 Objective: This study investigated the relationship between motor performance and social-communicative impairment in children with ADHD-combined type (ADHD-CT). Method: An upper limb Fitts’ aiming task was used as a measure of motor performance and the Social Responsiveness Scale as a measure of social-communicative/autistic impairment in the following groups: ADHD-CT (n = 11) and typically developing (TD) controls (n = 10). Results: Children with ADHD-CT displayed greater variability in their movements, reflected in increased error variance over repeated aiming trials compared with TD controls. Motor performance variability was associated with social-communicative deficits in the ADHD-CT but not in the TD group. Conclusion: Social-communicative impairments further complicate the clinical picture of ADHD-CT; therefore, further research in this area is warranted to ascertain whether a particular pattern of motor disturbance in children with ADHD-CT may be clinically useful in identifying and assessing children with a more complex ADHD presentation. © 2012 SAGE Publications.

This study assessed the validity of a tri-axial accelerometer worn on the upper body to estimate peak forces during running and change-of-direction tasks. Seventeen participants completed four different running and change-of-direction tasks (0°, 45°, 90°, and 180°; five trials per condition). Peak crania-caudal and resultant acceleration was converted to force and compared against peak force plate ground reaction force (GRF) in two formats (raw and smoothed). The resultant smoothed (10 Hz) and crania-caudal raw (except 180°) accelerometer values were not significantly different to resultant and vertical GRF for all running and change-of-direction tasks, respectively. Resultant accelerometer measures showed no to strong significant correlations (r = 0.00–0.76) and moderate to large measurement errors (coefficient of variation [CV] = 11.7–23.9%). Crania-caudal accelerometer measures showed small to moderate correlations (r = − 0.26 to 0.39) and moderate to large measurement errors (CV = 15.0–20.6%). Accelerometers, within integrated micro-technology tracking devices and worn on the upper body, can provide a relative measure of peak impact force experienced during running and two change-of-direction tasks (45° and 90°) provided that resultant smoothed values are used.

This study examined the trunk postures and upper-body muscle activations during four physically demanding wildfire suppression tasks. Bilateral, wireless surface electromyography was recorded from the trapezius and erector spinae muscles of nine experienced, wildfire fighters. Synchronised video captured two retroreflective markers to allow for quantification of two-dimensional sagittal trunk flexion. In all tasks, significantly longer time was spent in the mild and severe trunk flexion (p ≤ 0.002) compared to the time spent in a neutral posture. Mean and peak muscle activation in all tasks exceeded previously established safe limits. These activation levels also significantly increased through the performance of each task (p < 0.001). The results suggest that the wildfire suppression tasks analysed impose significant musculoskeletal demand on firefighters. Fire agencies should consider developing interventions to reduce the exposure of their personnel to these potentially injurious musculoskeletal demands.

  Remote human activity monitoring is critical and essential in physiotherapy with respect to the skyrocketing healthcare expenditure and the fast aging population. One of frequently used method to monitor human activity is wearing inertial sensors since it is low-cost and accurate. However, the measurements of those sensors are able only to estimate the orientation and rotation angles with respect to actual movement angles, because of differences in the body’s co-ordination system and the sensor’s co-ordination system. There were numerous studies being conducted to improve the accuracy of estimation, though there is potential for further discussions on improving accuracy by replacing heavy algorithms to less complexity. This research is an attempt to propose an adaptive complementary filter for identifying human upper arm movements. Further, this article discusses a feasibility of upper arm rehabilitation using the proposed adaptive complementary filter and inertial measurement sensors. The proposed algorithm is tested with four healthy subjects wearing an inertial sensor against gold standard, which is the VICON system. It demonstrated root mean squared error of 8.77◦ for upper body limb orientation estimation when compared to gold standard VICON optical motion capture system.

Effective telerehabilitation technologies enable patients with certain physiological disabilities to engage in rehabilitative exercises for performing Activities of Daily Living (ADLs). Therefore, training and assessment scenarios for the performance of ADLs are vital for the promotion for telerehabilitation. In this paper we investigate quantitatively and automatically assessing patient's kinematic ability to perform functional upper extremity reaching tasks. The shape of the movement trajectory and the instantaneous acceleration of kinematically crucial body parts, such as wrists, are used to compute the approximate entropy of the motions to represent stability (smoothness) in addition to the duration of the activity. Computer simulations were conducted to illustrate the consistency, sensitivity and robustness of the proposed method. A preliminary experiment with kinematic data captured from healthy subjects mimicking a reaching task with dyskinesia showed a high degree of correlation (Cohen's kappa 0.85 with p < 0.05) between a human observer and the proposed automatic classification tool in terms of assigning the datasets to various levels to represent the subjects' kinematic abilities to perform reaching tasks. This study supported the use of Microsoft Kinect to quantitatively evaluate the ability of individuals with involuntary movements to perform an upper extremity reaching task.

Tele-rehabilitation has been widely studied in recent year, although a number of crucial issues has not been addressed. Quantitatively assessing exercise performance is vital in monitoring the progress in exercise based rehabilitation. This allows physiotherapists not only to refine rehabilitation plans, but also provides instant feedback to patients and facilitate the exercise performance in non-clinical setting. In this paper, we propose to evaluate the performance of upper extremity reaching tasks with in a kinematic perspective by assessing the smoothness of motion trajectories with the entropy of shape model, including curvature and torsion. The simulation result confirms that approximate entropy of shape model is consistent with the change of the smoothness in motion trajectory while it is capable of classifying six levels of the ability to perform upper extremity reaching tasks with higher accuracy.

Purpose

Few qualitative studies have explored patients' experience of food and eating following major upper gastrointestinal cancer surgery. The aim of this article was to explore the longer-term impact of different types of major upper gastrointestinal surgeries on people's relationship with food.

Methods

Twenty-six people having had major upper gastrointestinal cancer surgery greater than 6 months ago participated in semi-structured interviews. These interviews aimed to explore a person's physical, emotional and social relationship with food and eating following surgery. Interviews were tape-recorded, transcribed and analysed using an inductive thematic analysis approach.

Results

Interview findings revealed a journey of adjustment, grieving and resignation. The physical symptoms and experiences of people differed between types of surgery, but the coping mechanisms remained the same.

Conclusions

The grieving and resignation people experienced suggest adjustment and coping similar to that of someone with a chronic illness. Remodeling of health services is needed to ensure this patient group receives ongoing management and support.

 This research constructs an approach to engagement with Bama, Indigenous people from the Upper Mitchell River Watershed to develop a model of increased engagement and improved care of natural resource environments. It demonstrates that using Indigenous knowledge and experience can lead to better outcomes for both the land and its peoples.

Esta investigação visa o melhor entendimento do fenômeno do crowding em lojas, dentro do contexto do mercado brasileiro, procurando identificar como as respostas dos consumidores variam entre os de renda baixa e renda alta, e buscando fatores que possam melhor explicar essas diferenças. Para atender esses objetivos, adotou-se uma metodologia experimental para testar como o aumento da densidade humana afeta diferentemente as respostas dos consumidores de alta e baixa renda. Como moderadoras da relação entre densidade humana e as respostas de consumidores, utilizaram-se as variáveis renda (alta e baixa) e familiaridade (alta e baixa). Três experimentos de desenhos fatoriais permitiram o teste das hipóteses propostas, por meio de análises multivariadas: ANOVAs, ANCOVAs e regressões lineares. Os estudos demonstraram que renda efetivamente modera as respostas dos consumidores, pois existe substancial diferença entre os consumidores de alta e baixa renda para situações de alta densidade humana em lojas. Foram verificadas as seguintes respostas dos consumidores com menor renda em situações de alta densidade humana: atitude mais positiva e maior nível de satisfação do que os de alta renda; percepção de maior valor hedônico em lojas mais cheias de gente versus os de maior renda. Os resultados contrariam a maioria dos achados de estudos realizados nos Estados Unidos nas últimas décadas. Enquanto a maioria desses estudos indica uma resposta negativa à densidade humana, os resultados desta pesquisa evidenciaram que existem respostas mais positivas para o segmento econômico de consumidores de baixa renda. Constatou-se também o efeito moderador do controle informacional (familiaridade) na relação entre densidade humana e as respostas dos consumidores, e o efeito mediador da percepção de valor hedônico na relação entre densidade e satisfação. Os achados oferecem uma contribuição teórica para enriquecer o entendimento do fenômeno de crowding e ressaltam a importância de se considerar a diversidade do contexto socioeconômico para se garantir o desenvolvimento de generalizações que corretamente reflitam as várias realidades do mercado. Do ponto de vista gerencial, o estudo reforça a necessidade de o varejo desenvolver uma maior distinção em ambientes de loja para melhor satisfazer as diferentes preferências dos diversos segmentos de mercado.

In order to achieve better postures and decrease musculoskeletal risks adequate design of hand/box couplings for manual materials handling (MMH) are still needed. No studies evaluating upper limb movement thorough direct measurements during box handling in workplace were identified in the literature. In this study we describe the types of grip and movements adopted by ten workers when handling redesigned boxes with cutout handles between different heights on industrial pallets. The new handles were used by 90% of the workers through different types of grip. Electrogoniometric measurements showed relatively safe forearm and wrist movements, although elbow inadequate range of movement was recorded. Despite the good acceptance of the cutout by workers, the new design requires extra internal space in the boxes reducing applications for this alternative of box.

Purpose: To describe spontaneous blink kinematics in Graves' upper eyelid retraction (UER).Methods: The magnetic search coil technique was used to record spontaneous blinks of 15 healthy subjects (aged 23-56 years, 15 eyelids) and 15 patients with Graves' UER (aged 22-62 years, 15 eyelids) during a 5-min period of video observation, and the signals were digitized at 200 Hz (12 bits). Overall, a total of 2,798 blinks were recorded for the controls and 1,860 for the patients. The distance between pupil center and upper eyelid margin in the primary position of gaze (MRD) was measured with the Image J software.Results: The blinking rate of patients was lower than that of control subjects, with a mean (+/-SEM) blinking rate (blinks/min) of 13.0 +/- 1.7 for patients and of 20.0 +/- 2.1 for the controls (t = 2.58, P = 0.016). There were no statistically significant differences in blink amplitude between controls (22.7 +/- 3.1 degrees) and Graves' patients (24.7 +/- 3.3 degrees). However, while only 22% of the blinks performed by controls were smaller than MRD, this rate was 78% for patients. In addition, in blinks larger than 25, patients showed lower down-phase velocity than controls.Conclusions: Patients with Graves' UER show reduced blinks rates and abnormal blink kinematics, which might be related to the development of exposure keratitis in this disease.
