Postnatal depression is a significant problem affecting 10-15% of mothers in many countries and has been the subject of an increasing number of publications. Prenatal depression has been studied less. The aims of the present investigation were: 1) to obtain information on the prevalence of prenatal and postnatal depression in low income Brazilian women by using an instrument already employed in several countries, i.e., the Edinburgh Postnatal Depression Scale (EPDS); 2) to evaluate the risk factors involved in prenatal and postnatal depression in Brazil. The study groups included 33 pregnant women interviewed at home during the second and third trimesters of pregnancy, and once a month during the first six months after delivery. Questions on life events and the mother's relationship with the baby were posed during each visit. Depressed pregnant women received less support from their partners than non-depressed pregnant women (36.4 vs 72.2%, P<0.05; Fisher exact test). Black women predominated among pre- and postnatally depressed subjects. Postnatal depression was associated with lower parity (0.4 ± 0.5 vs 1.1 ± 1.0, P<0.05; Student t-test). Thus, the period of pregnancy may be susceptible to socio-environmental factors that induce depression, such as the lack of affective support from the partner. The prevalence rate of 12% observed for depression in the third month postpartum is comparable to that of studies from other countries.

Tutkimuksen aiheena oli tutkia keskoslapsen vanhemman psyykkistä hyvinvointia ja kiintymyksen kehittymistä vauvaan keskosen sairaalahoidon aikana sekä kotiutumisen jälkeen. Tarkoituksena oli selvittää, oliko vanhempien välillä eroa masennuksen, tehohoitoympäristöstä aiheutuvan stressin ja kiintymyksen kehittymisessä keskoseen, ja muuttuivatko nämä ajan kuluessa. Lisäksi haluttiin selvittää liittyivätkö masennus, stressi ja kiintymys toisiinsa. Tutkimusaineisto koostui alle 34 raskausviikolla syntyneistä 55 keskosista ja heidän vanhemmistaan (48 äitiä ja 36 isää), jotka olivat osallistuneet Vanhemmat vahvasti mukaan (VVM) -evaluaatiotutkimukseen. Tutkimusaineistoa kerättiin kyselylomakkeilla kolmessa eri mittauspisteessä: 7±3 vuorokautta syntymästä, kotiutusajankohtana ja keskoslapsen korjatussa kuuden kuukauden iässä. Masennusta arvioitiin EPDS-mittarin (Edinburgh Postnatal Depression Scale) avulla kaikissa kolmessa aikapisteessä. Tehohoitoympäristöstä aiheutuvaa stressiä arvioitiin PSS:NICU-mittarilla (Parental Stressor Scale: Neonatal Intensive Care Unit) ja kiintymyksen kehittymistä keskosvauvaan BABY & YOU -mittarilla kahdessa ensimmäisessä mittauspisteessä. Vanhempien kokema masennus oli vähäistä, sillä keskoslapsen korjatussa kuuden kuukauden iässä 2,9 % äideistä oli masentuneita. Äidit kokivat isiin verrattuna enemmän masennusta, ja vanhempien masennus väheni syntymästä keskosen korjattuun kuuden kuukauden ikään. Vanhemmat kokivat tehohoitoympäristöstä aiheutuvan kokonaisstressin vähäisenä (äideillä ka=1,89 kh=0,52; isillä ka=1,56 kh=0,52). Äidit kokivat isiä enemmän stressiä syntymäaikapisteessä, ja äitien kokonaisstressi väheni keskosen kotiutusajankohtaan mennessä. Vanhempien kokonaisstressi tehohoitoympäristöstä korreloi hyvin vahvasti masennuksen kanssa kaikissa aikapisteissä, ja yksittäisenä eniten stressiä aiheuttavana tekijänä vanhemmat kokivat erossaolon vauvastaan. BABY & YOU -mittarilla arvioituna äidit kokivat isiä enemmän huolta ja eroahdistusta keskosvauvastaan. Syntymäaikapisteessä huoli keskosesta korreloi positiivisesti masennuksen kanssa. Äidit kokivat vähemmän masennusta, stressiä ja huolta keskosestaan, mikäli raskausaika oli ollut onnellinen. Vanhempien vähäinen masennus ja stressi voivat mahdollisesti liittyä VVM-koulutusprojektiin, jonka tavoitteena oli antaa vanhemmille vahva emotionaalinen tuki vauvansa hoitoon.

Syfte: Att bidra till förståelsen av positiv psykisk hälsa och dess samband med psykosociala faktorer och förekomsten av psykiatriska symptom. Sambandet mellan psykiskt välmående och faktorer såsom kön, ålder och språktillhörighet undersöks också i studien. Metod: Studien bygger på datamaterial från undersökningen Enkät om psykisk hälsa i västra Finland 2014 (Western Finland Mental Health Survey 2014 – WFMHS). Samplet bestod av 3 531 personer i åldrarna 15–80. I studien används Short Warwick-Edinburgh Mental Well-being Scale (Stewart-Brown et al., 2009) som instrument för att mäta positiv psykisk hälsa. Resultat: Undersökningen påvisade ett samband mellan höga värden för positiv psykisk hälsa, starkt socialt kapital, en stark känsla av bemästring och låg nivå av symptom på depression och psykisk belastning. Man fann även ett samband mellan att ha blivit utsatt för aga under barndomen och ett större alkoholmissbruk och sämre positiv psykisk hälsa. Personer med annat modersmål än svenska och finska hade signifikant sämre positiv psykisk hälsa i jämförelse med svensk- och finskspråkiga. Konklusion: Eftersom höga värden för positiv psykisk hälsa har konstaterats ha ett samband med låga värden för psykiatriska symptom stöder undersökningen tanken att samhället kunde arbeta hälsofrämjande i högre grad än tidigare för att förebygga psykisk ohälsa.

In a cross-sectional study conducted four years ago to assess the validity of the Brazilian version of the Eating Attitudes Test-26 (EAT-26) for the identification of abnormal eating behaviors in a population of young females in Southern Brazil, 56 women presented abnormal eating behavior as indicated by the EAT-26 and the Edinburgh Bulimic Investigation Test. They were each matched for age and neighborhood to two normal controls (N = 112) and were re-assessed four years later with the two screening questionnaires plus the Composite International Diagnostic Interview (CIDI). The EAT results were then compared to diagnoses originating from the CIDI. To evaluate the temporal stability of the two screening questionnaires, a test-retest design was applied to estimate kappa coefficients for individual items. Given the prevalence of eating disorders of 6.2%, the CIDI psychiatry interview was applied to 161 women. Of these, 0.6% exhibited anorexia nervosa and 5.6%, bulimia nervosa (10 positive cases). The validity coefficients of the EAT were: 40% sensitivity, 84% specificity, and 14% positive predictive value. Cronbach's coefficient was 0.75. For each EAT item, the kappa index was not higher than 0.344 and the correlation coefficient was lower than 0.488. We conclude that the EAT-26 exhibited low validity coefficients for sensitivity and positive predictive value, and showed a poor temporal stability. It is reasonable to assume that these results were not influenced by the low prevalence of eating disorders in the community. Thus, the results cast doubts on the ability of the EAT-26 test to identify cases of abnormal eating behaviors in this population.

D, Medica-Odontologica. Painosalama Oy, Turku, Finland, 2015. Dentistry must provide sustainable, evidence-based, and prevention-focused care. In Scotland oral health prevention is delivered through the Childsmile programme, with an increasing use of high concentration fluoride toothpaste (HCFT). Compared with other countries there is little knowledge of xylitol prevention. The UK government has set strict carbon emission limits with which all national health services (NHS) must comply. The purpose of these studies was firstly to describe the Scottish national oral health prevention programme Childsmile (CS), to determine if the additional maternal use of xylitol (CS+X) was more effective at affecting the early colonisation of mutans streptococci (MS) than this programme alone; secondly to analyse trends in the prescribing and management of HCFT by dentists; and thirdly to analyse data from a dental service in order to improve its sustainability. In all, 182 mother/child pairs were selected on the basis of high maternal MS levels. Motherswere randomly allocated to a CS or CS+X group, with both groups receiving Childsmile. Theintervention group consumed xylitol three times a day, from when the child was 3 months until 24 months. Children were examined at age two to assess MS levels. In order to understand patterns of HCFT prescribing, a retrospective secondary data analysis of routine prescribing data for the years 2006-2012 was performed. To understand the sustainability of dental services, carbon accounting combined a top-down approach and a process analysis approach, followed by the use of Pollard’s decision model (used in other healthcare areas) to analyse and support sustainable service reconfiguration. Of the CS children, 17% were colonised with MS, compared with 5% of the CS+X group. This difference was not statistically significant (P=0.1744). The cost of HCFT prescribing increased fourteen-fold over five years, with 4% of dentists prescribing 70% of the total product. Travel (45%), procurement (36%) and building energy (18%) all contributed to the 1800 tonnes of carbon emissions produced by the service, around 4% of total NHS emissions. Using the analytical model, clinic utilisation rates improved by 56% and patient travel halved significantly reducing carbon emissions. It can be concluded that the Childsmile programme was effective in reducing the risk for MS transmission. HCFT is increasing in Scotland and needs to be managed. Dentistry has similar carbon emissions proportionally as the overall NHS, and the use of an analytic tool can be useful in helping identify these emissions.
