La diffusion internationale des paiements pour services environnementaux (PSE) a été interprétée en 2010 par le gouvernement bolivien d'Evo Morales comme une réponse strictement néolibérale à la nécessité d'assurer une gestion durable des ressources naturelles. Supposée amener à terme à l'éviction de toute régulation autre que marchande - qu'elle s'applique à la nature ou aux rapports entre personnes -, la mise en place de PSE n'a pas été encouragée par les autorités nationales boliviennes. Des projets de PSE ont toutefois été lancés, dont les Acuerdos Reciprocos por el Agua (ARA), issus d'un partenariat public-privé dans le département de Santa Cruz. En analysant leur conception et leur fonctionnement au prisme du référentiel polanyien, nous montrons que, contrairement aux craintes gouvernementales, ces PSE ne font pas abstraction des logiques organisationnelles réciprocitaires et redistributives, ajustant au contexte local un objet global. The international dissemination of payments for ecosystem services (PES) has been interpreted in 2010 by the Bolivian government of Evo Morales as a strictly neo-liberal response to the need to ensure a sustainable management of natural resources. Supposed to contribute to the crowding-out of any other regulation than market - applied to the nature or the relationship between people - the implementation of PES was not encouraged by the Bolivian national authorities. However some PES projects stemming from a public-private partnership have been initiated at local level, as the Acuerdos Reciprocos por el Agua (ARA), in the department of Santa Cruz. Analysing their design and operating through the Polanyian framework, we show that, contrary to the government fears, these PES do not ignore the reciprocal and redistributive organisational logics, adjusting a global object to the local context.

BACKGROUND: Meticulous steps and procedures are proposed in planning guidelines for the development of comprehensive multiyear plans for national immunization programmes. However, we know very little about whether the real-life experience of those who adopt these guidelines involves following these procedures as expected. Are these steps and procedures followed in practice? We examined the adoption and usage of the guidelines in planning national immunization programmes and assessed whether the recommendations in these guidelines are applied as consistently as intended. METHODS: We gathered information from the national comprehensive multiyear plans developed by 77 low-income countries. For each of the 11 components, we examined how each country applied the four recommended steps of situation analysis, problem prioritization, selection of interventions, and selection of indicators. We then conducted an analysis to determine the patterns of alignment of the comprehensive multiyear plans with those four recommended planning steps. RESULTS: Within the first 3 years following publication of the guidelines, 66 (86%) countries used the tool to develop their comprehensive multiyear plans. The funding conditions attached to the use of these guidelines appeared to influence their rapid adoption and usage. Overall, only 33 (43%) countries fully applied all four recommended planning steps of the guidelines. CONCLUSIONS: Adoption and usage of the guidelines for the development of comprehensive multiyear plans for national immunization programmes were rapid. However, our findings show substantial variation between the proposed planning ideals set out in the guidelines and actual use in practice. A better understanding of factors that influence how recommendations in public health guidelines are applied in practice could contribute to improvements in guidelines design. It could also help adjust strategies used to introduce them into public health programmes, with the ultimate goal of a greater health impact.

The Spanish Barley Breeding Program is carried out by four public research organizations, located at the most representative barley growing regions of Spain. The aim of this study is to evaluate the program retrospectively, attending to: i) the progress achieved in grain yield, and ii) the extent and impact of genotype-by-environment interaction of grain yield. Grain yields and flowering dates of 349 advanced lines in generations F8, F9 and F10, plus checks, tested at 163 trials over 11 years were analized. The locations are in the provinces of Albacete, Lleida, Valladolid and Zaragoza. The data are highly unbalanced because the lines stayed at the program for a maximum of three years. Progress was estimated using relative grain yield and mixed models (REML) to homogenize the results among years and locations. There was evident progress in the program over the period studied, with increasing relative yields in each generation, and with advanced lines surpassing the checks in the last two generations, although the rate of progress was uneven across locations. The genetic gain was greater from F8 to F9 than from F9 to F10. The largest non-purely environmental component of variance was genotype-by-location-by-year, meaning that the genotype-by-location pattern was highly unpredictable. The relationship between yield and flowering time overall was weak in the locations under study at this advanced stage of the program. The program can be continued with the same structure, although measures should be taken to explore the causes of slower progress at certain locations.

BACKGROUND: Transactional sex is associated with the HIV epidemic among young people in Uganda. Few quantitative studies based on nationally representative survey data explored the relationship between sexual behaviors, HIV infection, and transactional sex. OBJECTIVE: This study aimed to determine the associations between risky sexual behaviors, participation in transactional sex, and HIV sero-status among men and women aged 15-24 in Uganda. DESIGN: The study uses data from the Uganda AIDS Indicator Survey, a cross-sectional national HIV serological study conducted in 2011. We analyzed data on 1,516 men and 2,824 women aged 15-24 who had been sexually active in the 12 months preceding the survey. Private, face-to-face interviews were also conducted to record the sociodemographics, sexual history, and experiences of sexual coercion. Logistic regression analysis was performed to measure associations between sexual behaviors and transactional sex, and associations between HIV sero-status and transactional sex. RESULTS: Among young people who had been sexually active in the 12 months prior to the survey, 5.2% of young men reported paying for sex while 3.7% of young women reported receiving gifts, favors, or money for sex. Lower educational attainment (ORadjusted 3.25, CI 1.10-9.60) and experience of sexual coercion (ORadjusted 2.83, CI 1.07-7.47) were significantly associated with paying for sex among men. Multiple concurrent sexual relationships were significantly associated with paying for sex among young men (ORadjusted 5.60, CI 2.08-14.95) and receiving something for sex among young women (ORadjusted 8.04, CI 2.55-25.37). Paying for sex among young men and having three to five lifetime sexual partners among young women were associated with increased odds of testing positive for HIV. CONCLUSIONS: Transactional sex is associated with sexual coercion and HIV risk behaviors such as multiple concurrent sexual partnerships among young people in Uganda. In addition, transactional sex appears to place young men at increased risk for HIV in Uganda. Both sexes appear equally vulnerable to risks associated with transactional sex, and therefore should be targeted in intervention programs. In addition, strengthening universal education policy and improving school retention programs may be beneficial in reducing risky sexual behaviors and transactional sex.

Background: Diverse projects and guidelines to assist hospitals towards the attainment of comprehensive smoke-free policies have been developed. In 2006, Spain government passed a new smoking ban that reinforce tobacco control policies and banned completely smoking in hospitals. This study assesses the progression of tobacco control policies in the Catalan Network of Smokefree Hospitals before and after a comprehensive national smoking ban. Methods: We used the Self-Audit Questionnaire of the European Network for Smoke-free Hospitals to score the compliance of 9 policy standards (global score = 102). We used two crosssectional surveys to evaluate tobacco control policies before (2005) and after the implementation of a national smoking ban (2007) in 32 hospitals of Catalonia, Spain. We compared the means of the overall score in 2005 and 2007 according to the type of hospital, the number of beds, the prevalence of tobacco consumption, and the number of years as a smoke-free hospital. Results: The mean of the implementation score of tobacco control policies was 52.4 (95% CI:45.4-59.5) in 2005 and 71.6 (95% CI: 67.0-76.2) in 2007 with an increase of 36.7% (p < 0.01). The hospitals with greater improvement were general hospitals (48% increase; p < 0.01), hospitals with > 300 beds (41.1% increase; p < 0.01), hospitals with employees' tobacco consumption prevalence 35-39% (72.2% increase; p < 0.05) and hospitals that had recently implemented smoke-free policies (74.2% increase; p < 0.01). Conclusion: The national smoking ban appears to increase tobacco control activities in hospitals combined with other non-bylaw initiatives such as the Smoke-free Hospital Network.

"This paper will discuss the major developments in the area of fingerprint" "identification that followed the publication of the National Research Council (NRC, of the US National Academies of Sciences) report in 2009 entitled: Strengthening Forensic Science in the United States: A Path Forward. The report portrayed an image of a field of expertise used for decades without the necessary scientific research-based underpinning. The advances since the report and the needs in selected areas of fingerprinting will be detailed. It includes the measurement of the accuracy, reliability, repeatability and reproducibility of the conclusions offered by fingerprint experts. The paper will also pay attention to the development of statistical models allow- ing assessment of fingerprint comparisons. As a corollary of these developments, the next challenge is to reconcile a traditional practice domi- nated by deterministic conclusions with the probabilistic logic of any statistical model. There is a call for greater candour and fingerprint experts will need to communicate differently on the strengths and limitations of their findings. Their testimony will have to go beyond the blunt assertion" "of the uniqueness of fingerprints or the opinion delivered ispe dixit."

The 2013 survey addressed the following objectives: Primary objectives : a) Distribution of health behaviors related to NCDs, particularly tobacco use, alcohol drinking, and physical activity ; b) Distribution of the main modifiable risk factors of NCDs, particularly blood pressure, adiposity markers, diabetes and blood lipids ; c) Rates of awareness, treatment and control of hypertension, diabetes and dyslipidemia ; d) Comparison of findings in the survey 2013 with results in previous similar NCD surveys in 1989, 1994, 2004 ; e) Dietary patterns ; f) Knowledge, attitudes and practices related to NCDs and NCD risk factors. Secondary objectives : g) Assessment of indicators of quality of health (e.g. SF‐12) ; h) Assessment of psychological stress and relation with NCD ; i) Assessment of several indicators of frailty (e.g. handgrip strength test, chair strand test, functional limitations) ; j) Assessment of knowledge and level of agreement with current policies on tobacco control ; k) Use of public and private health care services, particularly for NCDs ; l) Exposure to advice on health behaviors given by health professionals at health care level ; m) Burden of chronic diseases not related to the main NCDs (e.g. musculoskeletal, mental health, etc) ; n) Screening of selected cancers ; o) Assessment of the kidney function ; p) Frequency of heart arrhythmias (one‐lead ECG) and heart murmurs (auscultation) ; q) Assessment of bone mineral density (ultrasound of calcaneus) ; r) Exposure of the population to mass media, particularly in relation to health programs, and use by the population of new communication technologies ; s) Assessment of a number of social variables and their association with the variables measured in the survey ; t) More generally, the survey provides broad information (medical, social, environment, etc) that can be useful for tailoring NCD prevention and control programs.

The survey is also referred shortly as the "2013 Survey" in this report. Overall crude results were reported in a comprehensive report in November 2014. Further detailed analyses and recommendations on particular topics will be performed separately.

Avec l'avènement d'une économie du savoir de plus en plus compétitive, le paysage universitaire suisse a connu de profonds changements ces dernières décennies. Afin d'améliorer la compétitivité de nos universités, les élites politico-administratives et universitaires ont accordé plus d'autonomie de gestion aux directions de nos universités. En contrepartie, ces dernières se sont alors engagées à réaliser un certain nombre d'objectifs stratégiques et à en rendre compte au travers de processus d'évaluation. Dans ce contexte de « Nouvelle Gestion Publique », les enseignants-chercheurs sont donc sensés orienter leur travail de manière à faire coïncider leurs objectifs scientifiques personnels avec les objectifs stratégiques définis entre leur direction et l'Etat. Face à ces changements, dans quelle mesure ce groupe professionnel, jouissant traditionnellement d'une autonomie et d'un pouvoir discrétionnaire élevés, est-il encore capable de maintenir une capacité de régulation autonome ? C'est à cette question que nous avons voulu répondre dans le cadre de cette thèse. Pour ce faire, nous avons développé un modèle original d'analyse de la régulation professionnelle. Nous l'avons ensuite appliqué à trois facultés de l'Université de Lausanne. L'analyse de nos entretiens semi directifs nous a alors montré que les enseignants-chercheurs de ces trois facultés étaient encore en mesure de maintenir une capacité d'autorégulation de leurs pratiques professionnelles. Certes nous avons constaté une certaine « homogénéisation » de ces dernières, mais celle-ci concerne avant tout des aspects marginaux du travail des enseignants-chercheurs. Autrement dit, les enseignants-chercheurs ont su adapter des aspects secondaires de leurs pratiques aux attentes externes afin de préserver le « coeur » de leur activité professionnelle. Cela nous a donc permis de conclure que ce groupe professionnel est toujours en mesure de maintenir un niveau d'autorégulation élevé malgré la mise en place d'une nouvelle gouvernance universitaire et des attentes toujours plus grandes en terme d' « accountability ».

Le vieillissement de la population va de pair avec une progression constante des maladies chroniques dans notre société. Les modes de vie modernes favorisent l'apparition de ces maladies, notamment par le tabagisme, par le manque d'activité physique et par une alimentation déséquilibrée. Les maladies chroniques somatiques sont aujourd'hui dans la ligne de mire des responsables de la politique de la santé. Elles constituent, avec les maladies chroniques psychiques, dont les conséquences sont multiples, un défi de taille pour notre système de santé et un problème majeur de santé publique. La prise en charge des maladies chroniques requiert une bonne coordination du système de santé. La Confédération et les cantons y travaillent activement en s'engageant par exemple dans le domaine du personnel de santé et de l'organisation des structures de soins. Confédération et cantons contribuent à faire en sorte que les maladies chroniques psychiques ou somatiques deviennent moins fréquentes ou soient traitées suffisamment tôt. Tels sont les objectifs des politiques de promotion de la santé et de prévention des maladies, dont l'importance ira croissant à l'avenir. Le présent rapport national sur la santé donne une vue d'ensemble de la santé dans la population suisse; il met en lumière certains aspects importants des maladies chroniques dans notre pays. Puisse-t-il donner des impulsions nouvelles et aider les acteurs du système suisse de santé à maîtriser ensemble les défis liés à ces maladies.
