BACKGROUND: In published case reports, tocilizumab (TCZ) has shown good efficacy for AA amyloidosis in almost all patients. We investigated the efficacy and safety of TCZ in AA amyloidosis in a multicentre study of unselected cases. METHODS: We e-mailed rheumatology and internal medicine departments in France, Switzerland and North Africa by using the Club Rhumatismes Inflammation (CRI) network and the French TCZ registry, Registry RoAcTEmra (REGATE), to gather data on consecutive patients with histologically proven AA amyloidosis who had received at least one TCZ infusion. Efficacy was defined as a sustained decrease in proteinuria level and/or stable or improved glomerular filtration rate (GFR) and by TCZ maintenance. RESULTS: We collected 12 cases of AA amyloidosis treated with TCZ as monotherapy (mean age of patients 63 ± 16.2 years, amyloidosis duration 20.6 ± 31.3 months): eight patients had rheumatoid arthritis (RA), six with previous failure of anti-tumor necrosis factor α (anti-TNF-α) therapy. In total, 11 patients had renal involvement, with two already on hemodialysis (not included in the renal efficacy assessment). For the nine other patients, baseline GFR and proteinuria level were 53.6 ± 32.8 mL/min and 5 ± 3.3 g/24 h, respectively. The mean follow-up was 13.1 ± 11 months. TCZ was effective for six of the eight RA patients (87.5%) according to European League Against Rheumatism response criteria (four good and two moderate responders). As expected, C-reactive protein (CRP) level decreased with treatment for 11 patients. Renal amyloidosis (n = 9) progressed in three patients and was stabilized in three. Overall, three patients showed improvement, with sustained decrease in proteinuria level (42%, 82% and 96%). Baseline CRP level was higher in subsequent responders to TCZ than other patients (p = 0.02). Among the six RA patients with previous anti-TNF-α therapy, amyloidosis was ameliorated in one and stabilized in three. Three serious adverse events occurred (two diverticulitis and one major calciphylaxia due to renal failure). Finally, 7 of 12 (58%) patients continued TCZ. CONCLUSIONS: The efficacy of TCZ for AA amyloidosis varies depending on the inflammatory status at treatment onset. Discrepancies between our study of unselected consecutive patients and reported cases may be due to publication bias. These results support further prospective trials of TCZ for AA amyloidosis.

Cette thèse met en lumière les relations réciproques entre les deux artefacts‐clés de la culture russe et mondiale du XXe siècle : la théorie du carnaval et de la ménippée de Mikhaïl Bakhtine et le dernier roman de Mikhaïl Boulgakov. Les chercheurs n'ont pas tardé à remarquer leur étonnante synthonie après leur sortie de l'oubli qui a eu lieu presque en même temps et qui a été marquée par la publication des ouvrages de Bakhtine Problèmes de la poétique de Dostoïevski (1963), L'oeuvre de François Rabelais et la culture populaire au Moyen Ãge et sous la Renaissance (1965) et par la parution du roman de Boulgakov Le Maître et Marguerite (1966-1967). C'est déjà dans la postface de la première édition du roman que Le Maître et Marguerite reçoit le nom de «ménippée de Bakhtine». Un tel rapprochement entre la théorie de Bakhtine et le roman de Boulgakov s'explique par leur intérêt simultané pour différentes manifestations de la poétique du carnaval qui s'appuie sur des traditions portant l'empreinte des conceptions du temps cyclique et de l'éternel retour par le biais de la mort, propres aux rituels agraires de l'époque archaïque et aux fêtes du Nouvel An de l'Antiquité. A partir du début du XXe siècle et jusqu'aux années 40, la notion de carnavalesque en Russie est un ensemble d'idées unique, un concept doté d'un caractère d'universalité culturelle de son temps: le carnavalesque attire l'attention non seulement de Bakhtine et de Boulgakov, mais aussi de presque tous les modernistes russes et de certains philologues de l'époque. Dans notre travail, l'étude de la ressemblance des approches du problème du carnaval chez Boulgakov et chez Bakhtine s'organise selon deux axes de recherche. Le premier est lié à la théorie des genres moderne et à son développement dans la philosophie de Bakhtine. C'est dans ce contexte que nous examinons la notion antique de la satire ménippée (ou de la ménippée), considérée par Bakhtine en tant que genre littéraire qui reflète le mieux la poétique du carnaval. C'est également ce contexte qui nous permet d'analyser l'évolution de la théorie du carnaval et de la ménippée chez Bakhtine depuis ses travaux des années 30 jusqu'à la parution du « quatrième chapitre célèbre » de la deuxième édition de son ouvrage consacré à Dostoïevski. Le même contexte sert à délimiter l'emploi de la théorie du carnaval et de la ménippée dans la critique littéraire et à déterminer l'efficacité de cet outil théorique que nous utilisons lors de l'analyse du roman Le Maître et Marguerite. Le second axe de notre recherche est lié à l'étude des traditions du carnaval, à l'examen de leurs sources mythologiques et de leurs sources relatives aux mystères. Ces sources sont considérées telles qu'elles se présentent dans les manières de concevoir le carnaval propres à Bakhtine et à Boulgakov que nous avons nommées, respectivement, le carnaval de Bakhtine et le carnaval de Boulgakov. La reconstruction du carnaval de Bakhtine a permis de mettre en évidence plusieurs approches du thème du carnaval présentes dans ses ouvrages. La distinction de ces approches est d'autant plus importante que son oubli peut causer de fausses interprétations des idées de Bakhtine exprimées dans L'oeuvre de François Rabelais et mener à des critiques injustes de son auteur. Nous avons également tenu compte des théories avec lesquelles dialogue la philosophie du carnaval de Bakhtine : nous l'avons examinée en relation avec des études de la fête, du carnaval et du sacrifice caractéristiques du paysage scientifique des premières décennies du XXe siècle. Nous avons également comparée la philosophie du carnaval aux théories des philosophes européens contemporains de Bakhtine qui se sont intéressés à la même problématique, mais qu'on n'a pas l'habitude de mentionner à ce propos. En ce qui concerne l'étude du roman Le Maître et Marguerite et l'analyse du carnaval de Boulgakov, nous avons tâché d'unir deux approches. D'une part, nous avons mis au jour les sources de l'auteur ; d'autre part, nous avons examiné cette oeuvre dans une perspective dont le but est d'aider à comprendre la spécificité du carnavalesque chez Boulgakov en s'appuyant sur des travaux d'éminents spécialistes du folklore, de la mytho-archéologie, de la mytho-critique et de la sémiotique moderne. Notre recherche a permis de mettre en évidence les manifestations caractéristiques de la poétique du carnaval dans Le Maître et Marguerite, auparavant négligées par la critique. En outre, nous avons prêté une attention particulière à la façon dont Boulgakov utilise des éléments et modèles mythologiques « pré-carnavalesques » qui n'ont pas été étudiés de manière suffisante dans l'oeuvre de Bakhtine. Cela nous a permis non seulement de voir sous un autre jour les scènes-clés du roman, mais aussi d'avoir la connaissance plus approfondie du carnaval de Bakhtine à travers le carnaval de Boulgakov.

Amitriptyline is a tricyclic antidepressant, considered the treatment of choice for different types of chronic pain, including chronic myofascial pain. Its antinociceptive property is independent of its antidepressant effect. Although its analgesic mechanism is not precisely known, it is believed that the serotonin reuptake inhibition in the central nervous system plays a fundamental role in pain control. Although this medication is widely used in the prevention of chronic tension-type headache, few studies have investigated the efficacy of this treatment and the published results are contradictory. The objective of this article was to review the literature published on the use of amitriptyline in the prophylactic treatment of chronic tension-type headache, considering the level of scientific evidence of the different studies using the SORT criteria. From this review, 5 articles of evidence level 1, and another 5 articles of evidence level 2 were selected. Following analysis of the 10 studies, and in function of their scientific quality, a level A recommendation was made in favor of using amitriptyline in the treatment of chronic tension-type headache.

Many therapies have been proposed for the management of temporomandibular disorders, including the use of different drugs. However, lack of knowledge about the mechanisms behind the pain associated with this pathology, and the fact that the studies carried out so far use highly disparate patient selection criteria, mean that results on the effectiveness of the different medications are inconclusive. This study makes a systematic review of the literature published on the use of tricyclic antidepressants for the treatment of temporomandibular disorders, using the SORT criteria (Strength of recommendation taxonomy) to consider the level of scientific evidence of the different studies. Following analysis of the articles, and in function of their scientific quality, a type B recommendation is given in favor of the use of tricyclic antidepressants for the treatment of temporomandibular disorders.

Objective: The objective of this study was to collect data on the prevalence of smokers among Catalonian dentists (by age and sex) and compare them with existing data on the general population, doctors, registered nurses and pharmacists. The overall prevalence of smokers in Catalonia (2006) was 34.5% of men and 24.3% of women. Data available on the prevalence of smoking among doctors (26.3% men and 22.1% women), pharmacists (19.8% men and 20.6% women) and registered nurses (34.1% men and 35.3% women) relates to the year 2002. Study design: In September 2006, Catalonian dentists (n=3,799) were asked about their habits in relation to tobacco in a self-administered questionnaire, on use and opinions with respect to dental amalgam. Five hundred and seventynine questionnaires were received, of which 538 answered the question on smoking (14.2% of the sample universe). Results: The prevalence of smokers among dentists is lower (24.9% of men and 18.4% of women) than in the general population and other healthcare professionals. In dentists in the age group between 25 and 34 years, the prevalence was 26.1% in men and 14.9% in women, while the prevalence in this age group in the general population was 43.6% and 37.1%, respectively. Conclusion: Catalonian dentists have a much lower prevalence of tobacco use than the general population and lower even than other healthcare professionals. Given that non-smoking healthcare professionals have better awareness for providing recommendations for smoking prevention and cessation, Catalonian dentists may be a valid group for performing this task for which there is scientific evidence.

Inverted ductal papilloma of the oral cavity is an infrequent benign neoplasm of papillary appearance that originates in the secretory duct of a salivary gland. The etiology is unknown, though some authors have related it to human papillomavirus (HPV) infection. We present the case of a 40-year-old woman with a tumor of the lower lip mucosa. Histopathological study of the lesion diagnosed inverted ductal papilloma of the oral cavity. Human papillomavirus DNA detection and typing based on tumor lesion DNA amplification and posterior hybridization, revealed no presence of viral DNA. The antecedents of trauma reported by the patient could have played an important role in the development of this tumor

This paper reviews the policy learning literature in political science. In recent years, the number of publications on learning in the political realm increased dramatically. Researchers have focused on how policymakers and administrators adapt policies based on learning processes or experiences. Thereby, learning has been discussed in very different ways. Authors have referred to learning in the context of ideas, understood as deeply held beliefs, and, as change and adaptation of policy instruments. Two other strands of literature have taken into consideration learning, namely the diffusion literature and research on transfer, which put forward learning to understand who learns from whom and what. Opposed to these views, political learning emphasizes the adaptation of new strategies by policymakers over the transfer of knowledge or broad ideas. In this approach, learning occurs due to the failure of existing policies, increasing problem pressure, scientific innovations, or a combination of these elements.

Schwannomas of the sinonasal tract are rare entities that are amenable to local resection. We present a 69 year old woman with a schwannoma of the inferior turbinate that was successfully resected. Schwannomas arising from the inferior turbinate are very infrequent.

A group of family physicians in an outpatient clinic in Switzerland prospectively followed scientific literature for ten years. What to remember among the numerous articles retrieved and which paper really changed our practice? If many readings are quickly forgotten, some of them marked our minds and changed our habits. This article is a summary of our efforts to keep the essential tools in clinical practice.

BACKGROUND: Patient-centered care (PCC) has been recognized as a marker of quality in health service delivery. In policy documents, PCC is often used interchangeably with other models of care. There is a wide literature about PCC, but there is a lack of evidence about which model is the most appropriate for maternity services specifically. AIM: We sought to identify and critically appraise the literature to identify which definition of PCC is most relevant for maternity services. METHODS: The four-step approach used to identify definitions of PCC was to 1) search electronic databases using key terms (1995-2011), 2) cross-reference key papers, 3) search of specific journals, and 4) search the grey literature. Four papers and two books met our inclusion criteria. ANALYSIS: A four-criteria critical appraisal tool developed for the review was used to appraise the papers and books. MAIN RESULTS: From the six identified definitions, the Shaller's definition met the majority of the four criteria outlined and seems to be the most relevant to maternity services because it includes physiologic conditions as well as pathology, psychological aspects, a nonmedical approach to care, the greater involvement of family and friends, and strategies to implement PCC. CONCLUSION: This review highlights Shaller's definitions of PCC as the one that would be the most inclusive of all women using maternity services. Future research should concentrate on evaluating programs that support PCC in maternity services, and testing/validating this model of care.
