The international society for bipolar disorders (ISBD) task force report on antidepressant use in bipolar diosrders

Objective:
The risk-benefit profile of antidepressant medications in bipolar disorder is controversial. When conclusive evidence is lacking, expert consensus can guide treatment decisions. The International Society for Bipolar Disorders (ISBD) convened a task force to seek consensus recommendations on the use of antidepressants in bipolar disorders.

Method:  
An expert task force iteratively developed consensus through serial consensus-based revisions using the Delphi method. Initial survey items were based on systematic review of the literature. Subsequent surveys included new or reworded items and items that needed to be rerated. This process resulted in the final ISBD Task Force clinical recommendations on antidepressant use in bipolar disorder.

Results:  
There is striking incongruity between the wide use of and the weak evidence base for the efficacy and safety of antidepressant drugs in bipolar disorder. Few well-designed, long-term trials of prophylactic benefits have been conducted, and there is insufficient evidence for treatment benefits with antidepressants combined with mood stabilizers. A major concern is the risk for mood switch to hypomania, mania, and mixed states. Integrating the evidence and the experience of the task force members, a consensus was reached on 12 statements on the use of antidepressants in bipolar disorder.

Conclusions:
Because of limited data, the task force could not make broad statements endorsing antidepressant use but acknowledged that individual bipolar patients may benefit from antidepressants. Regarding safety, serotonin reuptake inhibitors and bupropion may have lower rates of manic switch than tricyclic and tetracyclic antidepressants and norepinephrine-serotonin reuptake inhibitors. The frequency and severity of antidepressant-associated mood elevations appear to be greater in bipolar I than bipolar II disorder. Hence, in bipolar I patients antidepressants should be prescribed only as an adjunct to mood-stabilizing medications.

Jocoserious ‘ignorance shifting’ or ‘aestho-psycho-eugenics’?: interrogating Joseph Furphy’s Bulletin ‘apprenticeship'

Annotating Joseph Furphy’s first publication for the Bulletin raises some intriguing questions: to what extent was Furphy the product of the Bulletin writing academy, the late nineteenth century equivalent of a writing course, or to what extent was his talent sui generis? I intend to put this question to an extreme test, by reading closely his first published contribution to the Bulletin in 1889, ‘The Mythical Sundowner’.

Exploring and expressing Italian musical heritage in Melbourne (Australia): a women’s community choir

Australia comprises many cultures, ethnicities, and languages. Belonging to community music groups by older people can enhance quality of life, offer a sense fulfilment, and provide a space through which cultural and linguistic identity may be shared and celebrated. This qualitative case study explores engagement by older members of La Voce Della Luna, an Italian women’s community choir based in Melbourne, Victoria. Older Australians, particularly those from culturally and linguistically diverse backgrounds frequently rely on voluntary community arts organisations to enhance quality of life. Singing together can provide ways for individuals and communities to express themselves, build community identity, improve quality of life, and celebrate cultural heritage. The members of the choir know that under their inspiring conductor they would learn new songs, new languages and new ways of performing. Their music director saw that the women’s singing together opened new horizons of social engagement and new ideas such as social justice and women’s rights.
This case is from the larger ongoing joint research project (2008 ongoing), Well-being and ageing: community, diversity and the arts in Victoria. Data were gathered from documentary sources and by individual and focus group semi-structured interviews (2013) and were analysed using interpretative phenomenological analysis. Significant themes emerged: social connection and combatting isolation, the maintenance and transmission of cultural heritage, and opening horizons about music making and social justice. This paper demonstrates that active music making makes it possible for older women to learn new skills, new ideas, and create for themselves a resilient community.

Interview with Joseph Kata

Joseph Kata is a Papua New Guinean who studied at the University of Wollongong in 1992-1993. He studied on an Australian government scholarship and completed an MBA. He has since contributed to the development of the University of Goroka and worked for several NGOs. The interview was conducted in English on 22 January 2015 by Dr. Jonathan Ritchie of Deakin University and Dr. Musawe Sinebare of the Pacific Adventist University. N’Oublie pas de vivre: Goethe et la tradition des exercises spirituels, which has remained untranslated, has hitherto attracted little scholarly recognition or critical notice, even in its native French. It is this situation that this review essay hopes to redress, in the small way permitted to any such piece of writing. In what follows, we examine in turn Hadot’s framing claims concerning the shaping ends and origins of Goethe’s species of neoclassicism (Part I), his claims concerning Goethe’s debt to the classical or Hellenistic tradition of spiritual exercises (Part II), before our closing remarks (Part III) challenge some of Hadot’s claims concerning the author of Faust, and then reflect on the significance of the fact that this last philosophical testament of Pierre Hadot’s was on a modern novelist, scientist, and poet, not an ancient philosopher.
