Instruction of haematopoietic lineage choices, evolution of transcriptional landscapes and cancer stem cell hierarchies derived from an AML1-ETO mouse model.

The t(8;21) chromosomal translocation activates aberrant expression of the AML1-ETO (AE) fusion protein and is commonly associated with core binding factor acute myeloid leukaemia (CBF AML). Combining a conditional mouse model that closely resembles the slow evolution and the mosaic AE expression pattern of human t(8;21) CBF AML with global transcriptome sequencing, we find that disease progression was characterized by two principal pathogenic mechanisms. Initially, AE expression modified the lineage potential of haematopoietic stem cells (HSCs), resulting in the selective expansion of the myeloid compartment at the expense of normal erythro- and lymphopoiesis. This lineage skewing was followed by a second substantial rewiring of transcriptional networks occurring in the trajectory to manifest leukaemia. We also find that both HSC and lineage-restricted granulocyte macrophage progenitors (GMPs) acquired leukaemic stem cell (LSC) potential being capable of initiating and maintaining the disease. Finally, our data demonstrate that long-term expression of AE induces an indolent myeloproliferative disease (MPD)-like myeloid leukaemia phenotype with complete penetrance and that acute inactivation of AE function is a potential novel therapeutic option.

The Research Strategy for Nursing and Midwifery in Ireland (Department of Health and Children, 2003) was a response to the Commission on Nursing’s (Government of Ireland, 1998: p114) suggestion that a national strategy be developed to guide and support the emerging need for the professions of nursing and midwifery to develop a research base as a fundamental to practice. The Commission itself had found a dearth of published Irish nursing and midwifery research (Government of Ireland, 1998; Condell, 1998) due to a lack of policy direction and funding availability. Turnover in Nursing and Midwifery

The Report of the Commission on Nursing (1998) identified a need to strengthen the workforce planning functions. The Nursing Policy Division of the Department of Health and Children acted on the recommendation by establishing a Study of the Nursing and Midwifery Resource (December 1998). The main objective of the study was to forecast future nursing and midwifery needs which included estimating the turnover rate among registered nurses and midwives employed in the health services. Guidelines for the safe administration of cytotoxic medical preparations in the treatment of patients with cancer

The Minister for Health’s Strategy for effective health care in the 1990s has emphasised quality of service as one of its key principles.  Within the medical sphere, almost no other field has developed and continues to expand as rapidly as that of cancer biology and treatment.  Cytotoxic therapies are a major part of these developments. Report of The Commission on Nursing

Nursing and midwifery have been one of the cornerstones of the modern Irish health service. The quality of care and public satisfaction with the health service is often related to the quality of the nursing service. Irish nurses not only enjoy the confidence of patients and clients of the health service but have an international reputation for their professionalism and the excellence of their care. However, the health services are in a state of constant and rapid development in response to technological, social and economic changes both domestically and internationally. The Commission is recommending a new framework which will give a secure basis for the further professional development of nursing and midwifery in the context of  anticipated changes in the health services, their organisation and delivery. Cápsula endoscópica de colon versus colonoscopia en el cribado familiar de cancer colorrectal

Els objectius van ser avaluar prospectivament la capacitat diagnòstica de la càpsula endoscòpica en comparació amb la colonoscòpia en 30 pacients amb història familiar de CCR, i avaluar la neteja colònica amb preparació amb Moviprep. La prevalença de pòlips detectats amb càpsula i colonoscòpia va ser de 20,7% i 24,1%, respectivament. La sensibilitat i especificitat en detecció de pòlips va ser de 71, 4 i 95,4%, respectivament, amb VPP i VPN de 83,3 i 91,3%. Es conclou que la càpsula endoscòpica suposa una tècnica alternativa precisa, fiable, ben tolerada i segura pel cribatge familiar de CCR.

Implementation of Recommendations of The Commission on Nursing - First Annual Progress Report of Monitoring Committee

The focus of this first annual report is on the progress made during 2000 in implementing the recommendations contained in the priority Action Plan. However, the Monitoring Committee acknowledges that certain other key recommendations of the Commission were implemented in 1998/1999.
