The purpose of this study was to identify preoperative predictors of discharge destination after total joint arthroplasty. A retrospective study of three hundred and seventy-two consecutive patients who underwent primary total hip and knee arthroplasty was performed. The mean length of stay was 2.9 days and 29.0% of patients were discharged to extended care facilities. Age, caregiver support at home, and patient expectation of discharge destination were the only significant multivariable predictors regardless of the type of surgery (total knee versus total hip arthroplasty). Among those variables, patient expectation was the most important predictor (P < 0.001; OR 169.53). The study was adequately powered to analyze the variables in the multivariable logistic regression model, which had a high concordance index of 0.969.

There is concern about the potential increase of hematological toxicity in elderly patients treated with chemotherapy. Recently, primary prophylaxis with colony-stimulating factors (CSFs) was proposed for elderly patients receiving moderately toxic chemotherapy. However, evidence for the benefits of this primary prophylaxis for elderly breast cancer patients is currently lacking. We retrospectively analyzed the incidence of febrile neutropenia (FN) and neutropenic infections in elderly breast cancer patients receiving anthracycline-based chemotherapy without primary prophylaxis with colony-stimulating factors. In addition, we assessed the direct costs of hospitalization for these complications. Febrile neutropenia or neutropenic infection occurred in 13% of the 46 patients. Further studies are needed to adequately evaluate the risk of neutropenic complications (NC) in elderly patients receiving standard-dose chemotherapy for breast cancer and the potential benefits of primary prophylaxis with colony-stimulating factors. © 2004 Elsevier Ireland Ltd. This article explores the under-researched field of self-guided trails. The focus of the research is on the experiential aspects of self-guided literary trails from the perspective of both the developer and user. An examination of existing literature on self-guided trails and literary tourism was undertaken and supplemented with a review of experiential design principles. Content analysis of a sample of literary heritage trails was then carried out and three distinctive typologies were developed, informed by aspects of experiential design. The research reveals that few literary trails developers utilise these principles and the article concludes with proposals for the design of more effective literary trail experiences.

The primary production in the Bristol Channel, U.K., was studied from 1973 to 1977: in this estuary, the euphotic zone extends from less than 0.5 m to greater than 10m and there is a large riverine input of inorganic nutrients. The standing stock of phytoplankton chlorophyll a was measured in 1973 and 1974 and was similar throughout the Bristol Channel but the rate of primary production was much greater where the water was less turbid. The estimated primary production was 6.8g C m−2 for the most turbid region and 164.9g C m−2 for the Outer Bristol Channel. A larger proportion of the annual primary production occurred in the spring in the Outer Channel than in the most turbid regions. Phaeocystis developed into blooms in some, but not all, years and exhibited a different light saturation curve to other phytoplankton populations. Serial incubations of short duration gave higher fixation rates than day-long incubations and it is argued that photoinhibition is probably insignificant in a mixed water column. Excretion rates of dissolved organic carbon by phytoplankton were always low.

A simulated in situ incubation box has been compared with in situ exposure for 14C production measurements in an estuarine environment. Measurements were made over the course of 14 months, mainly in the Tamar estuary; production rates ranged from less than 1 mg C m−2h−1 to 350 mg C m−2h−1 and there was no significant difference between results from the two methods. In the estuarine waters investigated, the simulated in situ incubator with neutral density filters, used with a Secchi disc to determine sampling depths, gives a satisfactory estimate of in situ primary production.
