Foram realizadas 248 técnicas regionais em 2012. Houve urn predominio de bloqueios do neuro-eixo (63%) em relação aos bloqueios dos nervos periféricos (BNP) (37%). A ultrassonografia foi essencial nos BNP realizados, correspondendoa 75% dos casos. Na anestesia do membro superior os bloqueios mais frequentes foram o bloqueio do plexo braquialvia supra-clavicular (61%) e os BNP na fossa antecubital (23%). No membro inferior os bloqueios mais comuns foram o bloqueio do nervo ciciticopopliteo (41%) e o bloqueio de nervo femoral (35%). Colocaram-se cateteres contínuos de bloqueio de nervo periférico em 5 bloqueios do nervo ciático popliteo. o uso de ecografo tornou-se preponderante para a realização de bloqueios dos nervos periféricos dos membros com as vantagens já amplamente descritas na literatura. Parece haver margem para diminuir o número de técnicas do neuro-eixo em relação a realização de BNP com uma maior taxa de colocação de cateteres contínuos de bloqueio de nervo periférico, diminuindo deste modo a invasibilidade do neuro-eixo. 0 número total de BNP realizados parece claramente satisfatório embora careça de estudos comparativos com outras instituições que o comprove.

Introduction: Maternal HIV infection and related co-morbidities may have two outstanding consequences to fetal health: mother-to-child transmission (MTCT) and adverse perinatal outcomes. After Brazilian success in reducing MTCT, the attention must now be diverted to the potentially increased risk for preterm birth (PTB) and intrauterine fetal growth restriction (IUGR). Objective: To determine the prevalence of PTB and IUGR in low income, antiretroviral users, publicly assisted, HIV-infected women and to verify its relation to the HIV infection stage. Patients and Methods: Out of 250 deliveries from HIV-infected mothers that delivered at a tertiary public university hospital in the city of Vitória, state of Espírito Santo, Southeastern Brazil, from November 2001 to May 2012, 74 single pregnancies were selected for study, with ultrasound validated gestational age (GA) and data on birth dimensions: fetal weight (FW), birth length (BL), head and abdominal circumferences (HC, AC). The data were extracted from clinical and pathological records, and the outcomes summarized as proportions of preterm birth (PTB, < 37 weeks), low birth weight (LBW, < 2500g) and small (SGA), adequate (AGA) and large (LGA) for GA, defined as having a value below, between or beyond the ±1.28 z/GA score, the usual clinical cut-off to demarcate the 10th and 90th percentiles. Results: PTB was observed in 17.5%, LBW in 20.2% and SGA FW, BL, HC and AC in 16.2%, 19.1%, 13.8%, and 17.4% respectively. The proportions in HIV-only and AIDS cases were: PTB: 5.9 versus 27.5%, LBW: 14.7% versus 25.0%, SGA BW: 17.6% versus 15.0%, BL: 6.0% versus 30.0%, HC: 9.0% versus 17.9%, and AC: 13.3% versus 21.2%; only SGA BL attained a significant difference. Out of 15 cases of LBW, eight (53.3%) were preterm only, four (26.7%) were SGA only, and three (20.0%) were both PTB and SGA cases. A concomitant presence of, at least, two SGA dimensions in the same fetus was frequent. Conclusions: The proportions of preterm birth and low birth weight were higher than the local and Brazilian prevalence and a trend was observed for higher proportions of SGA fetal dimensions than the expected population distribution in this small casuistry of newborn from the HIV-infected, low income, antiretroviral users, and publicly assisted pregnant women. A trend for higher prevalence of PTB, LBW and SGA fetal dimensions was also observed in infants born to mothers with AIDS compared to HIV-infected mothers without AIDS.

Electricity markets worldwide are complex and dynamic environments with very particular characteristics. These are the result of electricity markets’ restructuring and evolution into regional and continental scales, along with the constant changes brought by the increasing necessity for an adequate integration of renewable energy sources. The rising complexity and unpredictability in electricity markets has increased the need for the intervenient entities in foreseeing market behaviour. Market players and regulators are very interested in predicting the market’s behaviour. Market players need to understand the market behaviour and operation in order to maximize their profits, while market regulators need to test new rules and detect market inefficiencies before they are implemented. The growth of usage of simulation tools was driven by the need for understanding those mechanisms and how the involved players' interactions affect the markets' outcomes. Multi-agent based software is particularly well fitted to analyse dynamic and adaptive systems with complex interactions among its constituents, such as electricity markets. Several modelling tools directed to the study of restructured wholesale electricity markets have emerged. Still, they have a common limitation: the lack of interoperability between the various systems to allow the exchange of information and knowledge, to test different market models and to allow market players from different systems to interact in common market environments. This dissertation proposes the development and implementation of ontologies for semantic interoperability between multi-agent simulation platforms in the scope of electricity markets. The added value provided to these platforms is given by enabling them sharing their knowledge and market models with other agent societies, which provides the means for an actual improvement in current electricity markets studies and development. The proposed ontologies are implemented in MASCEM (Multi-Agent Simulator of Competitive Electricity Markets) and tested through the interaction between MASCEM agents and agents from other multi-agent based simulators. The implementation of the proposed ontologies has also required a complete restructuring of MASCEM’s architecture and multi-agent model, which is also presented in this dissertation. The results achieved in the case studies allow identifying the advantages of the novel architecture of MASCEM, and most importantly, the added value of using the proposed ontologies. They facilitate the integration of independent multi-agent simulators, by providing a way for communications to be understood by heterogeneous agents from the various systems.

When using the useful 2013 Fenton Chart, data should be interpreted with caution taking into account two aspects: the physiologic loss of body water after birth for the weight curves, and the questionable accuracy of the birth length curves considering the heterogeneity and reliability of the methods used in the original measurements.

PURPOSE: The aim of the this study was to determine the effect of intravitreal antivascular endothelial growth factor injections on intraocular pressure (IOP) and identify possible risk factors for the development of increased IOP. MATERIALS AND METHODS: This prospective study included a total of 106 eyes receiving intravitreal injection of bevacizumab as treatment for macular edema or active choroidal neovascularization. IOP was measured by Goldmann applanation tonometry immediately before the intravitreal injection and 5 min, 1 h and 15 days after the procedure. The records of the study patients were reviewed for age, gender, history of glaucoma, diabetes mellitus, phakic status, systemic and topical medication and number of previous injections. Subconjunctival reflux was registered. IOP elevation was defined as IOP ≥21 mm Hg and/or a change from baseline of ≥5 mm Hg recorded at least on two or more measurements on the same visit. RESULTS: Mean preoperative IOP was 15.31 ± 3.90 mm Hg and postoperative IOP values were 27.27 ± 11.87 mm Hg (after 5 min), 17.59 ± 6.24 mm Hg (after 1 h) and 16.86 ± 3.62 mm Hg (after 15 days). The IOP variation was statistically significant between pre- and postoperative measurements (p < 0.05). Subconjunctival reflux was recorded in 11.3%, and in this subgroup the IOP at 5 min and at 1 h was lower than preoperative IOP (p < 0.05). CONCLUSIONS: More than one third of the eyes achieved IOPs >30 mm Hg 5 min after injection. Subconjunctival reflux contributed to a lower mean postoperative IOP (p < 0.05). Considerations for the management include prophylactic IOP lowering with medical therapy and/or preinjection ocular decompression for patients with a history of glaucoma or ocular hypertension and switching to an as-needed injection protocol in patients suffering a marked IOP rise in previous injections. © 2015 S. RESUMO - O estabelecimento de prioridades determina a sustentabilidade de qualquer sistema de saúde, pelo que urge identificar os procedimentos, metodologias e critérios de priorização. Não existem critérios nem métodos universais de os combinar, sendo que a sua seleção depende do contexto de aplicação. O presente projeto de estudo exploratório-descritivo tem por finalidade a criação de uma proposta de metodologia a adotar na determinação de prioridades do Plano Regional de Saúde de Lisboa e Vale do Tejo 2011-2016, contextualizada à região, tempo e circunstâncias. O estudo está organizado em duas etapas metodológicas: uma revisão bibliográfica, dirigida à identificação do método e dos critérios de determinação de prioridades, e a realização de um painel de Delphi, para validação do método de determinação de prioridades proposto, definição dos critérios e suas ponderações. Tendo sido encontrada evidência na literatura sobre as vantagens da utilização da Análise Multicritério da Tomada de Decisão, através da utilização do Método Aditivo Linear, na determinação de prioridades em saúde, foi selecionada esta metodologia, que obteve a concordância de 85% dos participantes para a sua utilização no contexto em estudo, na primeira ronda do painel de Delphi. Os resultados preliminares do estudo, obtidos na primeira ronda, mostram que um dos onze critérios propostos foi excluído, tendo sido sugeridos sete novos critérios pelos participantes, que serão sujeitos a análise nas rondas subsequentes. Os resultados obtidos poderão servir de base a estudos mais aprofundados nesta área e contribuir para o debate sobre os critérios subjacentes ao processo de determinação de prioridades em saúde.
