O objetivo deste artigo é abordar a questão das vivências na perspectiva de L.S. Vigotski, tratando de suas fontes epistemológicas, de suas raízes na língua russa e de seus sentidos específicos, priorizando "A Tragédia do Hamlet, Príncipe da Dinamarca" (1916) e os textos pedológicos dos anos 1930, com destaque para "A questão do meio na pedologia". Destacam-se as transformações das ideias do autor sobre vivências e sua contribuição metodológica à análise da relação indivíduo-meio no desenvolvimento humano, articulada aos conceitos de consciência e sistema.

Este artigo visa explicitar as concepções freudiana e husserliana de “vivência” (Erlebnis). Por “vivência” entende-se genericamente um tipo fundamental de experiência do mundo. Esse tema, ainda que não diretamente explorado por Freud, tornou-se necessário em sua teoria desde a descoberta da etiologia da histeria no início dos anos 1890 à luz do método catártico: a da vivência traumática. Por outro lado, Husserl, a partir do problema filosófico de garantir a possibilidade do conhecimento universal e necessário, se viu obrigado a combater o naturalismo das ideias, em 1900, e o naturalismo da consciência, em 1913, em ambos os casos partindo de uma análise das vivências (intencionais). Mostrarei que a abordagem freudiana (natural-científica) visa explicar metapsicologicamente a vivência, ao passo que a abordagem husserliana pretende descrever a estrutura da vivência (intencional). Por fim, apontarei para algumas das grandes diferenças entre ambas as abordagens desse tema.

O presente estudo hipotetiza que as diferentes matrizes clínicas que Freud encontrava em sua prática, e que lhe possibilitavam acréscimos teóricos, direcionaram seus distintos olhares sobre a cultura, fazendo-o privilegiar alguns elementos éticos em detrimento de outros. Desse modo, indicaremos: (1) como a histeria gerou a questão do conflito entre sexualidade e moral na civilização; (2) como a neurose obsessiva possibilitou a entrada dos temas da agressividade e do ódio como entraves contra os quais a cultura se esforça por lutar, assim como a presença marcante no psiquismo da consciência moral e do sentimento de culpa; (3) por fim, como as ditas afecções narcísicas trouxeram a Freud o papel do egoísmo e da destrutividade como inimigos da cultura. Nesse percurso nos aproximaremos das questões ligadas à problematização ética na "psicologia" freudiana e, a partir daí, do destaque que terá a dimensão moral na concepção freudiana do sujeito.

Caracterizar a limitação funcional, de atividade, consciência de risco, e restrição à participação social em pessoas atingidas pela hanseníase no pós-alta. Estudo seccional-descritivo com 69 residentes em Sobral, Ceará, com alta entre 2003 a 2005. Foram realizados exame físico dermato-neurológico, avaliação demográfica, de limitação funcional-atividade-consciência de risco e de restrição à participação social. Vinte (28,9%) apresentaram escores SALSA 19 e 20 e escore EHF zero. O maior escore EHF foi alcançado por dois participantes, com 25 e 28 na escala SALSA. Na escala de participação 37 (53,6%) não apresentaram restrição e tinham escore EHF zero. Dois (2,9%) com escore EHF zero tinham leve restrição e 1 (1,5%), grande restrição. Reafirma-se a potencialidade destas ferramentas para a atenção integral aos portadores.

This article aims at presenting a reading of Vicente Huidobro's poem Altazor (1931,) assuming the epic genre postulations and the conditions of its development in the first half of the twentieth century. Important works were produced in that period, setting in motion a deployment of the epic genre as refashioned by the avant-garde movements and contesting the metaphysical formulations that had considered it impossible as a discursive space in modern times. Altazor is situated in this movement as a self-conscience of issues of language and its objective capacities. The work deals with the possible modes of enunciation of a great poet, Altazor, who lacks his former serenity of old and looks for a language able to transcend his mother tongue, because he does not accept its origin. His political revolution is performed on language, the artifice through which history is told.

Over half a million heroin misusers receive oral methadone maintenance treatment world-wide1 but the maintenance prescription of injectable opioid drugs, like heroin, remains controversial. In 1992 Switzerland began a large scale evaluation of heroin and other injectable opiate prescribing that eventually involved 1035 misusers. 2 3 The results of the evaluation have recently been reported.4 These show that it was feasible to provide heroin by intravenous injection at a clinic, up to three times a day, for seven days a week. This was done while maintaining good drug control, good order, client safety, and staff morale. Patients were stabilised on 500 to 600 mg heroin daily without evidence of increasing tolerance. Retention in treatment was 89% at six months and 69% at 18 months.4 The self reported use of non-prescribed heroin fell signifianctly, but other drug use was minimally affected. Background: The Royal Australian and New Zealand College of Psychiatrists is co-ordinating the development of clinical practice guidelines (CPGs) in psychiatry, funded under the National Mental Health Strategy (Australia) and the New Zealand Health Funding Authority. This paper presents CPGs for schizophrenia and related disorders. Over the past decade schizophrenia has become more treatable than ever before. A new generation of drug therapies, a renaissance of psychological and psychosocial interventions and a first generation of reform within the specialist mental health system have combined to create an evidence-based climate of realistic optimism. Progressive neuroscientific advances hold out the strong possibility of more definitive biological treatments in the near future. However, this improved potential for better outcomes and quality of life for people with schizophrenia has not been translated into reality in Australia. The efficacy-effectiveness gap is wider for schizophrenia than any other serious medical disorder. Therapeutic nihilism, under-resourcing of services and a stalling of the service reform process, poor morale within specialist mental health services, a lack of broad-based recovery and life support programs, and a climate of tenacious stigma and consequent lack of concern for people with schizophrenia are the contributory causes for this failure to effectively treat. These guidelines therefore tackle only one element in the endeavour to reduce the impact of schizophrenia. They distil the current evidence-base and make recommendations based on the best available knowledge. Method: A comprehensive literature review (1990-2003) was conducted, including all Cochrane schizophrenia reviews and all relevant meta-analyses, and a number of recent international clinical practice guidelines were consulted. A series of drafts were refined by the expert committee and enhanced through a bi-national consultation process. Treatment recommendations: This guideline provides evidence-based recommendations for the management of schizophrenia by treatment type and by phase of illness. The essential features of the guidelines are: (i) Early detection and comprehensive treatment of first episode cases is a priority since the psychosocial and possibly the biological impact of illness can be minimized and outcome improved. An optimistic attitude on the part of health professionals is an essential ingredient from the outset and across all phases of illness. (ii) Comprehensive and sustained intervention should be assured during the initial 3-5 years following diagnosis since course of illness is strongly influenced by what occurs in this 'critical period'. Patients should not have to 'prove chronicity' before they gain consistent access and tenure to specialist mental health services. (iii) Antipsychotic medication is the cornerstone of treatment. These medicines have improved in quality and tolerability, yet should be used cautiously and in a more targeted manner than in the past. The treatment of choice for most patients is now the novel antipsychotic medications because of their superior tolerability and, in particular, the reduced risk of tardive dyskinesia. This is particularly so for the first episode patient where, due to superior tolerability, novel agents are the first, second and third line choice. These novel agents are nevertheless associated with potentially serious medium to long-term side-effects of their own for which patients must be carefully monitored. Conventional antipsychotic medications in low dosage may still have a role in a small proportion of patients, where there has been full remission and good tolerability; however, the indications are shrinking progressively. These principles are now accepted in most developed countries. (vi) Clozapine should be used early in the course, as soon as treatment resistance to at least two antipsychotics has been demonstrated. This usually means incomplete remission of positive symptomatology, but clozapine may also be considered where there are pervasive negative symptoms or significant or persistent suicidal risk is present. (v) Comprehensive psychosocial interventions should be routinely available to all patients and their families, and provided by appropriately trained mental health professionals with time to devote to the task. This includes family interventions, cognitive-behaviour therapy, vocational rehabilitation and other forms of therapy, especially for comorbid conditions, such as substance abuse, depression and anxiety. (vi) The social and cultural environment of people with schizophrenia is an essential arena for intervention. Adequate shelter, financial security, access to meaningful social roles and availability of social support are essential components of recovery and quality of life. (vii) Interventions should be carefully tailored to phase and stage of illness, and to gender and cultural background. (viii) Genuine involvement of consumers and relatives in service development and provision should be standard. (ix) Maintenance of good physical health and prevention and early treatment of serious medical illness has been seriously neglected in the management of schizophrenia, and results in premature death and widespread morbidity. Quality of medical care for people with schizophrenia should be equivalent to the general community standard. (x) General practitioners (GPs)s should always be closely involved in the care of people with schizophrenia. However, this should be truly shared care, and sole care by a GP with minimal or no special Optimal treatment of schizophrenia requires a multidisciplinary team approach with a consultant psychiatrist centrally involved.

The main objective of this study was to see if older people could maintain their quality of life and independence after their homes had been modified and they were using community services as recommended by an occupational therapist. There were 167 study participants aged 69 to 94 years from the Northern Sydney Area, After being assessed at home by an occupational therapist, 105 were randomly allocated to one of two groups, to either have or not have the occupational therapist's recommendations carried out, They were assessed again after six months, A third group did not require any intervention, This group was followed up by telephone and postal questionnaire at six months. The main outcome measures used were the Sickness Impact Profile, the Philadelphia Geriatric Center Morale Scale, the Life Satisfaction Index, assessment of Activities of Daily Living, the Health Assessment Questionnaire and change in residence. After six months there were no difference in outcomes among the three groups. Most study participants remained at a satisfactory level on each measure. Three people had died, One had moved to hostel care and one had moved to a nursing home. A further 14 from the group having no intervention had withdrawn from the study, A secondary objective of this study was to indicate the responsiveness of these outcome measures to change in the short term (over six months) in an elderly population. Twelve-month assessments are in progress and may indicate what to expect from these outcome measures in the medium term.

A presente pesquisa, desenvolvida na linha Cultura, Currículo e Formação de Educadores, pelo Programa de Pós-Graduação da Universidade Federal do Espírito Santo, tem como objetivo investigar práticas docentes consideradas pelas professoras do 4º e 5º anos do EF como promotoras de leitura e escrita em escolas do Sistema Municipal de Educação Vitória(ES). Apoiamo-nos, para isso, na base teórica da perspectiva histórico-cultural a partir de Bakhtin (2003, 2010); Freitas (2007, 2012) e Fichtner (2012). Levamos em consideração os conceitos de sujeito, linguagem e texto desenvolvidos por Bakhtin (2003, 2010); de educação e dialogicidade em Freire (1967, 1987, 1997, 2010); e de parceria em Foerste (2005); dentre outros, no sentido de buscar respostas para as perguntas: que práticas docentes estão sendo consideradas pelas professoras desses dois anos de escolarização como promotoras de leitura e escrita? Que políticas têm promovido tais práticas? Quem são os beneficiados/prejudicados por tais formas de ensinar? A pesquisa se deu por meio de um estudo de caso realizado durante o ano letivo de 2013 com docentes que atuam junto às turmas em questão, em duas escolas do EF. Utilizamos como procedimentos para a produção de dados a observação participante, o questionário, a entrevista, o acompanhamento de atividades desenvolvidas nos cadernos de alunos e a análise documental. A análise dos dados foi organizada a partir das categorias sujeito, linguagem e texto. Os resultados evidenciam que, conforme a concepção bakhtiniana de linguagem e a freireana de educação, as práticas docentes observadas ainda não sinalizam a promoção da leitura e da escrita junto aos alunos do 4º e 5º anos EF, sendo necessário para isso que se repensem as políticas públicas, em especial as voltadas para a formação de professores, com o intuito de beneficiar os estudantes oferecendo-lhes uma formação que lhes permita ter a consciência de que o centro de gravidade da linguagem não reside nas normas, mas na significação que essa forma adquire no contexto, na interação com outros sujeitos.
