Background: As imatinib pharmacokinetics are highly variable, plasma levels differ largely between patients under the same dosage. Retrospective studies in chronic myeloid leukemia (CML) patients showed significant correlations between low levels and suboptimal response, and between high levels and poor tolerability. Monitoring of plasma levels is thus increasingly advised, targeting trough concentrations of 1000 μg/L and above. Objectives: Our study was launched to assess the clinical usefulness of systematic imatinib TDM in CML patients. The present preliminary evaluation questions the appropriateness of dosage adjustment following plasma level measurement to reach the recommended trough level, while allowing an interval of 4-24 h after last drug intake for blood sampling. Methods: Initial blood samples from the first 9 patients in the intervention arm were obtained 4-25 h after last dose. Trough levels in 7 patients were predicted to be significantly away from the target (6 <750 μg/L, and 1 >1500 μg/L with poor tolerance), based on a Bayesian approach using a population pharmacokinetic model. Individual dosage adjustments were taken up in 5 patients, who had a control measurement 1-4 weeks after dosage change. Predicted trough levels were confronted to anterior model-based extrapolations. Results: Before dosage adjustment, observed concentrations extrapolated at trough ranged from 359 to 1832 μg/L (median 710; mean 804, CV 53%) in the 9 patients. After dosage adjustment they were expected to target between 720 and 1090 μg/L (median 878; mean 872, CV 13%). Observed levels of the 5 recheck measurements extrapolated at trough actually ranged from 710 to 1069 μg/L (median 1015; mean 950, CV 16%) and had absolute differences of 21 to 241 μg/L to the model-based predictions (median 175; mean 157, CV 52%). Differences between observed and predicted trough levels were larger when intervals between last drug intake and sampling were very short (~4 h). Conclusion: These preliminary results suggest that TDM of imatinib using a Bayesian interpretation is able to bring trough levels closer to 1000 μg/L (with CV decreasing from 53% to 16%). While this may simplify blood collection in daily practice, as samples do not have to be drawn exactly at trough, the largest possible interval to last drug intake yet remains preferable. This encourages the evaluation of the clinical benefit of a routine TDM intervention in CML patients, which the randomized Swiss I-COME study aims to.

Euroopan Unionin hyväksymän IAS-asetuksen mukaan kaikkien julkisesti noteerattujen yritysten on siirryttävä käyttämään tilinpäätöksen laatimisessa IFRS-standardeja viimeistään vuodesta 2005 lähtien. Tämän tutkimuksen tavoitteena on selvittää onko IFRS-standardeihin siirtymisellä ollut vaikutusta osakkeiden hintatasoon Suomen Pörssissä noteerattujen metallitoimialan yritysten kohdalla. IFRS-standardien vaikutuksia osakkeiden epänormaaleihin tuottoihin tutkitaan ns. tapahtumatutkimuksen avulla. Tutkittava tapahtuma eli ns. event on hetki, jolloin yritys on ensimmäisen kerran julkaissut IFRSstandardien mukaista tilinpäätösinformaatiota. Valittujen tunnuslukujen (P/E-, P/B-, EV/EBIT- ja Grahamin tunnuslukujen) muutosten ja tapahtuman jälkeisten ylituottojen avulla puolestaan tutkitaan ovatko markkinat sopeutuneet muuttuneeseen tilinpäätösinformaatioon. Saatujen tutkimustulosten perusteella voidaantodeta, että etenkin liikearvopoistojen päättyminen sekä ns. eläkestandardi ovat vaikuttaneet yritysten tuloslaskelmiin. Taseen puolella suurimmat muutokset ovat aiheuttaneet rahoitusleasingiä ja tuotekehityksen aktivointia käsittelevät standardit. Tutkimuksen mukaan IFRS:n tuomat tase- ja tuloslaskelma muutokset ovat myös heijastuneet osakkeiden markkinahintoihin.

Introduction: « Osteo-Mobile Vaud » is a mobile osteoporosis (OP) screening program. The women > 60 years living in the region Vaud will be offered OP screening with new equipment installed in a bus. The main goal is to evaluate the fracture risk with the combination of clinical risk factors (CRF) and informations extracted by a single DXA: bone mineral density (BMD), vertebral fracture assessment (VFA), and micro-architecture (MA) evaluation. MA is yet evaluable in daily practice by the Trabecular Bone Score (TBS) measure. TBS is a novel grey-level texture measurement reflecting bone MA based on the use of experimental variograms of 2D projection images. TBS is very simple to obtain, by reanalyzing a lumbar DXA-scan. TBS has proven to have diagnosis and prognosis value, partially independent of CRF and BMD. A 55-years follow- up is planned. Method: The Osteo-Mobile Vaud cohort (1500 women, > 60 years, living in the region Vaud) started in July 2010. CRF for OP, lumbar spine and hip BMD, VFA by DXA and MA evaluation by TBS are recorded. Preliminary results are reported. Results: In July 31th, we evaluated 510 women: mean age 67 years, BMI 26 kg/m². 72 women had one or more fragility fractures, 39 had vertebral fracture (VFx) grade 2/3. TBS decreases with age (-0.005 / year, p<0.001), and with BMI (-0.011 per kg/m², p<0.001). Correlation between BMD and site matched TBS is low (r=0.4, p<0.001). For the lowest T-score BMD, odds ratio (OR, 95% CI) for VFx grade 2/3 and clinical OP Fx are 1.8 (1.1-2.9) and 2.3 (1.5-3.4). For TBS, age-, BMI- and BMD adjusted ORs (per SD decrease) for VFx grade 2/3 and clinical OP Fx are 1.9 (1.2-3.0) and 1.8 (1.2-2.7). The TBS added value was independent of lumbar spine BMD or the lowest T-score (femoral neck, total hip or lumbar spine). Conclusion: As in the already published studies, these preliminary results confirm the partial independence between BMD and TBS. More importantly, a combination of TBS and BMD may increase significantly the identification of women with prevalent OP Fx. For the first time we are able to have complementary information about fracture (VFA), density (BMD), and micro-architecture (TBS) from a simple, low ionizing radiation and cheap device: DXA. The value of such informations in a screening program will be evaluated.

As cultivares de macieira exigem diferentes requerimentos em frio, ou seja, o total de horas abaixo de um limite de temperatura do ar, porém são poucas as informações sobre quais temperaturas são mais eficientes para superar a dormência. As cultivares de macieira Condesa, Baronesa, Daiane, Imperatriz, Fuji e Gala foram estudadas quanto à quantidade de frio e as temperaturas do ar para a indução da brotação. O delineamento experimental foi o inteiramente casualizado, no esquema fatorial, com seis cultivares, cinco níveis de unidades de frio ( 300; 600; 900; 1200 e 1500 UF) e três temperaturas do ar ( 5; 10 e 15ºC). O tempo médio para brotação foi menor quando as cultivares foram submetidas a 1.500 unidades de frio, independentemente da temperatura. A temperatura efetiva para acumular frio varia com a cultivar, podendo chegar até 15ºC para cultivares de menor exigência em frio.
