The recent approval of crizotinib for the treatment of anaplastic lymphoma kinase (ALK)-rearranged advanced non-small cell lung cancer (NSCLC) in the US and other countries has provoked intense interest in ALK rearrangements as oncogenic drivers, and promises to revolutionise the way in which NSCLC is diagnosed and treated. Here, we review clinical data to date for the use of crizotinib to treat patients with advanced, ALK-positive NSCLC and consider issues surrounding the detection of ALK-positivity including the use of fluorescence in situ hybridisation and the other potential techniques available, and their suitability for ALK screening. We also discuss the emergence of resistance to crizotinib therapy and the range of other ALK inhibitors currently in development.

In diabetes mellitus, it is expected to see a common, mainly sensitive, distal symmetrical polyneuropathy (DPN) involving a large proportion of diabetic patients according to known risk factors. Several other diabetic peripheral neuropathies are recognized, such as dysautonomia and multifocal neuropathies including lumbosacral radiculoplexus and oculomotor palsies. In this review, general aspects of diabetic neuropathies are examined, and it is discussed why and how the general practionner has to perform a yearly examination. At the present time, some consensuses emerge to ask help from the specialist when faced to other forms of peripheral neuropathies than distal symmetrical DPN.

We present a silicon chip-based approach for the enhanced sensitivity detection of surface-immobilized fluorescent molecules. Green fluorescent protein (GFP) is bound to the silicon substrate by a disuccinimidyl terephtalate-aminosilane immobilization procedure. The immobilized organic layers are characterized by surface analysis techniques, like ellipsometry, atomic force microscopy (AFM) and X-ray induced photoelectron spectroscopy. We obtain a 20-fold enhancement of the fluorescent signal, using constructive interference effects in a fused silica dielectric layer, deposited before immobilization onto the silicon. Our method opens perspectives to increase by an order of magnitude the fluorescent response of surface immobilized DNA- or protein-based layers for a variety of biosensor applications.

γ-Hydroxybutyric acid (GHB) is an endogenous short-chain fatty acid popular as a recreational drug due to sedative and euphoric effects, but also often implicated in drug-facilitated sexual assaults owing to disinhibition and amnesic properties. Whilst discrimination between endogenous and exogenous GHB as required in intoxication cases may be achieved by the determination of the carbon isotope content, such information has not yet been exploited to answer source inference questions of forensic investigation and intelligence interests. However, potential isotopic fractionation effects occurring through the whole metabolism of GHB may be a major concern in this regard. Thus, urine specimens from six healthy male volunteers who ingested prescription GHB sodium salt, marketed as Xyrem(®), were analysed by means of gas chromatography/combustion/isotope ratio mass spectrometry to assess this particular topic. A very narrow range of δ(13)C values, spreading from -24.810/00 to -25.060/00, was observed, whilst mean δ(13)C value of Xyrem(®) corresponded to -24.990/00. Since urine samples and prescription drug could not be distinguished by means of statistical analysis, carbon isotopic effects and subsequent influence on δ(13)C values through GHB metabolism as a whole could be ruled out. Thus, a link between GHB as a raw matrix and found in a biological fluid may be established, bringing relevant information regarding source inference evaluation. Therefore, this study supports a diversified scope of exploitation for stable isotopes characterized in biological matrices from investigations on intoxication cases to drug intelligence programmes.

Esta monografia foi elaborada com o intuito do aprofundamento, aperfeiçoamento e o conhecimento académico, pesquisando um tema que contribuísse, de alguma maneira, para a melhoria da área em questão. Também teve a intenção de contribuir na análise da inter-relação entre a auditoria interna e externa numa perspetiva de complementaridade. Procura-se, na presente monografia, proporcionar uma reflexão sobre a relação existente entre a auditoria interna e a auditoria externa. Para tal, realizaram-se pesquisas bibliográficas sobre os principais conceitos, âmbito, métodos, objetivos, funções e procedimentos, normas pertinentes à auditoria interna e externa, relação entre as duas auditorias, complementaridade e cooperação entre elas e auditoria interna e externa na prevenção e deteção de fraudes. Paralelamente foram elaborados dois questionários enviados a 6 empresas que têm departamentos de auditoria interna retiradas do universo das empresas com departamento de auditoria interna e a 2 empresas que fazem auditoria externa nessas empresas, a atuarem no mercado caboverdiano durante o ano de 2013, com o objetivo de verificar a relação entre a auditoria interna e a externa, através da opinião dos auditores internos e cruzamento com a opinião dos auditores externos, a fim de verificar, efetivamente, como estabelecer essa relação. De posse desses elementos, o objetivo geral apresentado pode ser comprovado pela conclusão de que se pode estabelecer uma relação entre auditoria interna e externa e a mesma pode ser de complementaridade. This monograph was developed with the aim of deepening, improvement and academic knowledge, by researching a topic that would contribute in some way to improve the topic in question. It also intended to contribute to the analysis of the inter-relationship existent between the internal and external audit and the notion that both complement each other’s work. This monograph attempts to provide a reflection on the relationship between internal audit and external audit. For this purpose, a library research was conducted on key concepts, scope, methods, purpose, function and procedures, relevant standards for internal and external audit, the relationship between the two audits, complementarity and cooperation between them, and internal and external audit in the prevention and fraud detection. In parallel, we prepared two questionnaires that were sent to 6 companies that have an internal audit department that are part of the universe of companies with internal audit department; and to two companies that make external audits to the companies mentioned above and that operate in the Cape Verdean market during the year 2013. The objective was to verify the relationship between internal audit and external by reviewing the opinion n of internal auditors and to intersect that with the opinion of external auditor in order to verify effectively, how to establish this relationship. With these elements, the overall goal presented can be substantiated by the finding that a relationship establish between internal and external audit and this relationship can be complementary.

Front crawl is an alternating swimming stroke technique in which different phases of arm movement induce changes in acceleration of limbs and body. This study proposes a new approach to use inertial body worn sensors to estimate main temporal phases of front crawl. Distinctive features in kinematic signals are used to detect the temporal phases. These temporal phases are key information sources of qualitative and quantitative evaluation of swimming coordination, which have been assessed previously by video analysis. The present method has been evaluated upon a wide range of coordination and showed a difference of 4.9% with video based system. The results are in line with video analysis inter-operator variability yet offering an easy-to-use system for trainers.

The pericentric inversion on chromosome 16 [inv(16)(p13q22)] and related t(16;16)(p13;q22) are recurrent aberrations associated with acute myeloid leukemia (AML) M4 Eo. Both abberations result in a fusion of the core binding factor beta (CBFB) and smooth muscle myosin heavy chain gene (MYH11). A selected genomic 6.9-kb BamHl probe detects MYH11 DNA rearrangements in 18 of 19 inv(16)/t(16;16) patients tested using HindIII digested DNA. The rearranged fragments were not detectable after remission in two cases tested, while they were present after relapse in one of these two cases tested.

We introduce a simple new hypothesis testing procedure, which,based on an independent sample drawn from a certain density, detects which of $k$ nominal densities is the true density is closest to, under the total variation (L_{1}) distance. Weobtain a density-free uniform exponential bound for the probability of false detection.

This review covers two important techniques, high resolution nuclear magnetic resonance (NMR) spectroscopy and mass spectrometry (MS), used to characterize food products and detect possible adulteration of wine, fruit juices, and olive oil, all important products of the Mediterranean Basin. Emphasis is placed on the complementary use of SNIF-NMR (site-specific natural isotopic fractionation nuclear magnetic resonance) and IRMS (isotope-ratio mass spectrometry) in association with chemometric methods for detecting the adulteration.

During the development and testing of a radioreceptor assay (RRA) for human IL-1, we have detected and identified the presence of auto-antibodies to IL-1 in normal human plasma (NHP). The RRA is based on the competition between human 125I-labeled rIL-1 alpha and standard or unknown quantities of IL-1 alpha or IL-1 beta for binding to a limited amounts of IL-1 receptor (IL-1R) isolated from the EL4 mouse thymoma cell line. NHP from 20 out of 100 unselected blood donors were found to completely inhibit the binding of 125I-labeled IL-1 alpha to its receptor, suggesting the presence in these NHP samples of either abnormal amounts of IL-1 or of a factor binding to the 125I-labeled IL-1 alpha. Special care was taken to ascertain that the inhibitory factors were antibodies and not soluble IL-1 receptor antagonist. When plasma samples with inhibiting activity were incubated with labeled IL-1 alpha and chromatographed on a Sephadex G200 column, they were found to contain 125I-labeled complexes with an apparent molecular weight of 150-200kD. The IL-1 binding factor could be eliminated from plasma by incubation with protein A-Sepharose, suggesting that it consisted in IgG antibodies directed against IL-1. Furthermore, the antibody nature of the inhibiting factor was confirmed by its binding to purified rIL-1 coupled to Sepharose. Screening of 200 NHP samples by incubation with 100 pg of 125I-labeled IL-1 followed by precipitation with 12% of polyethylene glycol (PEG) confirmed that about 25% of NHP contain detectable IgG antibodies to IL-1 alpha, while only 2% of NHP contain antibodies to IL-1 beta. No correlation between the presence of these anti-IL-1 antibodies and any particular major histocompatibility complex or any pathological conditions was detected. We suggest that all serum samples assayed for IL-1 alpha or IL-1 beta content should be pretested with the PEG precipitation assay described here.

Olfactory systems are evolutionarily ancient, underlying the common requirement for all animals to sense and respond to diverse volatile chemical signals in their environment. Odor detection is mediated by odorant receptors (ORs) that, in most olfactory systems, comprise large families of divergent G protein-coupled receptors. Here, I discuss our and others' recent investigations of ORs in the fruit fly, Drosophila melanogaster, which have revealed insights into the distinct evolutionary origin and molecular function of insect ORs. I also describe a bioinformatics strategy that we developed to identify molecules that function with these insect-specific receptors in odor detection.
