Background Infant development is adversely affected in the context of postnatal depression. This relationship may be mediated by both the nature of early mother-infant interactions and the quality of the home environment. Aim To establish the usefulness of the Global Ratings Scales of Mother-Infant Interaction and the Infant-Toddler version of the Home Observation for the Measurement of the Environment (IT-HOME), and to test expected associations of the measures with characteristics of the social context and with major or minor depression. Method Both assessments were administered postnatally in four European centres; 144 mothers were assessed with the Global Ratings Scales and 114 with the IT-HOME. Affective disorder was assessed by means of the Structured Clinical Interview for DSM-IV Disorders. Results Analyses of mother-infant interaction indicated no main effect for depression but maternal sensitivity to infant behaviour was associated with better infant communication, especially for women who were not depressed. Poor overall emotional support also reduced sensitivity scores. Poor support was also related to poorer IT-HOME scores, but there was no effect of depression. Conclusions The Global Ratings Scales were effectively applied but there was less evidence of the usefulness of the IT-HOME. Declaration of interest None.

As lesões crônicas do fígado são resultantes de agressões persistentes, onde a desorganização e destruição do tecido podem desencadear processos de regeneração e fibrose. Para que a integridade e homeostase do órgão sejam restauradas, várias vias intracelulares e intercelulares são ativadas. Uma delas é a através da liberação de moléculas pró-fibrogênicas, a exemplo da lectina solúvel bgalactosídea, a galectina-3 (Gal-3). A alta expressão dessa lectina tem sido associada a fibrogênese no fígado. A descoberta de moléculas capazes de se ligar à Gal-3 e inibir a sua ação são importantes no desenvolvimento de terapias antifibrosantes. A pectina cítrica modificada (PCM) e a N-acetilactosamina (LacNAc) demonstraram ação benéfica no tratamento de doenças fibróticas, incluindo renais e cardíacas, contudo, pouco se sabe sobre suas eficácias na fibrose hepática. Diante disso, o objetivo deste estudo foi investigar os efeitos das administrações da PCM e LacNAc quanto aos níveis de Gal-3 e fibrose em modelo experimental de lesão hepática crônica. Inicialmente, a fibrose hepática foi induzida em camundongos C57BL/6 pela administração de tetracloreto de carbono a 20 por cento diluído em azeite de oliva. Grupos de camundongos com dois ou quatro meses de lesão foram tratados com PCM (1 por cento e 5 por cento, fornecida ad libitum) e com LacNAc, por via intraperitoneal. Adicionalmente, camundongos knockouts para o gene da Gal-3 (Gal-3-/-) foram utilizados como um controle. Subsequentemente às intervenções, análises morfométricas, bioquímicas, imunológicas e de biologia molecular foram realizadas. Na análise morfométrica, não se verificou alteração no percentual de tecido fibroso entre os grupos tratados com PCM (1 por cento e 5 por cento) ou LacNAc quando comparados com seus respectivos controles, o que foi confirmado pela dosagem dos níveis de hidroxiprolina. Adicionalmente, não foi observada alteração dos níveis de Gal-3, MMP-9 e TIMP-1 após tratamento com os inibidores. Os resultados obtidos indicaram que a PCM e a LacNAc não foram capazes de inibir a Gal-3 no fígado, e portanto, não interferiram na deposição de tecido fibroso nesse modelo experimental

O titânio e suas ligas são os materiais mais comumente utilizados na substituição de tecidos duros por possuírem resistência mecânica, biocompatibilidade, resistência à corrosão e fácil manipulação. Embora o titânio possua várias vantagens sobre outros biomateriais, seu uso em longo prazo pode ocasionar problemas de rejeição. A modificação da superfície do titânio a fim de criar microrrugosidades é uma estratégia efetiva para melhorar a adesão e proliferação celular sobre implantes. Quando um implante danifica ou invade as barreiras epitelial e das mucosas, pode servir como reservatório para microrganismos e desta forma predispor à infecção. Neste sentido, o objetivo deste trabalho foi modificar a superfície do titânio, utilizando nanopartículas de prata (Ag) e lectina, a fim de melhorar a sua biocompatibilidade e conferir propriedades antimicrobianas a este material. O racional por trás destas mudanças é que a criação de uma topografia em nanoescala pode contribuir para mimetizar o ambiente celular melhorando a osseointegração e diminuindo o risco de infecção. Em nosso estudo, nanotubos de dióxido de titânio (NTs-TiO2) com estrutura bem distribuída e organizada, com diâmetro em torno de 70–80nm, foram sintetizados por anodização eletroquímica e decorados com nanopartículas de Ag usando a técnica de layer-by-layer (LbL), enquanto a lectina do peixe Oreochromis niloticus (OniL) foi incorporada aos NTs-TiO2 por spin coating. Estas amostras foram caracterizadas e avaliadas quanto a sua citotoxidade, adesão celular, potencial osteogênico e atividade bactericida. Nossos resultados mostraram que tanto as nanopartículas de Ag, como a Onil foram incorporadas com sucesso à superfície dos NTs-TiO2. Entretanto nossas preparações de LbL não foram capazes de melhorar a biocompatibilidade ou inibir o crescimento de bactérias nos NTs-TiO2. Por outro lado, a funcionalização dos NTs-TiO2 com a OniL induziu eficientemente a adesão e proliferação dos osteoblastos. Nossos resultados apontam para o uso da lectina OniL para melhorar a qualidade dos implantes de NT-TiO2 existentes.

A leishmaniose tegumentar americana é uma doença que acomete a pele e as mucosas das vias aerodigestivas superiores. Os antimoniais pentavalentes vêm sendo empregados há muitas décadas como fármacos de primeira linha para o seu tratamento. Pacientes com poucas lesões cutâneas, com impossibilidade de receber medicação parenteral regular ou com sinais de toxicidade importante ao antimonial por via sistêmica, podem ser submetidos ao tratamento intralesional com antimoniato de meglumina. Objetivos: descrever a eficácia e a segurança do antimoniato de meglumina administrado por via intralesional, para o tratamento da leishmaniose cutânea. Método: Foi realizado um estudo descritivo, retrospectivo, tipo série de casos, de pacientes atendidos no Laboratório de Vigilância em Leishmanioses do Instituto de Pesquisa Clínica Evandro Chagas \2013 FIOCRUZ, de 2002 até julho de 2011, que tivessem sido tratados para leishmaniose cutânea com aplicação intralesional de antimoniato de meglumina, após tratamento sistêmico com o mesmo fármaco Além disso, relatamos dois casos especiais de pacientes tratados com antimoniato de meglumina intralesional: o primeiro, de um paciente que apresentou resistência ao antimoniato de meglumina sistêmico e intralesional, não tolerou anfotericina B e respondeu apenas à pentamidina. O outro, de paciente com eczema generalizado após antimoniato de meglumina sistêmico e que desenvolveu eczema bolhoso em local de aplicação intralesional do mesmo fármaco. Resultados: Pacientes com leishmaniose cutânea apresentaram boa resposta terapêutica ao antimoniato de meglumina administrado por via intralesional com um mínimo de efeitos adversos, geralmente sem necessidade de mudar o fármaco para outros de mais difícil administração e alto custo, e sem desenvolvimento de lesões mucosas. Medicamentos de segunda ou terceira escolha são eficazes, na ausência de resposta prévia ao antimoniato de meglumina sistêmico e intralesional. A via intralesional deve ser evitada em caso de farmacodermia ao antimoniato de meglumina sistêmico

Neste estudo comparamos esquemas de alta e baixa dose de antimoniato de meglumina (AM) para o tratamento da forma cutânea de leishmaniose tegumentar americana, em pacientes oriundos do estado do Rio de Janeiro. OBJETIVO: Comparar a eficácia representada pela cura imediata (epitelização em 120 dias), tardia (cicatrização em 360 dias) e definitiva (ausência de reativação ou lesão mucosa em 720 dias) e toxicidade (clínica, laboratorial e eletrocardiográfica) com duas diferentes doses de tratamento com AM para leishmaniose cutânea (LC) e comparar os critérios de cura clínica aqui adotados com aqueles estabelecidos pelo Ministério da Saúde. MÉTODO: Ensaio clínico de não inferioridade, controlado, randomizado, cego e de fase III, com 60 pacientes com LC alocados em dois grupos de tratamento: (A) 20mg Sb5+/kg/dia por 20 dias e (B) 5mg Sb5+/kg/dia por 30 dias administrados por via intramuscular. RESULTADOS: Pacientes dos grupos A e B apresentaram, respectivamente: Cura imediata 90,0% e 86,7%, com tempo médio de epitelização de 58,7 e 54,9 dias; cura tardia por intenção de tratar 76,7% e 73,3%; e cura tardia por análise de protocolo 84,6% e 75,9%. Dos 53 pacientes que apresentaram epitelização em até 120 dias, 44 (83,4%) evoluíram para cura tardia. Quando avaliados conjuntamente, os efeitos adversos (EA) clínicos, laboratoriais e eletrocardiográficos foram mais frequentes no grupo A que no grupo B (médias 7,9 e 4,7) e mais graves [RR= 2,22 (IC 95% 1,22-4,06) p=0,0045] no grupo A. Os EA clínicos graus 2 e 3 foram mais frequentes no grupo A; RR=6,5 (IC 95% 1,60-26,36) p=0,001 Os EA laboratoriais foram mais frequentes RR=1,39 (IC 95% 0,99-1,93) p=0,05 e mais graves (graus 2, 3 ou 4) RR=4,67 (IC 95% 1,49-14,59) p=0,0016 no grupo A. Hiperlipasemia foi a alteração laboratorial mais frequente e mais grave. Pacientes do grupo A apresentaram um RR=4,0, p=0,006 de desenvolver hiperlipasemia moderada a grave, com fração atribuível de 75%. Dez pacientes (16,7%) necessitaram suspender o tratamento temporariamente por apresentarem QTc >0,46ms, entretanto não houve diferença entre os grupos.CONCLUSÃO: A dose de 5mg Sb5+/kg/dia mostrou-se menos tóxica, mais segura e de menor custo no tratamento da LC, especialmente em idosos e pacientes com co-morbidades, atualmente com restritas opções terapêuticas. Entretanto, a hipótese de não inferioridade da dose baixa em relação a dose alta não pôde ser comprovada A epitelização com 120 dias, seguida de progressiva melhora no sentido da cicatrização em 360 dias sugere um possível benefício com a flexibilização dos critérios de cura da LC

Cladistic analyses begin with an assessment of variation for a group of organisms and the subsequent representation of that variation as a data matrix. The step of converting observed organismal variation into a data matrix has been considered subjective, contentious, under-investigated, imprecise, unquantifiable, intuitive, as a black-box, and at the same time as ultimately the most influential phase of any cladistic analysis (Pimentel and Riggins, 1987; Bryant, 1989; Pogue and Mickevich, 1990; de Pinna, 1991; Stevens, 1991; Bateman et al., 1992; Smith, 1994; Pleijel, 1995; Wilkinson, 1995; Patterson and Johnson, 1997). Despite the concerns of these authors, primary homology assessment is often perceived as reproducible. In a recent paper, Hawkins et al. (1997) reiterated two points made by a number of these authors: that different interpretations of characters and coding are possible and that different workers will perceive and define characters in different ways. One reviewer challenged us: did we really think that two people working on the same group would come up with different data sets? The conflicting views regarding the reproducibility of the cladistic character matrix provoke a number of questions. Do the majority of workers consistently follow the same guidelines? Has the theoretical framework informing primary homology assessment been adequately explored? The objective of this study is to classify approaches to primary homology assessment, and to quantify the extent to which different approaches are found in the literature by examining variation in the way characters are defined and coded in a data matrix.

Many studies have shown that farmers in developing countries often overuse pesticides and do not adopt safety practices. Policies and interventions to promote a safer use of pesticides are often based on a limited understanding of the farmers’ own perspective of pesticide use. This often results in ineffective policies and the persistence of significant pesticide-related health and environmental problems, especially in developing countries. This chapter explores potentials and limitations of different approaches to study pesticide use in agriculture from the farmers’ perspective. In contrast to the reductionist and mono-disciplinary approaches often adopted, this chapter calls for integrative methodological approaches to provide a realistic and thorough understanding of the farmers’ perspective on pesticide use and illustrates the added value of such an approach with three case studies of pesticide use in Iran, India, and Colombia.

Some vocal disorders in teachers are associated with occupational factors, but there are few studies that analyze the influence of vocal habits, fluid intake, mastication, and sleep on these disorders. The objective was to analyze the Occurrence of vocal fatigue, hoarseness, and dry throat in elementary and high school teachers and their association with vocal habits, fluid intake, mastication, and sleep. A sample of 422 elementary and secondary school teachers was Studied using a specific questionnaire. The multiple regression analysis showed that hoarseness was associated with absence of water intake (odds ratio (OR) = 1.7; P = 0.047), yelling/speaking loudly (OR = 1.6; P = 0.058), jaw-opening limitations (OR = 3.8; P = 0.003). average of: 6 hours of sleep/light (OR = 1.7; P = 0.039), and waking-up feeling replenished (OR = 2.0; P = 0.020). The presence of vocal fatigue was significantly associated with yelling/speaking loudly (OR = 2.2: P = 0.013), speaking excessively (OR = 2.4; P = 0.023), difficulty to open the mouth to masticate (OR = 6.6; P = 0.003), less than 6 hours of sleep (OR = 4.0; P = 0.008), and waking-up feeling replenished (sometimes OR = 2.8: P = 0.003; or never OR = 3.3 P = 0.002). The presence of dry throat was associated with being a former smoker (OR = 3.3; P = 0.011) and having jaw-opening limitations (OR = 3.9; P = 0.021). In recent years, speech and hearing interventions with teachers have focused on health-care promotion actions and prevention of vocal disorders, prioritizing issues related with hydration and healthy vocal use habits. However, the findings in the present study show the need to further focus on lifestyle habits related to sleep and eating habits.

This multicentric population-based study in Brazil is the first national effort to estimate the prevalence of hepatitis B (HBV) and risk factors in the capital cities of the Northeast. Central-West, and Federal Districts (2004-2005). Random multistage cluster sampling was used to select persons 13-69 years of age. Markers for HBV were tested by enzyme-linked immunosorbent assay. The HBV genotypes were determined by sequencing hepatitis B surface antigen (HBsAg). Multivariate analyses and simple catalytic model were performed. Overall. 7,881 persons were inculded < 70% were not vaccinated. Positivity for HBsAg was less than 1% among non-vaccinated persons and genotypes A, D, and F co-circulated. The incidence of infection increased with age with similar force of infection in all regions. Males and persons having initiated sexual activity were associated with HBV infection in the two settings: healthcare jobs and prior hospitalization were risk factors in the Federal District. Our survey classified these regions as areas with HBV endemicity and highlighted the risk factors differences among the settings.

Background The objectives were to estimate the prevalence of hepatitis A among children and adolescents from the Northeast and Midwest regions and the Federal District of Brazil and to identify individual-, household- and area-levels factors associated with hepatitis A infection. Methods This population-based survey was conducted in 20042005 and covered individuals aged between 5 and 19 years. A stratified multistage cluster sampling technique with probability proportional to size was used to select 1937 individuals aged between 5 and 19 years living in the Federal capital and in the State capitals of 12 states in the study regions. The sample was stratified according to age (59 and 10- to 19-years-old) and capital within each region. Individual- and household-level data were collected by interview at the home of the individual. Variables related to the area were retrieved from census tract data. The outcome was total antibodies to hepatitis A virus detected using commercial EIA. The age distribution of the susceptible population was estimated using a simple catalytic model. The associations between HAV infection and independent variables were assessed using the odds ratio and corrected for the random design effect and sampling weight. Multilevel analysis was performed by GLLAMM using Stata 9.2. Results The prevalence of hepatitis A infection in the 59 and 1019 age-group was 41.5 and 57.4, respectively for the Northeast, 32.3 and 56.0, respectively for the Midwest and 33.8 and 65.1 for the Federal District. A trend for the prevalence of HAV infection to increase according to age was detected in all sites. By the age of 5, 31.5 of the children had already been infected with HAV in the Northeast region compared with 20.0 in the other sites. By the age of 19 years, seropositivity was 70 in all areas. The curves of susceptible populations differed from one area to another. Multilevel modeling showed that variables relating to different levels of education were associated with HAV infection in all sites. Conclusion The study sites were classified as areas with intermediate endemicity area for hepatitis A infection. Differences in age trends of infection were detected among settings. This multilevel model allowed for quantification of contextual predictors of hepatitis A infection in urban areas.

Scrotal circumference data from 47,605 Nellore young bulls, measured at around 18 mo of age (SC18), were analyzed simultaneously with 27,924 heifer pregnancy (HP) and 80,831 stayability (STAY) records to estimate their additive genetic relationships. Additionally, the possibility that economically relevant traits measured directly in females could replace SC18 as a selection criterion was verified. Heifer pregnancy was defined as the observation that a heifer conceived and remained pregnant, which was assessed by rectal palpation at 60 d. Females were exposed to sires for the first time at about 14 mo of age (between 11 and 16 mo). Stayability was defined as whether or not a cow calved every year up to 5 yr of age, when the opportunity to breed was provided. A Bayesian linear-threshold-threshold analysis via Gibbs sampler was used to estimate the variance and covariance components of the multitrait model. Heritability estimates were 0.42 +/- 0.01, 0.53 +/- 0.03, and 0.10 +/- 0.01, for SC18, HP, and STAY, respectively. The genetic correlation estimates were 0.29 +/- 0.05, 0.19 +/- 0.05, and 0.64 +/- 0.07 between SC18 and HP, SC18 and STAY, and HP and STAY, respectively. The residual correlation estimate between HP and STAY was -0.08 +/- 0.03. The heritability values indicate the existence of considerable genetic variance for SC18 and HP traits. However, genetic correlations between SC18 and the female reproductive traits analyzed in the present study can only be considered moderate. The small residual correlation between HP and STAY suggests that environmental effects common to both traits are not major. The large heritability estimate for HP and the high genetic correlation between HP and STAY obtained in the present study confirm that EPD for HP can be used to select bulls for the production of precocious, fertile, and long-lived daughters. Moreover, SC18 could be incorporated in multitrait analysis to improve the prediction accuracy for HP genetic merit of young bulls.

The production of a healthy cloned calf is dependent on a multitude of successful steps, including reprogramming mediated by the oocyte, the development of a functional placenta, adequate maternal-fetal interaction, the establishment of a physiological metabolic setting and the formation of a complete set of well-differentiated cells that will eventually result in well-characterised and fully competent tissues and organs. Although the efficiency of nuclear transfer has improved significantly since the first report of a somatic cell nuclear transfer-derived animal, there are many descriptions of anomalies concerning cloned calves leading to high perinatal morbidity and mortality. The present article discusses some our experience regarding perinatal and neonatal procedures for cloned Zebu cattle (B. indicus) that has led to improved survival rates in Nellore cloned calves following the application of such `labour-intensive technology`.
