Financial institutions are turning their attentions to lean methodology. Strong competitive financial markets demand that organizations search for new ways to achieve competitive advantage. Lean thinking proposes a new method to reach high efficiency: the kaizen approach, which advocates a change in the mind set of all stakeholders towards continuous improvement. In this work project is represented the lean methodology applied at the bank, from a bottom-up spirit, as well as the evaluation of the benefits and the achievement of the proposed goals.

ABSTRACTINTRODUCTION: Hydatid cysts are rarely detected in muscle tissue (0.7-0.9%), even in endemic countries. The aim of this study was to present information regarding the clinical manifestations, diagnosis, and management of muscle echinococcosis.METHODS: Twenty-two patients with hydatid cysts in the muscle were followed from January 2006 through December 2014.RESULTS: Twenty-four sites of muscle involvement were observed in the 22 patients. Fifteen (68%) of our patients were women, while seven (32%) were men. The mean age was 28.1 ± 15.4 (6-61) years. The most frequent locations were the thigh (27.2%) and the paravertebral region (13.6%). Most patients reported a painless slow-growing mass with normal overlying skin. Most (90.2%) cases were treated by surgical excision and fine-needle aspiration.CONCLUSIONS: Primary muscle hydatid cyst should be considered in the differential diagnosis in cystic masses of the muscular system without pain and localized enlargement of soft tissue, especially in endemic areas. Hydatid cyst should be investigated using serological tests and imaging modalities. If possible, total surgical excision of hydatid cyst in the muscle should be performed.

Nos dias de hoje, são cada vez mais as organizações que procuram ser reconhecidas no mercado pela qualidade, pela eficiência e pela resposta rápida às mudanças sentidas. Neste contexto surgem muitos paradigmas e métodos de gestão que pretendem fazer face aos fatores externos que tornam os mercados tão voláteis. Dentre várias, destaca-se o Lean como forma de gestão, que advoga práticas e conceitos de melhoria contínua e redução de desperdícios em toda a cadeia de valor, visando sempre a satisfação do consumidor. Considerada uma forma de pensar, desenvolve as pessoas através da criação de uma cultura onde todos são encorajados a usar o pensamento, conhecimento e capacidades adquiridas para resolver problemas e melhorar continuamente. Mas apesar de o Lean ter surgido na indústria automóvel e ser passível de aplicar em todas as indústrias e serviços, não é uma mudança simples. Para tal, deve existir um sentido forte de liderança desde a gestão de topo até ao fim da estrutura hierárquica. Associada à implementação desta metodologia numa organização, surge também a necessidade de avaliar o desempenho destas nas organizações. Nesta ótica, são vários os modelos de avaliação desenvolvidos sendo que um dos mais reconhecidos é o Modelo de Shingo, criado pela Utah State University, que todos os anos atribui prémios a organizações que se queiram submeter a uma avaliação que reflita a adequação da organização à metodologia. No âmbito do tema e objetivos propostos pela organização que serviu de caso de estudo, foi então proposto e desenvolvido um Modelo de Avaliação da Sustentação da Metodologia Lean. O modelo teve como base princípios do Modelo de Shingo, tendo como objetivo identificar o estado do Lean implementado numa organização e de que forma este está intrínseco à gestão, baseando-se na perceção interna da organização e a perceção externa da entidade que o aplica, tendo sempre presente a visão do Lean enquanto metodologia de gestão. Os resultados obtidos demonstraram ser coerentes com as características da implementação da metodologia numa organização, confirmando mais uma vez, a importância da compreensão dos princípios da metodologia a par com a aplicação das ferramentas de apoio, de modo a alcançar uma implementação de sucesso. Foi também notável, o impacto que a liderança e o alinhamento numa organização têm numa transformação cultural desta dimensão. Apesar de a organização ter demonstrado um nível bom de adequação à metodologia, ainda tem um caminho a percorrer no sentido de tornar o Lean incorporado à cultura organizacional. Com o presente estudo é possível concluir que o Modelo de Avaliação proposto, é uma ferramenta útil no diagnóstico à implementação e na tomada de decisões no âmbito da sustentação da metodologia.

Abstract: An integrative literature review was conducted to synthesize available publications regarding the potential use of serological tests in leprosy programs. We searched the databases Literatura Latino-Americana e do Caribe em Ciências da Saúde, Índice Bibliográfico Espanhol em Ciências da Saúde, Acervo da Biblioteca da Organização Pan-Americana da Saúde, Medical Literature Analysis and Retrieval System Online, Hanseníase, National Library of Medicine, Scopus, Ovid, Cinahl, and Web of Science for articles investigating the use of serological tests for antibodies against phenolic glycolipid-I (PGL-I), ML0405, ML2331, leprosy IDRI diagnostic-1 (LID-1), and natural disaccharide octyl-leprosy IDRI diagnostic-1 (NDO-LID). From an initial pool of 3.514 articles, 40 full-length articles fulfilled our inclusion criteria. Based on these papers, we concluded that these antibodies can be used to assist in diagnosing leprosy, detecting neuritis, monitoring therapeutic efficacy, and monitoring household contacts or at-risk populations in leprosy-endemic areas. Thus, available data suggest that serological tests could contribute substantially to leprosy management.

In order to evaluate the obstetric care in the Obstetric Clinic of the Gynaecology and Obstetrics Department of University of São Paulo, the authors present a survey of the management of pregnancy during the 6-year period from 1993 to 1998. The number of deliveries increased during the study by 45% over the 6 years. During this same period the number of fetal deaths was 526 (4.48%), but there was a significant decrease (p < 0.05) in the incidence of fetal death. However, there was no concomitant increase in the proportion of pregnant women with prenatal care that could explain this improvement. Incidence of premature labor also decreased considerably. The authors believe that the increment in the number of deliveries was due mainly to the increasing number of pregnant women referred to our service. The efforts made by the service towards decreasing the time of hospitalization of both newborns in the nursery and the mothers in the hospital made this possible. Despite the increasing number of deliveries, there was a significant improvement in the management of pregnancy during the period of study. This improvement may be a consequence of the standardization of a protocol of management of pregnancy based on the recent progress in scientific and technological knowledge.

OBJECTIVES: To evaluate the use of inhaled nitric oxide (NO) in the management of persistent pulmonary hypertension of the newborn. METHODS: Computerized bibliographic search on MEDLINE, CURRENT CONTENTS and LILACS covering the period from January 1990 to March 1998; review of references of all papers found on the subject. Only randomized clinical trials evaluating nitric oxide and conventional treatment were included. OUTCOMES STUDIED: death, requirement for extracorporeal membrane oxygenation (ECMO), systemic oxygenation, complications at the central nervous system and development of chronic pulmonary disease. The methodologic quality of the studies was evaluated by a quality score system, on a scale of 13 points. RESULTS: For infants without congenital diaphragmatic hernia, inhaled NO did not change mortality (typical odds ratio: 1.04; 95% CI: 0.6 to 1.8); the need for ECMO was reduced (relative risk: 0.73; 95% CI: 0.60 to 0.90), and the oxygenation was improved (PaO2 by a mean of 53.3 mm Hg; 95% CI: 44.8 to 61.4; oxygenation index by a mean of -12.2; 95% CI: -14.1 to -9.9). For infants with congenital diaphragmatic hernia, mortality, requirement for ECMO, and oxygenation were not changed. For all infants, central nervous system complications and incidence of chronic pulmonary disease did not change. CONCLUSIONS: Inhaled NO improves oxygenation and reduces requirement for ECMO only in newborns with persistent pulmonary hypertension who do not have diaphragmatic hernia. The risk of complications of the central nervous system and chronic pulmonary disease were not affected by inhaled NO.

The Peutz-Jeghers syndrome is a hereditary disease that requires frequent endoscopic and surgical intervention, leading to secondary complications such as short bowel syndrome. CASE REPORT: This paper reports on a 15-year-old male patient with a family history of the disease, who underwent surgery for treatment of an intestinal occlusion due to a small intestine intussusception. DISCUSSION: An intra-operative fiberscopic procedure was included for the detection and treatment of numerous polyps distributed along the small intestine. Enterotomy was performed to treat only the larger polyps, therefore limiting the intestinal resection to smaller segments. The postoperative follow-up was uneventful. CONCLUSION: We point out the importance of conservative treatment for patients with this syndrome, especially those who will undergo repeated surgical interventions because of clinical manifestation while they are still young.

The way in which electricity networks operate is going through a period of significant change. Renewable generation technologies are having a growing presence and increasing penetrations of generation that are being connected at distribution level. Unfortunately, a renewable energy source is most of the time intermittent and needs to be forecasted. Current trends in Smart grids foresee the accommodation of a variety of distributed generation sources including intermittent renewable sources. It is also expected that smart grids will include demand management resources, widespread communications and control technologies required to use demand response are needed to help the maintenance in supply-demand balance in electricity systems. Consequently, smart household appliances with controllable loads will be likely a common presence in our homes. Thus, new control techniques are requested to manage the loads and achieve all the potential energy present in intermittent energy sources. This thesis is focused on the development of a demand side management control method in a distributed network, aiming the creation of greater flexibility in demand and better ease the integration of renewable technologies. In particular, this work presents a novel multi-agent model-based predictive control method to manage distributed energy systems from the demand side, in presence of limited energy sources with fluctuating output and with energy storage in house-hold or car batteries. Specifically, here is presented a solution for thermal comfort which manages a limited shared energy resource via a demand side management perspective, using an integrated approach which also involves a power price auction and an appliance loads allocation scheme. The control is applied individually to a set of Thermal Control Areas, demand units, where the objective is to minimize the energy usage and not exceed the limited and shared energy resource, while simultaneously indoor temperatures are maintained within a comfort frame. Thermal Control Areas are overall thermodynamically connected in the distributed environment and also coupled by energy related constraints. The energy split is performed based on a fixed sequential order established from a previous completed auction wherein the bids are made by each Thermal Control Area, acting as demand side management agents, based on the daily energy price. The developed solutions are explained with algorithms and are applied to different scenarios, being the results explanatory of the benefits of the proposed approaches.

Splenectomy is the best available treatment for severe forms of hereditary spherocytosis, idiopathic thrombocytopenic purpura, and other hematologic conditions when these prove refractory to conservative management. It has been employed for many decades with low mortality and favorable remission rates. The use of laparoscopic splenectomy in recent years has been rapidly and even enthusiastically adopted in this field. However, the exact role of laparoscopic versus open surgery for hematologic diseases is still debated. In this study of 58 adult patients, laparoscopic procedures were compared with conventional splenectomies for similar indications. METHODS: All patients were operated on within an 8-year period. Subjects underwent similar procedures under the supervision of the same surgical school and were compared regarding age, gender, body mass index, and diagnosis. Laparoscopically managed cases (Group I, n = 30) were prospectively followed according to a written protocol, whereas the same investigation was retrospectively done with regard to traditional laparotomy (Group II, n = 28). Methods included general and demographic findings, duration and technical steps of operation, blood loss, weight of spleen, need for conversion (in minimally invasive subjects), intraoperative and postoperative complications, time until realimentation, postoperative hospitalization, mortality, and late follow-up including recurrence rate. RESULTS: Idiopathic thrombocytopenic purpura was the surgical indication in over 50% of the patients in both groups, but familial spherocytosis, thalassemia, myelodysplasia, and lymphomas were also represented in this series. Laparoscopic procedures took more time to perform (P = 0.004), and postoperative hospitalization was 2 days shorter, but this difference was not statistically significant. Postoperative hematocrit and volume of blood transfusions was equivalent, although the laparoscopic cases had a somewhat lower preoperative hematocrit (NS) and displayed better recovery for this measurement (P = 0.03). More patients in Group I were able to accept oral food on the first day than subjects undergoing conventional operations (P < 0.05). Relatively few conversions were necessary during the minimally invasive surgeries (13.3%), and postoperative early and late complications as well as recurrences occurred in similar proportions. Also, the mean weight of the spleen was not statistically different between the groups, although there was a marked numerical tendency toward larger masses in conventional procedures. No spleen in Group I exceeded 2.0 kg, whereas in Group II values up to 4.0 kg occurred, and the mean weight was 50% higher in the latter group. CONCLUSIONS: 1) Minimally invasive splenectomy was essentially comparable to open surgery with regard to safety, efficacy, and late results; 2) Advantages concerning shorter postoperative hospitalization could not be shown, despite earlier food intake and a non-significant tendency toward earlier discharge; 3) This new modality should be considered an option in cases of hematologic conditions whenever the spleen is not hugely enlarged.

OBJECTIVE: To identify the prevalence of management plans and decision-making processes for terminal care patients in pediatric intensive care units. METHODOLOGY: Evidence-based medicine was done by a systematic review using an electronic data base (LILACS, 1982 through 2000) and (MEDLINE, 1966 through 2000). The key words used are listed and age limits (0 to 18 years) were used. RESULTS: One hundred and eighty two articles were found and after selection according to the exclusion/inclusion criteria and objectives 17 relevant papers were identified. The most common decisions found were do-not-resuscitation orders and withdrawal or withholding life support care. The justifications for these were "imminent death" and "unsatisfatory quality of life". CONCLUSION: Care management was based on ethical principles aiming at improving benefits, avoiding harm, and when possible, respecting the autonomy of the terminally ill patient.

O setor da cortiça representa no panorama industrial português um lugar de grande destaque devido à imagem de excelência e de rigor que os fabricantes nacionais transmitem ao mundo. As rolhas, produto originado por esta matéria-prima, encontram-se sujeitas a novas pressões do mercado pelo aparecimento de produtos alternativos que fornecem a mesma utilidade. O caso de Estudo é aplicado na Corticeira Amorim, atual líder mundial do setor, que utiliza a inovação e a melhoria dos seus processos e produtos como forma de manter o monopólio que atualmente possui. Faz parte da estratégia da Unidade Industrial Equipar, procurar as melhores práticas exploradas pelas empresas internacionais de topo. Atualmente a metodologia que apresenta melhor desempenho em termos de ganhos são os modelos que combinam a Teoria das Restrições com Lean e 6-Sigma, denominados pela sigla TLS. Foi desejado que se criasse um modelo com base nas filosofias anteriores e que fosse possível adaptar ao sistema produtivo de rolhas técnicas encontrado na secção Twin Top. Após uma revisão bibliográfica, construiu-se um modelo TLS com 5 fases com objetivo de atingir a melhoria contínua de um sistema produtivo. As fases do modelo proposto passam pela análise e identificação da restrição, exploração da mesma com as possíveis oportunidades de melhorias LSS, pela subordinação do sistema à restrição e por fim é feita uma reavaliação do desempenho da restrição, onde se a mesma estiver resolvida o ciclo fecha retornando ao primeiro passo novamente ou, caso contrário, é elevado o desempenho do processo com recurso a investimento monetário. Foram aplicados dois ciclos de melhoria contínua. Os resultados obtidos na resolução da primeira restrição foram extremamente satisfatórios onde o processo restritivo tornou-se o mais eficiente após as melhorias. Foi iniciada a exploração da restrição no segundo ciclo. Os objetivos definidos inicialmente na presente tese de mestrado foram alcançados.

This thesis aims to develop an alternative active managed portfolio strategy based on companies‟ Fundamental and Technical Analysis and analyze its finals results. There is a big distinction between the two approaches and the main objective is to understand if it is possible to take advantage of both. With this in mind a Hybrid investment strategy for the US stock market, due to its dimension and liquidity, which was able to outperform the S&P 500 index, the benchmark, during both Bear and Bull Markets between 2000 and 2015.

The main goals for the current dissertation is to research on how practices and concepts from Agile Project Management can be applied in a non-IT context and to discover which aspects should be considered when deciding if whether an Agile approach should be implemented or not. Previous studies reflect on the adoption for the identified context. However, the recognition of these practices and concepts by the Project Management field of studies still remains unresolved. The adoption of Agile Project Management emerges as a manifestation against traditional approaches, mainly due to their inability of accepting requirements’ changes. Therefore, these practices and concepts can be considered in order to reduce the risks concerning the increase of competition and innovation – which does not apply to the IT sector solely. The current study reviews the literature on Agile Project Management and its adoption across different sectors in order to assess which practices and concepts can be applied on a non-IT context. Nine different methods are reviewed, where two of these show a higher relevance – Scrum and Extreme Programming. The identified practices and concepts can be separated into four different groups: Cultural and Organizational Structures, Process, Practices, and Artefacts. A framework based on the work by Boehm & Turner in 2004 is developed in order to support the decision of adopting agile methods. A survey intended for project managers was carried in order to assess the implementation of the identified practices and concepts and to evaluate which variables have the highest importance on the developed decision support framework. It is concluded that New Product Development is the project type with the highest potential to implement an agile approach and that the Project Final Product’s Innovativeness, Competitiveness, and the Project Member’s Experience and Autonomy are the most important aspects to consider an implementation of an Agile approach.

As exigências crescentes e as rápidas mudanças nos mercados obrigam as organizações a pensarem cada vez mais na inovação e melhoria contínua. Hoje em dia qualquer estratégia de crescimento, e até mesmo a própria sobrevivência nos mercados, passa cada vez mais pela aplicação de novas abordagens e metodologias. A redução de desperdícios, melhoria contínua, inovação e a capacidade de resolução de problemas tornaram-se de extrema importância nas últimas décadas. Nesta linha estratégica, a metodologia Lean é reconhecida e divulgada dentro das organizações devido aos benefícios advindos da sua aplicação e dos resultados positivos na identificação de desperdícios. Identificados os desperdícios, a metodologia especializada na resolução de problemas é a TRIZ – Teoria da Resolução Inventiva de Problemas. A TRIZ tem vindo a ganhar popularidade tanto a nível académico, como a nível empresarial. A TRIZ promove a inovação sistemática e a procura de novas soluções para os problemas existentes, apresentando técnicas e ferramentas analíticas para a resolução de problemas dos produtos, processos e organizações, permitindo assim desenvolver soluções inovadoras. Esta metodologia permite também que a inovação deixe de ser esporádica, não dependendo apenas da capacidade individual de cada um e de momentos de inspiração, evoluindo para a inovação sistemática que pode ser gerida e ensinada. Torna-se de muita utilidade a realização de estudos de aplicação conjunta das duas metodologias. Nesta dissertação foi elaborada a proposta de um modelo, que permite efetuar a análise de sistemas com vista à identificação dos desperdícios/contradições e a sua eliminação/redução, através da utilização conjunta das ferramentas analíticas da TRIZ e do Lean. Foi demonstrado que as metodologias complementam-se. A TRIZ com a sua vertente de inovação e de resolução de problemas poderá constituir um contributo importante nos sistemas de aplicação do Lean.
