BACKGROUND: Promoting healthy gestational weight gain (GWG) is important for preventing obstetric and perinatal morbidity, along with obesity in both mother and child. Provision of GWG guidelines by health professionals predicts women meeting GWG guidelines. Research concerning women's GWG information sources is limited. This study assessed pregnant women's sources of GWG information and how, where and which women seek GWG information. METHODS: Consecutive women (n = 1032) received a mailed questionnaire after their first antenatal visit to a public maternity hospital in Melbourne, Australia. Recalled provision of GWG guidelines by doctors and midwives, recalled provided GWG goals, and the obtaining of GWG information and information sources were assessed. RESULTS: Participants (n = 368; 35.7 % response) averaged 32.5 years of age and 20.8 weeks gestation, with 33.7 % speaking a language other than English. One in ten women recalled receiving GWG guidelines from doctors or midwives, of which half were consistent with Institute of Medicine guidelines. More than half the women (55.4 %) had actively sought GWG information. Nulliparous (OR 7.07, 95 % CI = 3.91-12.81) and obese (OR 1.96, 95 % CI = 1.05-3.65) women were more likely to seek information. Underweight (OR 0.29, 95 % CI = 0.09-0.97) women and those working part time (OR 0.52, 95 % CI = 0.28-0.97) were less likely to seek information. Most frequently reported GWG sources included the internet (82.7 %), books (55.4 %) and friends (51.5 %). The single most important sources were identified as the internet (32.8 %), general practitioners (16.9 %) and books (14.9 %). CONCLUSION: More than half of women were seeking GWG guidance and were more likely to consult non-clinician sources. The small numbers given GWG targets, and the dominance of non-clinical information sources, reinforces that an important opportunity to provide evidence based advice and guidance in the antenatal care setting is currently being missed.

We report the synthesis of a series of novel epoxy endoperoxide compounds that can be prepared in high yields in one to three steps from simple starting materials. Some of these compounds inhibit the growth of Plasmodium falciparum in vitro. Structure-activity studies indicate that an endoperoxide ring bisubstituted with saturated cyclic moieties is the pharmacophore. To study the molecular basis of the action of these novel antimalarial compounds, we examined their ability to interact with oxidized and reduced forms of heme. Some of the compounds interact with oxidized heme in a fashion similar to chloroquine and other 4-aminoquinolines, while some of the compounds interact with reduced heme. However, the level of antimalarial potency is not well correlated with these activities, suggesting that some of the endoperoxides may exert their antimalarial activities by a novel mechanism of action.

BACKGROUND: Evidence suggests that women are failing to meet guidelines for nutrition, physical activity, and weight gain during pregnancy. Interventions to promote a healthy lifestyle in pregnancy demonstrate mixed results and many are time and resource intensive. mHealth-delivered interventions offer an opportunity to provide trusted source information in a timely and cost-effective manner. Studies regarding women's and health professionals' views of mHealth in antenatal care are limited.

OBJECTIVE: This study aimed to explore women's and health professionals' views regarding mHealth information sources and interventions to assist women to eat well, be physically active, and gain healthy amounts of weight in pregnancy.

METHODS: A descriptive qualitative research approach employed focus groups and in-depth interviews with 15 pregnant or postpartum women and 12 in-depth interviews with health professionals including two from each category: obstetricians, general practitioners, midwives, dietitians, physiotherapists, and community pharmacists. All interviews were transcribed verbatim and thematically analyzed.

RESULTS: Women uniformly embraced the concept of mHealth information sources and interventions in antenatal care and saw them as central to information acquisition and ideally incorporated into future antenatal care processes. Health professionals exhibited varied views perceiving mHealth as an inevitable, often parallel, service rather than one integrated into the care model. Four key themes emerged: engagement, risk perception, responsibility, and functionality. Women saw their ability to access mHealth elements as a way to self-manage or control information acquisition that was unavailable in traditional care models and information sources. The emergence of technology was perceived by some health professionals to have shifted control of information from trusted sources, such as health professionals and health organizations, to nontrusted sources. Some health professionals were concerned about the medicolegal risks of mHealth (incorrect or harmful information and privacy concerns), while others acknowledged that mHealth was feasible if inherent risks were addressed. Across both groups, there was uncertainty as to who should be responsible for ensuring high-quality mHealth. The absence of a key pregnancy or women's advocacy group, lack of health funds for technologies, and the perceived inability of maternity hospitals to embrace technology were seen to be key barriers to provision. Women consistently identified the functionality of mHealth as adding value to antenatal care models. For some health professionals, lack of familiarity with and fear of mHealth limited their engagement with and comprehension of the capacity of new technologies to support antenatal care.

CONCLUSIONS: Women exhibited positive views regarding mHealth for the promotion of a healthy lifestyle in antenatal care. Conversely, health professionals expressed a much wider variation in attitudes and were more able to identify potential risks and barriers to development and implementation. This study contributes to the understanding of the opportunities and challenges in developing mHealth lifestyle interventions in antenatal care.

High intrauterine cortisol exposure can inhibit fetal growth and have programming effects for the child's subsequent stress reactivity. Placental 11beta-hydroxysteroid dehydrogenase (11β-HSD2) limits the amount of maternal cortisol transferred to the fetus. However, the relationship between maternal psychopathology and 11β-HSD2 remains poorly defined. This study examined the effect of maternal depressive disorder, antidepressant use and symptoms of depression and anxiety in pregnancy on placental 11β-HSD2 gene (HSD11B2) expression. Drawing on data from the Mercy Pregnancy and Emotional Wellbeing Study, placental HSD11B2 expression was compared among 33 pregnant women, who were selected based on membership of three groups; depressed (untreated), taking antidepressants and controls. Furthermore, associations between placental HSD11B2 and scores on the State-Trait Anxiety Inventory (STAI) and Edinburgh Postnatal Depression Scale (EPDS) during 12-18 and 28-34 weeks gestation were examined. Findings revealed negative correlations between HSD11B2 and both the EPDS and STAI (r = -0.11 to -0.28), with associations being particularly prominent during late gestation. Depressed and antidepressant exposed groups also displayed markedly lower placental HSD11B2 expression levels than controls. These findings suggest that maternal depression and anxiety may impact on fetal programming by down-regulating HSD11B2, and antidepressant treatment alone is unlikely to protect against this effect.

Cape Wind has proposed a wind farm of 130 turbines on Horseshoe Shoal in the center of Nantucket Sound. A prominent concern about the project is the impact the visibility of the turbines will have on the region's tourism industry and property values. It is feared that their presence will diminish the value of the pristine coastline that has attracted vacationers to Cape Cod for generations. In this project, we assess the extent to which Cape Cod, Martha's Vineyard, and Nantucket will be visually affected by the wind farm. It was completed using a Viewshed Analysis in the GIS program, ArcMap, from the surface, mean, and maximum height of the towers. These Viewsheds were combined to give a comprehensive perspective of which areas are able to see the highest percent of the wind farm. Finally, a weighted land use value was applied to the Viewshed to account for the impact of land use on the ability to see the project. The objective of this analysis is to provide a visual representation of how great an influence the wind farm will in fact have on Cape Cod.

Briefly, West discusses student life and Winthrop traditions, but she provides insight into her life student teaching at Florence High School. Additionally, West includes her experience in required courses like English and Math, as well as experiments conducted in her Biology courses. The purpose of this study was make an utterance of the managerial style of the public executive from Santa Catarina and compare it with public executives from Minas Gerais and United States, considering their capacity in giving and receiving feedback and their opening or auto-expositions in the interrelations with subordinates. It began with a review of the literature about functions and managerial style from several contributions that exist in the theory of the organizations. The theoric countersignement used in this essay was stablished in “Johari’s Window”, of Luft, that brought the possibility of the analysis of the interpersonal relations, considering the feedback variants and opening. The research is descriptive and still comparative, being applied to public executives from Santa Catarina, between the occupants of the first and the second hierarchic rungs. The instrument used was the questionnaire “Personel Relations Surwey”, of John Hall and Martha Williams, the same was applied at the researches that had been made in Minas Gerais and in United States. It was evident, by the statistic analysis in coherence with the theoric model, several conclusions, among them we detach: a) The unexistence of a standart managerial style between public executives from Santa Catarina; b) Executives from this state use more feedback than executives from Minas Gerais and the same as north-americans; c) The executives from Santa Catarina have more opening (overture) than mineiros and north-americans; d) Some personel variants interfere in the managerial style of the public executive from Santa Catarina.

A disputa por um filho é uma situação tratada nas instituições judiciais através da nomeação do guardião mais apto para permanecer com a criança ou através de acordos entre as partes envolvidas, sempre considerando o princípio de priorizar o interesse superior da criança. Este estudo tem como objetivo elucidar aspectos da condição psicopatológica da disputa por um filho e suas repercussões no processo identificatório da criança. Para tanto, a pesquisadora partiu da pesquisa psicanalítica e da proposta de construção de caso psicanalítico para elaborar um ensaio metapsico(pato)lógico do caso. Foram realizados três estudos enfocando a criança na situação de disputa entre a família biológica e a família guardiã, na disputa entre uma dupla e mediante a separação conjugal. Ao finalizar o estudo levantou-se a questão sobre a importância da retomada do processo identificatório da criança disputada mediante a firmação do contrato identificatório. A posição dos pais na disputa pelo filho caracterizou a necessidade de exposição de um risco de perda deste filho e do desejo pelo mesmo. O compromisso identificatório da criança deverá ter suas cláusulas revistas e o desejo de um filho, para os pais, incluirá a assunção de um filho desejado e do não desejo de desejo por este filho. A possibilidade de mudança surgirá com a revelação de uma demanda que se autoriza e com a nova posição, que só poderá ser assumida pela criança com a participação da tolerância e flexibilidade daqueles que são os signatários de seu compromisso identificatório. O processo identificatório da criança, nos casos de disputa de guarda, terá a possibilidade de ser retomado através de sua mediação em situação psicanalítica de tratamento.

O objetivo deste estudo foi o de identificar que dimensões institucionais do circo preservadas nas organizações, podem ter sido responsáveis por sua sobrevivência e capacidade de adaptação diante das inúmeras dificuldades e adversidades enfrentadas ao longo de sua existência secular. Dedicada ao entretenimento e à diversão, esta instituição de origem pré-industrial, resguarda características corporativas e familiares. No entanto, sua estrutura organizacional e suas práticas administrativas aproximam-se das características hoje consideradas tendências das organizações do próximo século. Os resultados do estudo mostram que as organizações circenses guardam características pré e pós industriais. Soube preservar valores e identidade enquanto utilizou-se de todos os benefícios da contemporaneidade. Dessa forma, chega ao século xxi, sobrevivente, como uma organização sui generis e um dos mais bem sucedidos empreendimentos em sua categoria. Mostra também que num século onde a ocupação do tempo livre com atividades culturais, de lazer e entretimento será objetivo de muitas organizações, o circo pode vir a fornecer bom material de análise.
