Numerous studies have examined which individual defense mechanisms are related with mental health, and which are linked with psychopathology. However, the idea that a flexible use of defensive mechanisms is related to psychological wellbeing remained a clinical assumption, which this study sought to test empirically. A total of 62 (N = 62) outpatients participated in the study and were assessed with the Symptom Checklist-90R and the Social Adjustment Self-rated Scale. A subsample of 40 participants was further assessed using the Hamilton Depression (HAMD-21) and Anxiety scales (HAMA-21). The first therapy session of all participants was transcribed and rated using the Defense Mechanisms Ratings Scales (), and the Overall Defensive Functioning (ODF) score, which indicates the maturity of one's defensive functioning, was computed. An indicator of flexible use of defenses was also calculated based on the Gini Concentration C measure. Results showed that defensive flexibility, but not ODF, could predict anxiety scores. Symptom severity was predicted by both ODF and defensive flexibility, although in directions opposite to our predictions. Results suggest that defensive flexibility captures another aspect of an individual's functioning not assessed by the ODF, and that it is a promising new way of documenting defensive functioning.

A taxa de mineralização do nitrogênio (N) varia de um resíduo orgânico para outro, evidenciando-se dependente da sua composição química e interação do material orgânico com o solo. Este estudo foi realizado com o objetivo de avaliar a dinâmica de mineralização de N de resíduos orgânicos incubados em Latossolos. O experimento foi conduzido no período de julho de 2007 a abril de 2008, sendo incubadas amostras de estercos, lodos, compostos, substrato e turfa em areia lavada, Latossolo Vermelho-Amarelo (LVA) e Latossolo Vermelho distroférrico (LVdf), com 240 e 670 g kg-1 de argila, respectivamente. O N mineralizado foi avaliado medindo-se os teores de N-NH4+ e N-NO3- em lixiviados coletados aos 15, 30, 45, 60, 75, 90, 120, 150, 180, 210, 240 e 270 dias de incubação. A mineralização de N ocorreu a maiores taxas nos períodos iniciais de incubação. O N mineralizado (33 a 199,2 mg kg-1) após 270 dias de incubação é regulado pelos teores de N total e de carbono solúvel em água dos resíduos orgânicos. Independentemente do meio utilizado para incubação, os estercos de galinha e de codorna propiciaram os maiores teores de N mineralizado nos Latossolos. À exceção dos estercos de galinha e codorna, há imobilização líquida de N no LVA; mineralização líquida de N é verificada no LVdf, para todos os resíduos orgânicos. Em relação às formas de N disponibilizadas pelos resíduos, predomina o N-nitrato, sendo a proporção N-nítrico/N-amoniacal dependente da mistura solo-resíduo estudada.

OBJECTIVES: This study aimed to determine adjustments in spring-mass model characteristics, plantar loading and foot mobility induced by an exhaustive run. DESIGN: Within-participants repeated measures. METHODS: Eleven highly-trained adolescent middle-distance runners ran to exhaustion on a treadmill at a constant velocity corresponding to 95% of velocity associated with VO₂max (17.8 ± 1.4 kmh(-1), time to exhaustion=8.8 ± 3.4 min). Contact time obtained from plantar pressure sensors was used to estimate spring-mass model characteristics, which were recorded (during 30 s) 1 min after the start and prior to exhaustion using pressure insoles. Foot mobility magnitude (a composite measure of vertical and medial-lateral mobility of the midfoot) was measured before and after the run. RESULTS: Mean contact area (foot to ground), contact time, peak vertical ground reaction force, centre of mass vertical displacement and leg compression increased significantly with fatigue, while flight time, leg stiffness and mean pressure decreased. Leg stiffness decreased because leg compression increased to a larger extent than peak vertical ground reaction forces. Step length, step frequency and foot mobility magnitude did not change at exhaustion. CONCLUSIONS: The stride pattern of adolescents when running on a treadmill at high constant velocity deteriorates near exhaustion, as evidenced by impaired leg-spring behaviour (leg stiffness) and altered plantar loading.

This paper applies probability and decision theory in the graphical interface of an influence diagram to study the formal requirements of rationality which justify the individualization of a person found through a database search. The decision-theoretic part of the analysis studies the parameters that a rational decision maker would use to individualize the selected person. The modeling part (in the form of an influence diagram) clarifies the relationships between this decision and the ingredients that make up the database search problem, i.e., the results of the database search and the different pairs of propositions describing whether an individual is at the source of the crime stain. These analyses evaluate the desirability associated with the decision of 'individualizing' (and 'not individualizing'). They point out that this decision is a function of (i) the probability that the individual in question is, in fact, at the source of the crime stain (i.e., the state of nature), and (ii) the decision maker's preferences among the possible consequences of the decision (i.e., the decision maker's loss function). We discuss the relevance and argumentative implications of these insights with respect to recent comments in specialized literature, which suggest points of view that are opposed to the results of our study.

Idiopathic inflammatory myopathies, such as polymyositis and dermatomyositis, share common clinical features such as progressive, symmetrical muscle weakness prevailing in the lower limbs, associated sometimes with muscle pains. High CK and typical biopsy insure the diagnosis. Possible causes for secondary myopathies and associated diseases should be actively investigated. The search for autoantibodies helps to better classify inflammatory myopathies and to better define the prognosis of the myopathy. Glucocorticoids are the cornerstone of the early phase therapy. Glucocorticoid-sparing agents, such as azathioprine and methotrexate, are second line agents but can be readily prescribed. In case of therapeutic resistance, a rescue treatment (ciclosporine, immunoglobulins, rituximab, cyclophosphamide) could be considered.

When expressed in Xenopus oocytes, GLUT1, 2 and 4 transport glucosamine with V(max) values that are three- to four-fold lower than for glucose. The K(m)s for glucosamine and glucose of GLUT1 and GLUT4 were similar. In contrast, GLUT2 had a much higher apparent affinity for glucosamine than for glucose (K(m)=0.8+/-0.1 mM vs. approximately 17-20 mM). Glucosamine transport by GLUT2 was confirmed in mammalian cells and, using hepatocytes from control or GLUT2-null mice, HgCl(2)-inhibitable glucosamine uptake by liver was shown to be exclusively through GLUT2. These data have implications for glucosamine effects on impaired glucose metabolism and for structure-function studies of transporter sugar binding sites.

Cet article consiste en une discussion critique de la question de la communication en matière de prise en charge du patient atteint de cancer et de l'enseignement des aptitudes communicationnelles en oncologie. Nous avons d'abord essayé de définir les contours de ce qu'est (ou devrait être) une communication adéquate en oncologie, pour ensuite aborder les concepts sous-tendant les formations à la communication dans ce domaine, le problème des recommandations d'experts et du type dominant d'évaluation se rapportant à ces formations ainsi que les contenus enseignés et ainsi véhiculés.
