We estimated risk of suicide in adults in New South Wales (NSW) by sex, country of birth and rural/urban residence, after adjusting for age; we also examined youth suicide (age 15-24 years). The study population was the entire population of NSW, Australia, aged greater than or equal to 15 years during the period 1985-1994. Poisson regression was used to examine the relationship between predictor variables and the risk of suicide, with the focus on migrant status and area of residence. A significantly higher risk of suicide was found in male migrants from Northern Europe and Eastern Europe/former USSR, compared to Australian-born males; a significantly lower suicide risk occurred in males from Southern Europe, the Middle East and Asia. In female migrants, those from UK/Eire, Northern Europe, Eastern Europe/former USSR and New Zealand exhibited a significantly higher risk of suicide compared to Australian-born females. A significantly lower risk of suicide occurred in females from the Middle East. Male migrants overall were at significantly lower risk of suicide than the Australian-born, while female migrants overall had a significantly higher risk of suicide than Australian-born females. Among migrant males overall, the rural-urban suicide risk differential was significantly higher for those living in non-metropolitan areas (RR = 1.9; 95% CI: 1.7-2.1). Suicide risk was significantly higher in non-metropolitan male immigrants from the UK/Eire (RR = 1.4; 95% CI: 1.1-1.7), Southern Europe (RR = 1.7; 95% CI: 1.2-2.4), Northern/Western Europe (1.5; 95% CI: 1.2-1.9), the Middle East (RR = 3.8; 95% CI: 1.9-7.8), New :Zealand (RR = 1.4; 95% CI: 1.0-1.8) and 'other' (RR = 2.6; 95% CI: 1.9-3.5), when compared to their urban counterparts. There was no statistically significant difference in suicide risk between rural and urban Australian-born males. For female suicide, significantly lower risk was found in female immigrants living in non-metropolitan areas who were from Northern/Western Europe (RR = 0.7; 95% CI: 0.4-0.96), as well as the Australian-born (RR = 0.7; 95% CI: 0.6-0.8), when compared to their urban counterparts. The non-metropolitan/metropolitan relative risk for suicide in female migrants overall was not significantly different from one. Among male youth there was a significantly higher suicide risk in non-metropolitan areas, with a relative risk estimate of 1.4 for Australian-born youth (95% CI: 1.2-1.5) and 1.7 for migrant youth (95% CI: 1.2-2.4), when compared with metropolitan counterparts. We conclude that suicide among migrant males living in non-metropolitan areas accounts for most of the excess of male suicide in rural NSW, and the significantly lower risk of suicide for non-metropolitan Australian-born women does not apply to migrant women. Objective-To compare analgesic effects of tramadol, codeine, and ketoprofen administered alone and in combination and their effects on concentrations of blood glucose, serum cortisol, and serum interleukin (IL)-6 in dogs undergoing maxillectomy or mandibulectomy. Animals-42 dogs with oral neoplasms. Procedures-30 minutes before the end of surgery, dogs received SC injections of tramadol (2 mg/kg), codeine (2 mg/kg), ketoprofen (2 mg/kg), tramadol + ketoprofen, or codeine + ketoprofen (at the aforementioned dosages). Physiologic variables, analgesia, and sedation were measured before (baseline) and 1, 2, 3, 4, 5, and 24 hours after surgery. Blood glucose, serum cortisol, and serum IL-6 concentrations were measured 1, 3, 5, and 24 hours after administration of analgesics. Results-All treatments provided adequate postoperative analgesia. Significant increases in mean +/- SD blood glucose concentrations were detected in dogs receiving tramadol (96 +/- 14 mg/dL), codeine (120 +/- 66 mg/dL and 96 +/- 21 mg/dL), ketoprofen (105 +/- 22 mg/dL), and codeine + ketoprofen (104 +/- 16 mg/dL) at 5, 1 and 3, 5, and 3 hours after analgesic administration, respectively, compared with preoperative (baseline) values. There were no significant changes in physiologic variables, serum IL-6 concentrations, or serum cortisol concentrations. Dogs administered codeine + ketoprofen had light but significant sedation at 4, 5, and 24 hours. Conclusions and Clinical Relevance-Opioids alone or in combination with an NSAID promoted analgesia without adverse effects during the 24-hour postoperative period in dogs undergoing maxillectomy or mandibulectomy for removal of oral neoplasms. One hundred and twenty subjects with Chagas' cardiopathy and 120 non-infected subjects were randomly selected from first time claimants of sickness benefits in the National Institute of Social Security (INPS) in Goiás. Cases of Chagas' cardiopathy were defined based on serological test, history of residence in an endemic area and, clinical and/or electrocardiogram (ECG) alterations suggestive of Chagas' cardiomyopathy. Controls were defined as subjects with at least two negative serological tests. Case and controls were compared in the analysis for age, sex, place of birth, migration history, socio-economic level, occupation, physical exertion at work, age at affiliation and years of contribution to the social security scheme, clinical course of their disease and ECG abnormalities. Chagas' disease patients were younger than other subjects and predominantly of rural origin. Non-infected subjects presented a better socio-economic level, were performing more skilled activities and had less changes of job than cases. No important difference was observed in relation to age at affiliation to INPS. About 60% of cases have claimed for benefits within the first four years of contribution while among controls this proportion was 38.5%. Cases were involved, proportionally more than controls, in "heavy" activities. A risk of 2.3 (95%CL 1.5 - 4.6) and 1.8 (95%CL 1.2- 3.5) was obtained comparing respectively "heavy" and "moderate" physical activity against "light". A relative risk of 8.5 (95%CL 4.9 - 14.8) associated with the presence of cardiopathy was estimated comparing the initial sample of seropositive subjects and controls. A high relative risk was observed in relation to right bundle branch block (RR = 37.1 95%CL = 8.8 - 155.6) and left anterior hemiblock (RR = 4.4, 95%CL = 2.1 - 9.1).

OBJECTIVE: To assess the receptive vocabulary of children aged between two years and six months and five years and eleven months who were attending childcare centers and kindergarten schools. METHODS: An analytical cross-sectional study was carried out in the municipality of Embu, Southeastern Brazil. The Peabody Picture Vocabulary Test and analysis of factors associated with children's performance were applied. The sample consisted of 201 children of both genders, aged between two and six years. Statistical analysis was performed using multivariate analysis and logistic regression model. The dependent variable analyzed was test performance and the independent variables were child's age, mother's level of education and family socio-demographic characteristics. RESULTS: It was observed that 44.3% of the children had performances in the test that were below what would be expected for their age. The factors associated with the best performances in the test were child's age (OR=2.4; 95% CI: 1.6-3.5) and mother's education level (OR= 3.2; 95% CI: 1.3-7.4). CONCLUSIONS: Mother's education level is important for child's language development. Settings such as childcare and kindergarten schools are protective factors for child development in families of low income and education.

Neste seu livro, Fibonacci coloca um problema, a partir da observação do crescimento de uma população de coelhos: "num pátio fechado coloca-se um casal de coelhos. Supondo que em cada mês, a partir do segundo mês de vida, cada casal dá origem a um novo casal de coelhos, ao fim de um ano, quantos casais de coelhos estão no pátio?" A resolução desse problema deu origem à famosa sucessão (ou sequência) de Fibonacci (ou os números de Fibonacci): 1, 1, 2, 3, 5, 8, 13, 21, 34, 55, 89, 144,... outra razão apontada para a projeção mundial de Fibonacci. OBJETIVO: Avaliar os fatores de risco da mortalidade neonatal precoce. MÉTODOS: Estudo caso-controle de base populacional com 146 óbitos neonatais precoces e amostra de 313 controles obtidos entre os sobreviventes ao período neonatal, na região sul do município de São Paulo, no período de 1/8/2000 a 31/1/2001. As informações foram obtidas por meio de entrevistas domiciliares e prontuários hospitalares. Foi realizada análise hierarquizada em cinco blocos com características: 1) socioeconômicas das famílias e das mães; 2) psicossociais maternas; 3) biológicas e da história reprodutiva materna; 4) do parto; 5) do recém-nascido. RESULTADOS: Os fatores de risco para a mortalidade neonatal precoce foram: Bloco 1: baixa escolaridade do chefe da família (OR=1,6; IC 95%: 1,1;2,6); domicílio em favela (OR=2,0; IC 95%: 1,2;3,5), com até um cômodo (OR=2,2; IC 95%: 1,1;4,2); Bloco 2: mães com união recente (OR=2,0; IC 95%: 1,0;4,2) e sem companheiro (OR=1,8; IC 95%: 1,1;3,0), presença de maus tratos (OR=2,7;1,1-6,5); Bloco 3: presença de intercorrência na gravidez (OR=8,2; IC 95%: 5,0;13,5), nascimento prévio de baixo peso (OR=2,4; IC 95%: 1,2;4,5); pré-natal ausente (OR=16,1; IC 95%: 4,7;55,4) ou inadequado (OR=2,1; IC 95%: 2,0;3,5); Bloco 4: presença de problemas no parto (OR=2,9; IC 95%: 1,4;5,1), mães que foram ao hospital de ambulância (OR=3,8; IC 95%: 1,4;10,7); Bloco 5: baixo peso ao nascer (OR=17,3; IC 95%: 8,4;35,6), nascimento de pré-termo (OR=8,8; IC 95%: 4,3;17,8). CONCLUSÕES: Além dos fatores proximais (baixo peso ao nascer, gestações de pré-termo, problemas no parto e intercorrências durante a gestação), identificou-se a participação de variáveis que refletem exclusão social e de fatores psicossociais. Esse contexto pode afetar o desenvolvimento da gestação e dificultar o acesso das mulheres aos serviços de saúde. A assistência pré-natal adequada poderia minimizar parte do efeito dessas variáveis.
