The aim of this study was to determine the seroprevalence of the infection by Toxocara in the general population of the Amazonian city of Yurimaguas, Peru. From March to August 2008, a total of 300 subjects were sampled and tested by means of a Toxocara ELISA-IgG test. A clinical and epidemiological questionnaire was used to assess the symptomatology and risk factors associated with human toxocariasis. The overall rate of seropositivity was 35.66%, with a significant high proportion in children (p < 0.001). The clinical evaluation revealed that 95.33% of the seropositive group had some type of symptomatology: headache (66.36%), respiratory compromise (63.55%), abdominal pain (54.21%), cutaneous signs (40.19%) and ocular manifestations (36.45%), and almost all of them were statistically significant (p < 0.001). Furthermore, 56.07% of the seropositive subjects presented at least one intestinal pathogen parasite with predominance of helminthes, but without significant association (p = 0.334). The analysis of risk factors showed only that the use of public places and geophagia exhibited a significant association with the seropositivity (p < 0.001). Clinical, serological and epidemiological findings associated to infection with Toxocara were observed in the present study and future studies should be done to assess this serious health problem.

Formalin-killed promastigotes (FKP) of Leishmania major, in combination with Montanide ISA 720 (MISA), BCG or alum were used in vaccination of an inbred murine model against cutaneous leishmaniasis (CL). Significant and specific increases in anti-FKP IgG responses were detected for both alum-FKP and BCG-FKP compared to MISA-FKP (p < 0.001). Significant increases in splenic lymphocyte recall proliferation was obtained in the MISA-FKP vaccinated mice compared to alum-FKP or BCG-FKP vaccinated groups (p < 0.01). The highest interferon-γ responses were observed in the BCG-FKP group followed by the MISA-FKP while the alum-FKP gave the least responses. Significantly reduced lesion sizes were obtained in the MISA-FKP group compared to the BCG/alum adjuvants-FKP vaccinated groups. Although the BCG-FKP group showed the highest IFN-γ responses, it failed to control cutaneous lesions. Significant reductions in parasite numbers were observed in the MISA-FKP and BCG-FKP vaccinated groups (p < 0.001). There was a good correlation between parasite burden and IFN-γ level indicating IFN-γ response as a sensitive parameter of the immune status. In conclusion, MISA-FKP is the most efficacious vaccine formulation against murine cutaneous leishmaniasis.

Jorge Lobo's disease is a cutaneous and subcutaneous mycosis that affects patients in the Amazon region. The number of patients is relatively small, but the real situation of the disease as public health problem is not known, because Jorge Lobo's disease is not a notifiable disease. This study aims to report the clinical evolution in patients affected and to determine the prevalence and areas of occurrence of the disease. A retrospective study was carried out based on the analysis of the clinical records, which included a collection of photographs of patients in the Department of Sanitary Dermatology, in Rio Branco, and patients seen in the interior of the state. In a decade, in Rio Branco, 249 cases of the disease were reported, 30 were females and 219 males. Of these patients, 153 had localized lesions, 94 of them were on one ear, 55 had multifocal lesions and 41 had disseminated lesions. The average time between the onset of symptoms and diagnosis was 19 years. The average age at the time of diagnosis was 53 years, and ages ranged from 14 to 96 years.

In Amazonian Brazil, the Cebus apella monkey (Primates: Cebidae) has been associated with the enzootic cycle of Leishmania (V.) shawi, a dermotropic parasite causing American cutaneous leishmaniasis (ACL). It has also been successfully used as animal model for studying cutaneous leishmaniasis. In this work, there has been investigated its susceptibility to experimental Leishmania (L.) infantum chagasi-infection, the etiologic agent of American visceral leishmaniasis (AVL). There were used ten C. apella specimens, eight adult and two young, four males and six females, all born and raised in captivity. Two experimental infection protocols were performed: i) six monkeys were inoculated, intra-dermal via (ID), into the base of the tail with 2 x 10(6) promastigotes forms from the stationary phase culture medium; ii) other four monkeys were inoculated with 3 x 10(7) amastigotes forms from the visceral infection of infected hamsters by two different via: a) two by intravenous via (IV) and, b) other two by intra-peritoneal via (IP). The parameters of infection evaluation included: a) clinical: physical exam of abdomen, weigh and body temperature; b) parasitological: needle aspiration of the bone-marrow for searching of amastigotes (Giemsa-stained smears) and promastigotes forms (culture medium); c) immunological: Indirect fluorescence antibody test (IFAT) and, Delayed-type hypersensitivity (DTH). In the six monkeys ID inoculated (promastigotes forms) all parameters of infection evaluation were negative during the 12 months period of follow-up. Among the four monkeys inoculated with amastigotes forms, two IV inoculated showed the parasite in the bone-marrow from the first toward to the sixth month p.i. and following that they cleared the infection, whereas the other two IP inoculated were totally negative. These four monkeys showed specific IgG-antibody response since the third month p.i. (IP: 1/80 and IV: 1/320 IgG) toward to the 12th month (IP: 1/160 and IV: 1/5120). The DTH-conversion occurred in only one IV inoculated monkey with a strong (30 mm) skin reaction. Considering these results, we do not encourage the use of C. apella monkey as animal model for studying the AVL.

In this study we investigated the efficacy of hyperbaric oxygen (HBO) therapy, alone or combined with the pentavalent antimonial glucantime on Leishmania amazonensis infection. In parallel, the effect of Brazilian red propolis gel (propain) alone or combined with glucantime on L. amazonensis infection was evaluated. The inhibition of the infection in macrophages treated with glucantime in combination with HBO exposition was greater than that of macrophages treated with glucantime alone or HBO alone. The susceptible mouse strain BALB/c infected in the shaved rump with L. amazonensis treated with glucantime and exposed to HBO showed: time points in the course of the disease in which lesions were smaller than those of mice treated with glucantime alone and revascularization of the skin in the lesion site; interferon-gamma (IFN-g) levels were not elevated in lymph node cells from these animals. Propain alone was not efficient against lesions, although less exudative lesions were observed in animals treated with propain alone or combined with glucantime. These results reveal the potential value of HBO and red propolis in combination with glucantime for treating cutaneous leishmaniasis and encourage further studies on the effect of more aggressive HBO, propolis and glucantime therapies on different mouse models of leishmaniasis.

INTRODUCTION: The symptoms of Brazilian borreliosis resemble the clinical manifestations of Lyme disease (LD). However, there are differences between the two in terms of epidemiological and laboratory findings. Primers usually employed to diagnose LD have failed to detect Borrelia strains in Brazil. OBJECTIVE: We aimed to identify the Brazilian Borrelia using a conserved gene that synthesizes the flagellar hook (flgE) of Borrelia burgdorferi sensu lato. METHOD: Three patients presenting with erythema migrans and positive epidemiological histories were recruited for the study. Blood samples were collected, and the DNA was extracted by commercial kits. RESULTS: The gene flgE was amplified from DNA of all selected patients. Upon sequencing, these positive samples revealed 99% homology to B. burgdorferi flgE. CONCLUSION: These results support the existence of borreliosis in Brazil. However, it is unclear whether this borreliosis is caused by a genetically modified B. burgdorferi sensu stricto or by a new species of Borrelia spp.

OBJECTIVES: The aim of this study was to describe the pattern of expression of Toll-like receptor 2 (TLR2) and Toll-like receptor 4 (TLR4) in skin biopsies of patients with American tegumentary leishmaniasis (ATL) caused by Leishmania braziliensis. METHODS: This prospective study evaluated 12 patients with ATL caused by Leishmania braziliensis confirmed by polymerase chain reaction. Immunohistochemistry was performed to determine the expression of TLR2 and TLR4. The number of NK cells, dendritic cells and macrophages in the tissue were calculated. The cytokine expression was determined using the anti-TNF-α, anti-IFN-Γ, anti-IL-1 and anti-IL-6. Double immunostaining reactions were used to determine the cell expressing TLR2 and TLR4. RESULTS: The numbers of cells expressing TLR2 and TLR4 were 145.48 ± 82.46 cell/mm² and 3.26 ± 4.11 cell/mm² respectively (p < 0.05). There was no correlation of TLR2 and TLR4 with the amount of cytokines and the number of NK cells, dendritic cells or macrophages. The double immunostaining revealed that TLR2 was expressed by macrophages. CONCLUSION: In human cutaneous leishmaniasis caused by Leishmania braziliensis, TLR2 is the most common TLR expressed during active disease, mainly by macrophages although without correlation with the amount of cytokines and number of cells.

O oleotórax foi largamente utilizado entre 1930 e 1950 como tratamento da tuberculose pulmonar. Embora tenha sido abandonado a partir da década de 50, pelo sucesso terapêutico dos antibacilares e pela evolução da cirurgia torácica, continuaram a surgir, muitos anos mais tarde, complicações. Os autores apresentam o caso clínico de um doente com 80 anos de idade, com antecedentes de tuberculose pulmonar, tratada há 52 anos com oleotórax, o qual foi internado para esclarecimento de um tumor na região infra-clavicular direita, tendo a tomografia axial computorizada revelado a existência de uma fístula pleuro-cutânea e a punção mostrado que o conteúdo era oleoso.

Rodents are the most frequently used animals in surgical experimentation. It is estimated that guinea pigs in particular are the third most commonly used species in this context. To disinfect guinea pigs’ skin, either alcohol or surgical iodine are most often used. In the context of an animal research project, a Nissen operation was performed in an adult male guinea pig. Because of accidental contamination of the operative field, a 10% povidone-iodine cutaneous solution was applied to the serosa of the anterior wall of the stomach and to the gastric fundus. The guinea pig died 12 hours after surgery due to an acute necrotizing hemorrhagic gastritis. Although there have been a few reports of povidone-iodine toxicity in the guinea pig, as far as the authors could determine, this is the first time that such a serious abdominal complication is reported. The authors believe that the possibility of a similar event should be taken into consideration when planning, executing and interpreting experiments in the guinea pig.

A xerose cutânea é um motivo frequente de consulta de dermatologia. O seu tratamento passa pela identificação da causa subjacente. Os autores descrevem o caso clínico de um doente do sexo masculino, 49 anos, que recorre a consulta de dermatologia por xerose cutânea severa com início há cerca de 4 meses. Referia também prurido intenso, xerose bucal e cansaço fácil. Dos antecedentes pessoais destacava-se tiroidectomia total há 8 meses, estando apenas medicado com cálcio, sem hormonas tiroideias. À apresentação, o doente tinha voz grave, edema palpebral, macroglossia, xerose cutânea severa generalizada com áreas de eczema craquelé nos membros, hiperqueratose folicular dorsal, hiperlinearidade das linhas das mãos, tonalidade cutânea palmoplantar amarelada e bradicárdia. Analiticamente, registava-se elevação das transaminases, hipercolesterolémia, hipertrigliceridémia, elevação da TSH e diminuição da T3 e T4. Salienta-se este caso pela semiologia rica de uma causa endocrinológica iatrogénica de xerose cutânea severa.

Introdução: A designação de mastocitose engloba várias entidades clinicamente distintas caracterizadas pela acumulação tissular de mastócitos. A pele é o órgão mais frequentemente envolvido. Consideram-se 4 padrões clínicos de mastocitose cutânea: urticária pigmentosa (UP), mastocitose cutânea difusa, mastocitoma e telangiectasia macularis eruptiva perstans. Na infância, a doença é habitualmente autolimitada e exclusivamente cutânea. Material e Métodos: Apresenta-se um estudo retrospetivo dos doentes com mastocitose cutânea observados na Consulta de Dermatologia Pediátrica do Hospital de Curry Cabral entre 2001 e 2010. Resultados: Foram englobados 32 doentes (20 do sexo masculino e 12 do sexo feminino). Em 90.6% dos casos, as manifestações surgiram antes dos 2 anos. Apenas foram observadas UP (53,1%) e mastocitomas (46,9%). O tronco e a raíz dos membros foram as localizações preferenciais. O sinal de Darier estava presente em 87,5% dos casos (94,1% das UP e 80% dos mastocitomas). As manifestações associadas foram: prurido (40,6%), formação de bolha (28,1%), flushing (18,8%) e dermografismo(12,5%). Não foram documentados casos de mastocitose sistémica. Dois doentes tinham história familiar de mastocitose cutânea. Não foram detetadas alterações laboratoriais significativas. Nos 16 casos determinados, os níveis séricos de Triptase-alfa foram normais. Discussão: Estes resultados estão de acordo com a literatura, nomeadamente em relação às formas clínicas mais frequentes, localizações habituais, elevada frequência do sinal de Darier e idade precoce de início. Quando determinados, os níveis séricos de Triptase-alfa foram normais, o que está de acordo com a ausência de envolvimento sistémico. A maior prevalência no sexo masculino tem sido relatada em alguns estudos.

As reacções adversas a fármacos (RAF) representam um problema frequente na prática clínica. A alergia a fármacos resulta de mecanismos de hipersensibilidade imunológica e representa 6-10% do total de RAF. Clinicamente, as reacções alérgicas a fármacos podem ser classificadas como imediatas (tipo I) ou não-imediatas (com manifestações clínicas diversas e associadas sobretudo a reacções de tipo IV). Neste artigo são abordados aspectos gerais, nomeadamente os mecanismos imunopatogénicos implicados na alergia a fármacos e reactividade cruzada mas também as manifestações cutâneas mais relevantes, nomeadamente exantemas máculo-papulares, eritema fixo a fármacos (EFF), pustulose exantemática aguda generalizada (PEAG), síndrome de hipersensibilidade a fármacos (DRESS – drug rash with eosinophilia and systemic symptoms), síndrome de Stevens-Johnson/necrólise epidérmica tóxica (SSJ/NET). O papel dos testes cutâneos (epicutâneos ou intradérmicos de leitura tardia) na abordagem de reacções não-imediatas é também revisto. Os beta-lactâmicos (BL) são o grupo farmacológico mais frequentemente envolvido em reacções de hipersensibilidade imunológica e que mais dificuldades coloca na prática clínica diária, nomeadamente devido aos riscos de reactividade cruzada, pelo que é analisado em maior detalhe ao longo da revisão. A indução de tolerância a fármacos poderá ser considerada em casos selecionados, sobretudo quando na ausência de alternativas terapêuticas igualmente eficazes ou seguras.

O diagnóstico dos sarcomas cutâneos é desafiante, devido à complexidade e heterogeneidade deste grupo de lesões, cuja apresentação nos tecidos superficiais (pele e tecido celular subcutâneo) impõe diagnóstico diferencial obrigatório com tumores benignos e processos inflamatórios. Apesar de serem tumores pouco frequentes alguns tipos assumem grande importância por serem localmente invasivos, com elevada morbi-mortalidade associada, taxas de recidiva local e metastização significativas. Os autores apresentam cinco casos clínico-patológicos de sarcomas cutâneos – sarcoma de Kaposi, dermatofibrossarcoma protuberans, fibroxantoma atípico, leiomiossarcoma e angiossarcoma – e fazem uma breve revisão da literatura sobre os tipos de sarcoma cutâneo com maior interesse na Dermatologia, salientando os aspectos fisiopatológicos, clínicos, histopatológicos e terapêuticos particulares.

SUMMARY A food-borne trematode infection fascioliasis is one among common public health problems worldwide. It caused a great economic loss for the human race. Control of snail population below a certain threshold level is one of the important methods in the campaign to reduce the incidence of fascioliasis. The life cycle of the parasite can be interrupted by killing the snail or Fasciola larva redia and cercaria inside of the snail Lymnaea acuminata. In vitro toxicity of different binary combinations (1:1 ratio) of plant-derived larvicidal active components such as citral, ferulic acid, umbelliferone, azadirachtin and allicin against Fasciola redia and cercaria were tested. The mortality of larvae was observed at 2h, 4h, 6h and 8h of treatment. In in vitro condition azadirachtin + allicin (1:1 ratio) was highly toxic against redia and cercaria (8h LC50 0.006 and 0.005 mg/L). Toxicity of citral + ferulic acid was lowest against redia and cercaria larvae.
