Managerial discretion is the focal theme bridging the clash between two schools of thoughts; whether executives have greater influence on their firms’ outcomes or other factors restrain their actions (Hambrick & Finkelstein, 1987). It is argued that constraints come from inertial, normative and environmental forces (e.g. DiMaggio & Powell, 1983). Of these restraints is the institutional environment in which a firm is headquartered. Our paper falls within this research stream and provides an extension for Crossland and Hambrick (2007, 2011) work. We investigate the national level of discretion in new cross-cultural contexts, provide deeper understanding of its concept, and shed the light on undiscovered discretion’s antecedents and consequences. We adopt a quantitative approach in which questionnaires represent our data collection instrument. We anticipate that in high discretion countries firms tend to follow what Miles & Snow (1978) labeled ‘Prospector’ strategy as opposed to low discretion countries in which firms incline to implement a ‘Defender’ strategy.

Cities have become a focal point for efforts to transition towards a more sustainable, low-carbon society, with many municipal agencies championing ‘eco city’ initiatives of one kind or another. And yet, national policy initiatives frequently play an important – if sometimes overlooked – role, too. This chapters provides comparative perspectives on four recent national sustainable city programmes from France, India, Japan, and the United Kingdom. The analysis reveals two key insights: first, national policy is found to exercise a strong shaping role in what sustainable development for future cities is understood to be, which helps explain the considerable differences in priorities and approaches across countries. Second, beyond articulating strategic priorities, national policy may exercise a ‘soft’ governance function by incentivising and facilitating wider, voluntary governance networks in the effort to implement sustainable city projects locally. This innovative role, however, depends on the ability of national policy to produce resonance among societal actors and on its effective interaction with formal planning processes.

Partner selection is crucial to green supply chain management as the focal firm is responsible for the environmental performance of the whole supply chain. The construction of appropriate selection criteria is an essential, but often neglected pre-requisite in the partner selection process. This paper proposes a three-stage model that combines Dempster-Shafer belief acceptability theory and particle swarm optimization technique for the first time in this application. This enables optimization of both effectiveness, in its consideration of the inter-dependence of a broad range of quantitative and qualitative selection criteria, and efficiency in its use of scarce resources during the criteria construction process to be achieved simultaneously. This also enables both operational and strategic attributes can be selected at different levels of hierarchy criteria in different decision-making environments. The practical efficacy of the model is demonstrated by an application in Company ABC, a large Chinese electronic equipment and instrument manufacturer.

OBJECTIVE: To provide a detailed phenotype/genotype characterization of Bietti crystalline dystrophy (BCD). DESIGN: Observational case series. PARTICIPANTS: Twenty patients from 17 families recruited from a multiethnic British population. METHODS: Patients underwent color fundus photography, near-infrared (NIR) imaging, fundus autofluorescence (FAF) imaging, spectral domain optical coherence tomography (SD-OCT), and electroretinogram (ERG) assessment. The gene CYP4V2 was sequenced. MAIN OUTCOME MEASURES: Clinical, imaging, electrophysiologic, and molecular genetics findings. RESULTS: Patients ranged in age from 19 to 72 years (median, 40 years), with a visual acuity of 6/5 to perception of light (median, 6/12). There was wide intrafamilial and interfamilial variability in clinical severity. The FAF imaging showed well-defined areas of retinal pigment epithelium (RPE) loss that corresponded on SD-OCT to well-demarcated areas of outer retinal atrophy. Retinal crystals were not evident on FAF imaging and were best visualized with NIR imaging. Spectral domain OCT showed them to be principally located on or in the RPE/Bruch's membrane complex. Disappearance of the crystals, revealed by serial recording, was associated with severe disruption and thinning of the RPE/Bruch's membrane complex. Cases with extensive RPE degeneration (N = 5) had ERGs consistent with generalized rod and cone dysfunction, but those with more focal RPE atrophy showed amplitude reduction without delay (N = 3), consistent with restricted loss of function, or that was normal (N = 2). Likely disease-causing variants were identified in 34 chromosomes from 17 families. Seven were novel, including p.Met66Arg, found in all 11 patients from 8 families of South Asian descent. This mutation appears to be associated with earlier onset (median age, 30 years) compared with other substitutions (median age, 41 years). Deletions of exon 7 were associated with more severe disease. CONCLUSIONS: The phenotype is highly variable. Several novel variants are reported, including a highly prevalent substitution in patients of South Asian descent that is associated with earlier-onset disease. Autofluorescence showed sharply demarcated areas of RPE loss that coincided with abrupt edges of outer retinal atrophy on SD-OCT; crystals were generally situated on or in the RPE/Bruch's complex but could disappear over time with associated RPE disruption. These results support a role for the RPE in disease pathogenesis.

Purpose: This paper presents a combined multi-phase supplier selection model. The process repeatedly revisits the criteria and sourcing decision as the development process continues. This enables a structured adoption of product and production system innovation from strategic suppliers, where previously the literature purely focuses on product innovation or cost reduction. Design/methodology/approach: The authors adopted an embedded researcher style, inductive, qualitative case study of an industrial supply cluster comprising a focal automotive company and its interaction with three different strategic stamping suppliers. Findings: Our contribution is the multi-phased production and product innovation process. This is an advance from traditional supplier selection and also an extension of ideas of supplier-located product development as it includes production system development, and complements the literature on working with strategic suppliers. Specifically, we explicitly articulate the previously unreported issue of whether a supplier chosen for its innovation capabilities at the start of the new product development process will also be the most appropriate supplier during the production system development phase, when an ability to work collaboratively may be the most important attribute, or in the large-scale production phase when an ability to manufacture at low unit cost may be most important. Originality/value: The paper identifies a multi-phase approach to tendering within a fixed body of strategic suppliers which seeks to identify the optimum technological and process decisions as well as the traditional supplier sourcing choice. These areas have not been combined before and generate a valuable approach for firms to adopt as well as for researchers to extend our understanding of a highly complex process.

Introdução: A unidade de biofeedback de pressão, em indivíduos com dor lombo-pélvica, é utilizada durante exercícios de estabilização segmentar, no entanto ainda carece de evidência científica. Objetivo: Determinar a relação entre a unidade de biofeedback de pressão (UBFP), o deslocamento do centro de pressão no sentido médio-lateral (COPml) e a atividade eletromiográfica abdominal durante o active straight leg raising (ASLR) em indivíduos com e sem dor lombo-pélvica, bem como identificar diferenças entre os grupos. Metodologia: Estudo transversal analítico em 18 estudantes universitários voluntários com dor lombo-pélvica crónica inespecífica (GCD) e em 20 sem dor (GSD). Durante o ASLR (desafio postural dinâmico) foram avaliadas as variações máxima e média da pressão (recorrendo à UBFP) e do deslocamento do COPml (através da plataforma de forças), bem como a atividade muscular abdominal, bilateralmente, com recurso à eletromiografia de superfície. Estatisticamente recorreu-se à correlação de Spearman e ao teste Mann-Whitney U, ambos com um nível de significância de 0,05. Resultados: No GCD, ao contrário do GSD, não foi verificada uma relação entre a UBFP e a atividade do transverso abdominal/obliquo interno (TrA/OI) contra-lateral. Correlações moderadas, mas com sentidos opostos, foram evidenciadas em ambos os grupos, entre o deslocamento do COPml e a atividade do TrA/OI contra-lateral. Em ambos os grupos, a UBFP demonstrou estar fortemente correlacionada com o COPml. Não foram observadas diferenças significativas entre os grupos nas variáveis avaliadas. Conclusão: A UBFP, no GCD, não se apresentou relacionada com a atividade do TrA/OI. Contudo, demonstrou uma relação com o deslocamento do COPml, em ambos os grupos, sendo portanto um indicador de estabilidade do tronco e assim, uma ferramenta útil em ambiente clínico. No GCD observou-se que uma maior atividade muscular TrA/OI pressupõe maior deslocamento do COPml, sendo uma relação contrária à verificada no GSD, podendo ser um indicador da perda da sua ação tónica.

Introdução: O padrão de recrutamento temporal inerente a uma sequência de ativação muscular (SAM), permite a organização multi-segmentar para a realização de uma tarefa motora. Este depende da conexão neural entre estruturas corticais e sub-corticais, incluindo os núcleos da base (NB), podendo, assim, estar comprometido em indivíduos com Doença de Parkinson (DP). As SAM poderão ser melhoradas através de uma intervenção baseada no conceito de Bobath (CB). Objetivo: Estudar o potencial da intervenção, baseada no CB, a longo prazo, nas SAM ao nível da tibio-társica (TT), durante as tarefas motoras sit-to-stand (SitTS) e o stand-to-sit (StandTS), em quatro indivíduos com DP. Metodologia: O estudo apresenta quatro casos de indivíduos com DP, que realizaram intervenção em fisioterapia baseada no CB, durante 12 semanas. Antes e após a intervenção, foram avaliadas as sequências de ativação do gastrocnémio medial (GM), do solear (SOL) e do tibial anterior (TA), durante as tarefas SitTS e StandTS, recorrendo à eletromiografia de superfície e à plataforma de forças, para a divisão cinética das diferentes fases das tarefas. Avaliou-se ainda o equilíbrio funcional, através da Escala de Berg, e a percepção subjetiva dos indivíduos acerca da sua capacidade para realizar atividades sem cair, através da Modified Falls Efficacy Scale. Resultados: Após a intervenção, os indivíduos em estudo apresentaram, na sua maioria, uma diminuição da co-ativação muscular, bem como um aumento do equilíbrio funcional e diminuição da probabilidade de risco de queda, refletindo uma melhoria do controlo postural (CP). As modificações na percepção subjetiva dos indivíduos acerca da sua capacidade para realizar atividades sem cair não foram homogéneas. Conclusão: A intervenção baseada no CB teve efeitos positivos do ponto de vista do CP nos quatro indivíduos com DP. Pensa-se que uma intervenção mais duradoura poderá intensificar as melhorias observadas.

Introdução: Por mais de uma década assumiu-se que o transverso abdominal/oblíquo interno (TrA/OI) se comportava como um ventre muscular único, pré-ativando-se bilateralmente de forma simultânea para aumentar a stiffness da coluna lombar, conferindo-lhe estabilidade segmentar durante o movimento de um dos membros superiores. À luz dos estudos atuais sabe-se que tal mecanismo não ocorre, uma vez que o TrA/OI apresenta uma ativação por mecanismo de feedforward predominantemente contralateral ao membro superior movido. Apesar de morfologicamente o TrA/OI do lado não-dominante ser mais hipertrofiado do que o do lado dominante, nada se sabe acerca da influência da dominância nos timings ativação do TrA/OI. Objectivos: Confirmar se o TrA/OI tem um timing de ativação assimétrico durante o movimento rápido de flexão do membro superior (MRMS). Pretende-se ainda avaliar se o timing de ativação do TrA/OI é influenciado pela dominância de lateralidade manual. Métodos: Efectuou-se um estudo observacional descritivo, transversal e duplamente cego com 32 atletas de futebol voluntários com membro superior direito dominante, colocando-os apenas num grupo. Procedeu-se à recolha do sinal eletromiográfico de forma a avaliar os timings de ativação do TrA/OI bilateralmente aquando dos movimentos rápidos de flexão de ambos os membros superiores, à vez. Todos os dados foram tratados estatisticamente com o programa SPSS, versão 20.0 para Mac OS, com um grau de significância de 0,05. Resultados: Verificaram-se diferenças nos timings de ativação dos TrA/OI direito e esquerdo durante os MRMS direito e esquerdo (Teste ANOVA medidas repetidas: F=291,087; p<0,001). O timing de ativação do TrA/OI direito – 29,15(13,15)ms – foi superior ao esquerdo – 4,71(17,32)ms – durante MRMS direito (Teste Post Hoc Bonferroni: p<0,001). O timing de ativação do TrA/OI esquerdo – 31,98(12,50)ms – foi superior ao direito – 12,20(17,40)ms – durante MRMS esquerdo (p<0,001). O timing de ativação do TrA/IO direito aquando do MRMS direito foi superior ao observado durante MRMS esquerdo (p<0,001). O contrário sucedeu em relação ao timing ativação do TrA/IO esquerdo (p<0,001). O timing de ativação do TrA/IO esquerdo no MRMS direito foi inferior ao do TrA/IO direito aquando do movimento com o membro esquerdo (p<0,001). O TrA/IO direito possuiu um timing de ativação no MRMS direito inferior ao do TrA/IO esquerdo aquando do movimento com o membro esquerdo (p<0,001). Conclusão: Através deste estudo pôde-se concluir que o TrA/OI contralateral ao MRMS apresenta um timing de ativação inferior ao ipsilateral e ainda que durante o MRMS dominante o TrA/OI esquerdo e direito apresentam um timing de ativação inferior ao ocorrido durante o MRMS não-dominante.

Multifocal intraocular lenses (MF IOLs) have concentric optical zones with different dioptric power, enabling patients to have good visual acuity at multiple focal points. However, several optical limitations have been attributed to this particular design. The purpose of this study is to access the effect of MF IOLs design on the accuracy of retinal optical coherence tomography (OCT). Cross-sectional study conducted at the Refractive Surgery Department of Central Lisbon Hospital Center. Twenty-three eyes of 15 patients with a diffractive MF IOL and 27 eyes of 15 patients with an aspheric monofocal IOL were included in this study. All patients underwent OCT macular scans using Heidelberg Spectralis®. Macular thickness and volume values and image quality (Q factor) were compared between the two groups. There were no statistically significant differences between both groups regarding macular thickness or volume measurements. Retinal OCT image quality was significantly lower in the MF IOL group (p < 0.01). MF IOLs are associated with a significant decrease in OCT image quality. However, this fact does not seem to compromise the accuracy of spectral domain OCT retinal measurements.

Multiparametric Magnetic Resonance Imaging has been increasingly used for detection, localization and staging of prostate cancer over the last years. It combines high-resolution T2 Weighted-Imaging and at least two functional techniques, which include Dynamic Contrast–Enhanced Magnetic Resonance Imaging, Diffusion-Weighted Imaging, and Magnetic Resonance Imaging Spectroscopy. Although the combined use of a pelvic phased-array and an Endorectal Coil is considered the state-of-the-art for Magnetic Resonance Imaging evaluation of prostate cancer, Endorectal Coil is only absolute mandatory for Magnetic Resonance Imaging Spectroscopy at 1.5 T. Sensitivity and specificity levels in cancer detection and localization have been improving with functional technique implementation, compared to T2 Weighted-Imaging alone. It has been particularly useful to evaluate patients with abnormal PSA and negative biopsy. Moreover, the information added by the functional techniques may correlate to cancer aggressiveness and therefore be useful to select patients for focal radiotherapy, prostate sparing surgery, focal ablative therapy and active surveillance. However, more studies are needed to compare the functional techniques and understand the advantages and disadvantages of each one. This article reviews the basic principles of prostatic mp-Magnetic Resonance Imaging, emphasizing its role on detection, staging and active surveillance of prostate cancer.

OBJECTIVES: To investigate if the shading sign is an exclusive MRI feature of endometriomas or endometrioid tumors, and to analyze its different patterns. METHODS: Three hundred and fourty six women with adnexal masses who underwent 1.5/3-T MRI were included in this retrospective, board-approved study. The shading sign was found in 56 patients, but five cases were excluded due to lack of imaging follow-up or histological correlation. The final sample included 51 women. The type of tumor and the pattern of shading were recorded for each case. RESULTS: Thirty endometriomas and five endometrioid carcinomas were found. The remaining 16 cases corresponded to other benign and malignant tumors. The overall sensitivity, specificity, positive predictive value, and negative predictive value were 73%, 93%, 59%, and 96%, respectively. Restricting the analysis to cystic lesions without solid or fat component, sensitivity, specificity, positive predictive value, and negative predictive value were 73%, 96%, 94%, and 80%. Five shading patterns were identified: layering (15.7%), liquid-liquid level (11.8%), homogenous (45.1%), heterogeneous (11.8%), and focal/multifocal shading within a complex mass (19.6%). No significant correlation was found between these patterns and the type of tumor. CONCLUSIONS: The shading sign is not exclusive of endometriomas or endometrioid tumors. Homogenous shading was the most prevalent pattern in endometriomas and half of the cases with focal/multifocal shading within a complex mass were endometrioid carcinomas.

Presentemente, a Comunidade para o Desenvolvimento da África Austral (SADC) apresenta-se como uma das mais desenvolvidas tentativas de integração do continente africano. Esta organização passou a revestir-se, em 1992, de um caráter permanente, tornando-se produto de um esforço de institucionalização que se tem vindo a materializar no estabelecimento de metas e estágios para um ambicioso projeto de integração económica. As suas instituições, frequentemente percecionadas como sendo regidas por uma lógica intergovernamental, serão a unidade focal desta análise. Esta tem como principal objetivo a verificação do seu impacto no processo de integração económica desta organização, particularmente o estabelecimento de uma relação de causalidade entre a estrutura institucional e o atual estado da vertente económica do processo integrativo. Procura-se assim inferir se o quadro institucional da organização é capaz de gerar uma dinâmica autónoma em prol da integração ou se sucumbe perante o voluntarismo estatal dos seus membros. A conclusão a que se chegará, como se pretende advogar, não implica dimensões mutuamente exclusivas, podendo traduzir-se numa conceção híbrida acerca do papel institucional no curso evolutivo da SADC, porventura mais fiel à complexidade deste bloco regional. A hipótese explicativa para o atual estado integrativo (económico) da SADC que se testará nesta análise será a de que a relativa fragilidade institucional desta organização - caso tal se verifique - não a impede de se desenvolver num curto a médio-prazo e, pelo contrário, constitui um dos incentivos à participação dos Estados Membros. Argumentar-se-á adicionalmente que os interesses integrativos dos Estados que a compõem não derivam de um qualquer espírito pós ou antinacional, mas antes de um condicionamento histórico favorável à cooperação intra-africana e de um cálculo de interesses que redunda na aceção de que a ação concertada entre Estados lhes confere vantagens estratégicas. Por isso mesmo, os modelos teóricos que substanciarão a análise, ainda que recorram por vezes aos moldes seminais derivados do estudo da experiência integrativa europeia, abarcarão também um conjunto de escolhas plásticas e maleáveis à realidade regional. Dentro do leque de estudos disponíveis, primar-se-á igualmente pela flexibilidade e abrangência. A difícil conciliação entre profundidade e abrangência redundará numa escolha de relevantes e úteis correntes teóricas, na qual se destacam o institucionalismo histórico e o institucionalismo da escolha racional, bem como o intergovernamentalismo liberal, o neofuncionalismo e a teoria do dominó aplicada aos blocos regionais.

Pontine ischemia usually results in focal deficits such as hemiparesis, facial palsy, dysarthria, disorders of eye movements or vertigo. Although rarely described, involuntary abnormal movements and "convulsions" due to pontine lesions can also occur. Here we describe a 67-year-old woman with hypertension who presented with a tonic movement mimicking a versive seizure in the acute phase of bilateral pontine ischemia. Post-stroke movement disorders are well known. They are usually associated with supratentorial lesions and rarely occur in the acute phase, but "seizure-like" episodes can be seen in pontine ischemia. Awareness of this rare phenomenon is useful for the management of acute stroke patients.

PURPOSE: To present the long-term follow-up of 10 adolescents and young adults with documented cognitive and behavioral regression as children due to nonlesional focal, mainly frontal, epilepsy with continuous spike-waves during slow wave sleep (CSWS). METHODS: Past medical and electroencephalography (EEG) data were reviewed and neuropsychological tests exploring main cognitive functions were administered. KEY FINDINGS: After a mean duration of follow-up of 15.6 years (range, 8-23 years), none of the 10 patients had recovered fully, but four regained borderline to normal intelligence and were almost independent. Patients with prolonged global intellectual regression had the worst outcome, whereas those with more specific and short-lived deficits recovered best. The marked behavioral disorders resolved in all but one patient. Executive functions were neither severely nor homogenously affected. Three patients with a frontal syndrome during the active phase (AP) disclosed only mild residual executive and social cognition deficits. The main cognitive gains occurred shortly after the AP, but qualitative improvements continued to occur. Long-term outcome correlated best with duration of CSWS. SIGNIFICANCE: Our findings emphasize that cognitive recovery after cessation of CSWS depends on the severity and duration of the initial regression. None of our patients had major executive and social cognition deficits with preserved intelligence, as reported in adults with early destructive lesions of the frontal lobes. Early recognition of epilepsy with CSWS and rapid introduction of effective therapy are crucial for a best possible outcome.
