Plain film radiography often underestimates the extent of injury in children with epiphyseal fracture. Especially Salter-Harris V fractures (crush fracture of the epiphyseal plate) are often primarily not detected. MRI of the ankle was performed in 10 children aged 9-17 (mean 14) years with suspected epiphyseal injury using 1.0-T Magnetom Expert. The fractures were classified according to the Salter-Harris-Rang-Odgen classification and compared with the results of plain radiography. In one case MRI could exclude epiphyseal injury; in four cases the MRI findings changed the therapeutic management. The visualisation of the fracture in three orthogonal planes and the possibility of detection of cartilage and ligamentous injury in MR imaging makes this method superior to conventional radiography and CT. With respect to radiation exposure MRI instead of CT should be used for the diagnosis of epiphyseal injuries in children.

Einfluss von L-Carnitin-supplementierter total parenteraler Ernährung (TPE) auf die postoperative Fett- und Stickstoff-Utilisation [Effect of L-carnitine supplemented total parenteral nutrition on postoperative lipid and nitrogen utilization].

During episodes of trauma carnitine-free total parenteral nutrition (TPN) may result in a reduction of the total body carnitine pool, leading to a diminished rate of fat oxidation. Sixteen patients undergoing esophagectomy were equally and randomly divided and received isonitrogenous (0.2 gN/kg.day) and isocaloric (35 kcal/kg.day TPN over 11 days without and with L-carnitine supplementation (12 mg/kg.day). Compared with healthy controls, the total body carnitine pool was significantly reduced in both groups prior to the operation. Without supplementation carnitine concentrations were maintained, while daily provision of carnitine resulted in an elevation of total carnitine mainly due to an increase of the free fraction. Without supplementation the cumulative urinary carnitine losses were 11.5 +/- 6.3 mmol corresponding to 15.5% +/- 8.5% of the estimated total body carnitine pool. Patients receiving carnitine revealed a positive carnitine balance in the immediate postoperative phase, 11.1% +/- 19.0% of the infused carnitine being retained. After 11 days of treatment comparable values for respiratory quotient, plasma triglycerides, free fatty acids, ketone bodies, and cumulative nitrogen balance were observed. It is concluded that in the patient population studied here carnitine supplementation during postoperative TPN did not improve fat oxidation or nitrogen balance.

Die Regierungsreform im Bund : in kleinen Schritten und auf einem engen Pfad, Inkrementalismus und Pfadabhängigkeit

Die Bestrebungen für eine Regierungsreform in der Schweiz lassen sich weit zurückverfolgen, sie führten aber zu keinen weitreichenden Ände-rungen betreffend das Regierungsorgan. Hauptsächliche Folgen waren Verwaltungsreformen. Schrittweise oder inkrementalistische Änderungen sind feststellbar. Sie betreffen die sogenannte Organisationsgewalt, die Bundeskanzlei, die Führungsstäbe, vor allem aber die Staatssekretärinnen und Staatssekre-täre. Die Leitplanken und die Zusatzbotschaft aus dem Jahr 2010 des Bun-desrates für eine Regierungsreform richten sich nach den Ergebnissen des Beitrages nach inkrementalistischen Entwicklungen aus. Mit der Verlängerung der Amtsdauer der Bundespräsidentin oder des Bundes-präsidenten könnte zudem ein neuer Entwicklungspfad begründet wer-den. Dem aktuellen Reformanstoss werden auf Grund der verwendeten Erklärungsansätze erhöhte Chancen zugemessen. Les efforts pour une réforme du gouvernement en Suisse remontent loin, mais ils n'ont pas abouti à des modifications de longue portée concernant l'organe gouvernemental. Les résultats principaux étaient des réformes de l'administration. Des modifications graduelles ou incrémentales sont détectables. Elles concernent le pouvoir d'organisation, la Chancellerie fédérale, les états-majors, mais surtout les secrétaires d'État. Les axes principaux et le message additionnel pour une réforme du gouvernement sont orientés vers les développements incrémentales constatés selon les recherches effectuées. L'allongement de la durée de fonction du président de la Confédération pourrait en plus constituer un nouveau chemin du développement. L'impulsion actuelle pour une réforme a des chances élevées d'aboutir selon les essais d'explication.

Eosinophile Ösophagitis : Die Sicht des Gastroenterologen [Eosinophilic esophagitis : The gastroenterologist's perspective].

Eosinophilic esophagitis is characterized by symptoms of esophageal dysfunction and eosinophil-predominant esophageal inflammation. Eosinophilic inflammation in other parts of the gastrointestinal tract is absent and several differential diagnoses for esophageal eosinophilia have to be excluded before diagnosing eosinophilic esophagitis. Most patients are male and have concomitant atopic disorders. Therapeutic options are based on drugs, diet and dilation.

Wählen per Internet verändert die Schweiz

Der Autor befasst sich mit dem Thema e-Voting am Beispiel der Schweiz. Umwählen zu gehen, begibt sich der Bürger auf Wählerseiten im Internet. Dort beantwortet er zuerst 10 Fragen zu bedeutsamen politischen Themen. Je nach seinen Antworten werden die dafür politisch eintretenden Kandidaten vorgestellt. Der Wähler kann sich nun detailliert über sie und ihre politischen Positionen informieren und dann entscheiden, welchem Politiker er seine Stimme gibt. Voraussetzung hierfür ist, dass die Kandidaten präzise Angaben über ihre Person und die von ihnen vertretene Politik im Internet zugänglich machen, da sie ansonsten auf den Wahlseiten nicht zugelassen würden und somit ihre Chancen, gewählt zu werden, minimieren würden. Hat der Bürger sich festgelegt, schickt er das Dokument zur Auswertung an einen Server des Staates, der dann innerhalb von Stunden das neue Wahlergebnis präsentieren könnte. Die Wahlseiten selbst werden wahrscheinlich nicht vom Staat kontrolliert, da die Regierung diese manipulieren könnte. Wenn aber die Gestaltung der Seiten in private Hand gelegt wird, besteht die Gefahr, dass die Listen nicht als offizielle Wahllisten anerkannt werden. Hier besteht also noch Klärungsbedarf. Ein klarer Vorteil solcher Wahlseiten ist, dass die Ziele und Interessen der Politiker noch transparenter werden. Das Internet bietet dem Bürger die Möglichkeit, seine persönlichen Interessen mit denen der Politiker zu vergleichen und sich dann nach einem Abwägunsprozess zu entscheiden. ,,In einigen Jahren werden hier zu Lande Wahlurnen ganz verschwinden", prophezeit der Autor und verweist auf die Arbeit einer Projektgruppe, die die noch offenen Fragen des E-Votings klären will.

Identification of muscle-specific calpain and beta-sarcoglycan genes in progressive autosomal recessive muscular dystrophies.

The autosomal recessive forms of limb-girdle muscular dystrophies are encoded by at least five distinct genes. The work performed towards the identification of two of these is summarized in this report. This success illustrates the growing importance of genetics in modern nosology.
