BACKGROUND: Adalimumab (ADA) and certolizumab pegol (CZP) have demonstrated efficacy in Crohn's disease (CD) patients previously treated with infliximab (IFX). AIM: To assess the efficacy and tolerability of a third anti-TNF in CD after failure of and/or intolerance to two different anti-TNF antibodies. METHODS: Crohn's disease patients who received ADA or CZP after loss of response and/or intolerance to two anti-TNF agent were included in this retrospective study. Data were collected using a standardized questionnaire. Clinical response, duration, safety and reasons for discontinuation were assessed. RESULTS: Sixty-seven patients treated with CZP (n = 40) or ADA (n = 27) were included. A clinical response was observed in 41 (61%) at week 6 and 34 patients (51%) at week 20. The probability of remaining under treatment at 3 months, 6 months and 9 months was 68%, 60% and 45%, respectively. At the end of follow-up, the third anti-TNF had been stopped in 36 patients for intolerance (n = 13), or failure (n = 23). Two deaths were observed. CONCLUSIONS: The treatment with a third anti-TNF (CZP or ADA) agent of CD patients, who have experienced loss of response and/or intolerance to two anti-TNF antibodies, has favourable short-term and long-term efficacy. It is an option to be considered in patients with no other therapeutic options.

The Summer Olympic Games constitute the biggest concentration of human sports and activities in a particular place and time since 776 BCE, when the written history of the Olympic Games in Olympia began. Summer and Winter Olympic anti-doping laboratories, accredited by the International Olympic Committee in the past and the World Anti-Doping Agency in the present times, acquire worldwide interest to apply all new analytical advancements in the fight against doping in sports, hoping that this major human event will not become dirty by association with this negative phenomenon. This article summarizes the new analytical progresses, technologies and knowledge used by the Olympic laboratories, which for the vast majority of them are, eventually, incorporated into routine anti-doping analysis.

The biodistribution of simultaneous intra-arterial and intravenous injections of a radiolabelled anti-CEA MAb F(ab')2 fragment was studied in three patients with liver metastases from colorectal cancer. Identical MAb fragments, labelled with either 125I or 131I, were injected over a period of 30 min into the hepatic artery and into a peripheral vein. After 1 or 2 days, biodistribution was measured in the surgically removed metastases, normal tissue samples and blood. By tissue radioactivity counting, tumour uptake in the range 6.3-9.1% of injected dose per gram was found. Superimposable metastasis-to-blood and metastasis-to-normal liver ratios were obtained for both iodine isotopes in all three patients. The results indicate that the intra-arterial injection of MAb F(ab')2 fragments gives no measurable advantage over more convenient injections into a peripheral vein.

Lynch's (1980a) optimal-body-size model is designed to explain some major trends in cladoceran life histories; in particular the fact that large and littoral species seem to be bang-bang strategists (they grow first and the reproduce) whereas smaller planktonic species seem to be intermediate strategists (they grow and reproduce simultaneously). Predation is assumed to be an important selective pressure for these trends. Simocephalus vetulus (Müller) does not fit this pattern; being a littoral and relatively large species but an intermediate strategist. As shown by computer simulations, this species would reduce its per capita rate of increase by adopting the strategy predicted by the optimal-body-size model. Two aspects of the model are criticized: (1) the optimization criterion is shown to be incorrect and (2) the prediction of an intermediate strategy is not justified. Structural constraints are suggested to be responsible for the intermediate strategy of S.vetulus. Biotic interactions seem to have little effect on the observed life-history patterns of this species.

Rapid production of IL-4 by Leishmania homolog of mammalian RACK1 (LACK)-reactive CD4(+) T cells expressing the V beta 4-V alpha 8 TCR chains has been shown to drive aberrant Th2 cell development and susceptibility to Leishmania major in BALB/c mice. In contrast, mice from resistant strains fail to express this early IL-4 response. However, administration of either anti-IL-12 or -IFN-gamma at the initiation of infection allows the expression of this early IL-4 response in resistant mice. In this work we show that Leishmania homolog of mammalian RACK1-reactive CD4(+) T cells also expressing the V beta 4-V alpha 8 TCR chains are the source of the early IL-4 response to L. major in resistant mice given anti-IL-12 or -IFN-gamma Abs only at the onset of infection. Strikingly, these cells were found to be required for the reversal of the natural resistance of C57BL/6 mice following a single administration of anti-IL-12 or -IFN-gamma Abs. Together these results suggest that a deficiency in mechanisms capable of down-regulating the early IL-4 response to L. major contributes to the exquisite susceptibility of BALB/c mice to L. major.

The molecular chaperone Hsp90-dependent proteome represents a complex protein network of critical biological and medical relevance. Known to associate with proteins with a broad variety of functions termed clients, Hsp90 maintains key essential and oncogenic signalling pathways. Consequently, Hsp90 inhibitors are being tested as anti-cancer drugs. Using an integrated systematic approach to analyse the effects of Hsp90 inhibition in T-cells, we quantified differential changes in the Hsp90-dependent proteome, Hsp90 interactome, and a selection of the transcriptome. Kinetic behaviours in the Hsp90-dependent proteome were assessed using a novel pulse-chase strategy (Fierro-Monti et al., accompanying article), detecting effects on both protein stability and synthesis. Global and specific dynamic impacts, including proteostatic responses, are due to direct inhibition of Hsp90 as well as indirect effects. As a result, a decrease was detected in most proteins that changed their levels, including known Hsp90 clients. Most likely, consequences of the role of Hsp90 in gene expression determined a global reduction in net de novo protein synthesis. This decrease appeared to be greater in magnitude than a concomitantly observed global increase in protein decay rates. Several novel putative Hsp90 clients were validated, and interestingly, protein families with critical functions, particularly the Hsp90 family and cofactors themselves as well as protein kinases, displayed strongly increased decay rates due to Hsp90 inhibitor treatment. Remarkably, an upsurge in survival pathways, involving molecular chaperones and several oncoproteins, and decreased levels of some tumour suppressors, have implications for anti-cancer therapy with Hsp90 inhibitors. The diversity of global effects may represent a paradigm of mechanisms that are operating to shield cells from proteotoxic stress, by promoting pro-survival and anti-proliferative functions. La présente recherche analyse le concept mental d'identité personnelle et en propose une interprétation critique, qui se développe à partir des cas de troubles psychologiques se manifestant chez certains patients transplantés. Nous pensons que l'étude de ces cas offre la possibilité de repenser, et par là critiquer, certains présupposés qui conditionnent le débat philosophique contemporain sur l'identité personnelle de matrice essentiellement analytique. Nous proposons de ce concept mental une interprétation inusuelle, reposant sur une certaine compréhension de l'être (humain), que nous caractérisons par la formule «identité personnelle transductive ». Cette expression véhicule l'idée d'une «ontologie autre » ; autre que l'ontologie classique qui pense l'être comme déterminité, à l'instar d'un individu substantiel, stable et persistant. Nous nous éloignons ainsi de la conception de la déterminité telle qu'elle se développe depuis l'Antiquité grecque, parce que nous croyons que le changement ou le devenir réside dans la nature des choses. Nous avons ainsi essayé de présenter une interprétation de l'être différente, en empruntant à Gilbert Simondon la notion de « transductivité » -dans le sens d'« individuation en progrès »que cet auteur lui attribue. En comparant les témoignages des patients transplantés avec les cas imaginaires de transplantation de cerveau qui dominent les discussions philosophiques, nous avons montré la nécessité de refonder l'ontologie sur laquelle reposent les théories contemporaines de l'identité personnelle. Cette tentative répond à l'exigence de résoudre et tempérer les apories qui se font jour au sein des conceptions déterministes de la personne -soient-elles réductionnistes (à l'instar des partisans du critère de continuité psychologique et physique) ou non réductionnistes (à l'instar des conceptions simples ou Théories de l'ego basées sur une approche à la première personne, où la personne, comprise comme persistance substantielle d'un sujet, se fonde d'elle-même). Au coeur de notre démarche se trouve la notion d'« image du corps », véritable outil d'unification épistémologique et de renouvellement ontologique. Cette notion s'est révélée extrêmement utile non seulement pour comprendre et démêler les composantes qui interviennent dans le surgissement des troubles psychologiques lors des transplantations d'organes, ainsi que la variabilité des réactions individuelles, mais aussi, plus fondamentalement, pour rendre manifeste l'opération de «transduction perpétuée »qui est à la base de l'ontogenèse de l'être humain en tant que personne, donc du moi. La notion d'image du corps nous a donc permis de mettre en évidence ce que l'expérience de la transplantation d'organes bien comprise pourrait ou devrait apporter à la théorie de l'identité personnelle. C'est ainsi que, par approximation, s'est précisée l'idée d'une identité personnelle transductive, où le moi (la personne), étant interprété comme l'aspect subjectif d'une opération d'«individuation perpétuée », se donne sous le signe de la discordance.
