La ricerca proposta si pone l’obiettivo di definire e sperimentare un metodo per un’articolata e sistematica lettura del territorio rurale, che, oltre ad ampliare la conoscenza del territorio, sia di supporto ai processi di pianificazione paesaggistici ed urbanistici e all’attuazione delle politiche agricole e di sviluppo rurale. Un’approfondita disamina dello stato dell’arte riguardante l’evoluzione del processo di urbanizzazione e le conseguenze dello stesso in Italia e in Europa, oltre che del quadro delle politiche territoriali locali nell’ambito del tema specifico dello spazio rurale e periurbano, hanno reso possibile, insieme a una dettagliata analisi delle principali metodologie di analisi territoriale presenti in letteratura, la determinazione del concept alla base della ricerca condotta. E’ stata sviluppata e testata una metodologia multicriteriale e multilivello per la lettura del territorio rurale sviluppata in ambiente GIS, che si avvale di algoritmi di clustering (quale l’algoritmo IsoCluster) e classificazione a massima verosimiglianza, focalizzando l’attenzione sugli spazi agricoli periurbani. Tale metodo si incentra sulla descrizione del territorio attraverso la lettura di diverse componenti dello stesso, quali quelle agro-ambientali e socio-economiche, ed opera una sintesi avvalendosi di una chiave interpretativa messa a punto allo scopo, l’Impronta Agroambientale (Agro-environmental Footprint - AEF), che si propone di quantificare il potenziale impatto degli spazi rurali sul sistema urbano. In particolare obiettivo di tale strumento è l’identificazione nel territorio extra-urbano di ambiti omogenei per caratteristiche attraverso una lettura del territorio a differenti scale (da quella territoriale a quella aziendale) al fine di giungere ad una sua classificazione e quindi alla definizione delle aree classificabili come “agricole periurbane”. La tesi propone la presentazione dell’architettura complessiva della metodologia e la descrizione dei livelli di analisi che la compongono oltre che la successiva sperimentazione e validazione della stessa attraverso un caso studio rappresentativo posto nella Pianura Padana (Italia).

Introduzione L’attività fisica moderata seguita da improvvisa interruzione può influenzare le caratteristiche biologiche del tendine. Lo scopo del lavoro è stato quello di valutare l’attività cellulare, le caratteristiche istologiche, istomorfometriche e microstrutturali del tendine patellare e della sua entesi in condizioni di non allenamento (sedentarietà), allenamento ed improvviso arresto dell’attività fisica. E’ stato ipotizzato che un’iniezione peri-tendinea di acido ialuronico nelle settimane successive all’improvviso arresto dell’attività fisica potesse mantenere l’integrità strutturale e biologica del tendine patellare. Materiali e Metodi 24 ratti Sprague Dawley maschi di 8 settimane sono stati suddivisi in tre gruppi, allenati per 10 settimane, fino a 60-80% VO2max. I ratti sono stati suddivisi in tre gruppi: Non Allenati (6), Allenati (6), Disallenati (12). A 6 dei 12 ratti del gruppo Detrained, è stata praticata un’infiltrazione peri-tendinea a nel tendine patellare destro di 300 μl di acido ialuronico, mentre nei rimanenti 6, è stata praticata l’infiltrazione con soluzione fisiologica. I tendini rotulei espiantati sono stati valutati con coltura cellulare, valutazione biologica molecolare, valutazioni morfologiche microstrutturali, proliferazione, conta ed attività cellulare. Risultati I risultati in vitro hanno evidenziato vitalità e conta cellulare simili fra i Gruppi Trained e Detrained-HA con un incremento significativo del metabolismo cellulare rispetto agli altri Gruppi. La cellularità ha mostrato valori maggiori nei Gruppi Non Allenati e Detrained-NaCl ove si è osservata una biosintesi del collagene III superiore ai Gruppi Trained e Detrained-HA. Contrariamente, la produzione di collagene I e II presentava valori maggiori nei Gruppi Trained e Detrained-HA suggerendo una superiore efficienza tessutale e metabolica di questi ultimi. Conclusioni Questi risultati confermano che l’allenamento ed il suo improvviso arresto hanno effetti sulla struttura tendinea patellare di ratto e che l’iniezione peritendinea di acido ialuronico nel periodo di inattività ha effetti significativi su metabolismo cellulare e sul tendine rispetto al trattamento con soluzione fisiologica.

Der Forschungsgegenstand der vorliegenden Arbeit war die Identifikation und Interpretation von Traumata an menschlichen Skeletten. Neben einer umfassenden Darstellung des aktuellen Kenntnisstandes unter verschiedenen Gesichtspunkten wurden menschliche Überreste aus der Schlacht von Dornach 1499 n. Chr. untersucht. Ergänzend wurde eine Versuchsreihe mit Replika mittelalterlicher Waffen an Kunstköpfen durchgeführt. Für die Ansprache und Kategorisierung von Traumata an Skelettfunden existiert kein einheitliches und allgemein gebräuchliches System. Die verschiedenen Herangehensweisen und ihre Vor- und Nachteile wurden benannt und diskutiert. Nachfolgend wurden die Erscheinungsformen prä-, peri- und postmortaler Traumata bzw. Defekte sowie von Verletzungen durch stumpfe und scharfe Gewalt, Schussverletzungen und anderen Verletzungsarten dargestellt. Weitere besprochene Aspekte waren die Abgrenzung von Traumata gegen pathologische Veränderungen und anatomische Varianten sowie eine Diskussion der Methodik und Problematik der Erfassung von Verletzungsfrequenzen. Neben der Bestimmung von Geschlecht, Sterbealter und Körperhöhe wurden an den zur Untersuchung zur Verfügung stehenden Schädeln (N=106) und Femora (N=33) aus der Schlacht von Dornach 1499 n. Chr. pathologische und postmortale Veränderungen sowie als Schwerpunkt prä- und perimortale Traumata identifiziert und beschrieben. Die anthropologischen Befunde zeichneten das Bild einer in Hinsicht auf Sterbealter und Körperhöhe heterogenen Gruppe von Männern mit wenigen pathologischen Veränderungen. Die Ergebnisse wurden vor dem Hintergrund des spätmittelalterlichen Söldnerwesens diskutiert. An den Schädeln wurden insgesamt 417 perimortale Traumata identifiziert, wobei Hiebverletzungen stark überwogen. Die Entstehungsweise charakteristischer Merkmale von Hiebverletzungen konnte experimentell nachvollzogen werden. Weiter stellte sich heraus, dass Hiebverletzungen durch Schwerter und Hellebarden nur in Ausnahmefällen voneinander unterschieden werden können. Verletzungen durch punktuelle Einwirkungen und stumpfe Gewalt sowie Schussverletzungen wurden in weitaus geringerer Häufigkeit festgestellt. Experimentell konnte gezeigt werden, dass die Verletzungen durch punktuelle Einwirkungen mit einer Beibringung durch Langspiesse, Stossspitzen und Reisshaken von Hellebarden sowie Armbrustbolzen vereinbar sind, wobei beträchtliche Limitationen einer genaueren Waffenzuordnung offenkundig wurden. Die Verletzungen konnten als wohl typisch für die damalige Zeit bezeichnet werden, da sie das zeitgenössische Waffenspektrum deutlich widerspiegeln. Die Lokalisation der perimortalen Traumata am Schädel liess kein Muster erkennen, mit Ausnahme der Feststellung, dass grössere Schädelknochen mehr Verletzungen aufwiesen als kleinere. Diese regellose Verteilung wurde als Hinweis darauf verstanden, dass die Kampfweise keine „ritterliche“ gewesen sein dürfte, was in Einklang mit den damals geltenden Kriegsordnungen steht. Postmortale Veränderungen unterschiedlicher Art liessen vermuten, dass die untersuchten Individuen nicht bestattet wurden und dass die vom Schlachtfeld aufgesammelten Gebeine in Beinhäusern aufbewahrt wurden. Die Resultate bestätigten damit Angaben aus Schriftquellen und erlaubten die Zuordnung der Skelettreste zu Gefallenen des Reichsheeres. Beim Vergleich der Dornacher Stichprobe mit anderen mittelalterlichen Schlachtfeldserien traten sowohl hinsichtlich der anthropologischen Befunde als auch im Hinblick auf die Verletzungen und Verletzungsmuster deutliche Ähnlichkeiten zutage. Diese ergänzten nicht nur das lückenhafte Bild spätmittelalterlicher Heere und ihrer Kampfweise, sondern beleuchteten auch Unterschiede zwischen mittelalterlicher und neuzeitlicher Kriegsführung.

Co-culture systems, consisting of outgrowth endothelial cells (OEC) and primary osteoblasts (pOB), represent a prom¬ising instrument to mimick the natural conditions in bone repair processes and provide a new concept to develop constructs for bone replacement. Furthermore, co-culture of OEC and pOB could provide new insights into the molecular and cellular mechanisms that control essential processes during bone repair. The present study described several advantages of the co-culture of pOB and OEC for bone tissue engineering applications, including beneficial effects on the angiogenic activation of OEC, as well as on the assembly of basement membrane matrix molecules and factors involved in vessel maturation and stabilization. The ongoing angiogenic process in the co-culture system proceeded during the course of co-cultivation and correlated with the upregulation of essential angiogenic factors, such as VEGF, angiopoietins, basement membrane molecules and mural cell-specific markers. Furthermore the co-culture system appeared to maintain osteogenic differentiation capacity.rnrnAdditional treatment of co-cultures with growth factors or morphogens might accelerate and improve bone formation and furthermore could be useful for potential clinical applications. In this context, the present study highlights the central role of the morphogen, sonic hedgehog, which has been shown to affect angiogenic activation as well as osteogenic differentiation in the co-culture model of OEC and pOB. Treatment of co-cultures with sonic hedgehog resulted in an increased formation of microvessel-like structures as early as after 24 hours. This proangiogenic effect was induced by the upregulation of the proangiogenic factors, VEGF, angiopoietin1 and angiopoietin 2. In contrast to treatment with a commonly used proangiogenic agent, VEGF, Shh stimulation induced an increased expression of factors associated with vessel maturation and stabilization, mediated through the upregulation of growth factors that are strongly involved in pericyte differentiation and recruitment, including PDGF-BB and TGFbeta. In addition, Shh treatment of co-cultures also resulted in an upregulation of osteogenic differentiation markers like alkaline phosphatase, osteocalcin, osteonectin and osteopontin, as well as an increased matrix calcification. This was a result of upregulation of the osteogenic differentiation regulating factors, BMP2 and RUNX2 which could be assessed in response to Shh treatment. rn

Im Rahmen dieser Arbeit wurden neuartige funktionale Nanographene synthetisiert und hinsichtlich ihrer strukturellen und elektronischen Eigenschaften charakterisiert. Basierend auf dem Strukturmotiv des Graphens konnten anellierte polyzyklische aromatische Kohlenwasserstoffe (PAKs) mit unterschiedlichen Seitenverhältnissen strukturdefiniert erhalten und gezielt in der Peripherie funktionalisiert werden. Basierend auf dem Synthesekonzept einer „Vorplanarisierung“ konnten Nanographen-Scheiben mit einem Durchmesser von bis zu 3 nm in hoher Reinheit erhalten werden. Durch die Entwicklung von Polyphenylen-Vorläufern mit einem gewinkelten Rückgrat konnten erstmals defektfreie und lösliche Nanographen-Streifen (GNRs) mit Breiten von 1,0 - 2,1 nm und Längen von über 40 nm synthetisiert werden.rnrnAm Hexa-peri-hexabenzocoronen (HBC) war es möglich, durch die Einführung kurzer linearer Alkylreste in der Peripherie den inter- und intrakolumnaren Abstand nach Selbstorganisation zu reduzieren. In Mischungen mit Perylentetracarboxydiimid (PDI) als Akzeptor konnte durch eine erhöhte Dichte und eine verbesserte Ladungsträgermobilität eine relative Steigerung der Effizienz von Donor-Akzeptor-Heteroübergangs-Solarzellen um 9 % erreicht werden. Eine kovalente Verknüpfung von HBC und PDI erlaubte hier die vollständige Kontrolle der supramolekularen Organisation, des Phasenverhaltens sowie des Abstandes zwischen Donor und Akzeptor.rnrnBasierend auf den im Rahmen dieser Arbeit entwickelten Synthesekonzepten, eröffnen sich nun zahlreiche Möglichkeiten zur Entwicklung weiterer Nanographene, die entsprechend der gewünschten Anwendung funktionalisiert werden können und ein besseres Verständnis der Eigenschaften graphenartiger Materialien erlauben werden.

In einer retrospektiven Analyse wurden Daten von 161 pädiatrischen Patienten ausgewertet, denen im Zeitraum von Oktober 1997 bis Dezember 2008 ein Vorhofseptumdefekt vom sekundum Typ in der Kinderkardiologie des Zentrums für Kinder- und Jugendmedizin der Universitätsmedizin Mainz interventionell mittels Amplatzer Septal Occluder erfolgreich verschlossen wurde. In diesem Kollektiv wurden Alter, Shuntvolumen und IAS/Occluder Ratio als mögliche Einflussfaktoren auf die Ausbildung von prae-, peri- und postinterventionell dokumentierten atrialen Arrhythmien untersucht und unter dem Aspekt aufgetretener Arrhythmien auch die Anzahl an Langzeit-EKGs und die Dauer des Follow Up analysiert.rnIm untersuchten Kollektiv war die Anzahl der Kinder mit prae- (2,5%), peri- (2,5%) und postinterventionell (3,7%) Arrhythmien gering.rnIm untersuchten Kollektiv konnte kein Zusammenhang zwischen den dokumentierten Arrhythmien und dem Alter ermittelt werden. Allerdings war die Anzahl an L-EKGs bei den 6 Patienten mit postinterventionellen Arrhythmien in der Tendenz höher als bei den Patienten mit normalem Sinusrhythmus, ein Ergebnis, das sich in der Dauer des Follow Up nicht zeigte.rnAls Hauptergebnis der vorliegenden Arbeit zeigte sich eine Assoziation zwischen dem Shuntvolumen und auch der IAS/Occluder Ratio mit den aufgetretenen prae-, peri- und postinterventionellen Arrhythmien im untersuchten Patientenkollektiv und zwar in der Form, dass ein großer Defekt und eine niedrige IAS/Occluder Ratio die Ausbildung von atrialen Arrhythmien zu begünstigen scheinen. Festzuhalten bleibt, dass selbst bei Kindern mit großen Shuntvolumina Rhythmusstörungen früher auftreten und deshalb auch ein früherer Verschluss indiziert ist. Ebenso ist die Wahrscheinlichkeit, dass bei großen Defekten die atrialen Arrhythmien verschwinden geringer als bei kleinen Defekten. Aus diesem Ergebnis kann zumindest in der Tendenz eine Prädiktorfunktion dieser beiden Parameter für supraventrikuläre Arrhythmien abgeleitet werden.rnOffen bleibt in der vorliegenden Untersuchung die Frage nach der Ursache und Wirkung dieser Faktoren hinsichtlich der Pathogenese der Arrhythmien. Offen bleibt ebenfalls, ob eine Kombination mehrer unterschiedlicher Faktoren für die Pathogenese entscheidend ist. Ein tieferes Verständnis der Pathogenese der prae-, peri- und postinterventionellen Arrhythmien beim ASD II könnte Grundlage für eine bessere Prognose im Hinblick auf ihre Vermeidung bedeuten.

L’osteomielite associata all’impianto è un processo infettivo a carico del tessuto osseo spesso accompagnato dalla distruzione dell’osso stesso. La patogenesi delle osteomieliti associate all’impianto si basa su due concetti fondamentali: l’internalizzazione del patogeno all’interno degli osteoblasti e la capacità dei batteri di formare il biofilm. Entrambi i meccanismi consentono infatti di prevenire l’eliminazione del batterio da parte delle difese immunitarie dell’ospite e di ostacolare l’azione della maggior parte degli antibiotici (che non penetrano e non agiscono pertanto su microrganismi intracellulari), così sostenendo ed alimentando l’infezione. Il saggio di invasione messo a punto su micropiastra ha consentito di investigare in modo approfondito e dettagliato il ruolo ed il peso dell’internalizzazione nella patogenesi delle infezioni ortopediche peri-protesiche causate da S. aureus, S. epidermidis, S. lugdunensis ed E. faecalis. Lo studio ha evidenziato che l’invasione delle cellule MG-63 non rappresenta un meccanismo patogenetico delle infezioni ortopediche associate all’impianto causate da S. epidermidis, S. lugdunensis ed E. faecalis; al contrario, in S. aureus la spiccata capacità invasiva rappresenta un’abile strategia patogenetica che consente al patogeno di sfuggire alla terapia sistemica e alla risposta immunitaria dell’ospite. È stato studiato inoltre il ruolo dell’immunità innata nella difesa contro il biofilm batterico. In seguito all’incubazione del biofilm opsonizzato di S. epidermidis con i PMN è stato possibile osservare la formazione delle NETs. Le NETs rappresentano ottime armi nella difesa contro il biofilm batterico, infatti le trappole sono in grado di limitare la diffusione batterica e quindi di confinare l’infezione. La comprensione del ruolo dell’internalizzazione nella patogenesi delle osteomieliti associate all’impianto e lo studio della risposta immunitaria innata a questo tipo di infezioni, spesso caratterizzate dalla presenza di biofilm, sono presupposti per identificare e affinare le migliori strategie terapeutiche necessarie ad eradicare l'infezione.

In dieser Arbeit wurde eine neue Methode zur asymmetrischen Substitution der K-Regionen von Pyren entwickelt, auf welcher das Design und die Synthese von neuartigen, Pyren-basierten funktionalen Materialien beruht. Eine Vielzahl von Substitutionsmustern konnte erfolgreich realisiert werden um die Eigenschaften entsprechend dem Verwendungszweck anzupassen. Der polyzyklische aromatische Kohlenwasserstoff (PAK) Pyren setzt sich aus vier Benzolringen in Form einer planaren Raute mit zwei gegenüberliegenden K-Regionen zusammen. Der synthetische Schlüsselschritt dieser Arbeit ist die chemische Transformation der einen K-Region zu einem α-Diketon und der darauffolgenden selektiven Bromierung der zweiten K-Region. Dieser asymmetrisch funktionalisierte Baustein zeichnet sich durch zwei funktionelle Gruppen mit orthogonaler Reaktivität aus und erweitert dadurch das Arsenal der etablierten Pyren Chemie um eine vielseitig einsetzbare Methode. Aufbauend auf diesem synthetischen Zugang wurden fünf wesentliche Konzepte auf dem Weg zu neuen, von Pyren abgeleiteten Materialen verfolgt: (i) Asymmterische Substitution mit elektronenziehenden versus -schiebenden Gruppen. (ii) Darstellung von Pyrenocyaninen durch Anbindung von Pyren mit einer der K-Regionen an das Phthalocyanin Gerüst zur Ausdehnung des π-Systems. (iii) Einführung von Thiophen an die K-Region um halbleitende Eigenschaften zu erhalten. (iv) Symmetrische Annullierung von PAKs wie Benzodithiophen und Phenanthren an beide K Regionen für cove-reiche und dadurch nicht-planare Strukturen. (v) Verwendung des K-Region-funktionalisierten Pyrens als Synthesebaustein für das Peri-Pentacen. Neben der Synthese wurde die Selbstorganisation in der Festphase und an der flüssig/fest Grenzfläche mittels zweidimensionaler Weitwinkel-Röntgenstreuung (2D WAXS) bzw. Rastertunnelmikroskopie (STM) untersucht. Die halbleitenden Eigenschaften wurden in organischen Feld-Effekt Transistoren (OFETs) charakterisiert.

For the successful integration of bone tissue engineering constructs into patients, an adequate supply with oxygen and nutrients is critical. Therefore, prevascularisation of bone tissue engineering constructs is desirable for bone formation, remodelling and regeneration. Co-culture systems, consisting of human endothelial cells and primary osteoblasts (pOB) as well as osteosarcoma cell lines, represent a promising method for studying the mechanisms involved in the vascularisation of constructs in bone tissue en- gineering and could provide new insights into the molecular and cellular mechanisms that control essential processes during angiogenesis. The present study demonstrated the im- portant components of co-culture systems with a focus on bone tissue replacement and the angiogenic effects of pOB and osteosarcoma cell lines on human endothelial cells. Furthermore, the studies emphasised an overall approach for analysis of signal molecules that are involved in the angiogenic activation of human endothelial cells by the regulation of VEGF-related pathways at the transcriptional and translational levels. The osteosarcoma cell lines Cal-72, MG-63 and SaOS-2, as well as pOB from several donors, differed in their angiogenesis-inducing potential in 2-D and 3-D co-culture systems. SaOS-2 cells appeared to have a high osteogenic differentiation level with no detectable angiogenesis-inducing potential in co-culture with human endothelial cells. The angiogenic potential of the osteoblast-like cells is mainly correlated with the upregulation of essential angiogenic growth factors, such as VEGF, bFGF and HGF and the downregulation of the angiogenesis inhibitor, endostatin. However, other factors involved in angiogenic regulation were found to differ between SaOS-2 cells, compared to Cal-72 and MG-63. The present study focuses on VEGF pathway-effecting genes as key players in the regulation of angiogenesis. The levels of VEGF and VEGF-effecting genes, such as TGF-α and TIMP-2 are down-regulated in SaOS-2 cells. In contrast, direct regulators of VEGF, such as IL6, IL8 and TNF are strongly upregulated, which indicates disruptions in growth factor regulating pathways in SaOS-2 cells. Potential pathways, which could be involved include MEK, PI3K, MAPK, STAT3, AKT or ERK. Additional treatment of co-cultures with single growth factors did not accelerate or improve the angiogenesis-inducing potential of SaOS-2 cells. Knowledge of the detailed molecular mechanisms involved in angiogenesis control will hopefully allow improved approaches to be developed for prevascularisation of bone tissue engineering constructs.

Die Arbeit beschäftigt sich mit der Kontrolle von Selbstorganisation und Mikrostruktur von organischen Halbleitern und deren Einsatz in OFETs. In Kapiteln 3, 4 und 5 eine neue Lösungsmittel-basierte Verabeitungsmethode, genannt als Lösungsmitteldampfdiffusion, ist konzipiert, um die Selbstorganisation von Halbleitermolekülen auf der Oberfläche zu steuern. Diese Methode als wirkungsvolles Werkzeug erlaubt eine genaue Kontrolle über die Mikrostruktur, wie in Kapitel 3 am Beispiel einer D-A Dyad bestehend aus Hexa-peri-hexabenzocoronene (HBC) als Donor und Perylene Diimide (PDI) als Akzeptor beweisen. Die Kombination aus Oberflächenmodifikation und Lösungsmitteldampf kann die Entnetzungseffekte ausgleichen, so dass die gewüschte Mikrostruktur und molekulare Organisation auf der Oberfläche erreicht werden kann. In Kapiteln 4 und 5 wurde diese Methode eingesetzt, um die Selbstorganisation von Dithieno[2, 3-d;2’, 3’-d’] benzo[1,2-b;4,5-b’]dithiophene (DTBDT) und Cyclopentadithiophene -benzothiadiazole copolymer (CDT-BTZ) Copolymer zu steuern. Die Ergebnisse könnten weitere Studien stimulieren und werfen Licht aus andere leistungsfaähige konjugierte Polymere. rnIn Kapiteln 6 und 7 Monolagen und deren anschlieβende Mikrostruktur von zwei konjugierten Polymeren, Poly (2,5-bis(3-alkylthiophen-2-yl)thieno[3,2-b]thiophene) PBTTT und Poly{[N,N ′-bis(2-octyldodecyl)-naphthalene-1,4,5,8-bis (dicarboximide)-2,6-diyl]-alt-5,5′- (2,2′-bithiophene)}, P(NDI2OD-T2)) wurden auf steife Oberflächen mittels Tauchbeschichtung aufgebracht. Da sist das erste Mal, dass es gelungen ist, Polymer Monolagen aus der Lösung aufzubringen. The study was arranged to manifest its objectives through preceding it with an intro-duction. Particular attention was paid in the second part to detect the physical settings of the study area, together with an attempt to show the climatic characteristics in Libya. In the third part, observed temporal and spatial climate change in Libya was investigated through the trends of temperature, precipitation, relative humidity and cloud amount over the peri-ods (1946-2000), (1946-1975), and (1976-2000), comparing the results with the global scales. The forth part detected the natural and human causes of climate change concentrat-ing on the greenhouse effect. The potential impacts of climate change on Libya were ex-amined in the fifth chapter. As a case study, desertification of Jifara Plain was studied in the sixth part. In the seventh chapter, projections and mitigations of climate change and desertification were discussed. Ultimately, the main results and recommendations of the study were summarized. In order to carry through the objectives outlined above, the following methods and approaches were used: a simple linear regression analysis was computed to detect the trends of climatic parameters over time; a trend test based on a trend-to-noise-ratio was applied for detecting linear or non-linear trends; the non-parametric Mann-Kendall test for trend was used to reveal the behavior of the trends and their significance; PCA was applied to construct the all-Libya climatic parameters trends; aridity index after Walter-Lieth was shown for computing humid respectively arid months in Libya; correlation coefficient, (after Pearson) for detecting the teleconnection between sun spot numbers, NAOI, SOI, GHGs, and global warming, climate changes in Libya; aridity index, after De Martonne, to elaborate the trends of aridity in Jifara Plain; Geographical Information System and Re-mote Sensing techniques were applied to clarify the illustrations and to monitor desertifi-cation of Jifara Plain using the available satellite images MSS, TM, ETM+ and Shuttle Radar Topography Mission (SRTM). The results are explained by 88 tables, 96 figures and 10 photos. Temporal and spatial temperature changes in Libya indicated remarkably different an-nual and seasonal trends over the long observation period 1946-2000 and the short obser-vation periods 1946-1975 and 1976-2000. Trends of mean annual temperature were posi-tive at all study stations except at one from 1946-2000, negative trends prevailed at most stations from 1946-1975, while strongly positive trends were computed at all study stations from 1976-2000 corresponding with the global warming trend. Positive trends of mean minimum temperatures were observed at all reference stations from 1946-2000 and 1976-2000, while negative trends prevailed at most stations over the period 1946-1975. For mean maximum temperature, positive trends were shown from 1946-2000 and from 1976-2000 at most stations, while most trends were negative from 1946-1975. Minimum tem-peratures increased at nearly more than twice the rate of maximum temperatures at most stations. In respect of seasonal temperature, warming mostly occurred in summer and au-tumn in contrast to the global observations identifying warming mostly in winter and spring in both study periods. Precipitation across Libya is characterized by scanty and sporadically totals, as well as high intensities and very high spatial and temporal variabilities. From 1946-2000, large inter-annual and intra-annual variabilities were observed. Positive trends of annual precipi-tation totals have been observed from 1946-2000, negative trends from 1976-2000 at most stations. Variabilities of seasonal precipitation over Libya are more strikingly experienced from 1976-2000 than from 1951-1975 indicating a growing magnitude of climate change in more recent times. Negative trends of mean annual relative humidity were computed at eight stations, while positive trends prevailed at seven stations from 1946-2000. For the short observation period 1976-2000, positive trends were computed at most stations. Annual cloud amount totals decreased at most study stations in Libya over both long and short periods. Re-markably large spatial variations of climate changes were observed from north to south over Libya. Causes of climate change were discussed showing high correlation between tempera-ture increasing over Libya and CO2 emissions; weakly positive correlation between pre-cipitation and North Atlantic Oscillation index; negative correlation between temperature and sunspot numbers; negative correlation between precipitation over Libya and Southern Oscillation Index. The years 1992 and 1993 were shown as the coldest in the 1990s result-ing from the eruption of Mount Pinatubo, 1991. Libya is affected by climate change in many ways, in particular, crop production and food security, water resources, human health, population settlement and biodiversity. But the effects of climate change depend on its magnitude and the rate with which it occurs. Jifara Plain, located in northwestern Libya, has been seriously exposed to desertifica-tion as a result of climate change, landforms, overgrazing, over-cultivation and population growth. Soils have been degraded, vegetation cover disappeared and the groundwater wells were getting dry in many parts. The effect of desertification on Jifara Plain appears through reducing soil fertility and crop productivity, leading to long-term declines in agri-cultural yields, livestock yields, plant standing biomass, and plant biodiversity. Desertifi-cation has also significant implications on livestock industry and the national economy. Desertification accelerates migration from rural and nomadic areas to urban areas as the land cannot support the original inhabitants. In the absence of major shifts in policy, economic growth, energy prices, and con-sumer trends, climate change in Libya and desertification of Jifara Plain are expected to continue in the future. Libya cooperated with United Nations and other international organizations. It has signed and ratified a number of international and regional agreements which effectively established a policy framework for actions to mitigate climate change and combat deserti-fication. Libya has implemented several laws and legislative acts, with a number of ancil-lary and supplementary rules to regulate. Despite the current efforts and ongoing projects being undertaken in Libya in the field of climate change and desertification, urgent actions and projects are needed to mitigate climate change and combat desertification in the near future.

Abstract Objectives: To investigate the influence of protein incorporation on the resistance of biomimetic calcium-phosphate coatings to the shear forces that are generated during implant insertion. Materials and Methods: Thirty-eight standard (5 x 13 mm) Osseotite((R)) implants were coated biomimetically with a layer of calcium phosphate, which either lacked or bore a co-precipitated (incorporated) depot of the model protein bovine serum albumin (BSA). The coated implants were inserted into either artificial bone (n=18) or the explanted mandibles of adult pigs (n=12). The former set-up was established for the measurement of torque and of coating losses during the insertion process. The latter set-up was established for the histological and histomorphometric analysis of the fate of the coatings after implantation. Results: BSA-bearing coatings had higher mean torque values than did those that bore no protein depot. During the insertion process, less material was lost from the former than from the latter type of coating. The histological and histomorphometric analysis revealed fragments of material to be sheared off from both types of coating at vulnerable points, namely, at the tips of the threads. The sheared-off fragments were retained within the peri-implant space. Conclusion: The incorporation of a protein into a biomimetically prepared calcium-phosphate coating increases its resistance to the shear forces that are generated during implant insertion. In a clinical setting, the incorporated protein would be an osteogenic agent, whose osteoinductive potential would not be compromised by the shearing off of coating material, and the osteoconductivity of an exposed implant surface would not be less than that of a coated one. AIM: To compare the 10-year peri-implant bone loss (BL) rate in periodontally compromised (PCP) and periodontally healthy patients (PHP) around two different implant systems supporting single-unit crowns. MATERIALS AND METHODS: In this retrospective, controlled study, the mean BL (mBL) rate around dental implants placed in four groups of 20 non-smokers was evaluated after a follow-up of 10 years. Two groups of patients treated for periodontitis (PCP) and two groups of PHP were created. For each category (PCP and PHP), two different types of implant had been selected. The mBL was calculated by subtracting the radiographic bone levels at the time of crown cementation from the bone levels at the 10-year follow-up. RESULTS: The mean age, mean full-mouth plaque and full-mouth bleeding scores and implant location were similar between the four groups. Implant survival rates ranged between 85% and 95%, without statistically significant differences (P>0.05) between groups. For both implant systems, PCP showed statistically significantly higher mBL rates and number of sites with BL> or =3 mm compared with PHP (P<0.0001). CONCLUSIONS: After 10 years, implants in PCP yielded lower survival rates and higher mean marginal BL rates compared with those of implants placed in PHP. These results were independent of the implant system used or the healing modality applied.

OBJECTIVES: The aim of this study was to compare the long-term outcomes of implants placed in patients treated for periodontitis periodontally compromised patients (PCP) and in periodontally healthy patients (PHP) in relation to adhesion to supportive periodontal therapy (SPT). MATERIAL AND METHODS: One hundred and twelve partially edentulous patients were consecutively enrolled in private specialist practice and divided into three groups according to their initial periodontal condition: PHP, moderate PCP and severe PCP. Perio and implant treatment was carried out as needed. Solid screws (S), hollow screws (HS) and hollow cylinders (HC) were installed to support fixed prostheses, after successful completion of initial periodontal therapy (full-mouth plaque score <25% and full-mouth bleeding score <25%). At the end of treatment, patients were asked to follow an individualized SPT program. At 10 years, clinical measures and radiographic bone changes were recorded by two calibrated operators, blinded to the initial patient classification. RESULTS: Eleven patients were lost to follow-up. During the period of observation, 18 implants were removed because of biological complications. The implant survival rate was 96.6%, 92.8% and 90% for all implants and 98%, 94.2% and 90% for S-implants only, respectively, for PHP, moderate PCP and severe PCP. The mean bone loss was 0.75 (+/- 0.88) mm in PHP, 1.14 (+/- 1.11) mm in moderate PCP and 0.98 (+/- 1.22) mm in severe PCP, without any statistically significant difference. The percentage of sites, with bone loss > or =3 mm, was, respectively, 4.7% for PHP, 11.2% for moderate PCP and 15.1% for severe PCP, with a statistically significant difference between PHP and severe PCP (P<0.05). Lack of adhesion to SPT was correlated with a higher incidence of bone loss and implant loss. CONCLUSION: Patients with a history of periodontitis presented a lower survival rate and a statistically significantly higher number of sites with peri-implant bone loss. Furthermore, PCP, who did not completely adhere to the SPT, were found to present a higher implant failure rate. This underlines the value of the SPT in enhancing the long-term outcomes of implant therapy, particularly in subjects affected by periodontitis, in order to control reinfection and limit biological complications.

Objective: In South Africa, many HIV-infected patients experience delays in accessing antiretroviral therapy (ART). We examined pretreatment mortality and access to treatment in patients waiting for ART. Design: Cohort of HIV-infected patients assessed for ART eligibility at 36 facilities participating in the Comprehensive HIV and AIDS Management (CHAM) program in the Free State Province. Methods: Proportion of patients initiating ART, pre-ART mortality and risk factors associated with these outcomes were estimated using competing risks survival analysis. Results: Forty-four thousand, eight hundred and forty-four patients enrolled in CHAM between May 2004 and December 2007, of whom 22 083 (49.2%) were eligible for ART; pre-ART mortality was 53.2 per 100 person-years [95% confidence interval (CI) 51.8–54.7]. Median CD4 cell count at eligibility increased from 87 cells/ml in 2004 to 101 cells/ml in 2007. Two years after eligibility an estimated 67.7% (67.1–68.4%) of patients had started ART, and 26.2% (25.6–26.9%) died before starting ART. Among patients with CD4 cell counts below 25 cells/ml at eligibility, 48% died before ART and 51% initiated ART. Men were less likely to start treatment and more likely to die than women. Patients in rural clinics or clinics with low staffing levels had lower rates of starting treatment and higher mortality compared with patients in urban/peri-urban clinics, or better staffed clinics. Conclusions: Mortality is high in eligible patients waiting for ART in the Free State Province. The most immunocompromised patients had the lowest probability of starting ART and the highest risk of pre-ART death. Prioritization of these patients should reduce waiting times and pre-ART mortality.
