BACKGROUND: In a previous study we demonstrated that mild metabolic alkalosis resulting from standard bicarbonate haemodialysis induces hypotension. In this study, we have further investigated the changes in systemic haemodynamics induced by bicarbonate and calcium, using non-invasive procedures. METHODS: In a randomized controlled trial with a single-blind, crossover design, we sequentially changed the dialysate bicarbonate and calcium concentrations (between 26 and 35 mmol/l for bicarbonate and either 1.25 or 1.50 mmol/l for calcium). Twenty-one patients were enrolled for a total of 756 dialysis sessions. Systemic haemodynamics was evaluated using pulse wave analysers. Bioimpedance and BNP were used to compare the fluid status pattern. RESULTS: The haemodynamic parameters and the pre-dialysis BNP using either a high calcium or bicarbonate concentration were as follows: systolic blood pressure (+5.6 and -4.7 mmHg; P < 0.05 for both), stroke volume (+12.3 and +5.2 ml; P < 0.05 and ns), peripheral resistances (-190 and -171 dyne s cm(-5); P < 0.05 for both), central augmentation index (+1.1% and -2.9%; ns and P < 0.05) and BNP (-5 and -170 ng/l; ns and P < 0.05). The need of staff intervention was similar in all modalities. CONCLUSIONS: Both high bicarbonate and calcium concentrations in the dialysate improve the haemodynamic pattern during dialysis. Bicarbonate reduces arterial stiffness and ameliorates the heart tolerance for volume overload in the interdialytic phase, whereas calcium directly increases stroke volume. The slight hypotensive effect of alkalaemia should motivate a probative reduction of bicarbonate concentration in dialysis fluid for haemodynamic reasons, only in the event of failure of classical tools to prevent intradialytic hypotension.

The high complexity of cortical convolutions in humans is very challenging both for engineers to measure and compare it, and for biologists and physicians to understand it. In this paper, we propose a surface-based method for the quantification of cortical gyrification. Our method uses accurate 3-D cortical reconstruction and computes local measurements of gyrification at thousands of points over the whole cortical surface. The potential of our method to identify and localize precisely gyral abnormalities is illustrated by a clinical study on a group of children affected by 22q11 Deletion Syndrome, compared to control individuals.

O estudo da eficiência nutricional de plantas enxertadas de cafeeiro é importante para a seleção de combinações enxerto/porta-enxerto, visando ao desenvolvimento e produção máximos. O presente trabalho teve como objetivo avaliar, em cultivo hidropônico, a eficiência de absorção, translocação e uso de Ca, Mg e S por mudas enxertadas de Coffea arabica L., influenciada pelo porta-enxerto. O experimento foi realizado em casa de vegetação por um período de 170 dias, em vasos que continham areia, como substrato, e solução nutritiva circulante. Utilizaram-se, como enxerto, quatro genótipos de Coffea arabica L.: as variedades Catuaí Vermelho IAC 15 e Oeiras MG 6851 e as linhagens H 419-10-3-1-5 e H 514-5-5-3, e, como porta-enxerto, três genótipos de C. canephora Pierre et Froenher: Apoatã LC 2258, Conillon Muriaé-1, Robustão Capixaba (EMCAPA 8141) e um genótipo de Coffea arabica L.: Mundo Novo IAC 376-4, além da utilização de quatro pés-francos. Utilizou-se o delineamento experimental de blocos casualizados com quatro repetições. Os contrastes entre médias compararam as mudas de pé-franco com as associações enxerto/porta-enxerto. A eficiência de absorção, translocação e uso de Ca, Mg e S por mudas enxertadas de cafeeiro variou de acordo com a combinação enxerto/porta-enxerto. Somente a eficiência de translocação de Ca não foi alterada pela combinação enxerto/porta-enxerto. A linhagem H 514-5-5-3 foi beneficiada na eficiência de uso de Mg e produção de matéria seca pelos porta-enxertos Mundo Novo IAC 376-4 e Apoatã LC 2258, e na eficiência de uso de Ca e S apenas pelo Mundo Novo IAC 376-4.

IMPORTANCE: Owing to a considerable shift toward bioprosthesis implantation rather than mechanical valves, it is expected that patients will increasingly present with degenerated bioprostheses in the next few years. Transcatheter aortic valve-in-valve implantation is a less invasive approach for patients with structural valve deterioration; however, a comprehensive evaluation of survival after the procedure has not yet been performed. OBJECTIVE: To determine the survival of patients after transcatheter valve-in-valve implantation inside failed surgical bioprosthetic valves. DESIGN, SETTING, AND PARTICIPANTS: Correlates for survival were evaluated using a multinational valve-in-valve registry that included 459 patients with degenerated bioprosthetic valves undergoing valve-in-valve implantation between 2007 and May 2013 in 55 centers (mean age, 77.6 [SD, 9.8] years; 56% men; median Society of Thoracic Surgeons mortality prediction score, 9.8% [interquartile range, 7.7%-16%]). Surgical valves were classified as small (≤21 mm; 29.7%), intermediate (>21 and <25 mm; 39.3%), and large (≥25 mm; 31%). Implanted devices included both balloon- and self-expandable valves. MAIN OUTCOMES AND MEASURES: Survival, stroke, and New York Heart Association functional class. RESULTS: Modes of bioprosthesis failure were stenosis (n = 181 [39.4%]), regurgitation (n = 139 [30.3%]), and combined (n = 139 [30.3%]). The stenosis group had a higher percentage of small valves (37% vs 20.9% and 26.6% in the regurgitation and combined groups, respectively; P = .005). Within 1 month following valve-in-valve implantation, 35 (7.6%) patients died, 8 (1.7%) had major stroke, and 313 (92.6%) of surviving patients had good functional status (New York Heart Association class I/II). The overall 1-year Kaplan-Meier survival rate was 83.2% (95% CI, 80.8%-84.7%; 62 death events; 228 survivors). Patients in the stenosis group had worse 1-year survival (76.6%; 95% CI, 68.9%-83.1%; 34 deaths; 86 survivors) in comparison with the regurgitation group (91.2%; 95% CI, 85.7%-96.7%; 10 deaths; 76 survivors) and the combined group (83.9%; 95% CI, 76.8%-91%; 18 deaths; 66 survivors) (P = .01). Similarly, patients with small valves had worse 1-year survival (74.8% [95% CI, 66.2%-83.4%]; 27 deaths; 57 survivors) vs with intermediate-sized valves (81.8%; 95% CI, 75.3%-88.3%; 26 deaths; 92 survivors) and with large valves (93.3%; 95% CI, 85.7%-96.7%; 7 deaths; 73 survivors) (P = .001). Factors associated with mortality within 1 year included having small surgical bioprosthesis (≤21 mm; hazard ratio, 2.04; 95% CI, 1.14-3.67; P = .02) and baseline stenosis (vs regurgitation; hazard ratio, 3.07; 95% CI, 1.33-7.08; P = .008). CONCLUSIONS AND RELEVANCE: In this registry of patients who underwent transcatheter valve-in-valve implantation for degenerated bioprosthetic aortic valves, overall 1-year survival was 83.2%. Survival was lower among patients with small bioprostheses and those with predominant surgical valve stenosis.

In this open, 30-day trial, the pharmacokinetics, safety and tolerability of a combination therapy of risperidone (4 or 6 mg/day)and fluoxetine (20mg/day from day 6) were evaluated in 11 psychotic inpatients. CYP2D6 genotyping revealed that 3 and 8 patients were poor metabolizers (PMs) and extensive metabolizers (EMs) of debrisoquine, respectively. The mean (+/- SD) AUC of risperidone increased from 83.1 +/- 46.8 ng.h/ml and 398.3 +/- 33.2 ng.h/ml (monotherapy) to 345.1 +/- 158.0 ng.h/ml (p < 0.05) and 514.0 +/- 144.2 ng.h/ml (p < 0.001) when coadministered with fluoxetine in EMs and PMs, respectively. The AUC of the active moiety (risperidone plus 9-hydroxy-risperidone) increased from 470.0 +/- 170.0 ng.h/ml to 663.0 +/- 243.3 ng.h/ml (p < 0.05)and from 576.3 +/- 19.6 ng.h/ml to 788.0 +/- 89.1 ng.h/ml (ns) in EMs and PMs, respectively. In EMs, the AUC of 9-hydroxy-risperidone remained similar (monotherapy vs. combination therapy: 386.8 +/- 153.0 ng.h/ml vs. 317.7 +/- 125.2 ng.h/ml, ns),whereas it increased in PMs (178.3 +/- 23.5 ng.h/ml vs. 274.0 +/- 55.1 ng.h/ml (p < 0.05)). Ten of the 11 patients showed a clinical improvement (reduction of 20% or more in total PANSS score and 70% on the mean MADRS score compared to baseline). The severity and incidence of extrapyramidal symptoms and adverse events did not significantly increase when fluoxetine was added.
Solos do topo da Serra São José (Minas Gerais) e suas relações com o paleoclima no Sudeste do Brasil

A diversidade de ecossistemas do sudeste do Brasil nem sempre pode ser relacionada com fatores edáficos, geomorfológicos ou hidrológicos. Topos de elevações, onde os solos são caracterizados pela unicidade de material de origem, podem constituir ambiente especial para estudos de gênese de solos e datações de eventos cíclicos relacionados com a dinâmica do clima regional. Depois de um levantamento detalhado de solos no topo da Serra São José (Prados - Minas Gerais), dois perfis de solo (P1 e P2), originados de metarenitos da Formação Tiradentes e caracterizados por deposições sucessivas de camadas arenosas alternadas com camadas arenosas enriquecidas com matéria orgânica, foram estudados, com intuito de encontrar testemunhos de paleoambientes. O pequeno platô localiza-se a 1.350 m acima do nível de mar e 350 m acima do nível topográfico regional dominante. No P1, foram identificadas trinta e três camadas enriquecidas com matéria orgânica, alternadas com camadas de areia. Três camadas no P1 (20-30, 70-80 e 100-110 cm), com conteúdo de C orgânico respectivamente de 0.5, 7 e 1 dag kg-1, apresentam idades radiocarbônicas < 40, 180 ± 60 e 350 ± 80 anos AP, respectivamente, e taxas de deposição de 0,177 cm ano-1 entre 110 e 70 cm e de 0,357 cm ano-1 entre 70 e 20 cm de profundidade. No P2, as camadas enriquecidas com matéria orgânica são mais espessas (entre 10 e 130 mm) e apresentam descontinuidades abruptas. Situam-se entre 20-30, 80-90, 110-120 e 170-180 cm de profundidade, têm um conteúdo de C orgânico de 3, 2.5, 21 e 1.5 dag kg-1 e idade radiocarbônica de 3580 ± 80, 3750 ± 80, 21210 ± 180 e 24060 ± 130 anos AP, respectivamente. Suas taxas de deposição são de 0,352 cm ano-1, entre 20 e 80 cm; de 0,002 cm ano-1, entre 80 e 110 cm, e de 0,021 cm ano-1, entre 110 e 170 cm de profundidade. Nos dois perfis, a relação C/N aumenta com a profundidade e com a idade das camadas. Os teores de Ti e Zr, elementos de baixa mobilidade, são mais elevados nas camadas mais antigas dos perfis, enquanto o Cu e o Pb concentram-se nas camadas mais ricas em matéria orgânica. Um fragmento de planta de 5 cm de diâmetro e 62 cm de comprimento, situado na base de P2, foi datado de 32220 ± 290 anos AP e relacionado com o início da gênese deste perfil. Os solos do topo da Serra São José são formados a partir de metarenitos da Formação Tiradentes, sem aporte de materiais de outra litologia. A água pluvial é o principal fator que adiciona energia a este ambiente, relacionando os atributos dos solos com o clima. P1 é um solo holocênico (Neossolo Flúvico Psamítico típico), formado a partir de deposições episódicas de areia, alternadas com material enriquecido com matéria orgânica. A formação de P2 (Paleossolo) iniciou-se no Pleistoceno, prolongou-se até o Holoceno e a morfologia de suas camadas enterradas de turfa relaciona-se com a oscilação do espelho d'água de uma lagoa, decorrente de fases mais secas e mais úmidas do clima. As idades radiocarbônicas encontradas estão relacionadas com alternâncias climáticas pleistocênicas e holocênicas em P2 e holocênicas em P1. O perfil P2 está situado em um local propício para realização de estudos palinológicos, com intuito de identificar ecótipos que ocuparam a área a partir do Pleistoceno tardio, relacionando-os com os paleoclimas.

O objetivo deste trabalho foi avaliar o aporte de nutrientes pela deposição de serapilheira em uma área degradada, que sofreu empréstimo de solo, após dez anos da sua revegetação com as leguminosas arbóreas: Mimosa caesalpiniifolia (sabiá), Acacia auriculiformis (acácia) e Gliricidia sepium (gliricídia); e outra área vizinha, um fragmento da mata Atlântica em crescimento secundário (capoeira). O trabalho foi realizado no campo experimental da Embrapa-Agrobiologia, Km 47, Seropédica, Rio de Janeiro. Na amostragem, utilizaram-se coletores circulares do material formador da serapilheira com área de 0,25 m², determinando-se os teores de nutrientes (N, P, K, Ca e Mg) e polifenóis. A quantidade de serapilheira depositada foi influenciada pelas espécies de leguminosas utilizadas na revegetação, variando de 5,7 Mg ha -1 ano-1 de matéria seca (MS), onde predominava gliricídia, até 11,2 Mg ha-1 ano-1 , na faixa formada pela sabiá com contribuição do material de acácia. A deposição na capoeira foi de 9,2 Mg ha-1 ano-1 de MS. O material de gliricídia foi o mais rico em nutrientes (N, P, Ca e Mg) e o que apresentou os menores teores de polifenóis; qualitativamente formou a serapilheira mais favorável ao processo de decomposição. O aporte de nutrientes correlacionou-se com a quantidade de serapilheira depositada. Na revegetação, o aporte anual de nutrientes, em kg ha-1 ano-1 , variou: para o N, de 130 a 170; para o P, de 4,9 a 7,9; para K, de 24 a 31; para o Ca, de 150 a 190, e para o Mg, de 28,6 a 40,0. Estes valores foram similares ou superiores aos observados para a capoeira, que foram para o N, 140, para o P, 4,9, para o Ca, 110, e para o Mg, 31,7, exceto para o K, 63. A revegetação com leguminosas, em áreas degradadas, adiciona, relativamente, em pouco tempo, grande quantidade de matéria orgânica e N por meio da produção de serapilheira, favorecendo a ciclagem de nutrientes e o processo de recuperação.

Purpose/Objective(s): Mammary adenoid cystic carcinoma (ACC) is a rare breast cancer variant. It accounts for less than 0.1% of all invasive breast malignancies. Typically, it presents as a small breast lump with a low propensity to metastasize to regional lymph nodes or distant sites. The aim of this retrospective multicenter Rare Cancer Network study is to assess prognostic factors and patterns of failure in ACC, as well as the role of radiation therapy (RT) in this rare disease. Materials/Methods: Between January 1980 and December 2007, 61 women with breast ACC were included in this study. Median age was 59 years (range, 28-94 years). The majority of the patients had good performance status (49 patients with WHO 0, 12 patients with WHO 1), and 70% of the patients (n = 42) were premenopausal. Surgery consisted of tumorectomy in 35 patients, mastectomy in 20, or quadrantectomy in 6. Median tumor size was 20 mm (range, 6-170 mm). Surgical margins were clear in 50 (82%) patients. Axillary dissection (n = 41) or sentinel node assessment (n = 10) was realized in the majority of the patients. There were 53 (87%) pN0 and 8 pNx (13%) patients. Estrogen (ER) and progesterone receptor (PR) was negative in 43 (71%) and 42 (69%) patients, respectively. In 16 patients (26%), the receptor status was unknown. Adjuvant chemotherapy or hormonotherapy was administered in 8 (13%) and 7 (12%) patients, respectively. Postoperative RT with a median total dose of 50 Gy (1.8-2.0 Gy/fraction; range, 44-70 Gy) was given in 40 patients. Results: With a median follow-up of 79 months (range, 6-285 months), 5-year overall and disease-free survival (DFS) rates were 94% (95% confidence interval [CI]: 88-100%) and 82% (95% CI: 71-93%), respectively. Five-year locoregional control rate was 95% (95% CI: 89-100%). There were only 4 patients with local relapse who were all salvaged successfully, and 4 other patients developed distant metastases. According to the Common Terminology Criteria for Adverse Events v3.0, late toxicity consisted of grade 2-3 cutaneous fibrosis in 4 (10%) patients, grade 1-2 edema in 2 (5%), and grade 3 lung fibrosis in 2 (5%). In univariate analyses, the outcome was influenced neither by the type of surgery nor the use of postoperative RT. However, positive receptor status had a negative influence on the outcome. Multivariate analysis (Cox model) revealed that negative ER (p = 0.006) or PR (p = 0.04) status was associated with improved DFS. Conclusions: ACC of the breast is a relatively indolent disease with excellent local control and survival. The prognosis of patients with ACC is much better than that for patients with other breast cancers, especially those who are ER and PR negative. The role of postoperative RT is not clear. More aggressive treatments may be warranted for patients with positive receptor status.

Comparativamente ao pé-franco, a combinação enxerto/porta-enxerto altera os complexos mecanismos de "feedback" entre parte aérea e raízes, afetando de maneira positiva ou negativa a eficiência nutricional da planta. Este trabalho teve como objetivo avaliar, em cultivo hidropônico, a eficiência da absorção, translocação e utilização de Zn, Cu e Mn por mudas de Coffea arabica L., de acordo com o porta-enxerto utilizado. O experimento foi realizado em casa de vegetação, por um período de 170 dias, em vasos que continham areia como substrato, recebendo solução nutritiva circulante. Utilizaram-se, como enxerto, quatro genótipos de C. arabica: os cultivares Catuaí Vermelho IAC 15 e Oeiras MG 6851 e os híbridos 'H 419-10-3-1-5' e 'H 514-5-5-3' , e, como porta-enxerto, quatro genótipos, sendo três de Coffea canephora Pierre ex Froenher: Apoatã LC 2258, Conilon Muriaé-1 e RC EMCAPA 8141 (recombinação entre clones da variedade Robustão Capixaba - EMCAPA 8141) e uma linhagem de Coffea arabica L.: Mundo Novo IAC 376-4, além de quatro pés-francos. O delineamento experimental utilizado foi em blocos casualizados com 20 tratamentos, quatro repetições e uma planta por parcela. A eficiência nutricional das mudas quanto ao Zn, Cu e Mn variou de acordo com a combinação enxerto/porta-enxerto. A progênie 'H 514-5-5-3' foi mais eficiente quanto à utilização de Zn, Cu e Mn e produção de matéria seca, quando combinada com os porta-enxertos Apoatã LC 2258 e Mundo Novo IAC 376-4. O Catuaí Vermelho IAC 15 foi mais eficiente na utilização de Cu e Mn quando combinado com Apoatã LC 2258.

The seven cytologically known species of the Sorex araneus complex each show a different karyotype. The three European species are genetically and morphologically very close, and mostly allopatric, thus having similar ecological requirements. It is suggested that chromosomic changes occasion a stasipatric speciation, the formation of chromosomic races in S. araneus being an illustration of this process. Neither small demes nor high inbreeding are likely in Sorex; thus the Robertsonian changes should spread out by meiotic drive alone, and the hybrids for a change should be almost fully fertile. Chromosomic speciation occurs when two incompatible metacentrics meet together. This model explains the separation between S. araneus and S. coronatus, but geographical isolation must have occured in the case of S. granarius, which keeps a primitive karyotype.

Concentrations of the enantiomers of unconjugated and of total (unconjugated plus conjugated) mianserin, desmethylmianserin and 8-hydroxymianserin were measured in 12 patients before and after the introduction of carbamazepine. The dose of mianserin was 60 mg/d, carbamazepine was coadministered at 400 mg/d for 4 weeks, and blood samples were taken at weekly intervals after the introduction of carbamazepine. Each week, carbamazepine significantly decreased plasma concentrations of unconjugated and total (S)-mianserin (the more potent enantiomer) and of unconjugated and total (R)-mianserin. On average, plasma concentrations of unconjugated and total (S)-mianserin and of unconjugated and total (R)-mianserin were 55%, 56%, 66%, and 55%, respectively, of the corresponding values before introduction of carbamazepine. These results strongly suggest the involvement of CYP3A4, the major CYP enzyme induced by carbamazepine, in the metabolism of both enantiomers of mianserin. A strong decrease in the concentrations of (S)-8-hydroxymianserin was also measured (on average, the concentrations were 69% of the corresponding values before carbamazepine introduction). Conversely, plasma concentrations of unconjugated and of total (S)-desmethylmianserin, (R)-desmethylmianserin, and (R)-8-hydroxymianserin were only slightly modified by carbamazepine. From a clinical point of view, as a therapeutic window for (S)-mianserin has been recently suggested, the dose of racemic mianserin for a patient whose (S)-mianserin concentrations have been stabilized within this therapeutic window would need to be approximately doubled if carbamazepine, at 400 mg/d, is introduced as a comedication.

Hepatitis C virus (HCV) is an important human pathogen, persistently infecting more than 170 million individuals worldwide. Studies of the HCV life cycle have become possible with the development of cell culture systems supporting the replication of viral RNA and the production of infectious virus. However, the exact functions of individual proteins, especially of nonstructural protein 4B (NS4B), remain poorly understood. NS4B triggers the formation of specific, vesicular membrane rearrangements, referred to as membranous webs, which have been reported to represent sites of HCV RNA replication. However, the mechanism of vesicle induction is not known. In this study, a panel of 15 mutants carrying substitutions in the highly conserved NS4B C-terminal domain was generated. Five mutations had only a minor effect on replication, but two of them enhanced assembly and release of infectious virus. Ten mutants were replication defective and used for selection of pseudoreversions. Most of the pseudoreversions also localized to the highly conserved NS4B C-terminal domain and were found to restore replication competence upon insertion into the corresponding primary mutant. Importantly, pseudoreversions restoring replication competence also restored heterotypic NS4B self-interaction, which was disrupted by the primary mutation. Finally, electron microscopy analyses of membrane alterations induced by NS4B mutants revealed striking morphological abnormalities, which were restored to wild-type morphology by the corresponding pseudoreversion. These findings demonstrate the important role of the C-terminal domain in NS4B self-interaction and the formation of functional HCV replication complexes.

The trabecular bone score (TBS) is a new parameter that is determined from gray-level analysis of dual-energy X-ray absorptiometry (DXA) images. It relies on the mean thickness and volume fraction of trabecular bone microarchitecture. This was a preliminary case-control study to evaluate the potential diagnostic value of TBS as a complement to bone mineral density (BMD), by comparing postmenopausal women with and without fractures. The sample consisted of 45 women with osteoporotic fractures (5 hip fractures, 20 vertebral fractures, and 20 other types of fracture) and 155 women without a fracture. Stratification was performed, taking into account each type of fracture (except hip), and women with and without fractures were matched for age and spine BMD. BMD and TBS were measured at the total spine. TBS measured at the total spine revealed a significant difference between the fracture and age- and spine BMD-matched nonfracture group, when considering all types of fractures and vertebral fractures. In these cases, the diagnostic value of the combination of BMD and TBS likely will be higher compared with that of BMD alone. TBS, as evaluated from standard DXA scans directly, potentially complements BMD in the detection of osteoporotic fractures. Prospective studies are necessary to fully evaluate the potential role of TBS as a complementary risk factor for fracture.
