Objetivo. Analizar los resultados de la fijación transarticular e interarticular en dos series de pacientes. Métodos. Se analizaron 36 pacientes con inestabilidad C1-C2, 20 tratados mediante fijación transarticular, y 15 con fijación interarticular. Se obtuvieron datos sobre las complicaciones, evolución radiológica y resultados clínicos. Resultados. De los tratados con fijación transarticular se obtuvieron resultados buenos en 17 casos, regular en 2 y malo en 1. En la fijación interarticular en 14 se obtuvieron un buen resultado, y regular en 2. Conclusiones. La fijación C1-C2 transarticular e interarticular resulta segura, con presentando altas tasas de buenos resultados con pocas complicaciones.

OBJETIVO: Identificar factores predictivos de afectación unilateral en el cáncer de próstata (CaP) de bajo riesgo. MATERIAL Y METODOS: 95 pacientes con CaP de bajo riesgo. Evaluamos la presencia de CaP unilateral en la pieza de prostatectomía. Se realizó análisis uni y multivariante estudiando las variables: edad, PSA, volumen prostático, porcentaje de cilindros afectos y presencia de biopsias previas. RESULTADOS: el 36% presentaron CaP unilateral en la pieza. No se han encontrado diferencias significativas entre el grupo de pacientes con CaP unilateral y el bilateral. CONCLUSION: No hemos identificado ningún factor predictivo de CaP unilateral en pacientes de bajo riesgo.

En este estudio se determinó los niveles de expresión del gen TGF-β en muestras de 117 pacientes con CECC. El tejido tumoral contó con un nivel de expresión de TGF-β superior al correspondiente a las mucosas sanas. En el grupo de pacientes con unos niveles bajos de expresión del TGF-β (n=16, 13.7%) contaron con un control local de la enfermedad del 100%, y en el grupo de pacientes con unos niveles superiores al punto de corte (n=101, 86.3%), un 36.6% de los pacientes contaron con una recidiva del tumor a nivel local después de realizado el tratamiento con radioterapia o quimio-radioterapia.

Como parte del proyecto A.P.A.T. (Asistencia y seguimiento al Paciente Anciano Traumatizado), desarrollamos un protocolo para optimizar la hemoglobina preoperatoria mediante ferroterapia endovenosa, en pacientes con fractura de cadera susceptible de intervención quirúrgica. Los objetivos del estudio fueron describir el perfil anémico de estos pacientes y determinar la influencia de la administración de hierro endovenoso sobre la transfusión de sangre alogénica (TSA), el número total de concentrados de hematíes administrados y la estancia media hospitalaria.

INTRODUCCIÓN: La asociación entre la arteriopatía periférica y los nuevos eventos cardiovasculares ya es conocida, no así, en función del Índice tobillo-brazo (ITB). OBJETIVOS: Identificar los valores a partir de los que un ITB patológico se correlaciona con la aparición de eventos cardiovasculares. PACIENTES Y MÉTODOS: Se analiza el resgistro FRENA de pacientes a los que se realiza una única medición del ITB. CONCLUSIONES: No hemos encontrado un valor a partir del cual el ITB se asocie significativamente a un peor pronóstico pero sí una asociación entre un ITB & 0.6 y el riesgo de IAM.

Objetivo: Valorar el efecto sobre el grosor foveal de retirar el tratamiento con análogos de las prostaglandinas previamente a la cirugía de cataratas en pacientes con glaucoma o hipertensión ocular. Material y métodos: Estudio prospectivo y aleatorizado. Se incluyeron 90 ojos de 80 pacientes. Grupo A: Suspendió el tratamiento una semana antes de la cirugía y grupo B: Mantuvo el tratamiento. Resultados: No se encontraron diferencias significativas en el grosor foveal en ambos grupos (p&0.05). Conclusiones: La interrupción del tratamiento con análogos de prostaglandinas previa a la cirugía no influye en cambios en el grosor foveal en estos pacientes. BACKGROUND It has been identified differences of medical care practice in primary care related to physician's sex. Simultaneously, there are gender inequalities in the assignment of health resources. Both aspects give rise to an increasing growing interest in the management and provision of health services. OBJECTIVES To examine the differences in the referral practice made by female and male primary care physicians working in health centers in Andalusia, to consider whether there are disparities in referrals received by men and women, and to examine the interaction between patient's sex and physician's sex. METHODS Observational, cross-sectional, and multicenter study. POPULATION 4 health districts in Andalucía and their physicians. SAMPLE 382 physicians. MEASUREMENTS referral rate per visit (RV), referral rate per patient quota (RQ), patient's sex, physician: sex, age, postgraduate family medicine specialty, size of the patient quota by sex, mean number of patients/day by sex, mean age of the patient quota by sex, and proportion of men in the quota. Health center: urban / rural, size of the team, enrolled population, and postgraduate family medicine specialty's accreditation. SOURCES databases of health districts. PERIOD OF STUDY 2010. ANALYSIS Bivariate and multivariate multilevel analysis of the referral rate per visit with mixed Poisson model. RESULTS In 2010 382 physicians made 129,161 referrals to specialized care. The RQ was 23.47 and the RV was 4.92. The RQ in women and men was 27.23 and 19.78 for women physicians, being 27.37 and 19.51 for male physicians. The RV in women and men was 4.92 and 5.48 for women physicians, being 4.54 and 4.93 for male physicians. CONCLUSION There are no differences in referral according to physician's sex. However, there are signs that might indicate the existence of gender inequality, and women patient received less referrals. There are no physician-patient's sex interaction.

En este estudio prospectivo observacional se evaluaron 60 pacientes diagnosticados de glioma cerebral y sometidos a algún tipo de intervención quirúrgica como parte de su tratamiento. En ellos se analizaron las variables demográficas, clínicas, la localización de las lesiones, el volumen tumoral basal, así como, escalas de calidad de vida y el efecto del grado de resección tumoral (biopsia, resección parcial y resección total/subtotal) sobre las mismas. Los hallazgos de nuestro estudio confirmaron el beneficio de las resecciones tumorales amplias en la calidad de vida de los pacientes. Se discuten las dificultades técnicas de este tipo de estudio y su importancia de la incorporación de estas escalas en futuros estudios neuro-oncológicos.

: Para estudiar la historia natural del ictus isquémico por oclusión de gran vaso de la circulación anterior se seleccionó una muestra prospectiva de pacientes con ictus agudo que no recibieron terapias de reperfusión en los que se confirmó una oclusión de gran vaso de la circulación anterior mediante Dúplex transcraneal. El 74% de los pacientes tuvo mal pronóstico. Los pacientes con oclusión de la a.carótida interna terminal y de la a.cerebral media proximal tuvieron peor pronóstico funcional y mayor porcentaje de mortalidad a los tres meses que los pacientes con oclusión de la a.cerebral media distal.

INTRODUCTION: The metabolic syndrome (MS) consists of a set of clinical and biochemical changes. It is very common among chronic hemodialysis patients, being the leading cause of death in these patients, 44% of all patients undergoing this therapy. AIMS: The aim of this study was to investigate the prevalence of MS and risk factors associated with its development, as well as the prevalence of obesity in HD patients. METHODS: This study has followed 90 patients of both sexes with chronic renal failure (CRF) who were treated with hemodialysis periodically in our unit for ten years. All patients were performed quarterly measurements of plasma albumin (A1b) and other biochemical analysis; besides, they underwent some anthropometric measurements like weight, height and body mass index (BMI). This was calculated using weight / size2 formula and grouped in BMI values according to WHO criteria. The data concerning hypertension and glucose were also considered. RESULTS: The prevalence of MS was 25% and obesity was presented as follows: 45% with type I overweight; 30.8% with type II overweight and 12 patients (2%) were obese. Being statistically significant as risk factors, BMI, overweight, triglycerides, total cholesterol, HDL cholesterol as well as hypertension and elevated glucose levels were obtained. CONCLUSIONS: The metabolic syndrome compromises the patient survival causing a high prevalence in these patients. The principal risk factors in MS are monitoring weight, BMI, triglycerides, HDL cholesterol, hypertension and diabetes.

INTRODUCTION: Weight gain and malnutrition after kidney transplantation is common and the resulting overweight and obesity is associated with serious health complications. By contrast, the prevalence of malnutrition in patients with renal transplantation and its impact on the outcome of kidney transplantation is underestimated. OBJECTIVES: The aim of this study was to evaluate the nutritional status of renal transplant patients and determine if the five-year follow-up, these patients undergo alterations that suggest nutritional deterioration. METHODS: The sample consisted of 119 renal transplant patients who attended for five years post-transplant consultation. All patients measurements of total cholesterol, low density lipoprotein (LDL), high density lipoprotein (HDL), triglycerides and ferritin (Ft) were performed. and anthropometric measurements were made of weight, height and BMI. Patients were divided into three groups according to GFR Group 1: <60 mL/min, Group 2: 89-60 mL/min Group 3: ≥ 90 mL/min. RESULTS: The weight and BMI tended to decrease in group 3 while increasing in the other groups. A decrease in total cholesterol, HDL, LDL, Triglycerides and Ferritin less pronounced in group 3 occurs. CONCLUSIONS: After five years you can see a significant reduction in nutritional biochemical parameters in general, likewise the nutritional status is closely related, and is directly proportional to the function of the graft.

INTRODUCTION: The onset of post-transplant diabetes mellitus (PTDM) among kidney recipients is associated with an increased risk of graft failure and high rates of morbidity and mortality. Minimize the risk of PTDM is a priority for improving long-term survival rates. Aims. This study aims to assess the prevalence of PTDM in a renal transplant patient population, to identify risk factors and assess the graft and patient survival. METHODS: The sample consisted of 112 renal transplant patients , 69 men and 43 women , renal transplant , who attended for five years post-transplant consultation. Were analyzed as potential risk factors for PTDM : age , sex, body mass index (BMI ) , obesity , VHC , hypertension, dyslipidemia , total cholesterol (TC) , serum triglyceride and immunosuppressive therapy ( cyclosporine , tacrolimus , mycophenolate mofetil and sirolimus ), also the prevalence of acute rejection episodes was evaluated. RESULTS: The prevalence of PTDM was 24.2 %, compared with 85 patients (75.8%) with standard glucose (PGN) . PTDM patients showed a higher BMI , a higher percentage of overweight , dyslipidemia , total cholesterol levels , triglycerides and performed a greater percentage of patients with PDMPT including Mycophenolate mofetil was administered. CONCLUSIONS: There is a high incidence of PTDM in kidney recipients , the importance of weight control and strict adherence to all identified risk factors , as well as in minimizing the doses of immunosuppressive therapies to prevent the onset of PTDM.

Introduction: The quality of life assessment means investigating how patients perceive their disease. Malnutrition-specific characteristics make patients more vulnerable, so it is important to know how these factors impaction patients’ daily life. Aim: To assess the quality of life in malnourished patients who have had hospital admission, and to determine the relationship of the quality of life with age, body mass index, diagnosis of malnutrition, and dependency. Method: Multicenter transversal descriptive study in 106 malnourished patients after hospital admission. The quality of life (SF-12 questionnaire), BMI, functional independency (Barthel index), morbidity, and a dietary intake evaluation were assessed. The relationship between variables was tested by using the Spearman correlation coefficient Results: The patients of the present study showed a SF-12 mean of 38.32 points. The age was significantly correlated with the SF-12 (r= -0.320, p= 0.001). The BMI was correlated with the SF-12 (r= 0.251, p= 0.011) and its mental component (r= 0.289, p= 0.03). It was also reported a significant correlation between the Barthel index and the SF-12 (r= 0.370, p< 0.001). Conclusions: The general health perception in malnourished patients who have had a hospital admission was lower than the Spanish mean. Moreover, the quality of life in these patients is significantly correlated with age, BMI and functional independency.
