Hamartoma is a proliferation of normal tissues that are considered endogenous to the site of occurrence. In the head and neck region, hamartomas composed of other tissues different from blood and lymphatic vessels (hemangiomas and lymphangiomas) are very uncommon. We report an unusual case of upper lip angiomyolipomatous hamartoma in an 8-month-old patient. The patient underwent surgical treatment and the 1-year follow-up revealed no signs of recurrence. Angiomyolipomatous hamartoma is a very rare condition in the paediatric population group, especially in the head and neck region. It should be considered in the differential diagnosis of congenital lesions in childhood. (C) 2010 European Association for Cranio-Maxillo-Facial Surgery.

The aim of this study was to verify the association between sleep bruxism (SB) and temporomandibular disorders (TMD) in a sample of 14 TMD patients and 12 healthy control subjects. All participants were evaluated using a clinical questionnaire, visual analog scale (VAS) for TMJ/muscle palpation, and by functional examination. The experimental group was divided into three TMD subgroups: joint sounds and pain, muscular tenderness, and mixed diagnosis. All participants underwent polysomnographic recording (PSG). A second clinical examination was then carried out to verify the relationship between rhythmic masticatory muscle activity and pain/tenderness on the following morning. e experimental and control groups presented VAS mean scores of 36.85 +/- 23.73 mm and 0 mm, respectively. The presence of SB was neither associated with TMD (p>0.05) nor with pain on palpation (p>0.05). Further research with a more representative sample of each TMD subgroup is necessary to elucidate its interaction with SB.

The objective of this study was to analyze the electromyographic (EMG) data, before and after normalization. One hundred (100) normal subjects (with no signs and symptoms of temporomandibular disorders) participated in this study. A surface EMG of the masticatory muscles was performed. Two different tests were performed: maximum voluntary clench (MVC) on cotton rolls and MVC in intercuspal position. The normalization was done using the mean value of the EMG signal of the first examination. The coefficient of variation CV showed lower values for the standardized data. The standardization was effective in reducing the differences between records from the same subject and in different subjects.

The aim of this study was to analyze the immediate effect of resilient splints through surface electromyography testing and to compare the findings with the electromyographic profiles of asymptomatic subjects. The participants were 30 subjects, 15 patients with TMD (TMD Group) and 15 healthy subjects (Control Group), classified according to Research Diagnostic Criteria (RDC/TMD) Axis I. A resilient occlusal splint was made for each patient in the TMD Group from two mm thick silicon to cover all teeth. The EMG examination was performed before and immediately after installing the splint. Three tests were performed as follows: 1. Maximum Voluntary Contraction (MVC) using cotton rolls (standards test); 2. MVC in maximal intercuspation position; and 3. MVC with the splint in position. The EMG signal was recorded for five seconds. EMG indices were calculated to assess muscle symmetry, jaw torque, and impact. There was a statistically significant difference when comparing the results among the study groups. The symmetry index values in the Control Group were higher than the TMD Initial Group and similar to the TMD Group after the installation of the splint. The index values of torque were higher in TMD Initial Group when compared with the Controls. Impact values were lower than normal values in the TMD Initial Group and restored upon installation of the splint. The resilient occlusal splints may be used as complementary or adjunctive treatment of temporomandibular disorders.

The purpose of this study was to evaluate the effect of diode laser (GaAlAs - 780 nm) on pain to palpation and electromyographic (EMG) activity of the masseter and anterior temporalis muscles. The laser was applied on the temporalis and masseter muscles twice a week (four weeks). Forty-eight (48) patients with myofascial pain were randomly assigned between actual and placebo treatments and between the energetic doses of 25 J/cm(2) and 60 J/cm(2), and were evaluated using VAS before, immediately after the final application, and 30 days after the laser treatment. Surface electromyography was performed with maximum dental clenching before and after laser therapy. The results show there were no significant statistical differences in the EMG activity between the groups before and after laser treatment. With regard to the pain at palpation, although both groups presented a significant difference in the symptoms before and after the treatment, only the active doses showed statistically significant reductions in pain level in all the regions of the palpated muscles. However, there was no significant statistical difference between groups (experimental and placebo). In conclusion, low level laser did not promote any changes in EMG activity. The treatment did, however, lessen the pain symptoms in the experimental groups.

The purpose of this study was to evaluate the analgesic effect of Low Intensity Laser Therapy (LILT) and its influence on masticatory efficiency in patients with temporomandibular dysfunction (TMD). This study was performed using a random, placebo-controlled, and double-blind research design. Fourteen patients were selected and divided into two groups (active and placebo). Infrared laser (780 nm, 70 mw, 60s, 105J/cm(2)) was applied precisely and continuously into five points of the temporomandibular joint (TMJ) area: lateral point (LP), superior point (SP), anterior point (AP), posterior point (PP), and posterior-inferior point (PIP) of the condylar position. This was performed twice per week, for a total of eight sessions, To ensure a double-blind study, two identical probes supplied by the manufacturer were used: one for the active laser and one for the inactive placebo laser. They were marked with different letters (A and B) by a clinician who did not perform the applications. A Visual Analogue Scale (VAS) and a colorimetric capsule method were employed. Data were obtained three times: before treatment (Ev1), shortly after the eighth session (Ev2), and 30 days after the first application (Ev3). Statistical tests revealed significant differences at one percent (1%) likelihood, which implies that superiority of the active group offered considerable TMJ pain improvement. Both groups presented similar masticatory behavior, and no statistical differences were found. With regard to the evaluation session, Ev2 presented the lowest symptoms and highest masticatory efficiency throughout therapy. Therefore, low intensity laser application is effective in reducing TMD symptoms, and has influence over masticatory efficiency [Ev2 (0.2423) and Ev3 (0.2043), observed in the interaction Evaluations x Probes for effective dosage].

Recent findings that spinal manual therapy (SMT) produces concurrent hypoalgesic and sympathoexcitatory effects have led to the proposal that SMT may exert its initial effects by activating descending inhibitory pathways from the dorsal periaqueductal gray area of the midbrain (dPAG). In addition to hypoalgesic and sympathoexcitatory effects, stimulation of the dPAG in animals has been shown to hal e a facilitatory effect on motor activity. This study sought to further investigate the proposal regarding SMT and the FAG by including a test of motor function in addition to the variables previously investigated, Using a condition randomised, placebo-controlled, double blind, repeated measures design, 30 subjects with mid to lon er cervical spine pain of insidious onset participated in the study. The results indicated that the cervical mobilisation technique produced a hypoalgesic effect as revealed by increased pressure pain thresholds on the side of treatment (P = 0.0001) and decreased resting visual analogue scale scores (P = 0.049). The treatment technique also produced a sympathoexcitatory effect with an increase in skin conductance (P < 0.002) and a decrease in skin temperature (P = < 0.02). There was a decrease in superficial neck flexor muscle activity (P < 0.0002) at the lower levels of a staged cranio-cervical flexion test. This could imply facilitation of the deep neck flexor muscles with a decreased need for co-activation of the superficial neck flexors, The combination of all findings,would support the proposal that SMT may, at least initially, exert part of its influence via activation of the PAG, (C) 2000 Harcourt Publishers Ltd.

Dysfunction in the motor system is a feature of persistent whiplash associated disorders. Little is known about motor dysfunction in the early stages following injury and of its progress in those persons who recover and those who develop persistent symptoms. This study measured prospectively, motor system function (cervical range of movement (ROM), joint position error (JPE) and activity of the superficial neck flexors (EMG) during a test of cranio-cervical flexion) as well as a measure of fear of re-injury (TAMPA) in 66 whiplash subjects within 1 month of injury and then 2 and 3 months post injury. Subjects were classified at 3 months post injury using scores on the neck disability index: recovered (30). Motor system function was also measured in 20 control subjects. All whiplash groups demonstrated decreased ROM and increased EMG (compared to controls) at 1 month post injury. This deficit persisted in the group with moderate/severe symptoms but returned to within normal limits in those who had recovered or reported persistent mild pain at 3 months. Increased EMG persisted for 3 months in all whiplash groups. Only the moderate/severe group showed greater JPE, within 1 month of injury, which remained unchanged at 3 months. TAMPA scores of the moderate/severe group were higher than those of the other two groups. The differences in TAMPA did not impact on ROM, EMG or JPE. This study identifies, for the first time, deficits in the motor system, as early as 1 month post whiplash injury, that persisted not only in those reporting moderate/severe symptoms at 3 months but also in subjects who recovered and those with persistent mild symptoms. A 13-year-old Labrador cross dog was presented with progressive abdominal distension of three to four months duration. A large abdominal mass displacing the intestines in a cranio-dorsal direction was diagnosed radiographically. A 4.5kg intra-abdominal lipoma was surgically removed from the lesser omentum near the splenic pedicle. This condition has been infrequently reported in the dog.

Este estudo tem por objectivos determinar a Dose Glandular Média - Mean Glandular Dose (MGD) - em 3 sistemas de Mamografia e comparar os valores obtidos com os referenciais internacionais. O estudo foi realizado num sistema analógico de Écran-Película (EP) e em dois sistemas de imagem digital (CR e DR). Foi efectuado o cálculo da Entrance Surface Air Kerma (ESAK) e da MGD em três equipamentos a partir de uma amostra de dados referentes a 30 mulheres assintomáticas, com idades compreendidas entre os 40 e 64 anos. Em cada equipamento objecto de análise, foram recolhidos os dados referentes a 10 mulheres. Foram consideradas as projecções crânio-caudal (CC) e oblíqua médio-lateral (MLO). A análise de resultados revelou que o valor de MGD varia quando se compara os três sistemas. Nas incidências CC os valores de MGD obtidos foram de 1,54 mGy (EP), 1,78 mGy (CR) e 0,82 mGy (DR). Nas incidências MLO o valor de MGD foi de 1,53 mGy no sistema EP, de 1,78 mGy no CR e 0,87 mGy no sistema DR. Constata-se que o valor de MGD na incidência de CC é inferior ao valor de MGD na incidência MLO, excepto para o sistema EP. Verifica-se também que o sistema EP apresenta maior variabilidade nos dados de MGD comparativamente com os restantes sistemas. O sistema DR é o que apresenta a menor variabilidade de valores MGD e também valores de MGD mais baixos. Comparando os resultados deste estudo com as referências internacionais, verifica-se que a MGD se encontra abaixo do limite de 2 mGy recomendado. ABSTRACT - This study aims to estimate the Mean Glandular Dose (MGD) associated with three different mammographic systems and compare the results with recommended international reference values. The systems included in the study included a conventional Screen-Film (SF) system and two digital mammography systems (CR and DR). Entrance Surface Air Kerma (ESAK) and MGD associated with each equipment were calculated. A sample of 30 healthy women (age ranging from 40 to 64 years old) undertaking screening mammography was considered in this study. The mammographic exam includes two projections, cranio-caudal (CC) and medio-lateral oblique (MLO). The MGD results obtained for CC projection were 1,54 mGy (SF), 1,78 mGy (CR) and 0,82 mGy (DR). MGD values for the MLO projection were 1,53 mGy (SF), 1,78 mGy (CR) and 0,87 mGy (DR). Results show that MGD value is slightly lower in the CC projection than in MLO, except for the SF system (1,54 mGy; 1,53 mGy). In addition the MGD for the SF system varied more than that associated with the digital systems. The DR system allows a narrow variation of MGD values and also lower MGD values. Comparing this study results with the international references we concluded that MGD values are below the 2 mGy recommended value for the three systems evaluated.

Introdução: Na criança, a etiologia do acidente vascular cerebral (AVC) é conhecida em 75% dos casos, sendo a anemia de células falciformes (ACF) a mais frequente na criança de raça negra. O interesse deste caso clínico reside na forma de apresentação pouco habitual e curso evitável. Caso clínico: Criança de raça negra com 27 meses de idade, sem antecedentes relevantes, admitida por sinais neurológicos focais de instalação súbita. A tomografia computorizada cranio-encefálica e ressonância magnética evidenciaram lesão isquémica aguda extensa e alterações compatíveis com AVC silencioso prévio. Analiticamente apresentava anemia normocítica, muitos drepanocitos de formação espontânea e 87% de hemoglobina S. Neste contexto, foi submetida a transfusão-permuta. Conclusão: O AVC como complicação da ACF pode acontecer em idades precoces e surgir como quadro inaugural. Pensamos que se justifica divulgar o rastreio pré-natal e realizar um estudo da relação custo-benefício para a implementação de um rastreio neonatal desta patologia em Portugal.

Os autores apresentam o caso clínico de uma doente de 35 anos, sexo feminino, que recorreu ao hospital da área da residência por parésia do membro superior direito, tendo sido diagnosticado AVC isquémico por Ressonância Magnética Nuclear Cranio-Encefálica (RMN-CE). A investigação da fonte embólica levou ao diagnóstico, por ecocardiograma transesofágico, de foramen oval patente (FOP), com shunt direito-esquerdo espontâneo. Em Maio de 2009 foi efectuado encerramento percutâneo do FOP com dispositivo bioreabsorvível “BioSTAR®”, sob controlo ecocardiográfico. Efectuou ecocardiograma transtorácico após o primeiro, terceiro e sexto mês pós-procedimento e ecocardiograma transesofágico aos nove meses, mostrando sempre adequada colocação do dispositivo, sem presença de shunt residual, sem qualquer intercorrência clínica durante este período. Os autores discutem a importância do desenvolvimento de dispositivos bioabsorvíveis para encerramento de FOP, e as suas vantagens em comparação com os dispositivos sintéticos previamente utilizados.

Dependendo da localização e do número de ossos envolvidos, a Displasia Fibrosa (D.F.) crânio-facial pode ser responsável por síndromes dismórficos e por sintomatologia otológica, oftalmológica ou rinológica. Este artigo tem por objectivo i1ustrar dois casos clínicos de D. F. Poliostótica com envolvimento predominante dos ossos temporal a etmóide. No primeiro caso clínico o envolvimento do osso temporal é responsável por síndrome vertiginoso resultante de hipofunção vestibular esquerda e da obliteração do aqueduto vestibular homolateral. Neste coso o doente foi submetido a neurectomia dos nervos vestibulares, por via retrosigmóide. O segundo caso clínico é um caso de D.F. predominantemente do osso etmóide, acompanhado de proptose e obstrução nasal, em que se procedeu à excisão total por via paralateronasal sob controlo endoscópico. Os autores fazem uma revisão da literatura sobre a clínica, o diagnóstico, e a terapêutica do da doença a nível crânio-facial.

RESUMO - Portugal atravessa um contexto socioeconómico conturbado onde se têm imposto várias reformas, nomeadamente ao nível da Saúde. Atualmente, o financiamento do internamento hospitalar é feito por grupos de diagnóstico homogéneo com base num sistema prospetivo, reunindo os episódios em grupos clinicamente coerentes e homogéneos, de acordo com o consumo de recursos necessário para o seu tratamento, tendo em conta as suas características clínicas. Apesar do objetivo deste sistema de classificação de doentes, é aceite que existe variabilidade no consumo de recursos entre episódios semelhantes, sendo que a mesma variabilidade pode representar uma diferença significativa nos custos de tratamento. Os Traumatismos Cranio-encefálicos são considerados um problema de saúde pública, pelo que os episódios selecionados para este estudo tiveram por base os diagnósticos mais comuns relacionados com esta problemática. Procurou-se estudar a relação entre o consumo esperado e o observado bem como, a forma em que esta relação é influenciada por diferentes variáveis. Para verificar a existência de variabilidade no consumo de recursos, bem como as variáveis mais influentes, foi utilizada a regressão linear e constatou-se que variáveis como a idade, o destino pós-alta e o distrito têm poder explicativo sobre esta relação. Verificou-se igualmente que na sua generalidade as instituições hospitalares são eficientes na prestação de cuidados. Compreender a variabilidade do consumo de recursos e as suas implicações no financiamento poderá suscitar a dúvida se a utilização de GDH será o mais adequado à realidade portuguesa, de forma a ajustar as políticas de saúde, mantendo a eficiência e a qualidade dos cuidados.
