Objectives. A large-scale survey of doses to patients undergoing the most frequent radiological examinations was carried out in health services in Sao Paulo (347 radiological examinations per 1 000 inhabitants), the most populous Brazilian state. Methods. A postal dosimetric kit with thermoluminescence dosimeters was used to evaluate the entrance surface dose (ESD) to patients. A stratified sampling technique applied to the national health database furnished important data on the distribution of equipment and the annual number of examinations. Chest, head (skull and sinus), and spine (cervical, thoracic, and lumbar) examinations were included in the trial. A total of 83 rooms and 868 patients were included, and 1 415 values of ESD were measured. Results. The data show large coefficients of variation in tube charge, giving rise to large variations in ESD values. Also, a series of high ESD values associated with unnecessary localizing fluoroscopy were detected. Diagnostic reference levels were determined, based on the 75th percentile (third quartile) of the ESD distributions. For adult patients, the diagnostic reference levels achieved are very similar to those obtained in international surveys. However, the situation is different for pediatric patients: the ESD values found in this survey are twice as large as the international recommendations for chest radiographs of children. Conclusions. Despite the reduced number of ESD values and rooms for the pediatric patient group, it is recommended that practices in chest examinations be revised and that specific national reference doses and image quality be established after a broader survey is carried out.

The protective shielding design of a mammography facility requires the knowledge of the scattered radiation by the patient and image receptor components. The shape and intensity of secondary x-ray beams depend on the kVp applied to the x-ray tube, target/filter combination, primary x-ray field size, and scattering angle. Currently, shielding calculations for mammography facilities are performed based on scatter fraction data for Mo/Mo target/filter, even though modern mammography equipment is designed with different anode/filter combinations. In this work we present scatter fraction data evaluated based on the x-ray spectra produced by a Mo/Mo, Mo/Rh and W/Rh target/filter, for 25, 30 and 35 kV tube voltages and scattering angles between 30 and 165 degrees. Three mammography phantoms were irradiated and the scattered radiation was measured with a CdZnTe detector. The primary x-ray spectra were computed with a semiempirical model based on the air kerma and HVL measured with an ionization chamber. The results point out that the scatter fraction values are higher for W/Rh than for Mo/Mo and Mo/Rh, although the primary and scattered air kerma are lower for W/Rh than for Mo/Mo and Mo/Rh target/filter combinations. The scatter fractions computed in this work were applied in a shielding design calculation in order to evaluate shielding requirements for each of these target/filter combinations. Besides, shielding requirements have been evaluated converting the scattered air kerma from mGy/week to mSv/week adopting initially a conversion coefficient from air kerma to effective dose as 1 Sv/Gy and then a mean conversion coefficient specific for the x-ray beam considered. Results show that the thickest barrier should be provided for Mo/Mo target/filter combination. They also point out that the use of the conversion coefficient from air kerma to effective dose as 1 Sv/Gy is conservatively high in the mammography energy range and overestimate the barrier thickness. Background Along the internal carotid artery (ICA), atherosclerotic plaques are often located in its cavernous sinus (parasellar) segments (pICA). Studies indicate that the incidence of pre-atherosclerotic lesions is linked with the complexity of the pICA; however, the pICA shape was never objectively characterized. Our study aims at providing objective mathematical characterizations of the pICA shape. Methods and results Three-dimensional (3D) computer models, reconstructed from contrast enhanced computed tomography (CT) data of 30 randomly selected patients (60 pICAs) were analyzed with modern visualization software and new mathematical algorithms. As objective measures for the pICA shape complexity, we provide calculations of curvature energy, torsion energy, and total complexity of 3D skeletons of the pICA lumen. We further measured the posterior knee of the so-called ""carotid siphon"" with a virtual goniometer and performed correlations between the objective mathematical calculations and the subjective angle measurements. Conclusions Firstly, our study provides mathematical characterizations of the pICA shape, which can serve as objective reference data for analyzing connections between pICA shape complexity and vascular diseases. Secondly, we provide an objective method for creating Such data. Thirdly, we evaluate the usefulness of subjective goniometric measurements of the angle of the posterior knee of the carotid siphon.

We discuss potential caveats when estimating topologies of 3D brain networks from surface recordings. It is virtually impossible to record activity from all single neurons in the brain and one has to rely on techniques that measure average activity at sparsely located (non-invasive) recording sites Effects of this spatial sampling in relation to structural network measures like centrality and assortativity were analyzed using multivariate classifiers A simplified model of 3D brain connectivity incorporating both short- and long-range connections served for testing. To mimic M/EEG recordings we sampled this model via non-overlapping regions and weighted nodes and connections according to their proximity to the recording sites We used various complex network models for reference and tried to classify sampled versions of the ""brain-like"" network as one of these archetypes It was found that sampled networks may substantially deviate in topology from the respective original networks for small sample sizes For experimental studies this may imply that surface recordings can yield network structures that might not agree with its generating 3D network. Purpose: To obtain cerebral perfusion territories of the left, the right. and the posterior circulation in humans with high signal-to-noise ratio (SNR) and robust delineation. Materials and Methods: Continuous arterial spin labeling (CASL) was implemented using a dedicated radio frequency (RF) coil. positioned over the neck, to label the major cerebral feeding arteries in humans. Selective labeling was achieved by flow-driven adiabatic fast passage and by tilting the longitudinal labeling gradient about the Y-axis by theta = +/- 60 degrees. Results: Mean cerebral blood flow (CBF) values in gray matter (GM) and white matter (WM) were 74 +/- 13 mL center dot 100 g(-1) center dot minute(-1) and 14 +/- 13 mL center dot 100 g(-1) center dot minute(-1), respectively (N = 14). There were no signal differences between left and right hemispheres when theta = 0 degrees (P > 0.19), indicating efficient labeling of both hemispheres. When theta = +60 degrees, the signal in GM on the left hemisphere, 0.07 +/- 0.06%, was 92% lower than on the right hemisphere. 0.85 +/- 0.30% (P < 1 x 10(-9)). while for theta = -60 degrees, the signal in the right hemisphere. 0.16 +/- 0.13%, was 82% lower than on the contralateral side. 0.89 +/- 0.22% (P < 1 x 10(-10)). Similar attenuations were obtained in WM. Conclusion: Clear delineation of the left and right cerebral perfusion territories was obtained, allowing discrimination of the anterior and posterior circulation in each hemisphere.

Purpose: We present an iterative framework for CT reconstruction from transmission ultrasound data which accurately and efficiently models the strong refraction effects that occur in our target application: Imaging the female breast. Methods: Our refractive ray tracing framework has its foundation in the fast marching method (FNMM) and it allows an accurate as well as efficient modeling of curved rays. We also describe a novel regularization scheme that yields further significant reconstruction quality improvements. A final contribution is the development of a realistic anthropomorphic digital breast phantom based on the NIH Visible Female data set. Results: Our system is able to resolve very fine details even in the presence of significant noise, and it reconstructs both sound speed and attenuation data. Excellent correspondence with a traditional, but significantly more computationally expensive wave equation solver is achieved. Conclusions: Apart from the accurate modeling of curved rays, decisive factors have also been our regularization scheme and the high-quality interpolation filter we have used. An added benefit of our framework is that it accelerates well on GPUs where we have shown that clinical 3D reconstruction speeds on the order of minutes are possible.

Modern medical imaging techniques enable the acquisition of in vivo high resolution images of the vascular system. Most common methods for the detection of vessels in these images, such as multiscale Hessian-based operators and matched filters, rely on the assumption that at each voxel there is a single cylinder. Such an assumption is clearly violated at the multitude of branching points that are easily observed in all, but the Most focused vascular image studies. In this paper, we propose a novel method for detecting vessels in medical images that relaxes this single cylinder assumption. We directly exploit local neighborhood intensities and extract characteristics of the local intensity profile (in a spherical polar coordinate system) which we term as the polar neighborhood intensity profile. We present a new method to capture the common properties shared by polar neighborhood intensity profiles for all the types of vascular points belonging to the vascular system. The new method enables us to detect vessels even near complex extreme points, including branching points. Our method demonstrates improved performance over standard methods on both 2D synthetic images and 3D animal and clinical vascular images, particularly close to vessel branching regions. A radioproteção dos pacientes submetidos a exames radiográficos está diretamente ligada à qualidade e à repetição das radiografias realizadas. Esta dissertação é apresentada sob a forma de três artigos. O artigo I avaliou a qualidade de 300 radiografias panorâmicas enviadas a clínicas de ortodontia. As radiografias foram classificadas como excelentes (não se observam erros), aceitáveis para diagnóstico (observam-se erros, contudo os mesmos não impedem o diagnóstico) e inaceitáveis (imagem sem valor diagnóstico). Um total de 16,33% das radiografias foi considerado excelente, 78,66% aceitáveis para o diagnóstico e 5% inaceitáveis. Encontrou-se uma média de 1.54 erros por radiografia, sendo os mais freqüentemente encontrados a falta de contato da língua com o palato (21%), aparecimento de imagens fantasma (19,66%), mento inclinado para cima (15,66%), paciente à frente do plano de foco (13,33%), cabeça girada (13,33%), imagens com alta densidade (10,33%) e com baixa densidade (8,66%). Concluiu-se que os padrões de qualidade da amostra estão de acordo com o preconizado em Guidelines on Radiology Standards for Primary Dental Care, segundo os quais se admite até 10% de imagens inaceitáveis. Contudo, cabe salientar que não se conhece o índice de repetições nas clínicas onde as radiografias da amostra foram obtidas e, tendo passado por um controle de qualidade prévio, todas as imagens deveriam ser classificadas como excelentes ou aceitáveis. O artigo II avaliou a freqüência dos erros que levaram à repetição de radiografias panorâmicas realizadas no Serviço de Radiologia da FO-UFRS. O livro de registros do Serviço mostrou um total de 3815 radiografias panorâmicas realizadas no período de junho de 2002 a junho de 2005. No mesmo período o Serviço apresentou 330 radiografias panorâmicas repetidas, resultando em índice de repetição de 8,65% dos exames. Os erros mais freqüentemente encontrados foram: paciente posicionado à frente do plano de foco (25,15%); cabeça girada para direita ou esquerda (24,84%); cabeça inclinada para frente (21,21%); paciente posicionado atrás do plano de foco (20,30%); imagem com a alta densidade (19,69%); imagem com baixa densidade (17,27%); imagem com baixo contraste (16,96%); imagem com alto contraste (12,72%); cabeça inclinada para direita ou esquerda (12,42%); corte do côndilo na radiografia (11,21); corte do mento na radiografia (8,48%); ausência de contato da língua com o palato (7,27%); paciente se moveu durante a exposição (4,94%); cabeça inclinada para trás (2,72%) e aparecimento de imagem fantasma (2,12%). Encontrou-se uma média de 2,07 erros por radiografia. Conclui-se que os erros mais freqüentemente cometidos foram classificados como erros de posicionamento do paciente e que o Serviço de Radiologia da FO-UFRGS apresenta um índice de repetição de radiografias panorâmicas satisfatório, estando de acordo com os padrões de qualidade estabelecidos pelo Guidelines on Radiology Standards for Primary Dental Care. O artigo III teve por objetivo verificar o efeito da radiação emitida em radiografias panorâmicas sobre as células da borda lateral direita da língua, através da avaliação das alterações nucleares, antes e depois da exposição aos raios X. A amostra foi constituída de 42 indivíduos adultos jovens do gênero masculino, sendo 22 deles pertencentes ao grupo que realizou uma radiografia panorâmica (grupo I) e os outros 20 pacientes pertencentes ao grupo II, que realizou duas radiografias panorâmicas. O exame citopatológico das células esfoliadas da mucosa da língua foi realizado antes da incidência radiográfica e 10 dias após. As células foram obtidas através da raspagem e as lâminas foram coradas pela técnica de Feulgen. Para observação das alterações citopatológicas foram analisadas 2.000 células em cada lâmina e quantificados os micronúcleos, buds, broken eggs, cariorrexes e células binucleadas. As lâminas foram analisadas por um único observador. Constatou-se que existe diferença significativa (p=0,01) para as variáveis broken egg, bud, cariorrexe e célula binucleada antes e depois da exposição à radiação ionizante. Na comparação entre os grupos, verificou-se que as variáveis cariorrexe e célula binucleada apresentaram diferença significativa (p=0,01), ambas com valores superiores para o grupo II.

The people of Ceará state are descended from miscegenation between the Portuguese colonizers and the native population, resulting in a different facial pattern from other populations. It is important that this pattern be thoroughly understood, along with its minimum and maximum values so that they can be assessed and respected, allowing professionals who deal with the craniofacial complex to work more efficiently and scientifically. Aim: To characterize the morphological pattern of individuals from Ceará state, whose father and grandfather are also native from Ceará, in the 10-12 year age group, not submitted to previous orthodontic treatment, in order to determine: 1) the prevalence of occlusal pattern; 2) the prevalence of dental anomalies (DA) and, 3) the skeletal and dental cephalometric characteristics of individuals that present with normal occlusion and harmonious facial pattern. Methodology: A list of 10-12 year-olds was obtained from 515 schools containing 162,713 students (Education Secretariat of Ceará State), from which 234 individuals were examined (107 boys and 157 girls). The assessment criteria adopted were: 1) Angle s Classification System to determine occlusal pattern. The occlusal characteristics were measured through overbite, overjet, crowding and interincisal diastema. 2) DA are anomalies of number, shape, size, eruption and structure and, 3) in the group that presented with normal occlusion, we used cephalometric analysis measures proposed by Downs, Steiner, Tweed, Holdaway, Jacobson and McNamara. Results: 1) 25.8% of the schoolchildren had normal occlusion, 47.5% class I malocclusion, 22.3% class II malocclusion and 4.2% class III malocclusion. No statistically significant difference was found between the age group studied and sex. Thirty percent of the individuals had normal overbite, while 36.7% and 19.7% had increased and reduced overbite, respectively. Normal overjet was found in 33.7% of the individuals, increased overjet in 50% and reduced in 16.3%. Dental crowding was observed in 62.5% of the individuals and the presence of interincisal diastema in 14.8%. 2) The prevalence of DA was 56.1%, 6.8% in the number, 10.8% in shape, 4.1% in size, 34.5% in eruption, 26.4% in structure and 17.4% had more than one DA. No association was found between DA and sex, but DA was significantly associated to malocclusion (p<0.05); 3) there was no association between sex or facial type between the measures of nasal-labial angle, position and effective maxillary length, effective mandibular length and the sagittal relationship between the molars, overjet and overbite, position of upper incisors, lower incisors and between the incisors themselves. There was a difference between sex, on the VERT index and in lower anterior facial height, upper incisor inclination and line-H, between facial types for the occlusal plane angles, mandibular plane, facial axis, lower incisor inclination, mandibular position, upper incisor position, lower anterior facial height, ANB and line-H. It was concluded that: 1) the most prevalent occlusal type was class I malocclusion, with no distinction for sex or age group, and the assessment of occlusal characteristics showed that excessive overbite and overjet were the most predominant findings, along with a high occurrence of tooth crowding; 2) a high prevalence of DA was found, particularly eruption anomalies, not influenced by sex but significantly associated to malocclusion and 3) individuals from Ceará are predominantly brachyfacial, exhibiting a number of similarities inherent to their facial pattern, such as a convex profile, retracted jaw, reduced lower third and protruded lower incisors. This study was multidisciplinary, involving researchers from the areas of epidemiology, radiology and dentistry, thereby meeting the multidisciplinarity requirements of the Postgraduate Program in Health Sciences

Descrevem-se 16 casos de osteodistrofia hipertrófica (OH) entre 13.483 casos levantados nos arquivos do Setor de radiologia do Hospital Veterinário Governador Laudo Natel da FCAVJ-UNESP, entre 1° de janeiro de 1977 a 31 de dezembro de 1995. A enfermidade mostrou-se de baixa manifestação clínica, atingindo cães jovens em fase de crescimento, pertencentes às raças de grande porte e supernutridos. Destaca-se ainda, a importância do exame radiográfico seriado no diagnóstico da enfermidade e principalmente como método de avaliação da reversão clínica do processo, mormente quando ocorre bacteremia associada.
