Os benzodiazepínicos são medicamentos utilizados principalmente como ansiolíticos e hipnóticos e estão entre as drogas mais prescritas no mundo. Atualmente, apesar de possuírem indicações precisas, a realidade é que eles continuam sendo prescritos de modo indiscriminado. No município de Ponte Nova, o Programa Saúde da Família Abdalla Felício é uma Unidade de Atenção Primária à Saúde que convive com essa realidade, devido a causas diversas. É preocupação de toda sua equipe a alta prevalência do uso de benzodiazepínicos pela comunidade assistida, sem controle principalmente dos médicos. Este trabalho teve como objetivo elaborar um projeto de intervenção com a finalidade de organizar a prescrição e a distribuição de benzodiazepínicos aos pacientes residentes na área de abrangência da Unidade Básica de Saúde Abdalla Felício. Após o diagnóstico situacional, concluiu-se que é possível elaborar um plano de intervenção, com o intuito de estabelecer controle mais efetivo do uso desses fármacos na comunidade. Para tanto foi realizada uma revisão bibliográfica para buscar as evidências já existentes sobre o tema e assim contribuir na elaboração da proposta de intervenção O plano de ação propõe a implantação de um protocolo na unidade. Este protocolo consiste na obrigatoriedade de consulta médica regular, a cada dois meses, pelo paciente usuário de benzodiazepínicos que almeja ter sua receita renovada na Unidade. Utilizando essa estratégia, pretende-se, a cada consulta, reavaliar o caso, levantando os fatores que levaram à prescrição de tal medicação, avaliando, assim, a resposta ao tratamento até o momento da consulta e reavaliando a dose utilizada. A partir da análise e de acordo com o caso, tratamentos alternativos serão propostos

No Brasil as parasitoses intestinais continuam sendo um grave problema de saúde pública, assim como no restante dos países em desenvolvimento. São causadas principalmente pela falta de saneamento básico, precárias condições de sobrevida e outros fatores sociais, econômicos e políticos. A esquistossomose no município de Igreja Nova, Alagoas, incide principalmente em agricultores que trabalham diretamente no cultivo do arroz, pescadores e pessoas que se banham em águas contaminadas. Nesse sentido, este plano de intervenção tem como objetivo realizar ações educativas voltadas para prevenção da esquistossomose no município. Para subsidiar a construção do plano foram utilizadas informações disponíveis na Secretaria Municipal de Saúde de Igreja Nova (relatórios de gestão), informações obtidas dos profissionais de saúde da UBS, além de textos disponíveis na Biblioteca Virtual do Programa Ágora (Núcleo de Educação em Saúde Coletiva, NESCON; Faculdade de Medicina, UFMG). Por sua vez, foram consultados os bancos de dados da Vigilância Epidemiológica do município, utilizando-se os descritores de busca: "Doenças infecciosas e parasitárias"; "Parasitologia humana"; "Bases de parasitologia médica"; "Vigilância epidemiológica e controle da Esquistossomose: normas e instruções" entre outros. A construção da revisão bibliográfica sobre o tema Esquistossomose foi realizada por meio de busca de artigos em periódicos, consultas em livros textos, e publicações disponibilizadas pelo Ministério da Saúde. As atividades serão direcionadas para trabalhadores da agricultura, pescadores e toda a comunidade que de uma forma ou de outra entra em contato com águas estagnadas contaminadas. As ações educativas contarão com reuniões, capacitações e oficinas educativas. Ao se trabalhar com este plano de intervenção focado em educação para saúde devemos pensar nas articulações que deverão ser feitas. Neste caso, além da participação da população, os órgãos responsáveis pela qualidade da água e do ambiente no município, e outros setores deverão ser envolvidos nesta atividade.

Meu TCC consta de temas extremamente presentes em nosso meio , diabetes , obesidade e parasitoses intestinais, este último abordado dentro do portfólio , a prevalência e relato de caso foram os modelos apresentados , de modo simples e claro , sobre um pouco da minha vivência dentro da USF de Vila Nova no município de Vigia - PA.

Diabetes é um grupo de doenças metabólicas caracterizadas por hiperglicemia e associadas a complicações micro e macro vascular, que necessita de uma série de mudanças nos hábitos de vida e alimentação deveriam acontecer para o devido controle da doença. O Diabetes Mellitus, assim como a Hipertensão arterial sistêmica, está dentre as doenças crônicas não transmissíveis mais frequentes no mundo, são condições clínicas de caráter multifatorial, de alta prevalência e de baixo controle. Este trabalho, estudo de caso clinico, foi desenvolvido, baseado em revisão bibliográfica, visando à necessidade de diagnóstico precoce e acompanhamento de pacientes portadores de diabetes mellitus na atenção primaria de saúde com finalidade de manter o controle glicêmico, com medidas não farmacológicas (dieta e atividades físicas) e farmacológicas, e com adesão efetiva ao tratamento pelo paciente, minimizando assim o desenvolvimento de complicações micro e macrovasculares.

The occurrence of de novo psychogenic seizures after epilepsy surgery is rare, and is estimated in 1.8% to 3.6%. Seizures after epilepsy surgery should be carefully evaluated, and de novo psychogenic seizures should be considered especially when there is a change in the ictal semiology. We report a patient with de novo psychogenic seizures after anterior temporal lobe removal for refractory temporal lobe epilepsy. Once psychogenic seizures were diagnosed and psychiatric treatment was started, seizures stopped.

The individual affective-cognitive evaluations are important factors that control the way he feels the disease impact in his life. Then, the perception of seizure control is a more important factor to evaluate Quality of Life (QoL) than the illness characteristics, such as the severity, type, sickening period and seizure frequency. This study searched for the relationship among the subjective variables (perception of seizure control) and the illness characteristics to evaluate QoL. The sample consisted of 60 individuals with chronic epilepsy, aging 18 to 70 (M=37.05; SD=11.25), chosen at randon from the ambulatory of epilepsy - HC/UNICAMP, by the Questionnaire 65. The illness characteristics were not significant, except the seizures frequency, when associated to the impairment in QoL among controlled seizures and seizures with frequency higher than 10 per month (p=0.021). The perception of control was significantly associated to QoL (p=0.005).

By mid 2003, the Brazilian people accompanied astonished, in the press, the news about the death of more than 20 persons due to ingestion of a pharmaceutical product containing a suspension of barium sulfate (Celobar®) commonly used as a radiological contrast. Analysis of the product indicated the presence of barium carbonate (about 13% weight/weight) which reacts easily with the hydrochloric acid in the stomach liberating barium ions, a severe poison. In this article, we briefly discuss the possible economic, personal and technical causes that led to this disaster.

An absolute method is described via mass spectrometry (MS) for the structural assignment of isomers within the class of methylpiperidines. The method explores both the unimolecular and bimolecular gas phase behavior of structurally diagnostic fragment ions (SDFI). For the methylpiperidnes, the isomeric 2-methyl, 3-methyl and 4-methyl 2-azabutadienyl cations are found to function as SDFI. These fragment ions are expected to be formed from all members within the class, to be stable and to retain the structural information of the precursor molecule, and to not interconvert into one another. To characterize these SDFI, both the collision induced dissociation (CID) in argon and bimolecular ion/molecule chemistry with ethyl vinyl ether were compared.

The aim of this study was to analyze the prevalence of hypertension and control practices among the elderly. The survey analyzed data from 872 elderly people in São Paulo, Brazil, through a cluster sampling, stratified according to education and income. A Poisson multiple regression model checked for the existence of factors associated with hypertension. The prevalence of self-reported hypertension among the elderly was 46.9%. Variables associated with hypertension were self-rated health, alcohol consumption, gender, and hospitalization in the last year, regardless of age. The three most common measures taken to control hypertension, but only rarely, are oral medication, routine salt-free diet and physical activity. Lifestyle and socioeconomic status did not affect the practice of control, but knowledge about the importance of physical activity was higher among those older people with higher education and greater income. The research suggests that health policies that focus on primary care to encourage lifestyle changes among the elderly are necessary.

The copolymer poly (L-co-D,L lactic acid), PLDLA, has gained prominence in the field of temporary prostheses due to the fact that their time of degradation is quite compatible with the requirement in the case of osseous fracture. In this work the in vivo degradation of devices from copolymer, as a system of plates and screws, used in fixation of the tibia of rabbits was studied. The devices were implanted in 15 adult rabbits, albinos, New Zealand race, and they were used as control devices of alloys of titanium (Ti-6Al-4V/ V grade). The use of copolymers, synthesized in the laboratory, was tested in the repair of fracture in rabbits'tibias, being assessed in the following times: 2 weeks, 2 months and 3 months. Morphological analysis of tissue surrounding the plate and screw system, for 2 weeks of implantation, showed the presence of osteoblasts, indicating a pre bone formation. After 2 months there was new bone formation in the region in contact with the polymer. This bone growth occurred simultaneously with the process of PLDLA degradation, invading the region where there was polymer and after 3 months there was an intense degradation of the copolymer and hence greater tissue invasion compared to 2 months which characterized bone formation in a region where the polymer degraded. The in vivo degradation study of the devices for PLDLA by means of histological evaluations during the period of consolidation of the fracture showed the efficiency of plate and screw system, and it was possible to check formation of bone tissue at the implantation site, without the presence of inflammatory reaction

The aim of this study was to test fear, anxiety and control related to dental treatment. The subjects were 364 children with ages between 7 and 13 years. Three questionnaires with multiple choice questions were applied in groups of 10 children. The first instrument was the 15-item dental subscale from the Childrens Fear Survey Schedule9. The subjects rated their level of fear on a 5-point scale. The second survey instrument was the 20-item subscale from the State Trait Anxiety Inventory for Children16. This measure was used to capture how anxious the child was, in general. The third instrument was the Child Dental Control Assessment19. It contained 20 items to assess perceived control and 20 items to assess desired control. The results of the survey indicated that dental fear and anxiety were slightly higher for females when compared with male subjects (P < 0.05). Older children (11 to 13 years old) obtained higher fear scores than younger ones (7 to 9 years old). Concerning perceived control, the results indicate that younger children perceive more control than older ones. For desired control, the results indicate that younger children reported higher percentages than older ones. In this study, patients who had undergone anesthesia during treatment revealed higher fear scores when compared with those who had not. Dental fear etiology seems to be related to a procedure that may involve pain or lack of control.

A case report of an atipical inferior rectus congenital fibrosis associated with a generalized muscular hypotonia is presented. It shows mild hipotropia with a normal visual acuity and binocularity in infraversion position contrasting with the severe hipotropia and ambliopy found. The surgical procedure took these facts into consideration, so we performed a small retrocess of the muscle with the objective to correct the head position without changes in the reading position. The postoperative results were satisfactory.

PURPOSE: To collect information and opinions from a group of diabetic patients regarding diabetic retinopathy and its treatment, in order to get reliable information that can help to improve programs and actions to control and prevent this ocular disease. METHODS: A cross-sectional study was performed. The sample was from 980 diabetic patients seen in a diabetic association. A previous questionnaire was made with general questions about the main subject. Thereafter, an appropriate questionnaire was prepared. RESULTS: The sample showed that among 299 patients with age ranging from 16 to 83 years, with a mean of 57 years, mainly female (67.91%) did not know how severe their disease was (30.8%), or believed that it was not a serious problem (19.7%). The laser technique to solve diabetic retinopathy was known by 60.2% of the patients. It was reported as the only treatment available by 24.1%. Among the reasons for no treatment 59.8% reported that they did not think it was necessary and 29.7% could not afford it. CONCLUSIONS: Patients showed lack of knowledge about how serious is diabetic retinopathy, the possibility of using laser technique for it and the severity of the disease. Some patients believed in the efficacy of the treatment and some patients did not, but all of them reported that they were afraid of submitting to it.

PURPOSE: To report a case of Nocardia asteroides scleritis in a patient without risk factors for infeccious scleritis. METHODS: A 38-year old woman was initially examined for pain, discharge, photophobia of 1 month duration in her right eye. Her medical and ophthalmological history were unremarkable. The results of laboratory tests were normal. Surgical debridement of necrotic tissue was performed and material was sent for biopsy and culture confirmed as Nocardia asteroides. Treatment consisted of amikacin eyedrops, and systemic trimethropim-sulfamethoxazole. The infection resolved leaving scleral thinning and a subconjunctival fibrovascular scar. Best corrected visual acuity two months after referral had improved to LE, 20/20. CONCLUSION: Prompt evaluation and treatment is essential for successful management of Nocardia asteroides infectious scleritis.

PURPOSES: 1) To verify the impact of the creation of the Single Technical Record (STR) at the University of Campinas (Unicamp) Hospital das Clínicas, on the preservation period of corneas which were used in elective penetrating keratoplasties, and 2) to compare the primary failure incidence in cornea penetrating keratoplasties regarding the periods before and after the creation of STR. METHODS: A retrospective study was conducted at the Unicamp Hospital, which evaluated 15 consecutive cornea penetrating keratoplasties between January 1st and April 30th, 2000 and 24 consecutive penetrating keratoplasties between May 1st and September 20th of the same year (corneas under the control of the STR), totaling 39 keratoplasties. RESULTS: The mean time between cornea preparation and transplantation was 3.8 days (±1.78) in the period before STR creation, and 6.0 days (±2.97) after STR creation, representing a 36.7% increase in the preservation time. There was a statistically significant difference (p=0.02) between the two groups. No corneal primary failure was observed among the 39 transplanted patients in both groups. CONCLUSION: Based on the results of this study, it can be concluded that this new concept of the State Transplantation System has caused a statistically significant increase in the conservation period of corneas, which may reduce the period of a clear transplant due to an increased loss of endothelial cells, as well as increase the primary failure incidence or result in a high number of corneas that cannot be used due to having exceeded the preservation time recommended by the literature.
