CONTEXT: New trial data and drug regimens that have become available in the last 2 years warrant an update to guidelines for antiretroviral therapy (ART) in human immunodeficiency virus (HIV)-infected adults in resource-rich settings. OBJECTIVE: To provide current recommendations for the treatment of adult HIV infection with ART and use of laboratory-monitoring tools. Guidelines include when to start therapy and with what drugs, monitoring for response and toxic effects, special considerations in therapy, and managing antiretroviral failure. DATA SOURCES, STUDY SELECTION, AND DATA EXTRACTION: Data that had been published or presented in abstract form at scientific conferences in the past 2 years were systematically searched and reviewed by an International Antiviral Society-USA panel. The panel reviewed available evidence and formed recommendations by full panel consensus. DATA SYNTHESIS: Treatment is recommended for all adults with HIV infection; the strength of the recommendation and the quality of the evidence increase with decreasing CD4 cell count and the presence of certain concurrent conditions. Recommended initial regimens include 2 nucleoside reverse transcriptase inhibitors (tenofovir/emtricitabine or abacavir/lamivudine) plus a nonnucleoside reverse transcriptase inhibitor (efavirenz), a ritonavir-boosted protease inhibitor (atazanavir or darunavir), or an integrase strand transfer inhibitor (raltegravir). Alternatives in each class are recommended for patients with or at risk of certain concurrent conditions. CD4 cell count and HIV-1 RNA level should be monitored, as should engagement in care, ART adherence, HIV drug resistance, and quality-of-care indicators. Reasons for regimen switching include virologic, immunologic, or clinical failure and drug toxicity or intolerance. Confirmed treatment failure should be addressed promptly and multiple factors considered. CONCLUSION: New recommendations for HIV patient care include offering ART to all patients regardless of CD4 cell count, changes in therapeutic options, and modifications in the timing and choice of ART in the setting of opportunistic illnesses such as cryptococcal disease and tuberculosis.

Since World War II there have been about fifty episodes of large-scale mass killings of civilians and massive forced displacements. They were usually meticulously planned and independent of military goals. We provide a model where conflict onset, conflict intensity and the decision to commit mass killings are all endogenous, with two main goals: (1) to identify the key variables and situations that make mass killings more likely to occur; and (2) to distinguish conditions under which mass killings and military conflict intensity reinforce each other from situations where they are substitute modes of strategic violence. We predict that mass killings are most likely in societies with large natural resources, significant proportionality constraints for rent sharing, low productivity and low state capacity. Further, massacres are more likely in a civil than in an interstate war, as in the latter group sizes matter less for future rents. In non polarized societies there are asymmetric equilibria with only the larger group wanting to engage in massacres. In such settings the smaller group compensates for this by fighting harder in the first place. In this case we can talk of mass killings and fighting efforts to be substitutes. In contrast, in polarized societies either both or none of the groups can be ready to do mass killings in case of victory. Under the "shadow of mass killings" groups fight harder. Hence, in this case massacres and fighting are complements. We also present novel empirical results on the role of natural resources in mass killings and on what kinds of ethnic groups are most likely to be victimized in massacres and forced resettlements, using group level panel data.

Fanon, Senghor, and Ela took a radical stance in criticising the structures and mechanisms of power in hegemonic situations and relations between colonial subjects and colonial masters. They aimed to liberate African societies by decolonising the mind, culture and religion of colonial subjects. In this respect, we are concerned with the continuities and ruptures of the colonial encounter and its unequal relationships. Switzerland does not have an official colonial history and yet, Swiss companies and migrants were and are part of the world's colonies. In our contribution, we question what makes an event postcolonial : in other words, how are postcolonial relations negotiated in Switzerland? We discuss this question by analysing two annual sacred journeys in Switzerland that have been invented for and by African Christians (clerics and laity) together with the leaders of the Swiss Catholic church : one to the relics of African saints in St. Maurice, canton Valais and the other to the Black Madonna, the Virgin Mary of Einsiedeln, in the canton Schwyz. These events are empowered by the performance of African choirs - their music, dance, and costumes - but to which end and in which way?

Is there a link between decentralized governance and conflict prevention? This article tries to answer the question by presenting the state of the art of the intersection of both concepts. Provided that social conflict is inevitable and given the appearance of new threats and types of violence, as well as new demands for security based on people (human security), our societies should focus on promoting peaceful changes. Through an extensive analysis of the existing literature and the study of several cases, this paper suggests that decentralized governance can contribute to these efforts by transforming conflicts, bringing about power-sharing and inclusion incentives of minority groups. Albeit the complexity of assessing its impact on conflict prevention, it can be contended that decentralized governance might have very positive effects on the reduction of causes that bring about conflicts due to its ability to foster the creation of war/violence preventors. More specifically, this paper argues that decentralization can have a positive impact on the so-called triggers and accelerators (short- and medium-term causes).

The 2009 International Society of Urological Pathology Consensus Conference in Boston, made recommendations regarding the standardization of pathology reporting of radical prostatectomy specimens. Issues relating to surgical margin assessment were coordinated by working group 5. Pathologists agreed that tumor extending close to the 'capsular' margin, yet not to it, should be reported as a negative margin, and that locations of positive margins should be indicated as either posterior, posterolateral, lateral, anterior at the prostatic apex, mid-prostate or base. Other items of consensus included specifying the extent of any positive margin as millimeters of involvement; tumor in skeletal muscle at the apical perpendicular margin section, in the absence of accompanying benign glands, to be considered organ confined; and that proximal and distal margins be uniformly referred to as bladder neck and prostatic apex, respectively. Grading of tumor at positive margins was to be left to the discretion of the reporting pathologists. There was no consensus as to how the surgical margin should be regarded when tumor is present at the inked edge of the tissue, in the absence of transected benign glands at the apical margin. Pathologists also did not achieve agreement on the reporting approach to benign prostatic glands at an inked surgical margin in which no carcinoma is present.

The population of industrialized societies has increased tremendously over the last century, raising the question on how an enhanced age affects cognition. The relevance of two models of healthy aging are contrasted in the present study that both target the functioning of the two cerebral hemispheres. The right hemi-aging model (RHAM) assumes that functions of the right hemisphere decline before those of the left hemisphere. The Hemispheric Asymmetry Reduction in Older Adults (HAROLD) Model suggests that the contralateral hemisphere supports the normally superior hemisphere in a given task resulting in a reduced hemispheric asymmetry overall. In a mixed design, 20 younger and 20 older adults performed both a task assessing a left (lateralized lexical decisions) and a right (sex decisions on chimeric faces) hemisphere advantage. Results indicated that lateralized performance in both tasks was attenuated in older as compared to younger adults, in particular in men. These observations support the HAROLD model. Future studies should investigate whether this reduced functional hemispheric asymmetry in older age results from compensatory processes or from a process of de-differentiation

El objetivo de este estudio es contribuir al entendimiento, desde un punto de vista histórico, del papel que juega el cine en los procesos de popularización de la ciencia, la medicina y la tecnología. El punto de partida es la consideración de la medicina y del cine como conjuntos complejos y multidimensionales de “prácticas y discursos” de carácter científico-tecnológico y, por tanto, como formas particulares de acción e interacción cotidiana entre personas, colectivos e instituciones que juegan un papel básico en los procesos de construcción y funcionamiento de las sociedades contemporáneas. Así, mediante el análisis de la representación y articulación de las prácticas y los discursos médico-sanitarios en el cine documental colonial español del primer franquismo, es decir, en el caso particular de la construcción, legitimación y consolidación de un régimen totalitario, se aportan algunas claves explicativas sobre las múltiples relaciones entre diferentes colectivos (expertos y no-expertos) en cuanto a la naturaleza y el nivel de intervención en los procesos de generación y gestión de conocimiento científico-tecnológico. Como resultado, el cine se revela como un espacio no sólo de evasión y entretenimiento, sino también de ciencia y educación, donde la capacidad de definir y solucionar problemas cotidianos de la población, y por tanto, su impacto en la construcción de la arquitectura social, ideológica, económica, política y cultural de las sociedades contemporáneas, se fundamenta en la combinación de sus aspectos como espectáculo narrativo y científico-tecnológico.

Curated databases are an integral part of the tool set that researchers use on a daily basis for their work. For most users, however, how databases are maintained, and by whom, is rather obscure. The International Society for Biocuration (ISB) represents biocurators, software engineers, developers and researchers with an interest in biocuration. Its goals include fostering communication between biocurators, promoting and describing their work, and highlighting the added value of biocuration to the world. The ISB recently conducted a survey of biocurators to better understand their educational and scientific backgrounds, their motivations for choosing a curatorial job and their career goals. The results are reported here. From the responses received, it is evident that biocuration is performed by highly trained scientists and perceived to be a stimulating career, offering both intellectual challenges and the satisfaction of performing work essential to the modern scientific community. It is also apparent that the ISB has at least a dual role to play to facilitate biocurators' work: (i) to promote biocuration as a career within the greater scientific community; (ii) to aid the development of resources for biomedical research through promotion of nomenclature and data-sharing standards that will allow interconnection of biological databases and better exploit the pivotal contributions that biocurators are making. La formación investigadora en las distintas áreas del conocimiento es una desafío, una responsabilidad y una proyección para aquellas sociedades que busquen el desarrollo equilibrado y sostenible para el conjunto de sus ciudadanos. La investigación y la construcción del conociminento en los distintos campos del saber es el motor de cualquier región, o país. La memoria que aquí se expone refleja por un lado el proceso de formación de una investigadora novel en el área educativa, en el marco del programa de doctorado "“Diversidad y Cambio en Educació: Políticas y Prácticas" del Departamento de Didáctica y Organización Educativa, de la Universidad de Barcelona. Específicamente el área de investigación es la docencia universitaria y la consturcción de su conocimiento. A través de ella se revelan los distintos engranajes que van conformando el proceso formación. Además, por otro lado se presentan los puntos más relevantes del proyecto y del proceso de investigación, denominado "La Docencia Universitaria: su saber pedagógico desde distintas áreas del conocimiento. Un estudio de casos con docentes de la Universidad de Barcelona.

La meva investigació s’articula entorn a l’obra del filòsof alemany Jürgen Habermas. L’aportació filosòfica més important de Habermas ha estat l’elaboració d’una racionalitat discursiva. Aquesta nova manera d’entendre la raó com a discurs ha tingut importants repercussions en l’àmbit de la filosofia i de les ciències socials en general. La primera part de la meva investigació analitza l’obra de Habermas, posant èmfasi en el procés de configuració d’aquesta racionalitat discursiva. Aquest primer període culmina amb la presentació de la tesina “Introducció al pensament i l’obra de Jürgen Habermas”. Com a resultat d’aquesta investigació, la meva recerca es centrarà, primer, en les nocions habermasianes de publicitat (Öffentlichkeit) i d’opinió pública (öffentliche Meinung) i, finalment, en l’anomenada democràcia deliberativa. Per tant, la recerca consta de dos part estretament relacionades: (1) un anàlisi del procés de configuració d’una raó discursiva en l’obra de Habermas; (2) una valoració crítica dels límits i les possibilitats de la democràcia deliberativa en les societats contemporànies. Aquesta darrera fase d’investigació –en la qual treballo actualment– s’endinsa en el debat dels “teòrics deliberatius”. Entre d’altres, aprofundeix en les repercussions ètiques i polítiques d’una relació problemàtica entre esfera pública i esfera privada. Desfer aquesta problemàtica passa per repensar l’exigència d’una racionalitat imparcial que, alhora, sigui Desfer aquesta problemàtica capaç de garantir la participació democràtica de la ciutadania en el context actual, on el pluralisme de valors ha esdevingut un dels fenòmens vertebradors de les societats contemporànies.
