BACKGROUND:
Musculoskeletal conditions are a common reason for consultation to General Practitioners (GPs)/family physicians in primary care. Osteochondromas are the most common benign bone tumours and usually occur in the metaphyseal region of long bones. Despite the distal femur being the commonest location to find these benign bone tumours, this is the first case report in the literature specifically describing vastus medialis muscle pain as the presenting symptom due to underlying bursa formation secondary to local pressure effects.

CASE PRESENTATION:
Twenty nine year old female of white British ethnic origin, presenting to a primary care clinic with a three year history of intermittent left distal medial thigh pain.

CONCLUSION:
The benign bone tumour, femoral exostosis/osteochondroma, was diagnosed via Magnaetic Resonance Imaging (MRI) and treated conservatively, with surgical excision an option if not resolving. GPs/family physicians need to be aware of this diagnosis and that femoral exostosis/osteochondroma can present to primary care physicians, particularly within the second decade of life.

This paper presents findings based on a palynological investigation of artificially accreting (plaggen) soils from the settlement of Village Bay, Hirta, in the St Kilda archipelago, which was perhaps the most distant and inhospitable outpost of sustained human habitation in the British Isles. The soils were developed principally through the addition of turf ash and seabird waste, although some ash may have been derived from upland peats. It is assumed that the woodland pollen signal (much lower in the soils than in an upland peat site nearby) represents off-island sources. Corylus avellana-type pollen (frequent in upland sites), along with Potentilla-type, may provide markers in the Village Bay profiles for the addition of ashed hillside turf, and possibly peat, to the plaggen soils. Cereal-type pollen is well represented through the profiles and is often strongly associated with the record for Chrysanthemum segetum (corn marigold), a frequent indicator of arable land. The Brassicaceae signal may partly reflect the cultivation of cabbages; Chelidonium majus (greater celandine) may have been grown for medicinal use. Soil mixing has rendered radiocarbon dating meaningless at this site, but the establishment of a change in cultivation regime before AD 1830 may have been identified from the patterns of pollen concentration and preservation in the profiles. © 2008 Elsevier B.V. The region of the Northeast Atlantic represents a gradient between two oceanographic regimes: (i) the subtropical waters of southern Portugal and southwestern Spain, and (ii) the temperate waters characterizing the northwestern and northern Iberian coasts (Cantabrian Sea) and the rest of the Bay of Biscay along the French coast.

São ainda muito incipientes os estudos sobre a vida e obra do médico olhanense Francisco Fernandes Lopes (1884-1969), uma das figuras mais interessantes do quadro cultural português da primeira metade do século XX. Na verdade, dificilmente poderemos caracterizá-lo apenas como “médico”, pois era um intelectual multifacetado e interessado nas mais diversas áreas do saber, assim como também é impossível descrevê-lo apenas como “olhanense”, pois a sua actividade intelectual extravasou largamente o âmbito local, regional e mesmo nacional. A presente dissertação, intitulada Francisco Fernandes Lopes (1884-1969), historiador do Algarve: contributo para a historiografia henriquina, tem dois objectivos principais, patentes no próprio título: compreender o homem, ou seja, elaborar um estudo biográfico sobre Fernandes Lopes, e analisar uma das suas facetas, a de historiador, mais concretamente no que concerne à sua obra acerca do Infante D. Henrique e a sua relação ao Algarve. No que respeita à biografia, pretende-se compreender a vida e obra de Francisco Fernandes Lopes através de uma análise concertada do seu percurso de vida, dos seus principais interesses e da sua actividade cultural e intelectual. Neste âmbito, é essencial ter em consideração que a vida de Fernandes Lopes se desenrolou mormente em Olhão, vila algarvia singular devido ao seu panorama arquitectónico único de açoteias, e que ele tanto se empenhou em divulgar, procurando que Olhão ocupasse um lugar de destaque no mapa do Algarve e, também, de Portugal. Relativamente à produção historiográfica tem particular destaque a temática henriquina, que foi a que mais ocupou Francisco Fernandes Lopes entre as décadas de 30 a 60 do século XX. Os seus estudos devem ser entendidos no âmbito de um período histórico muito específico – o Estado Novo – e tendo por base a evolução do pensamento historiográfico, assim como outros estudos similares e coetâneos que permitam formular uma comparação.
