Only half of hypertensive patients has controlled blood pressure. Chronic kidney disease (CKD) is also associated with low blood pressure control, 25-30% of CKD patients achieving adequate blood pressure. The Community Preventive Services Task Force has recently recommended team-based care to improve blood pressure control. Team-based care of hypertension involves facilitating coordination of care among physician, pharmacist and nurse and requires sharing clinical data, laboratory results, and medications, e.g., electronically or by fax. Based on recent studies, development and evaluation of team-based care of hypertensive patients should be done in the Swiss healthcare system.

Les progrès continus des connaissances réalisés dans le domaine de l'oncohématologie depuis quelques décennies ont permis une amélioration considérable du pronostic de la plupart des formes de cancer. Toutefois, la morbidité et la mortalité attribuables aux infections apparaissent actuellement comme les principaux facteurs limitant l'agressivité des traitements de la maladie cancéreuse, et un meilleur contrôle de ces dernières est devenu l'un des éléments essentiels de la prise en charge de ce type de patients. Une recherche clinique intense a permis d'en identifier les grands principes qui sont exposés dans cet article. Un algorithme thérapeutique susceptible de guider le clinicien face au développement d'un état fébrile, toujours suspect d'infection chez le patient cancéreux neutropénique, est ensuite proposé.

BACKGROUND: Drug-resistant human immunodeficiency virus type 1 (HIV-1) minority variants (MVs) are present in some antiretroviral therapy (ART)-naive patients. They may result from de novo mutagenesis or transmission. To date, the latter has not been proven. METHODS: MVs were quantified by allele-specific polymerase chain reaction in 204 acute or recent seroconverters from the Zurich Primary HIV Infection study and 382 ART-naive, chronically infected patients. Phylogenetic analyses identified transmission clusters. RESULTS: Three lines of evidence were observed in support of transmission of MVs. First, potential transmitters were identified for 12 of 16 acute or recent seroconverters harboring M184V MVs. These variants were also detected in plasma and/or peripheral blood mononuclear cells at the estimated time of transmission in 3 of 4 potential transmitters who experienced virological failure accompanied by the selection of the M184V mutation before transmission. Second, prevalence between MVs harboring the frequent mutation M184V and the particularly uncommon integrase mutation N155H differed highly significantly in acute or recent seroconverters (8.2% vs 0.5%; P < .001). Third, the prevalence of less-fit M184V MVs is significantly higher in acutely or recently than in chronically HIV-1-infected patients (8.2% vs 2.5%; P = .004). CONCLUSIONS: Drug-resistant HIV-1 MVs can be transmitted. To what extent the origin-transmission vs sporadic appearance-of these variants determines their impact on ART needs to be further explored.

Introduction :¦Une grossesse chez l'adolescente est définie selon l'Office Fédéral de la Statistique (OFS) comme une grossesse survenant avant l'âge de 20 ans. Dans notre étude, nous avons choisi d'étudier plus particulièrement les grossesses d'adolescentes de moins de 18 ans, l'âge de la majorité civile étant fixé à 18 ans selon le Code pénal suisse. En Suisse, en 2009, ont eu lieu 647 naissances d'enfants de mères de moins de 20 ans (OFS). Plus rares sont les naissances de mères de moins de 18 ans ; entre 2005 et 2009, elles étaient 461 à donner naissance(OFS). Dans la littérature, la controverse est grande au sujet d'un éventuel risque augmenté de complications obstétricales et néonatales lors de grossesses à l'adolescence. Des programmes adaptés à la¦Prise en charge de l'adolescente enceinte ont été mis en place dans de nombreux pays et ont démontré des bénéfices par l'amélioration des perspectives sociales, éducationnelles et médicales de ces jeunes filles.¦But de l'étude :¦Cette recherche se divise en trois parties dont les objectifs sont bien distincts :¦I. Analyse Statistique des données suisses du point de vue médical et psychosocial concernant les grossesses et les accouchements chez les adolescentes.¦II. Identifier La présence en Suisse De spécialistes et/ou de programmes spécifiques pour la prise en charge de l'adolescente enceinte.¦III. Explorer En parallèle le vécu individuel par rapport au suivi de grossesse d'une jeune fille concernée.¦Méthode:¦I. Analyse Statistique des données de l'Association des Cliniques De Gynécologie-ˇ‐Obstétrique Suisse (AGOS).¦II. Envoi De questionnaires via programme LimeSurvey A 85 professionnels en Suisse, Répartis selon quatre corps de métiers: médecins cadre en gynécologie-ˇ‐obstétrique sans formation spécifique relative à l'adolescente, médecin gynécologue-ˇ‐obstétricien ayant une formation spécifique en gynécologie de l'adolescente, sages-°©‐femmes, assistantes sociales (taux de réponses: 63%, n=54).¦III. Entretien Structuré avec une jeune femme ayant accouché avant l'âge de 18 ans.¦Résultats:¦I.Selon Les données de l'Association des Cliniques De Gynécologie-ˇ‐Obstétrique Suisse, Il n'y a pas d'augmentation significative des complications obstétricales et néonatales lors de grossesses chez les adolescentes. Toutefois, Les différences se situent plutôt sur le plan des dépendances aux substances et des issues psychosociales des grossesses.¦II. Une part importante des adolescentes enceintes est prise en charge par des professionnels non spécifiquement formés en médecine de l'adolescence. Malgré cela, nous constatons une volonté générale de porter une attention particulière à ces grossesses avec la mise en place d'une prise en charge individualisée. De plus, le suivi est le plus souvent multidisciplinaire. La Plupart des professionnels non spécifiquement formés prenant en charge ces patientes pensent que les structures non spécialisées actuelles suffisent. En Suisse, Les principaux obstacles au développement de structures spécifiques sont le petit nombre de patientes potentielles et les faibles moyens financiers à disposition pour de tels projets.¦III. Organiser Les rencontres avec les adolescentes s'est avéré difficile, plusieurs adolescentes ne s'étant pas présentées aux rendez‐vous convenus malgré un consentement initial spontané. L'unique Adolescente qui a finalement pu être interviewée a bénéficié d'une prise en charge multidisciplinaire, principalement au CHUV. Questionnée Sur le regard qu'elle portait sur le suivi, elle s'est dite satisfaite tout en souhaitant que plus d'attention soit accordée à ses désirs. Cependant, La nécessité de protéger l'enfant est souvent un facteur limitant l'accès aux demandes des adolescentes.

Post-thrombotic syndrome (PTS) is the most frequent chronic complication of deep vein thrombosis with an estimated prevalence of 30-50%. PTS is a significant cause of disability, especially when complicated by venous ulcers. Therefore, it has important socio-economic consequences for both the patient and the health care system. Aim of this review is to resume state of the art literature on the management of PTS.

OBJECTIVES: Street-based sex workers (SSWs) in Lausanne, Switzerland, are poorly characterised. We set out to quantify potential vulnerability factors in this population and to examine SSW healthcare use and unmet healthcare requirements. METHODS: We conducted a cross-sectional questionnaire-based survey among SSWs working in Lausanne's red light district between 1 February and 31 July 2010, examining SSW socio-demographic characteristics and factors related to their healthcare. RESULTS: We interviewed 50 SSWs (76% of those approached). A fifth conducted their interviews in French, the official language in Lausanne. 48 participants (96%) were migrants, of whom 33/48 (69%) held no residence permit. 22/50 (44%) had been educated beyond obligatory schooling. 28/50 (56%) had no health insurance. 18/50 (36%) had been victims of physical violence. While 36/50 (72%) had seen a doctor during the preceding 12 months, only 15/50 (30%) were aware of a free clinic for individuals without health insurance. Those unaware of free services consulted emergency departments or doctors outside Switzerland. Gynaecology, primary healthcare and dental services were most often listed as needed. Two individuals (of 50, 4%) disclosed positive HIV status; of the others, 24/48 (50%) had never had an HIV test. CONCLUSIONS: This vulnerable population comprises SSWs who, whether through mobility, insufficient education or language barriers, are unaware of services they are entitled to. With half the participants reporting no HIV testing, there is a need to enhance awareness of available facilities as well as to increase provision and uptake of HIV testing.

Intrinsic connections in the cat primary auditory field (AI) as revealed by injections of Phaseolus vulgaris leucoagglutinin (PHA-L) or biocytin, had an anisotropic and patchy distribution. Neurons, labelled retrogradely with PHA-L were concentrated along a dorsoventral stripe through the injection site and rostral to it; the spread of rostrally located neurons was greater after injections into regions of low rather than high characteristic frequencies. The intensity of retrograde labelling varied from weak and granular to very strong and Golgi-like. Out of 313 Golgi like retrogradely labelled neurons 79.6% were pyramidal, 17.2% multipolar, 2.6% bipolar, and 0.6% bitufted; 13.4% were putatively inhibitory, i.e. aspiny or sparsely spiny multipolar, or bitufted. Individual anterogradely labelled intrinsic axons were reconstructed for distances of 2 to 7 mm. Five main types were distinguished on the basis of the branching pattern and the location of synaptic specialisations. Type 1 axons travelled horizontally within layers II to VI and sent collaterals at regular intervals; boutons were only present in the terminal arborizations of these collaterals. Type 2 axons also travelled horizontally within layers II to VI and had rather short and thin collateral branches; boutons or spine-like protrusions occurred in most parts of the axon. Type 3 axons travelled obliquely through the cortex and formed a single terminal arborization, the only site where boutons were found. Type 4 axons travelled for some distance in layer I; they formed a heterogeneous group as to their collaterals and synaptic specializations. Type 5 axons travelled at the interface between layer VI and the white matter; boutons en passant, spine-like protrusions, and thin short branches with boutons en passant were frequent all along their trajectory. Thus, only some axonal types sustain the patchy pattern of intrinsic connectivity, whereas others are involved in a more diffuse connectivity.

We include solvation effects in tight-binding Hamiltonians for hole states in DNA. The corresponding linear-response parameters are derived from accurate estimates of solvation energy calculated for several hole charge distributions in DNA stacks. Two models are considered: (A) the correction to a diagonal Hamiltonian matrix element depends only on the charge localized on the corresponding site and (B) in addition to this term, the reaction field due to adjacent base pairs is accounted for. We show that both schemes give very similar results. The effects of the polar medium on the hole distribution in DNA are studied. We conclude that the effects of polar surroundings essentially suppress charge delocalization in DNA, and hole states in (GC)n sequences are localized on individual guanines

Several airline consolidation events have recently been completed both in Europe and in the United States. The model we develop considers two airlines operating hub-and-spoke networks, using different hubs to connect the same spoke airports. We assume the airlines to be vertically differentiated, which allows us to distinguish between primary and secondary hubs. We conclude that this differentiation in air services becomes more accentuated after consolidation, with an increased number of flights being channeled through the primary hub. However, congestion can act as a brake on the concentration of flight frequency in the primary hub following consolidation. Our empirical application involves an analysis of Delta s network following its merger with Northwest. We find evidence consistent with an increase in the importance of Delta s primary hubs at the expense of its secondary airports. We also find some evidence suggesting that the carrier chooses to divert traffic away from those hub airports that were more prone to delays prior to the merger, in particular New York s JFK airport. Entre 2006 et 2009, 774 cas de cancer colorectal in situ ou invasif ont été diagnostiqués en Valais. La proportion des hommes (59%) est plus élevée que celle des femmes (41%). L'âge moyen au diagnostic est de 70 ans. 79% des tumeurs sont invasives. Le côlon est la localisation la plus fréquente (71%). 20% des cas sont de stade 0, 36% de stade I et II, 18% de stade III et 22% de stade IV. Le mode de présentation le plus fréquent est de loin la consultation pour symptômes non urgents (75%). Toutefois, 9% des patients sont pris en charge en urgence. 95% des patients traités le sont par de la chirurgie seule ou en combinaison avec d'autres traitements. 82% des patients avec un cancer colorectal invasif ont été traités dans les 30 jours. En première intention, 95% des cancers du côlon invasifs ont un traitement chirurgical alors que 53% des cancers du rectum invasifs ont un traitement chirurgical et 36% une radio-chimiothérapie. La survie du cancer colorectal invasif est de 95% à 30 jours et de 79% à 1 an. La survie est plus basse chez les personnes de 70 ans et plus (à 30 jours: 92%; à 1 an: 70%) que chez les personnes de moins de 70 ans (à 30 jours: 99%; à 1 an: 90%). Elle est également moins bonne pour les stades IV de la maladie (à 30 jours: 91%; à 1 an: 54%) que pour les stades I-II (à 30 jours: 97%; à 1 an: 91%) ou III (à 30 jours: 98%; à 1 an: 92%). Ces observations indiquent que l'épidémiologie du cancer colorectal dans la population valaisanne est similaire à ce qui est décrit dans d'autres populations en Europe, que les modalités de prise en charge sont proches de celles proposées dans les guidelines et que la survie est similaire à celle observée en Suisse et dans d'autres pays européens.

Report for the scientific sojourn carried out at the Paul Drude Institut für Festkörperelektronik of the Stanford University, USA, from 2010 to 2012. The objective of this project is the transport and control of electronic charge and spin along GaAs-based semiconductor heterostructures. The electronic transport has been achieved by taking advantage of the piezolectric field induced by surface acoustic waves in non-centrosymmetric materials like GaAs. This piezolectric field separates photogenerated electrons and holes at different positions along the acoustic wave, where they acummulate and are transported at the same velocity as the wave. Two different kinds of structures have been studied: quantum wells grown along the (110) direction, both intrinsic and n-doped, as well as GaAs nanowires. The analysis of the charge acoustic transport was performed by micro-photoluminescence, whereas the detection of the spin transport was done either by analyzing the polarization state of the emitted photoluminescence or by Kerr reflectometry. Our results in GaAs quantum wells show that charge and spin transport is clearly observed at the non-doped structures,obtaining spin lifetimes of the order of several nanoseconds, whereas no acoutically induced spin transport was detected for the n-doped quantum wells. In the GaAs nanowires, we were able of transporting successfully both electrons and holes along the nanowire axis, but no conservation of the spin polarization has been observed until now. The photoluminescence emitted by these structures after acoustic transport, however, shows anti-bunching characteristics, making this system a very good candidate for its use as single photon emitters.
